A five-step synthetic method of ambrisentan starting from benzophenone was developed.The reaction methods of resolution and substitution were optimized.A stable,facilitating method of detection and efficient resolution reagent,L-phenylglycinamide,was used and helped to obtain highly optical pure intermediates.In the substitution reaction,NaOH was chosen as the base,which is safe and suitable for the industry.This method was mild,easily operated with a total yield up to 31.0%,and may have a good application prospect.

Based on the big data of Electronic Medical Records (EMR),information retrieval and the deep learning method,an auxiliary diagnosis Question Answering (QA) system is designed.The paper introduces the design of general framework,EMR database,value network and policy-based network of the system,provides the system operation process.This system can not only help patients to examine their conditions independently,but also provide reference for doctors when they make the diagnosis and treatment schemes.
3.Clinical Effect Addition and Subtraction Therapy of Si Junzitang Combined with Simotang to Outlet Obstructive Constipation After Stapled Trans-anal Rectal Resection

Objective:To observe clinical effect of addition and subtraction therapy of Si Junzitang combined with Simotang to outlet obstructive constipation (OOC) after stapled trans-anal rectal resection (STARR). Method:One hundred and twenty-four patients were randomly divided into control group (62 cases) and observation group (62 cases) by random number table. Patients in control group got Qirong Ruichang oral liquid, 20 mL/time, 3 times/day. After operation, patients in observation group got addition and subtraction therapy of Si Junzitang combined with Simotang, 1 dose/day. And courses of treatment in two groups were 4 weeks, and 8 weeks' follow-up was recorded. Before the operation and at the second and fourth week after treatment, and the eighth week of follow-up, scores of main symptoms of constipation and Longo ODS were graded. Before the operation and at the fourth week after treatment, levels of superoxide dismutase (SOD), malondialdehyde (MDA), constipation patients quality of life self-assessment scale (PAC-QOL), anorectal pressure, anal resting pressure (ARP), maximum anal systolic pressure (MSP), rectal defecation pressure (RSP), FSV, CRS and MTV were recorded. And incidence, recurrence, normal defecation, satisfaction at the fourth week after the operation and safety were evaluated. 4.Analysis of Genomic Landscape in Patients with Acute Myeloid Leukemia.

OBJECTIVE:
To investigate the gene mutation occurved in AML patients with 29 kinds of fusion genes and 51 kinds of tumor gene.
METHODS:
Next-generation sequencing (NGS) was used to detected the 49 kinds of targeted gene. FLT3 internal tandem duplication (FLT3-ITD), CALR, NPM1 and CEBPA mutation were detected by DNA-based PCR and Sanger sequencing. Twenty-nine kinds of fusion genes were dected by multiplex nested RT-PCR.
RESULTS:
The total gene mutation rate was 91% (109/121) in all the 121 patients. On average, 2.1 mutated genes per patient were identified, among these 121 patients, coexistence of ≥ 3 mutations was frequent (34.7%). The most commonly mutated genes were NRAS (23.96%, n=29), followed by NPM1 (14.04%, n=17), CEBPA double mutations (14.04%, n=17), KRAS (11.57%, n=14),FLT3-ITD (10.74%, n=13), CSF3R (10.74%, n=13), TET2 (9.92%, n=12) and IDH1 (9.1%, n=11). Overall, fusion genes were detected in 47 (37.3%) patients, including AML/ETO (n=12), CBFβ/MYH11 (n=11), PML/RARa (n=12), MLL rearranagement realated mutation MLL-X (n=10). TLS/ERG (n=1) and DEK/CAN (n=1) in an order of decreasing frequency. Patients with normal karyotype (NK)- AML exhibited more mutations in CEBPA, NPM1, TET2, RUNX1 and IDH1, comparing with abnormal karyotype patients. KRAS mutation in abnormal kayotype patients was significantly higher than that in normal kayotype patients (P=0.014). TP53 mutations were predominantly associated with complex cytogenetics (P=0.199). KRAS mutations were more frequent in core binding factor (CBF) acute myeloid leukemia (AML) and 11q23/MLL rearrangement leukemia, compared with NK-AML (P=0.006 and 0.003, respectively). KIT mutations predominated in CBF-AML (P=0.006). JAK2V617F mutations were detected in two patients and co-occurred with AML-ETO fusions.
CONCLUSION
At least one mutation is observed in more than 90% patients. On average, more than 2 mutated genes per patient are identified. Some gene mutations are associated with gene rearrangement.

