1.Analysis of the hypokalemia complication of dexamethasone used for hyperthyroidism preoperative preparation

Objective Analyzed the cause and time of hypokalemia complication that dexamethasone used for hyperthyroidism preoperative preparation.Methods The study consisted of 161 patients with hyperthyroidism disease,who were used the dexamethasone in his or her preoperative preparation at the First Affiliated Hospital of Zhengzhou University between August 2011 and July 2013.Results In the 161 hyperthyroidism patients,composed of 41 men and 120 women,twenty-seven patients happened hypokalemia and 92.6% occurred after the first.Twenty-six men patients developed hypokalemia complication when used dexamethasone for hyperthyroidism preoperative preparation,while only 1 woman patient occurred the hypokalemia complication.Among the 11 patients who with a history of thyrotoxic periodic paralysis,10 cases occurred after given dexamethasone hypokalemia,only 1 case has no hypokalemia.Conclusion Dexamethasone and other drug used in hyperthyroidism preoperative preparation,which was rapid,safe and effective,but it can lead to hypokalemia complication,especial in the 24 hours after the first use,should be brought clinical doctors attentions,and give timely diagnosis and treatment.
2.Pharmaceutical Care in the Management of Nausea and Vomiting Induced by Adjuvant Chemotherapy for a Postoperative Patient with Gastric Cancer

Objective To provide reference for clinical pharmacist participating in management of nausea and vomiting induced by tumor chemotherapy. Methods The process of pharmaceutical care for a patient with severe vomiting caused by adjuvant chemotherapy after gastric cancer operation was described. Antiemetic application and drug adverse reactions were analyzed. A new treatment plan was given by clinical pharmacist. Results The suggestions were adopted by clinician. The vomiting was controlled and drug adverse reactions were dealt with. Conclusion To reduce the risk and improve the income of antiemetic,clinical pharmacists should pay more attention to clinical practice guideline,drug interaction and adverse reactions, provide the most suitable suggestions for clinicians according to pharmacology and evidence-based medicine.
3.Early diagnosis of keratoconus with Orbscan-II anterior system.

Orbscan-II anterior system was used for early diagnosis of keratoconus. 48 Eyes of 24 patients with suspicious keratoconus were examined by Orbscan-II anterior system from Dec. 1999 to Dec. 2000 and followed up. The values of Diff and anterior chamber depth (ACD) were recorded. Results indicated that values of Diff and ACD were increased in 4 eyes of 2 patients with keratoconus trend during follow-up. Taking advantage of Orbscan-II anterior system to observe the values of Diff and ACD can early diagnose the sub-clinical keratoconus. The values of Diff and ACD can sensitively report the progression of keratoconus.

4.Effects of modified fixed reverse Twin-block appliance on soft tissue in treatment of adult Angle Ⅲ anterior crossbite

Objective To evaluate the curative effects of the modified fixed reverse Twin-block ap-pliance (TBA)combined Edgewise in the treatment of adult Angle Ⅲ anterior crossbite. Methods The appliance was used in 18 subjects with adult Angle Ⅲ skeletal malocclusion and clinical characters and X-ray cephalometry were then compared before and after treatment. Results The mandibular soft tissue changes could be achieved fast and the profile had changed effectively. Moderate labioclination of maxil-lary anterior teeth played a role in the improvement of axillary soft tissue protrusion. The cases were fol-lowed up as long as 3 years. No relapse occurred in all the patients. Anterior occlusion was normal. Con-clusion All findings indicate that adult Angle Ⅲ anterior crossbite can be quickly corrected with the mod-ified fixed reverse TBA combined Edgewise. The effect on mandibular soft tissue is significant.
5.The relationship of angiotension-Ⅱ to portal hypertension in patients with liver cirrhosis

Objective:To study the relationships of portal hypertensions to vasoactive substances-angiotensin(AT-Ⅱ)in cirrhosis patients.Methods:48 patients with liver cirrhosis(LC)(including 18 patients with compensated LC and 30 patients with decompensated LC) and 32 normal controls(NC) were studied. Color duplex doppler ultrasonography was used to study portal venous and splenic venous blood flow(PVBF and SVBF).Also,the blood concentration of AT-Ⅱ was detected.Then,analyze the correlation between PVBF and SVBF to AT-Ⅱ.Results:Portal hyperdynamic exists in cirrhosis patients from beginning to the end. Low level of AT-Ⅱ in compensated LC patient has no relation with portal hyperdynamic, high level of AT-Ⅱ in decompensated LC has positive relation with portal hyperdynamic.Conclusion:This study has directed significance of clinical treatment and diagnosis.

