3.Antimicrobial resistance profile of the clinical isolates in Fuzhou First People's Hospital, Jiangxi during 2015

Objective To investigate the antimicrobial resistance profile of clinical isolates in Fuzhou First People's Hospital during 2015. Methods? The?bacterial?isolates?were?identified?using?ATB?Reader?semi-automatic?equipment?via?colorimetric?method.?The antimicrobial susceptibility was tested by disk diffusion method. The results were interpreted according to the CLSI breakpoints issued in 2015. All data were analyzed by WHONET 5.5 software. Results A total of 1215 non-duplicate clinical isolates were collected during 2015, including gram negative bacteria (70.6%) and gram positive bacteria (29.4%). The prevalence of MRSA was 50.0 % in S. aureus and the prevalence of MRCNS was 84.1 % in coagulase-negative Staphylococcus. Most MRSA strains (80.3?%)?were?resistant?to?erythromycin,?tetracycline?and?norfloxacin.?No?staphylococcal?isolate?was?found?resistant?to?vancomycin?or teicoplanin. The prevalence of ESBLs was 52.5% in E. coli and 34.7% in K. pneumoniae. Ten strains of Enterobacteriaceae were resistant to carbapenems. Less than10% of the P. aeruginosa isolates were resistant to amikacin, cefoperazone-sulbactam, meropenem and tobramycin. About 2.4% and 51.8% of the Acinetobacter isolates were resistant to polymyxin E and cefoperazone-sulbactam, respectively, while more than 75% of these strains were resistant to other commonly used antimicrobial agents. Conclusions Most of the clinical isolates are gram-negative bacilli in our hospital during 2015. Antimicrobial resistance remains an important clinical concern, which should be addressed seriously.
4.Therapeutic Observation of Hypersalivation in Cerebral Palsy Predominantly Treated with Acupuncture at Three Tongue Points plus Tuina

Objective To observe the clinical efficacy of acupuncture at the three tongue points plus tuina in treating hypersalivation in cerebral palsy.Method Sixty patients with hypersalivation due to cerebral palsy were randomized into a treatment group and a control group, 30 cases in each group. The treatment group was intervened predominantly by acupuncture at the three tongue points plus tuina, while the control group was by tuina alone. The clinical efficacies were compared between the two groups.Result The total effective rate was 96.7% in the treatment group versus 83.3% in the control group, and the difference was statistically significant (P<0.05).Conclusion Acupuncture at the three tongue points plus tuina is an effective approach in treating hypersalivation in cerebral palsy.
5.Reoperation of Laparoscopic Common Bile Duct Exploration in the Management of Choledocholithiasis

Objective To investigate the feasibility and safety of reoperation of laparoscopic common bile duct exploration in the management of choledocholithiasis in patients with biliary surgical history. Methods Thirty-one patients with choledocholithiasis and biliary surgical history were performed by reoperation of laparoscopic common bile duct exploration between January 2012 and January 2014 in our hospital.The adhesion was disconnected under the laparoscopy, and then the common bile duct was opened to introduce an endoscope for stone removal.A T-tube was placed for drainage. Results The operations were successfully completed in all the 31 cases, without conversion to open surgery.The vital signs of those patients were smooth during the operation.The operation time was 65-125 min, with a mean of (118 ±21) min.The volume of blood loss was 10-75 ml during operations, with a mean of (39 ±19) ml.The patients could take food normally 24-48 h after operation, with a mean of (27 ±13) h.The drainage tubes were pulled out 48 -72 hours after operation.No severe complications, such as residual stones, bleeding, gastrointestinal leakage, or pancreatitis happened during hospitalization, except for 1 case of biliary leakage and 1 case of incision infection.The patients stayed in hospital for 6-9 days after operation, with a mean of (7.0 ±1.5) days.No biliary stricture or recurrence was found during a 3-24 months of follow-up (mean, 15.6 months) in 31 cases. Conclusion Reoperation of laparoscopic common bile duct exploration by experienced laparoscopic specialists for choledocholithiasis in patients with biliary surgical history is relatively safe and effective.

8.Repair of cleft hard palate with absorbable membrane of poly-DL-lactic acid: A clinical report

Objective:To study the effects of poly-DL-lactic acid(PDLLA) biomembrane in the repair of cleft hard palate. Methods:Thirty-two cleft palate patients, age 2.67 to 12.83 years old, were treated. The traditional surgical method was used to close the cleft soft palate, and the PDLLA biomembrane was implanted into the surgical gap between the periosteum and bone at the hard palate and fixed with suture to close the cleft hard palate. Clinical follow-up was conducted for 6 months.Results:Operations on all 32 patients were completed successfully.The average surgical time was not prolonged, and post-operative complication was not increased. Wound healing of soft palate, uvula and hard palate was uneventful with no incidence of fistula or dehiscence. Conclusion: PDLLA absorbable biomembrane can be used to repair cleft hard palate.
9.Prevention of posterior capsule opacification by adenovirus-mediated herpes simplex virus thymidine kinase/ganciclovir system

Background There is no effective preventing way for the posterior capsular opacification(PCO) after cataract surgery.Objective The aim of this study was to investigate the preventing effect of adenovirus-mediated herpes simplex virus thymidine kinase/ganciclovir(HSV-tk/GCV) system on PCO.Methods Twelve New Zealand white rabbits were divided into BSS+BSS group,BSS+GCV group,HSV-tk+BSS group and HSV-tk+GCV group at random.Phacoemulsification was performed in bilateral eyes of rabbits,and 0.1ml of BSS or adenovirus vector with HSV-tk gene were injected into lens capsular bag during the operation based on the grouping.In 12 hours after operation,0.1ml BSS was injected into the anterior chamber daily for 3 times in BSS+BSS group and HSV-tk+BSS group,and 0.1ml GCV was used in the same way in BSS+GCV group and HSV-tk+GCV group.The situations of the anterior segment and the posterior capsule were examined under the slit lamp postoperatively.All the wet weight of capsular bags was assessed using electronic balance.The capsular membrane was histopathologically examined in the 4th week after injection.PCO was graded according to the method of Couderc.Results Mild corneal edema and aqueous humor opacity were observed in all the eyes within 3 days and returned to normal level gradually within 2 weeks postoperatively.PCO scores were higher in BSS+BSS group,BSS+GCV group,HSV-tk+BSS group than the HSV-tk+GCV group,showing the significant differences between them(H=2.647,P=0.008;H=2.939,P=0.003;H=2.884,P=0.004).No obvious difference was seen in PCO score among BSS+BSS group,BSS+GCV group,HSV-tk+BSS group(P>0.05).The wet weight of the capsular bags in HSV-tk+GCV group was reduced in comparison with BSS+BSS group,BSS+GCV group,HSV-tk+BSS group with the statistically significant difference between them(q=9.93,q=10.15,q=10.07,P<0.05).Histopathological examination revealed that mild proliferation of lens epithelial cells(LECs) at the capsular equator and migration toward the posterior capsule to form a thin soemmering ring in the periphery of the capsular bags and thin fibromembrane at the center of the capsular bags without cells or cortices in HSV-tk+GCV group.However,in BSS+BSS group,BSS+GCV group and HSV-tk+BSS group,the proliferation and migration of LECs were more obvious to form a dense soemmering ring in the periphery of the capsular bags and thick fibromembrane in the center of capsular bags.Conclusion Adenovirus-mediated HSV-tk/GCV system can effectively prevent PCO.
