1.Influence of waling training on pulmonary function and blood glucose in aged patients with type 2 dia-betes mellitus

Objective:To explore influence of walking training on pulmonary function and blood glucose in aged pa‐tients with type 2 diabetes mellitus (T2DM) .Methods :A total of 163 T2DM patients treated in our department were selected .According to number table ,they were randomly divided into routine group (n= 80 ,performed DM diet and routine insulin therapy strictly ,and lived as usual) and intensive group (n=83 ,received intensive walking train‐ing based on strict DM diet and routine insulin therapy ,namely walking ≥10000 steps/d for eight weeks) .Pulmona‐ry function and blood glucose indexes were compared between two groups before and after intervention .Results:Compared with before treatment ,after treatment ,there was significant rise in pulmonary function and significant reductions in blood glucose indexes in intensive group , P<0.01 all;compared with routine group after treatment , there were significant rise in vital volume [(2622.0 ± 323.3)ml vs .(2987.0 ± 405.6)ml] ,forced expiratory volume in one second [(2003.0 ± 279.7)ml比(2392.0 ± 323.6)ml]and vital volume/body weight [(44.0 ± 4.2) ml/kg vs . (47.0 ± 4.7) ml/kg] ,and significant reductions in levels of glycosylated hemoglobin A1c [(5.63 ± 2.63)mmol/L vs . (3.82 ± 1.41) mmol/L] ,fasting blood glucose [(7.90 ± 1.41) mmol/L vs .(5.28 ± 1.18) mmol/L]and 2h postpran‐dial blood glucose [(10.48 ± 1.43) mmol/L vs .(8.09 ± 1.68) mmol/L]in intensive group , P<0.01 all .Compared with before treatment ,there were no significant changes in above indexes in routine group after treatment , P>0.05 .Conclusion:As a low intensity aerobic exercise ,walking training can effectively improve pulmonary function , glucose metabolism in aged patients with type 2 diabetes mellitus ,it is help to improve quality of life .
2.Experimental evaluation of the ventilatin heat dissipation of high altitude partial pressure suit and FTF-2 air ventilation suit

Objective In order to provide important theoretical basis for further improving design of DC-7/7A high altitude partial pressure suit and FTF-2 air ventilation suit (named as A suit and B suit), the performance of ventilating heat dissipation of DC-7/7A and FTF-2 was studied. Methods Five healthy volunteers were enrolled to took participate in A group and B group tests. Environmental weather conditions for A and B suits were set at Tdb 40℃ and 45℃,rh 45% and 40% respectively, and the test time was 90min and 120 min respectively. Results During the test for A group and B group, Ts ranged from 31.5℃ to 34.5℃ for human comfort. Average skin temperatures of the lower part of the body were from 33.1℃ to 33.8℃. Skin temperatures of the legs did not obviously change or slightly lowered. Heart rate showed an increase of 12 beats/min and 14 beats/min. Sweat quantities were measured to be 0.288?0.056kg/1.5h and 0.445kg?0.077 kg/2h respectively. Evaporation rates were 77.4?3.6% and 84.2?2.4% respectively. Subjective uncomfortable sensation was more obvious as test time went on. Conclusion The results of the ventilating heat dissipation for both groups were quite good, but the findings suggested that the number of the ventilating holes for some important sites should shall be re-considered. Pilots wearing A group and B group with ventilation function would still suffer from mild heat stress and obvious subjective discomfort in hot enviroment.
3.The effect of the′cocktail′wet compress on the prevention of arm swelling after trans radial percutaneous coronary procedures