7.Prokaryotic expression and characterization of truncated mutant capsid protein of genotype IV hepatitis E virus.

A truncated mutant of the Open Reading Frame 2 (ORF2, aa384-606) was amplified from cDNA of genotype IV hepatitis E virus (HEV) by polymerase chain reaction (PCR), subcloned to expression plasmid pTO-T7, and expressed in Escherichia coli. SDS-PAGE and Western blotting were used to detect and identify the recombinant protein, namely rP24. After washing of inclusion bodies, dissolving in denaturing agents, refoldeding by dialysis, ion exchange chromatography and gel chromatography, dynamic light scatter was used to study the hydrated radius of rP24. Western blotting was applied to detect the immunoreactivity of rP24, and mouse immunity test and indirect enzyme linked immunosorbent assay (ELISA) were applied to evaluate the immunogenicity and the detection rate of HEV positive and negative serum. SDS-PAGE and Western blotting show that rP24 was highly expressed in the form of inclusion bodies after induction, and had strong immunoreactivity to monoclonal antibody (McAb) 15B2. After a multi-step purification of rP24, Western blotting indicated that the purified rP24 also had strong immunoreactivity to neutralizing McAb 8C11 and HEV positive serum, suggesting that rP24 simulated the nature structure of HEV capsid protein. Dynamic light scatter demonstrated that the average hydration radius of purified rP24 was 7.48 nm. The mouse immunity test showed that the purified rP24 also had good immunogenicity, and the period of serum antibodies converted from negative to positive was very short, but the antibodies maintained more than 20 weeks. Indirect ELISA tests showed that the detection rate of was the same as anti-HEV-IgG diagnostic kit (Wan Tai corporation). Taken together, the rP24 simulated the neutralizing epitopes of natural HEV, and had strong immunoreactivity and immunogenicity. It provided a basis for the further investigation of the difference of infection mechanism between genotype I and genotype IV HEV.

9.Framework, Measurement, and Social Support of Intellectual Disability Based on ICF

Objective To explore the definition, structure, terminology, framework, and models of social support and services, and mea-surement of intellectual disability based ICF approach. Methods The ICF approach and definition and approach proposed by American Asso-ciation on Intellectual and Developmental Disabilities (AAIDD) were adopted. Results The characteristics of intellectual disability are sig-nificant limitations in both intellectual functioning and adaptive behavior as expressed in conceptual, social, and practical adaptive skills, and onset before 18 years old. The framework, domains and contents of services and support for intelligence disability were proposed, as well as the updated measurement of intelligence disability based on new framework and definition. Conclusion The new framework of intel-ligence disability, system of service and support, and measurement are in accordance with the bio-psycho-social model of disability of ICF. The new paradigm will provide framework and tool of measurement, rehabilitation, education, and social services for people with intelli-gence disability.
10.Primary nephrotic syndrome in children:a clinical analysis of 964 cases

Objective To summarize clinical characteristics of primary nephrotic syndrome (PNS) in children. Methods The clinical and laboratory data of 964 children with PNS (including 747 new cases) were retrospectively analyzed,who were admitted between 1 August,1996 to 31 July,2006. Results Of 747 cases,568 were males and 179 females,with a median age of 4.7 years (0.3-17.3). The total number of children with PNS aged from 1 to 5 years was 385 (51.5%). Compared with children with nephritic type nephrotic syndrome (NNS),children with simple type nephrotic syndrome (SNS) had lower albumin (P0.05); Compared with normal values,children with PNS had lower IgG (P0.05). Main motivation of PNS relapse in children was upper respiratory infection,followed by irregular administration of glucocorticoids. Conclusion Children aged from 1 to 5 years are susceptible to PNS. There is humoral immunological dysfunction in PNS children. In the treatment of PNS,to relieve hyperlipidemia and to prevent and control infections is the key point. To strengthen propaganda and guidance about PNS is important as well.
