Objective To investigate the application value of the Mini-Nutritional Assessment (MNA) in assessing the nutritional status in middle-aged women with diabetes.Methods Using the normal range of traditional nutritional index as the reference standard,two hundred and forty middle-aged women with diabetes were recruited and their nutritional status were assessed by MNA and laboratory test (including PAB,Fib,TLC,CHOL),respectively.Finally,the results were analyzed and compared.Results The correlation coefficient of MNA value and BMI、PAB,Hb,TLC,CHOL were 0.571,0.614,0.781,0.568,0.218.MNA had good correlations with several traditional laboratory indicators regarding with nutritional status,with signifi-cant difference.In this study,the results of investigation by MNA showed that:the rates of malnutrition,potential malnutrition,and normal nutritional were 30％(72 cases),65％(156 cases),and 5％(12 cases),respectively.The sensitivity and specificity of MNA were 96.1％ and 98.2％ respectively.Laboratory testing resuits:malnutrition and normal nutrition were 31％ (74 cases) and 4％ (10 cases).Kappa test(Kappa=0.922):the results of MNA and laboratory testing had strong consistency.Conclusions MNA is feasible in assessing nutritional status in the domestic middle-aged women with diabetes.

Traditional medicinal plant cultivation and collection have also faced with challenges, such as lack of resources, deterioration of environment, germplasm of recession and a series of problems. Genetic engineering can be used to improve the disease resistance, insect resistance, herbicides resistant ability of medicinal plant, also can improve the medicinal plant yield and increase the content of active substances in medicinal plants. Thus, the potent biotechnology can play an important role in protection and large area planting of medicinal plants. In the development of medicinal plant genetic engineering, the safety of transgenic medicinal plants should also be paid attention to. A set of scientific safety evaluation and judgment standard which is suitable for transgenic medicinal plants should be established based on the recognition of the particularity of medicinal plants.

AlM: To observe the influence of phacoemulsification combined with intraocular lens implantation on retinal thickness changes of macular center fovea using optical coherence tomography ( OCT) in cataract patients.METHODS: Surgical eyes of 60 cases with cataract were as surgery group, the contralateral eye were as controls group. There was no complication before and during the surgery. Macular thickness was examined by OCT preoperatively and 1wk, 1, 3, and 6mo after surgery. The retinal thickness changes of macular center fovea were compared after surgery. The SPSS 17. 0 software was used for statistical analysis. The paired t-test and variance analysis were used in two groups before and after surgery for relevant statistical data analysis (α=0. 05).RESULTS: ln surgery group, the retinal thickness changes of macular center fovea at 1wk, 1, 3mo after treatment had significant differences compared with preoperation (P<0. 05). The retinal thickness changes of macular center fovea at 6mo after treatment had no statistical significance compared with preoperation ( P>0. 05). The retinal thickness changes of macular center fovea was significantly higher than other groups at 1mo after treatment (P<0. 05). The center fovea appeared to be abnormally thickened in 12 eyes, including 10 eyes of single fovea thickening and 2 eyes of cystoid macular edema in 2~4wk following surgery. Eleven eyes of them were resolved till 6mo postoperatively. ln control group, preoperation or after operation for 1wk, 1, 3, and 6mo respectively, P values were without statistical differences ( P > 0. 05 ). There were no statistical significant differences on retinal thickness of macular center fovea between two groups before and 6mo after surgery ( P>0. 05), while there were significant differences at 1wk, 1 and 3mo after surgery (P<0. 05) CONCLUSlON: The retinal thickness changes of macular center fovea increases significantly after phacoemulsification combined with intraocular lens implantation. ln a few of cases appear macular edema, but the majority of patients can be restored, prognosis is good.

Objective To explore the clinical situation of ankle arthroscopic surgery combined with sodium hyaluronate in the treatment of osteochondral lesions of the talus.Methods 40 cases of osteochondral lesions of the talus were treated in our hospital from June 2014 to October 2016 were treated with ankle arthroscopic surgery combined with sodium hyaluronate,the situation after the treatment were observed, and according to the patient's rehabilitation the rehabilitation exercises were conducted according to the patient's condition, and the therapeutic effect was analyzed.Results The total effective rate was 87.5%;The mean value of ankle function score before treatment was(34.26±2.13)points,the mean value of ankle function score after treatment was(74.35±2.07)points, the difference was statistically significant before and after treatment(P<0.05).Conclusion The clinical observation of ankle arthroscopic surgery combined with sodium hyaluronate in the treatment of osteochondral lesions of the talus, the treatment effect is better, and can help the patient's ankle joint function get improve, and ultimately improve the prognosis, it is worthy of reference.

The college students innovative test project,which advocates student-centered teaching pattern focusing on questions and subjects is an important component of the project on undergraduate teaching quality and teaching reform in medical college of Shanghai Jiao Tong university.The innovative test project has effectively fostered the innovative spirits,practical abilities,scientific attitudes and learning initiatives of the college students after being implementing for more than four years.Besides,it has changed the teaching philosophy of teachers and created a favorable atmosphere of innovative practice.Several problems have emerged during the implementation of the project including broad topics,limited sample size and incorrect motives for doing tests.Exclusive mechanism aiming at the long-term effect of the project should be established, the concept of the project should be promoted and the communication platform should be built in order to consummate this project.

