Objective:To study the analgesic effect of rofecoxib on patients with advanced carcinoma.Methods:In a randomized clinical trial,108 pathologically diagnosed cancer patients were treated with rofecoxib(dose 25～50mg/d) and the pain relief was observed among all the patients.Results:Rofecoxib was effective in control of cancer related pain in an oral dose of 25～50mg/day with a total effective rate of 84.26%.The main adverse reactions such as drowsiness,dizziness,nausea,vomiting,discomfort in stomach and constipation were slight.Conclusion:Rofecoxib is effective for relieving cancer related pain with sustained duration and slight in adverse reaction and convenient for administration.

Objective; To study the efficacy, side effect and economic value of morphine controlled - release tablets plus diclofenac sodium and antidepressive drugs in the treatment of pain caused by metastatic bone carcinoma. Methods: 83 patients with pain of meta-static bone carcinoma were randomly divided into two groups. Group I was treated only with morphine controlled - release tablets, Group II with morphine controlled - release tablets, diclofenac sodium and amitriptyline. The dosage of morphine controlled - release tablets, the adverse reaction and the medical cost were observed. Results: The dosage of morphine controlled - release tablets and the medical cost in Group II were obviously lower than those in Group I. The side effects were not significantly different between the two groups. Conclusions: Morphine controlled - release tablets plus diclofenac sodium and antidepressive drugs can treat the pain of metastatic bone carcinoma effectively, relieve the economic burden of the patient while not to increase the side effects.

Objective To look for a better way to assay vascular endothelial growth factor(VEGF) and explore the correlation of serum VEGF levels with platelet and leucocyte count in breast cancer by comparison of peripheral blood plasma, serum and P-APR. Methods Blood samples were obtained from 42 patients with breast cancer and 20 healthy volunteers. Serum, plasma and P-APR samples were analyzed for VEGF using a commercially available sandwich ELISA, and assayed for semiquantitative measurement using microplatereader. At the same time, samples were analyzed by hematology analyzer. Results The level of VEGF in peripheral blood positively correlated to platelet (health control group: r=0.424, P=0.063) and leucocyte (health control group: r=0.443, P=0.050). The difference in VEGF values of P-APR between the localized breast cancer group and the health control group was statistically significant. Conclusion The level of VEGF in peripheral blood positively correlates to platelet and leucocyte. Platelet and leucocyte possibly secrete VEGF in vitro. Compared with serum and plasma, P-APR is the best section assaying VEGF in peripheral blood.

Objective To investigate the significance of blood donation in preventing metabolic syndrome (MS)by analyzing the changes of the blood pressure and the related biochemical indexes before and after blood donation in the blood donors with high risk of MS.Methods The blood specimens of 102 blood donors,aged 45 -55 years old with the systolic blood pressure of 110 -140 mmHg and the diastolic blood pressure of 75 -90 mmHg,the body mass index> 24 kg/m2 ,were selected as the subjects.The blood pressure,heart rate,iron reserves,blood glucose,blood lipids,and the other indicators were determined before blood donation. All the above indicators were tested again on 42 d after blood donation.Results The blood pressure,iron reserves,blood glucose and LDL/HDL ratio after donation showed significant changes compared with before blood donation(P <0.05 ).Conclusion The changes of the MS related blood indexes exist before and after blood donation,blood donation has a certain significance for preven-ting MS in the blood donors with high risk of MS.

ObjectiveTo investigate the teaching effect of problem-based learning(PBL) in clinical probation of gynecology and obstetrics.Methods48 students who majored in obstetrics and took internship in Xiamen Maternity and Child Care Hospital were selected as study subjects and were randomly assigned to control group and observation group.Traditional teaching pattern and PBL were performed in the above-mentioned groups respectively.The score of final test on maternity nursing and self-evaluation on capacity advancement were compared between two groups.ResultsThe score of observation group were significantly higher than control group (P＜0.05); Similarly,the self-evaluation of observation group after internship was also elevated with significance (P＜0.05) when being compared with control group.ConclusionEvidence showed that PBL was capable of improving the knowledge of maternity nursing,and it was also been proved effective in fostering the ability of self-learning,communication,and solving problem.Therefore,the PBL was appropriate to be promoted in the internship of nursing education.