5.Study on the electrophysiological features of shoulder hand syndrome after stroke

Objective To analyze the nerve electrophysiological characteristics of stroke occurrence shoulder hand syndrome patients.Methods A total of 70 stroke patients hospitalized in the Department of Neurology of Ruian City Red Cross Hospital were selected.According to whether the occurrence of shoulder hand syndrome,the patients were defined as the observation group (32 cases) and the control group (38 cases).All patients during hospitalization received the median nerve needle electromyography and nerve conduction determination.The needle pole electromyo-gram examination results, sensory nerve action potential changes were observed and compared in the two groups. Results (1) After needle electromyography,all patients in the observation group showed abnormal potential ( such as fibrillation potentials was sharp waves).Among them,fibrillation potential observed in 2 cases(5.26%) in the control group,which was significantly lower than 21 cases (65.63%) in the observation group (χ2 =6.954,P=0.026).Positive sharp wave observed in 1 case(2.63%) in the control group,which was significantly lower than 11 cases (34.38%) in the observation group (χ2 =8.323, P =0.014 ) .Nerve conduction measurement results showed that the sensory nerve action potential amplitude of the observation group was significantly lower than that of the control group, and amplitude of sensory nerve in the observation group decreased more significantly than the control group.The average sensory nerve action potential of the observation group was (7.48 ±4.12) ms,which was significantly lower than (12.53 ±1.61)ms of the control group,amplitude of wave in the observation group decreased more significantly than the control group ( t =12.064, P =0.017 ) .Conclusion Shoulder hand syndrome nerve electrophysiological parameters changed significantly after stroke occurrence.Therefore,it can be used further for the early found brain after stroke shoulder hand syndrome and provide a theoretical basis.
6.Application of WeChat public platform in pathological experiment teaching

Objective To explore the advantages and characteristics of pathology experimental teaching based on WeChat public platform. Method Through the establishment ofhomogeneous pathol-ogy experiment platform—WeChat public platform, the students of clinical medicine major of Class 1 and 2 of Grade 2014 who participated in the pathological experiment course were divided into two groups. The experimental group (65 people) used the auxiliary teaching based on the WeChat public platform, and the control group (65 people) used the traditional teaching method. The students in the experimental group used the WeChat public platform in combination with pathology experimental teaching, pushing the experimental teaching by WeChat public platform, including the change of specimen and eye diseases under the micro-scope of typical pictures and videos, and at the same time pushing the typical disease image and the related text introduction, and auxiliary pathology experimental teaching. The effect of teaching was evaluated by student experimental examination and electronic questionnaire. The data were collated after the entry of SPSS 19.0, and the data comparison was performed by t test. Result The average score of the experimental group (14.80±0.24) was significantly higher than that of the traditional teaching group (13.79±0.33), and the difference was statistically significant (P=0.031). The experimental group students' evaluation on the learning of WeChat based public platform was higher than the control group's evaluation on traditional teaching in the aspects of learning interest, reducing learning pressure and feedback. Conclusion The application of WeChat public platform for the teaching of pathological experiments is feasible compared with the traditional teaching model and can improve the teaching effect effectively, and solve the problems of the pathology experiment teaching sample being insufficient or the teaching sample being not typical, which creates conditions for students to study independently and use specimens and pictures.
7.Detection and Characteristics of Hepatitis C Virus Infection among Blood Donors in Qingyang City.

OBJECTIVE:
To deeply understand the epidemic status of hepatitis C virus (HCV) among blood donors in Qingyang City, and prevent the risk of transmission of HCV via blood.
METHODS:
To compare the application of nucleic acid and enzyme immunoassay adopted in HCV screening of volunteer blood donors in Qingyang City. The characteristics of positive population and the status of co-infection were studied.
RESULTS:
From 2015 to 2020, the positive rate of HCV detection among blood donors in Qingyang City was 3.2‰. Among the HCV positive co-infections, treponema pallidum (TP) accounted for the largest proportion, followed by alanine aminotransferase (ALT) and HBsAg. Blood donation was mainly based on individual voluntariness, and the main age group was 26-45 years old. The distribution of blood type showed that 31.08% for type B, 28.31% for type O, 27.38% for type A and 13.23% for type AB, the population of type AB was lower than that of type B and type O. The distribution of sex showed that male was more than female. The marital status was concentrated in married population. The regional distribution was concentrated in Xifeng District. Qingcheng County was higher than Huachi County and other places, while Ning County, Heshui County, Zhenyuan County and Huan County were close. The occupation was dominated by unemployed. Their nationalities were all Han. The overall awareness rate of HCV knowledge in young volunteers was 67.2%.
CONCLUSION
There are more HCV infected people with TP infection in voluntary blood donors, and the cognitive level of HCV knowledge in young volunteers needs to be improved.

8.Screening of HBV and Positive Population Distribution Characteristics of Voluntary Blood Donors in Qingyang.

OBJECTIVETo study and analyse the application of ELISA and nucleic acid test (NAT) to HBV detection of voluntary blood donors in Qingyang city and the characteristics and social attributes of HBV positive population.
METHODSThe HBV of 70 659 blood donors in Qingyang city from January 2013 to December 2016 was detected by using ELISA and NAT, and the statistical analysis of results was performed by means of Xdetection and FIsher' s Exact test.
RESULTSThe HBV positive rate detected by ELISA was 0.47% in 70 659 blood samples, and the HBV positive rate detected by NAT was 0.05% in 47 913 HBV negative samples detected by ELISA, moreover, the number of HBV negative by ELISA detection yet HBV positive by NAT detection increased year by year. The statistical analysis of positive population characteristics showed that the age distoibution was mainly between 26-45 years old, however the difference of detected rate between 26-45 years old and 18-25 years old gradually decreased on near 2 years, showing that the positive population tended to be younger. The distribution of blood type was B>O>A>AB, while the sex distribution was male> female, but this difference showed gradually nairowing trend due to increment of female voluntary donors with HBV positive; for marital status, HBV positive concentrated in married population. The regional distribution of HBV positive mainly concentrated in Xifeng region, mainly in jobless population; the HBV positive comparison betwwen anationalities showed Han natienatity was > minoring nationalitie.
CONCLUSIONThe application of ELISA combined with NAT for the HBV screaning of blood voluntary donors in Qiangyang city can prevent the detection failed and occult HBV, and reduce the potential risk of blood transfution. The characteristis analysis of HBV positive population provides practical basis for the safe use and transfution of blood.
9.Effects of ligustilide on chemotherapy resistance of cervical cancer cells

