1.Study on Mechanism and Clinical Significance of the Precordial ST Segment Deviations in Acute Inferior Myocardial Infarction Involving Adjacent Area

Objective:To study the mechanism and clinical significance of the precordial ST segment deviations in acute inferiorwall myocardial infarction (AIMI) involving adjacent area. Methods: Characteristics of electrocardiogram were analyzed in118 patients with AIMI. Results: (1)The magnitude of ST segment elevation of inferior wall leads (Ⅱ, Ⅲ and aVF) wassignificantly negatively correlated with that of precordial leads (P<0.01). (2)The changed magnitude of ST segment oflead V2 (0.63±1.82 mm) and V2/aVF ratio (0.84±1.61) in 16 cases associated with right ventricular infarction weresignificantly higher than those of only inferior wall myocardial infarction group (V2:0.35±1.65 mm,V2/aVF ratio:0.29±1.28)(P>0.05 and P>0.05). (3)The magnitude of ST segment fall of lead V2(-1.20±1.52 mm) and V2/aVFratio (-0.33±1.15) in 38 cases associated with lateral-posterior wall in farction were higher than those of only inferiorwall infarction group (P<0.05). (4)If the cases associated with right ventricular and lateral-posterior wall infarction werenot included, the ST segment correlation coefficient between inferior and precordial leads would significantly increase inAIMI (r =-0.797, P<0.01). Conclusion:The directions of ST segment deviations of precordial leads elevate in casesassociated with right ventricular infarction and fall in eases associated with lateral-posterior wall infarction in AIMI.
2.Radical operation of gastric cancer with labelling MM_C-CH_(40)

Objective:To explore the clinical significance of labelling MM C-CH 40 in the radical operation of gastric cancer.Methods:Fifty seven patients with gastric cancer were randomly divided into two groups-labelling group and control group.In the labelling group,MM C-CH 40 was injected into the periphery of tumor through gastroscope before operation,then performed radical gastrectomy,the dyed lymph nodes were regarded as marker of the operation.Results:(1)In the labelling group,the average number of eliminated lymph nodes(41.5) was far higher than that of control group (24.4).(2)In the labelling group,of the total eliminated lymph nodes,dyed lymph nodes were 67.1%.(3)In the case of lymph node metastasis,the average number eliminated in the labelling group was 10.05,and that of the control group was 6.74.(4) Inject MM C-CH 40 into the periphery of tumor through gastroscope one week before operation could get a satisfactory result.Conclusion:MM C-CH 40 labelled lymph nodes can guide the radical gastrectomy.
3.The effect of high-frequency electrotome on the healing of perineal incision in patients undergoing Miles′ procedure for rectal carcinoma

0.50).ConclusionsThe use of high-frequency electrotome significantly increases the postoperative perineal complications. The packing of greater omentum and perineal electromagnetic wave irradiation reduce the occurrence of the side effects.
4.Significance of aberrant melanosomes in the diagnosis of malignant melanoma

Seventeen cases of malignant melanoma (MM) were observed with transmission electron microscopy in order to clarify the significance of abberant melanosomes in its diagnosis.30 cases of nevus were also studied to serve as control.It was found that aberrant melanosomes were present in 14 cases of MM including 2 cases of amelanotic MM and absent in 2 cases of pigmented MM and one case of amelanotic MM.In addition,aberrant melanosomes were found in 4 cases of congenital nevus.The findings suggest that the significance of aberrant melanosomes in the diagnosis of MM has been overemphasized.It is believed that the diagnosis of MM must be judged comprehensively on all the ultrastructural changes under transmission electron microscope.
5.A study on the labeling of melanocytic tumors and nevi with anti-human proliferative cell antibody Ki-67

Fifteen cases of malignant melanoma,17 cases of acquired nevus,18 cases of congenital small nevus,and 3 cases of congenital giant nevus were labeled with anti-human prolifera-tive cell antibody Ki-67 with ABC labeling method.It was found that 13 out of the 15 cases of malignant melanoma were positive and the positive rate of the tumor cells was ranged between 2% to 33%.After culture,the positive rate of the melanoma cells was 88%.But all the 3.types of melanocytic nevi were negative.The findings suggest that the labeling of anti-human proliferative cell antibody Ki-67 is a sensitive and specific method to identify melanocytic tumors and it is not sensitive enough to differentiate the minor changes of the 3 types of nevi.
6.Isolation of epidermal desmosomes and preliminary analysis of desmosomal proteins