Objective To compare the emergence characteristics between propofol total intrave-nous anesthesia (TIVA)and sevoflurane induction and maintenance anesthesia (VIMA)under the guidance of Narcotrend,and to see if the recovery of consciousness after different anesthesia regimen will be of identical Narcotrend index (NTI).Methods Sixty ASA Ⅰ or Ⅱ patients undergoing elective transsphenoid pituitary tumor resection were randomized to receive deep anaesthesia (D)or light anaesthsia (L)with propofol (P)/remifentail (group PD and group PL)or sevoflurane (S)/remifentanil (group SD and group SL).During surgery,anesthesia was maintained at a depth of NTI at 46-37 or 56-47 for deep or light anesthesia respectively.After surgery,group comparisons of the NTI at extubation,recovery time,and the time interval between NTI 80 and extubation (Tdelay )were performed by analysis of variance.Results The NTI at consciousness was significantly lower in group PD (84.2±2.7)and PL(84.9±2.5)compared with group SD (88.6 ±3.4)and group SL (89.9 ± 3.3)(P < 0.05 ).The Tdelay was significantly longer in group SD (9.4 ± 4.6 )min compared with group SL (4.7±2.4)min (P <0.05),but there was no difference between group PD and group PL. Compared with other groups,the recovery time was longer in group SD (P <0.05).Conclusion The results indicate that the NTI values at recovery of consciousness are not identical between patients re-ceived proposfol or sevoflurane anesthesia.

ObjectiveTo study the relationship between carotid sclerosis and lipoprotein (LP) (a) in patients with coronary artery disease. MethodsOne hundred and fifty-nine coronary artery disease patients with coronary arteriongraphy (CAG) positive were selected as the study group, and Gensini points were taken in these patients. Forty CAG normal patients was in control group, and all of them were examined with ultrasound, and serum levels of LP (a) were tested. According to ultrasound, study group were divided into 4 subgroups: normal intima group(24 cases), mild stenosis group(41 cases), moderate stenosis group (82 cases) and severe stenosis group ( 12 cases). ResultsThe rate of carotid sclerosis in study group [84.9％ (135/159)] was significantly higher than that in control group [42.5％ ( 17/40 )] (P ＜ 0.01 ). Gensini points in moderate and severe stenosis group were significantly higher than those in normal intima and mild stenosis group (P ＜ 0.05 or ＜ 0.01 ), Gensini points in severe stenosis group were significantly higher than those in moderately group(P ＜ 0.01 ), but there was no significant difference between mild stenosis group and normal intima group(P ＞ 0.05 ). The level of LP(a) in study group[(35.27 + 23.38) mg/L] was significantly higher than that in control group[(23.13 ± 16.56) mg/L](P＜ 0.01 ). The level of LP(a) in mild stenosis group was significantly lower than that in moderate and severe stenosis group(P＜ 0.05 or ＜ 0.01 ), and there was no significantly difference compared with that in normal intima group(P＞ 0.05 ). ConclusionsCarotid sclerosis ultrasonic testing helps to predict the risk degree of coronary artery disease. The level of LP(a ) is closely related with carotid sclerosis and has important value in judging the condition of patients with coronary artery disease.

Objective To study the therapeutic effect of cardura on lower urinary tract symptoms (LUTS) and ejectile dystunction(ED) in men with benign prostatic hyperplasia (BPH).Methods One hundred and one cases with BPH and typical LUTS were investigated using the International Prostate Symptom Score (IPSS) and International Index of Erectile Function 5 (IIEF-5)before and after treatment. The patients were given cardura 4 mg, once a day for 8 weeks. The LUTS and ED were was analyzed after treatment. Results The IPSS was scores ( 14.3 ± 4.0) after treatment, and it was lower than before (mean scores: 19.9±6.5, t= 7.534, P＜0.001). The IIEF2-5 was(14.9±8.8)scores after treatment, and it was higher than before (mean scores: 9.5±8. 5, t = 5. 335, P＜ 0. 001 ), respectively. There was statistically significant correlation between IIEF2-5 and IPSS score (r= -0. 327, P＜0. 001 ). Various indexes were significantly improved after cardura treatment as compared with pretreatment (P＜0.001). Conclusions LUTS is dangerous factor for sexual dysdysfunction, and the severity of LUTS is closely related to the development of sexual dysfunction. Cardura can at once improve the sexual function and LUTS of BPH patients.

Objective To summariza the clinical and histological features of linear cutaneous lupus erythematosus (LCLE).Methods Six cases of LCLE were analyzed retrospectively.Results LCLE was often clinically characterized by unilateral.zonal or linear,dark-erythematous patches with telangiectasia,and some lesions were covered with adherent Scales.Histologically,there were patchy perivascular and periadnexal lymphoid inflammatory infiltrate occurring in the superficial and deep dermis.Some cases were pathologically manifested by parakeratosis of superficial dermis,follicular plugging and hydropic degeneration of the basal layer of the epidermis,which was characteristic of discoid lupus erythematosus.The preferred treatment was chloroquine for LCLE.Low dose of corticosteroids were recommended for widespread lesions,especially for those pathologically manifested by lymphocyte infiltrates in the lower dermis and fat tissue.Conclusion The diagnosis of LCLE should be based on clinical and histopathological manifestations.