Objective To test the effect and safety of′cocktail′wet compression on alleviate the swelling arm after the inter‐vention through the radial artery .Methods Patients whose arm were swollen after the intervention through radial artery were di‐vided into two groups (experiment group ,47 patients vs .control group ,46 patients) .The experiment group adopt the′cocktail′wet compress plus traditional therapy (including elevate and immobile the swelling arm);the control group only use the traditional ther‐apy .The levels of pain ,swelling index and the number of the blister were compared between two groups .Results None of the limbs necrosis was observed in two groups .The level of pain(2 .62 ± 0 .86 vs .5 .46 ± 1 .02) ,swelling index (3 .3% ± 2% vs .5 .8% ± 3% ) of the arm ,and the number of blister (0 vs .4) in experiment group were lower than in the control group (P<0 .05) .Conclusion The bundle of therapy strategy which named'cocktail'wet compress can lessen the swelling of the arm induced by intervention through radial artery ;it is both safe and effective thus deserves widely application .
4.Construction and Identification of NBS1-targeting microRNA Expressing Eukaryotic Vector

Objective:To construct NBS1 microRNA expressing eukaryotic recombinants,and identify biological activity of recombinants in Hela cell after transfection.Methods:According to sequence of NBS1mRNA,the NBS1 pre-microRNA was designed and synthesized,then cloned into the GFP reporter pcDNA6.2-GW/EmGFP-miR vector and transfected into Hela cell line.To detect integrity of inset fragment through colony PCR and sequencing analysis.The biological activity of recombinants through identify interference efficiency of NBS1 microRNA recombinants by way of Real-Time PCR and Western blot were determined.Results:Sequences of inset fragment in four microRNA expressing recombinants were correct.NBS1 mRNA and protein expression of four microRNA recombinants were decrease,which is the lowest in the NBS1mi-2 group.Conclusion:Four NBS1-targeting microRNA expressing recombinants all have biological activity in Hela cell line,and NBS1mi-2 recombinant has the most interference efficiency.The microRNA expressing plasmid which were successfully constructed and lay foundation for the studies on the tumor gene therapy of microrna targeting NBS1.

Diabetic gastroparesis (DGP) is one of the most common complications of diabetes. Acupuncture is an ef-fective treatment for DGP in recent years. Literatures on acupuncture therapy for DGP treatment were screened for the past 10 years. Statistical analysis was made on acupoints used in the literatures in order to find the most com-monly used acupoints and combinations. This article was aimed to explore and summarize the use of the acupuncture law for the better service of clinical practice.
7.Free anteromedial thigh perforator flap transplantation for repairing soft tissue defect in lower extremity

Objective To explore the clinical effect of anteromedial thigh perforator flap for repairing soft tissue defect in lower extremity.Methods From January 2009 to December 2012,12 patients with soft tissue defect of lower extremity were treated with free anteromedial thigh perforator flap.Among them,there were 5 cases of wound in front of tibia,4 cases of dorsal foot wound,3 cases of heel wound;the wound size was 3.0 cm ×3.5 cm-7.0 cm × 9.5 cm;3 cases were repaired with free perforator flap of descending genicular artery,2 cases with free anteromedial thigh perforator flap,and 7 cases with medial vastus muscle perforator flap.Results All the transplants survived.Among them,1 case of anteromedial thigh perforator flap and 1 case of descending genicular artery perforator flap appeared vascular crisis postoperative.After anticoagulation and antispasmodic treatment aggressively,the flaps survived.One case of descending genicular artery perforator flap necrosis in edge,and then were cured by changing dressing.Twelve cases were followed up 6-12 months after operation (average 8.6 months),and the texture of flaps were good,the appearance appropriate ;7 sensation of 5 free flaps after nerve anastomosis came back to S; the donor sites in all cases were sutured directly and healed,leaving only the linear scar,and no effect on function.Conclusion The anatomy of anteromedial thigh perforator flap is constant,and the operation is convenient.The clinical result of repairing soft tissue defect of lower extremity is satisfying.
8.Study on inhibitory effects and mechanism of lipophilic components in Salvia miltiorrhiza on angiogenesis in vitro.