Desmosomes with well-preserved architecture were isloated from the epidermis of bovine muzzle.The thickness of the dense plaque was about 15 nm and the width of desmoso-mal interspace 30~40 nm.The midline in the desmosomal interspace was shown as a moderately electrondense material.The component proteins of the desmosomes were analyzed with SDS-PAGE.It was found that both DP Ⅰ and DP Ⅱ consisted of 3 polypeptides and the molecular weight of DP 1 a,b and c was 235 kd,226 kd and 215kd and that of DP Ⅱ a,b and c was 206 kd,198 kd and 186 kd respectively ; and DP Ⅲ was composed of 2 polypeptides and the molecular weight of DP I a and b was 87 kd and 86 kd.The above mentioned data were different from those of other reports,while the data of the other component proteins were similar to those of others.The molecular weight of DG I was 150 kd.that of DG Ⅱ a and b was 135 kd and 118 kd,that of DG Ⅲ was probably 22 kd.and that of DP Ⅳ was 82 kd.According to our findings,it is concluded that both DP Ⅰ and DP Ⅱ consist of 3 polypeptides and DP Ⅲ of 2 polypepties.

8.A Study on Clinical Parameters of Patients with Severe Acute Respiratory Syndrome and Fever-clinic Patients

Objective To investigate clinical features of patients with severe acut respiratory syndrome (SARS) and fever-clinic patients in SARS prevalence days. Methods Clinical data of 45 SARS patients, 150 medical observation patients and 1200 home observation patients from fever-clinic were analyzed retrospectively. Results All the 45 SARS patients had the medical history of close contact with SARS, and their body temperatures were higher than the fever-clinic patients' ones. Leucopenia and lymphopenia in SARS patients were more common than those in the home observation patients, but were similar with those in the medical observation patients. All the SARS patients' chest radiographs showed large area of interstitial infiltration or shadows, and 46 7% patients developed rapidly in a short time. Conlusion Epidemic history and chest radiograph findings may be the major evidences to diagnose SARS, while the fever symptom and blood routine examination may be helpful to screen SARS.
9.Compared research of selective culture media for Legionella and clinical practicability

Objective To develop a selective isolation media which has a high isolation ratio for Legionella.Methods We compared the growth of Legionella pneumonia on BCYE? plate (with DGVP), BCYE?(with GVPC) and BCYE? (with CCCV) by colony count and compared with BCYE? plate. The comparison of isolation ratio of Legionella pneumonia from air-conditions cooling water with 3 kinds of BCYE? plate containing different anti-reagents was performed.Results The results of colony count on BCYE? plate (with DGVP) and BCYE?(with GVPC) was identical with that on BCYE? plate at same concentration. But the result of colony count on BCYE? plate (with CCCV) was half of the former 2 kinds of plate. 5 strains of Legionella pneumonia isolated from 9 central air-condition cooling water were used by BCYE? plate (with DGVP) and BCYE? (with GVPC), while only 2 strains of Legionella pneumonia were isolated by BCYE? plate (with CCCV). Legionella pneumonia wasn′t detected by BCYE? plate.Conclusion BCYE? plate with antibiotics system of CCCV showed obvious suppression to Legionella pneumonia, and so that there was a low isolation ratio to Legionella pneumonia. But BCYE? plate with antibiotics system of DGVP and GVPC had a few suppression and showed a high isolatio ratio for Legionella pneumonia. Moreover, BCYE? plate with antibiotics system of DGVP and GVPC can effectively inhibit non-Legionella and increase isolation ratio of Legionella. They should be the first selection media for Legionella from environment and clinical samples.
10.Clinical characteristics and prognostic factors of esophageal spindle cell carcinoma

Objective Esophageal spindle cell carcinoma is a rare type of esophageal squamous cell carcinoma , which was usually named esophageal carcinosarcoma .This article was to discuss the clinical characteristics , treatment and prognostic factors of e-sophageal spindle cell carcinoma . Methods The clinical and pathological data of 28 cases of esophageal spindle cell carcinoma were retrospectively analyzed .The prognostic factors was analyzed for various factors including gender , age, smoking history , tumor loca-tion, tumor size, lymph node metastasis, histological type, pathological staging and adjuvant treatment regimen .Kaplan-Meier method was applied to calculate the survival rate , log rank method for single prognostic factor analysis and Cox proportional hazard regression model for multivariate analysis of prognosis . Results One-year, three-year, and five-year survival rates were 89.3%(25/28), 67.1%(19/28), 38.5%(10/28)respectively.Single factor analysis showed that the pathological type , N stage, T stage, pTNM stage were the factors influencing the prognosis(P﹤0.05), while gender, age, smoking and drinking history, tumor location, tumor size, operation incision selection , adjuvant treatment regimen were not .Multivariate analysis showed pTNM stage were the independent fac-tors affecting the prognosis of esophageal spindle cell carcinoma (χ2 =13.104, P=0.000). Conclusion Esophageal spindle cell carcinoma is a rare kind of esophageal squamous cell carcinoma with less invasiveness , low rate of lymph node metastasis and good progno-sis.Radical operation is the preferred treatment .
