1.VEGF in the treatment and prognosis of lung cancer

Angiogenesis plays a critical role in growth and metastasis of lung cancer.The strongest tumor angiogenesis factor known is the vascular endothelial growth factor (VEGF).Therefore,anti-VEGF therapy which can restrain tumor angiogenesis is an important form of the targeted therapy of lung cancer.The expression of VEGF is also significant to the prognosis of lung cancer.
2.Complex Bacteria Infections in Severe Hepatitis and Hepatocirrhosis

OBJECTIVE To investigate the susceptible factors and pathogenic bacteria in severe hepatitis and hepatocirrhosis cases infected by complex bacteria.METHODS We reviewed the severe hepatitis and hepatocirrhosis cases admitted to our hospital during 2004-2007,who infected by complex bacteria,to investigate the risk factors and pathogenic bacteria.RESULTS In 216 cases of severe hepatitis and hepatocirrhosis,27 cases were infected by complex bacteria(12.5%).After testing in 66 strains of bacteria,there were 22 strains of Gram-negative bacteria(33.3%),10 strains of Gram-positive bacteria(15.2%),and 34 strains of fungi(51.5%),most of the cases were at terminal stage,with long-term hospitalization,with different underlying diseases,repeated use of broad-spectrum antibiotic and hormone,and invasive operations.All of the factors might induce the complex bacteria infections.CONCLUSIONS The complex bacteria infections in the severe hepatitis and hepatocirrhosis cases are critical and difficult to cure.The underlying diseases should be treated actively;the detection of pathogenic bacteria and drug sensitive test should be carried out;the strict measurement of disinfection and isolation are strongly suggestedto decrease the rate of infection of bacteria and prevent the hazards.
3.Serum TFAR19 Activity in Preeclampsia Patients

Objective To explore serum TFAR19 activity in preeclampsia patients.Methods 43 of cesarean delivery patients with preeclampsia in the Huairou Maternal and Child Health Hospital of Beijing were recruited and the peripheral venous blood was collected.Among them,severe preeclampsia patients were 18,mild preeclampsia were patients 25.In addition,40 unrelated healthy cesarean puerpera from a routine health survey were enrolled as controls.Enzyme-linked immunosorbent assay(ELISA)was used to detect serum TFAR19 activity in preeclampsia patients.That was absorbancy-A catalytic genera-ted by TFAR19.Results The activity of TFAR19 severe preeclampsia patients,mild preeclampsia patients,and healthy con-trols were 43.56±2.93,33.38±2.32 and 23.63±2.94,respectively.The TFAR19 activity in each group was statistically significant difference (F = 10.65,P <0.01).Compared with healthy controls,the TFAR19 activity in severe preeclampsia group (q=6.384,P <0.001),mild preeclampsia group (q = 3.478,P =0.016)was statistically significant difference.The TFAR19 activity in preeclampsia patients subgroups (severe preeclampsia group,mild preeclampsia group)was statistically significant difference (q = 2.993,P = 0.037).Age between each groups was no-statistically significant difference (F =0.091,P =0.913).Gestation age and neonatal weight betweeneach groups was statistically significant difference (F=4.37, P =0.016;F=35.06,P <0.001).Conclusion The increase of TFAR19 could promote apoptosis,participating the occur-rence of preeclampsia.The content of TFAR19 could be increased with the severity of the preeclampsia.
4.Market Positioning and Marketing Strategies for Shanghai Ruijin-Harvard Heart Center

Objective To perform the research on the domestic hospital marketing. Methods Through deep discussion,data analysis and marketing survey,we conducted a research on the market positioning and marketing strategies for Shanghai Ruijin-Harvard Heart Center which is a Sino-foreign joint venture. Results The demand is much bigger than the current supply in the market for cardiovascular diseases.The establishment of the center meets the demands of the market. Conclusion The center will focus on the adult patients in Shanghai and Yangtze Delta,and combine the external,internal and interactive marketing strategies.It may provide some useful experience for the Chinese hospital management,especially for hospital marketing.
5.Relationship between cervical curvature index and cervical spine angle

BACKGROUND: How to evaluate the cervical curvature correctly has been a greatly concern of spine surgeons. In recent studies, cervical curvature index(CCI) and cervical spine angle(C2-7) were often used by foreign researchers.OBJECTIVE: Ishihara method and spine angle(C2-7) method were applied to measure the cervical curvature, and the correlation between the two methods was compared as well.DESIGN: An observational controlled trial based on the patients.SETTING: Department of Orthopaedics of Peking Union Medical College Hospital.PARTICIPANIS: Totally 63 cases including 34 male and 29 female patients with cervical spondylopathy were involved in the Department of Orthopaedics of Peking Union Medical Clooefe Hospital from July 2002 to July 2003.METHODS: The 63 cervical spondylopathy patients' cervical spine lateral X-ray films were evaluated.MAIN OUTCOME MEASURES: Main outcomes: ① The patients' CCI and the cervical spine angle; ② Relationship between their CCI and the cervical spine angle. Secondary outcomes: ① Regressive equation derived from their CCI and the cervical spine angle; ② Comparison of CCI and the cervical spine angle between male and female. RESULTS: The CCI(Ishihara method) was averaged 10. 1 (SD, 11.6), and the cervical spine angle(C2 -7) 17. 5 °(SD, 13.5). A highly significant correlation was found between the two indexes( P<0.01). Sex and age factors were found to be irrelevant to them.CONCLUSION: The CCI (Ishihara method) and the cervical spine angle (C2-7) are significantly correlated with each other.