In this study, the human umbilical vein endothelial cell model was used to study the regulating effect of lipophilic components in Salvia miltiorrhiza on angiogenesis, and explore its possible mechanism. The cell model was established to determine the effect of lipophilic components in S. miltiorrhiza on the proliferative activity and migration capacity of endothelial cells. Then the realtime fluorescence quantification PCR technology was applied to detect the changes in the gene expressions of angiogenesis-related cytokines VEGF-A, VEGF-C and MMP-9. The results showed that 5 mg x L(-1) lipophilic components in S. miltiorrhiza could inhibit the proliferation and migration of endothelial cells, and reduce the expression of VEGF-A and MMP-9 genes. It indicated that lipophilic components in S. miltiorrhiza may inhibit the proliferation and migration of endothelial cells by inhibiting the expression of VEGF-A and MMP-9 genes, so as to show the inhibitory effect on angiogenesis.

9.Analgesic effects of electroacupuncture combined with Celebrex on rats with tibial cancer pain

OBJECTIVE: To establish a proper experimental model of bone cancer pain in rat for acupuncture research, and observe the pain-relieving effect of electroacupuncture (EA) and/or Celebrex on bone cancer pain in rats. METHODS: The rat model of bone cancer pain was established by percutaneous direct puncture technique and inoculating the rat mammary gland carcinoma cells Walker 256 into tibial medullary cavity directly, and evaluated by detecting the bone tumor growth and mechanical allodynia. The effects of daily EA treatment and/or Celebrex treatment on the rat mechanical allodynia after intratibial Walker 256 inoculation was observed in this study. RESULTS: Significant mechanical allodynia in ipsilateral hind paw and tumor growth in proximal end of tibial bone of rats in the untreated group were observed after intratibial Walker 256 inoculation. The mechanical allodynia thresholds in rats that received EA or 5 mg/(kg.d) Celebrex treatment showed no significant difference as compared with that of rats in the untreated group. However, the mechanical allodynia thresholds of rats in 10 mg/(kg.d) Celebrex group showed significant increase after 22- and 26-day treatment as compared with that in the methyl cellulose (MC) group. There was significant difference between rats with EA combined with 5 mg/(kg.d) Celebrex treatment and rats in the untreated group after 10-, 18- and 23-day treatment. CONCLUSION: EA and 5 mg/(kg.d) Celebrex have synergistic effect on pain relieving and their combined use may enhance the analgesic effect on bone cancer pain.
10.Treatment of infectious bone and soft tissue defects following tibial shaft fracture of Gustilo type Ⅲ B using free flap and Ilizarov bone transport

Objective To report treatment of infectious bone and soft tissue defects caused by tibial shaft fracture of Gustilo type Ⅲ B using free flap and Ilizarov bone transport.Methods Nineteen patients who had suffered from infectious bone and soft tissue defects following tibial shaft fracture of Gustilo type Ⅲ11 B were treated from May 2010 to February 2015.They were 15 men and 4 women,aged from 21 to 58 years (average,45.3 years).Their course of disease ranged from 16 to 21 months,averaging 17.9 months.The area of their infectious defects ranged from 10 cm × 6 cm to 21 cm × 12 cm,and the length of their bone defects from 5 to 11 cm (average,7.4 cm).They were treated with debridement,simple external fixation to reconstruct bony support,coverage of wounds with free flap,and stuffing the dead space with antibiotic concrete beads,followed by Ilizarov bone transport and bone graft after control of infection to reconstruct the defective tibia and function of the affected limb.Results All the flaps survived.Necrosis occurred at the distal margin of one flap but responded to dressing.Pin tract infection occurred in 4 cases but also responded to dressing and antibiotic therapy.Autografts of iliac cancellous bone were implanted into the gliding and traction ends of the bone fragments one month after bone transport had come to rest.All the patients achieved direct bony union.The patients were followed up for an average of 25 months(range,from 19 to 36 months).No secondary fractures or angular deformity was observed.The total treatment time averaged 17.9 months,with no recurrence of infection.According to the Puno score system for functional evaluation at the last follow-up,7 cases were rated as excellent,6 as good and 6 as fair.Conclusion The infectious bone and soft tissue defects caused by tibial shaft fracture of Gustilo type Ⅲ B can be treated by free flap and Ilizarov bone transport,resulting in definitely positive outcomes.