Objective To investigate the clinical results of microsurgical free tissue transfers in pediatric reconstruction.Methods Between Janua~ 2004 and December 2010,twenty-seven free tissue transfers for reconstruction of various defects in 27 pediatric patients were performed.The average age at the time of reconstruction was 10.8 years(range:5-14 years).Fourteen patients were boys and 13 were girls.There were 5 tumor resection defects,sixteen posttraumatic defects and 6 burn contractures which consist of 19 fasciocutaneous flaps,six osteoseptocutaneous flaps and 2 muscle flaps.The mean operative time was 4 hours and 20 minutes.Results All flaps survived except 1 partial necrosis,with 96.3％ success rate.All patients were followed up 3 months to 4 years (mean,one and a half years).The appearance of the flaps was slightly overstaffed,but the color and texture were satisfactory.Conclusion Free tissue flap transfer is a safe,reliable,cost-and time-effective method for the reconstruction of various defects in children.

Objective To study the clinical application of endoscopic retrograde cholangiopancre-atography(ERCP)combined with biliary microlithiasis exploration in acute cholangitis.Methods The clinical data of consecutive 54 cases of acute cholangitis were analyzed.Before ERCP,all cases were examined by routine examinations,including abdominal uhrasonography,CT or magnetic resonance cholangiopancre atography(MRCP).The bile Was collected through ERCP or endoscopic mtsobiliary drainage(ENBD),then all the samples were choked by polarization microscope.Results Thirty-six cases(66.67%)out of 54 patients with common bile duct(CBD)stones or dilation of common bile duct were diagnosed by routine examination.ERCP Was performed in all patients,only 3 cases failed,and the rest were successful.Through ERCP,39 cases with CBD stones(76.47%),4 cases with CBD inflamnmtory smcmre were diagnosed,and 12 cases were normal.Among the 12 cases,9 cages were found with biliary microlithiasis by polarization microscope.The diagnostic effectiveness of ERCP combined with biliary microlithiasis exploration was highest(94.12%),and there Was significant difference between them(P＜0.05).Conclusions It may improve the diagnostic accuracy to combine ERCP with biliary microlithiasis exploration in CBD stones.Biliary microlithiasis may play an important role in the pathogenesis in unknown aetiology acute unexplained cholangitis.

Objective To investigate the pathological significance of expression of IL-23 in colon tissues of patients with steroid-dependent uncreative colitis at remission.Methods Expression of IL-23 was measured by means of Western Blot and immunohistochemistry SABC in inflammation repairing areas from 15 patients with steroid-dependent ulcerative colitis at remission, 30 patients with common ulcerative colitis at remission (15 treated with SASP and 15 with prednisone) and 10 normal colon tissues.The results were analyzed by SPSS 16.0.Results Compared with normal control, expression of IL-23 in patients of SASP maintenance therapy and prednisone with common ulcerative colitis were slightly increased ( P > 0.05), which was significantly lower that of steroid-dependent specimens ( P < 0.01).Conclusion Over-expression of IL-23 may plas a key role in the pathogenesis of steroid-dependent ulcerative colitis.

Objective To study whether the protective effects of fenofibrate on insulin resistance in mice of nonal-coholic fatty liver disease(NAFLD) are related to endoplasmic reticulum stress (ERS). Methods Male C57BL/6 mice were fed with high-calorie and high-cholesterol diet ( HCD) to induce a model of NAFLD, then one of those two groups from HCD was treated with fenofibrate 40 mg/( kg · d ) for 2 weeks ( HCF ) . Glucose tolerance test (GTT) and insulin tolerance test (ITT) were used to analyze the improvement on insulin resistance. The serum levels of triglyceride ( TG) , total cholesterol ( TC) , high density lipoprotein-cholesterol ( HDL-C) , low density lip-oprotein-cholesterol ( LDL-C) , alanine aminotransferase ( ALT) and aspartic transaminase ( AST) were detected. The pathological changes of livers were detected with HE and Oil Red O staining, the mRNA and protein expression of peroxisome proliferator-activated receptorα( PPARα) , glucose regulated protein 78 ( GRP78 ) and transcription factors GADD153 ( CHOP) were detected with real-time quantification PCR and Western blot analysis respectively. Results Compared with the SCD mice, the HCD mice showed significant insulin resistance, higher serum levels of TG, TC and LDL-C (P<0. 01, P<0. 05), lower serum level of HDL-C (P<0. 01), micro-and macrovesicular steatosis, ballooned hepatocytes and infiltration of inflammatory cells were observed in the liver, the expressions of PPARα and GRP78 at mRNA and protein levels were decreased (P<0. 05), however, the expressions of CHOP at mRNA and protein levels were increased ( P<0 . 05 ) . Fenofibrate intervention significantly improved insulin resist-ance and decreased the serum level of TG ( P<0. 05 ) . Fenofibrate also alleviated hepatic steatosis and inflammato-ry infiltration, and increased the mRNA and protein expressions of PPARα and GRP78, decreased the mRNA and protein expression of CHOP ( P<0 . 05 ) . Conclusion Fenofibrate significantly improves insulin resistance and de-creases the serum level of TG in NAFLD mice, which may be related to activating PPARα and relieving ERS.