7.Autogenous bone graft harvested from trephination for arthrodesis of one single cervical intervertebral space following decompression

Objective To investigate whether the bone harvested from Cloward discectomy with trephine could substitute for iliac-crest bone grafts in arthrodesis procedure. Methods We reviewed the 30 patients with cervical spondylotic myelopathy involving a single level, which had been managed with anterior trephination discectomy and arthrodesis by the bone chips harvested from within the trephine. The bone chips harvested with trephine were then trimed and implanted erectedly similar to Robinson arthrodesis procedure. All cases had been followed-up for an average of 4.75 years. The latest results were analyzed according to JOA score system and recovery rate. The fusion outcome were assessed by anteroposterior and flexion and extension lateral radiographs of the cervical spine. Results At the latest followed-up examination, clinical results were excellent in fifteen patients(50% ), good in eleven(36.7% ), fair in three(10% ) and poor in one (3.3% ). X-ray showed solid fusion in all, and no dislodgment of the grafts. However, the cervical curves had a little loss. There was no significant bone grafts subsidence. Conclusion In cervical anterior decompression the bone chips obtained in trephination could substitute for other type of bone grafts for arthrodesis. The postoperative outcomes were not affected by the loss of the cervical curves.
8.Evaluation of radiographic methods for predicting scoliosis correction

Objective To evaluate the standing bending, traction and fulcrum bending radiographies in the prediction of scoliosis correction. Methods 27 patients with thoracic scoliosis with an average of 16.4 years old were selected for the evaluation. All the patients were treated with the posterior correction and bone grafting fusion, and fixed with the third generation system of the segmental vertebral fixation, such as TSRH, CD, or CD+Horizon. Measurement of the Cobbs angle was carried out in the preoperative posterior anterior radiographies with the natural position, standing bending, traction and fulcrum bending and in the postoperative posterior anterior radiographies. Results Mean of Cobbs angle at the thoracic curve after operation was 31 degrees, which decreased significantly in comparison with the preoperative data. Average correction rate was 51.6%. The postoperative Cobbs angle at the thoracic curve was positively correlated with the preoperative Cobbs angle in the standing bending, traction and fulcrum bending radiographies. It was significantly different from the preoperative Cobbs angle in the standing bending and traction radiographies, while no difference was seen with the fulcrum bending radiography. Conclusion The fulcrum radiography can be used to evaluate the flexibility of the thoracic scoliosis, and is better than the standing bending and traction radiographies. The three methods should be used together for the analysis of the scoliosis to predict the result of the scoliosis correction.
9.Effect of Preoperative Regional Intra-Arterial Infusion Chemo therapy on the Prognosis of Advanced Gastric Cancer

Objective To evaluate the effect of p reoperative regional intra-arterial infusion chemotherapy on the prognosis of a dvanced gastric cancer. Methods The clinical data of 80 patients who u nderwent curative resection for advanced gastric cancers were summarized.Among t hem,33 patients carried out preoperative regional intra-arterial infusion chemo therapy were as the interventional chemotherapy group,and the remaining 47 patie nts were the control group.Eleven factors including clinical and pathological da ta,treatment procedures and molecular biological makers that contributed to the long-term survival rate were analyzed by Cox multivariate regression analysis.Results The 5-year survival rate of the interv entional group was 59.3%,and the control group 47.6% .There was significant difference between two groups ( P
10.Biomechanical study of various pedicle screw revision techniques

Objective To determine the best method of revision for failed pedicle screw by investigating the change in maximum insertional torque and axial pullout strength after placing a larger diameter and/or longer screw or augmenting the failed hole with bone shims or PMMA. Methods Six fresh male adult cadaveric spines from T10-L5 were harvested. These specimens, aging from 23 to 51 years with an average of 36.7 years, were divided into six groups: 1)Using a larger diameter screw; 2)Using a longer screw; 3)Using a larger and longer screw; 4)Augmenting with bone shims; 5)Augmenting with PMMA; and 6)Reinsertion after being backed out. The first three groups were subdivided into two groups. Maximum insertional torque and axial pullout strength of each original screw were recorded as control data. Change of maximum insertional torque and axial pullout strength between original and corresponding revision screws were noted. Measurements were analyzed using one-way ANOVA statistically by SPSS10.0. Insertional torque change after simply removing and replacing a 5.0 mm?40 mm screw was also measured. Results Among the changes in pedicle dimensions, the greatest improvement in peak insertional torque and axial pullout strength occurred when using a 2 mm larger and 10 mm longer screw, with an increase of 37.06% and 18.22%; a 2 mm larger screw increased peak insertional torque and axial pullout strength by 20.15% and 19.99% respectively, while a 1 mm larger and 5 mm longer screw increased by 19.23% and 10.07% respectively; use of a 5 mm or 10 mm longer screw decreased peak insertional torque by 32.80% and 14.02% respectively, with axial pullout strength down by 27.36% and up by 43.25% respectively. Use of bone shims caused a decrease of the insertional torque and axial pullout strength by 14.99% and 29.34% respectively. Hole augmentation with PMMA lead to a significant increase in insertional torque but a decrease in axial pullout strength by 37.40%. Simply removing and replacing an original screw resulted in a decrease in insertional torque by 34.22%. Conclusion Revision for pedicle screw is most effective when using a 2 mm larger diameter screw, next by using a 1 mm larger diameter and 5 mm longer screw. Use of a bone shim should be avoided. The efficacy of hole augmentation with PMMA need to be further investigated.