OBJECTIVE To investigate the effects of ligustilide on chemotherapy resistance of cervical cancer cells based on Hippo-Yes-associated protein (YAP) signaling pathway. METHODS Human cervical cancer cisplatin-resistant cells HeLa/DDP were divided into control group, cisplatin group (10 μmol/L cisplatin), cisplatin+ligustilide low-, medium- and high-concentration groups (10 μmol/L cisplatin+25, 50, 100 μmol/L ligustilide). The proliferation, apoptosis, migration and invasion of HeLa/DDP cells were all detected in each group. The mRNA expressions of YAP and transcriptional coactivator with PDZ binding motif (TAZ) as well as the protein expressions of YAP, TAZ, matrix metalloproteinase 2 (MMP2), Ki67, cleaved-caspase-3 and caspase-3 were determined in HeLa/DDP cells. RESULTS Compared with control group, the inhibitory rate, apoptotic rate and cleaved- caspase-3/caspase-3 of cisplatin group were increased significantly; scratch healing rate, the number of invasive cells, the mRNA expressions of YAP and TAZ, and the protein expressions of YAP, TAZ, MMP2 and Ki67 were decreased significantly in cisplatin group (P<0.05). Compared with cisplatin group, the inhibitory rate of cell proliferation, apoptotic rate and cleaved-caspase-3/ caspase-3 were further increased in cisplatin+ligustilide low-, medium- and high-concentration groups, while scratch healing rate, the number of invasive cells, the mRNA expressions of YAP and TAZ, and the protein expressions of YAP, TAZ, MMP2 and Ki67 were further decreased, in a dose-dependent manner (P<0.05). CONCLUSIONS Ligustilide can increase the sensitivity of drug-resistant cervical cancer cells to cisplatin by inhibiting Hippo-YAP signaling pathway.
10.Analysis of risk factors of diabetic retinopathy inpatients with type 2 diabetes in the city of Dongguan

Background Diabetic retinopathy (DR) has become the main cause of blindness in the world,But the etiology of DR is still not clear,and the results of the studies on the risk factors of DR are not completely consistent.Fully understanding the risk factors of DR has an important clinical value for the prevention and treatment of DR.Objective This study was to analyze the prevalence rate and risk factors of DR inpatients with type 2 diabetes and provide a basis for the establishment of ophthalmic intervention programs and measures for diabetic inpatients.Methods Cross sectional study was performed.Four hundred and seventy three patients with type 2 diabetes in the department of endocrinology,Dongguan People's Hospital from July 2011 to July 2012 were included.The patients were divided into DR group and non diabetic retinopathy (NDR) group.The DR group was subdivided into mild,moderate and severe non-proliferative DR (NPDR) group and proliferative DR (PDR) group.The sex and age of patients,course of diabetes,body mass index (BMI),systolic blood pressure,diastolic blood pressure,fasting blood glucose (FBG),2 hours postprandial blood glucose (2 h PBG),glycosylated hemoglobin (HbA1 c),fasting insulin,2 hours postprandial insulin,high density lipoprotein cholesterol,low density lipoprotein cholesterol,apolipoprotein A1 (APOA1),APOB,alpha lipoprotein,total cholesterol,three glycerol,urea nitrogen,creatinine,uric acid,percentage of neutrophil,24 hours urinary albumin total (ALBU-24 h) and ALBU were detected.Logistic regression was used to analyze the relationship between DR and various factors,and the risk factors of DR were screened out.Results The prevalence of DR inpatients with type 2 diabetes was 28.33%,the prevalence of mild,moderate and severe NPDR were 2.54%,16.28% and 4.23%,respectively,the prevalence of PDR was 5.29%.The prevalence of DME was 10.36% in the DR patients.The course of diabetes,the levels of serum lipoprotein,creatinine,ALBU-24 h and ALBU were statistically significant between DR group and NDR group (all at P<0.05).By stepwise Logistic regression analysis,the course of disease,FBG were identified as the independent risk factors of DR (course of disease:odds ratio [OR] =1.155,95 % confidence interval [CI]:1.067-1.251;FBG:OR =1.313,95% CI:1.071-1.610).Conclusions The course of diabetes,lipoprotein,creatinine,ALBU-24h,ALBU are closely related to the occurrence and development of DR.The course of diabetes and FBG are the independent risk factors of DR.
