1.NET LEVELS IN SALIVA AND SERUM AFTER MULTIPLE INJECTIONS OF NET-OEN PLUS EV AND THEIR RELATION

Since saliva has been widely used as a fluid for monitoring drug concentrations, both saliva and serum samples were collected after multiple injections of Norethisterone Enanthate ( NET-OEN ) 50mg and Estradiol Valerate ( EV ) 5mg from 10 women. The concentration of NET in saliva was measured by RIA using diluted antisera working solution and serum NET level was measured by RIA according to our lab. regular method. The assay was running very well. The phar- macokinetics of NET in saliva were observed from the data obtained. The time to reach peak levels in saliva was 5d after injection, which was similar to the Tmax of NET in blood. Mean peak values of NET in saliva were about 220, 218, and 158 ng/L which were almost 1/30 to 1/40 of the Cmax of NET in serum (8506, 6888, and 5619 ng/L) for 1st, 6th, and 12th injections. The concentration of NET in saliva showed to be very closely related to the serum NET concentration in the same subject, and the correlation coefficients were 0.86, 0.83, and 0.80 ( P
2.Influencing factors of ankylosing spondylitis patients’life quality and medical coping styles

Objective To investigate the influencing factors of ankylosing spondylitis(AS)patients’life quality and medical coping styles.Method Two hundred patients visiting at the outpatient department of our hospital took part in the assessment on AS life quality and medical coping styles to look into the living quality of AS patients and its medical coping methods and influencing factors.Results The total score on life quality was(66.82±5.57),among whose dimensions the psychological function was scored the lowest by(10.65±4.05). Pearson correlation analysis showed that the scores on confrontation of coping styles were positively related with the dimension of social function in life quality(r=0.18,r=0.13,P<0.05),the scores on the avoidance of coping style were positively related to the dimension of physiological function in the life quality(r=0.12,r=0.14,P<0.05),the scores on yielding in coping styles was negatively correlated to physiological function of the life quality(r=-0.20,P<0.05).Regression analysis results showed that the AS patients’life quality was impacted by the course of the disease,coping styles and modes for paying medical expenses(P<0.05).Conclusions The AS patients’life quality is low and it is related to their coping style,the course of the disease and modes for paying medical expenses.Medical personnel should implement targeted interventions to guide patients into taking correct medical coping styles according to the specific condition of patients,in order to improve their quality of life.
3.Reconstruction of portal vein in liver surgery

As an essential technique involved in complicated liver surgery,portal vein reconstruction results in eradication of macro- or microscopic tumor residual on surgical margins when combined with precise hepatectomy,improving both the living quality and the survival rate of patients.The application of this reconstruction technique needs precise evaluation of pre-operational image data,clearly dissection of portal vessels and tremendous amount of collaborative effort by the surgery team. Other techniques performed during the surgical procedure include intra-operative ultrasound scan,revitalizing the cryopreserved vessels,and angioplasty.
4.Survey and analysis in organizational commitment and influencing factors of clinical nurses at tertiary hospital in Shenzhen

Objective To study the organizational commitment and influencing factors of clinical nurses at tertiary hospital in Shenzhen. Methods 347 clinical nurses from a tertiary hospital were select-ed by stratified random sampling and investigated and analyzed for organizational commitment with ques-tionnaires. Results Total score for organizational commitment of clinical nurses was (60.45±9.99),in which, from the highest to the lowest, the scores for five aspects were normative commitment (14.49±2.97), affective commitment (13.50±3.57), ideal commitment (11.69±3.20), economic commitment (11.27±3.23), opportunity commitment (9.50±3.30),respectively, their influencing factors included age, nursing age, mari-tal status, professional tide. Conclusions The organizational commitment of clinical nurses is at higher-middle level, their influencing factors are multi-dimensional, demographic characteristics have different im-pact on five aspects of organizational commitment.
5.Impact of nursing intervention on self- care ability and health behavior of cancer patients with PICC catheter

Objective To discuss the impact of nursing intervention on self- care ability and health behavior of cancer patients with indwelling PICC in order to improve their life. Methods 218 cases of cancer patients with PICC catheter were randomly divided into the intervention group (107 patients)and the control group (111 patients),the control group were treated with conventional care, the intervention group was treated with targeted cognitive intervention by the PICC nurse specialists on the basis of conventional care. Interventions included improving health knowledge, self-concept, self-care responsibility and self-care skills in four areas, the intervention lasted 3 months. A questionnaire survey was conducted before and after intervention. Self-care skills and health behavior levels were compared between the two groups. Results Self-care skills and health behavior levels of the intervention group were significantly higher thanthose of the control group after intervention. Conclusions Targeted interventions can effectively improve the self-care capacity of outpatient oncology patients with PICC catheter and improve their health behaviors.
6.Analysis of reasons for abnormal extubation in chemotherapy patients with PICC and nursing countermeasure

Objective To analyze the reasons of abnormal extubation for the chemotherapy patients with PICC,and put forward preventive measures. Methods A retrospective investigation was carried out about the reasons for abnormal extubation in 583 chemotherapy patients with PICC in our hospital from December 2007 to September 2009. Results There were 63 cases of abnormal extubation in 583 patients, accounted for 10.8% of the total number. The reasons of abnormal extubation included: catheter-related infection, misplacement,prolapse, dilapidation and fracture, catheter occlusion,thrombosis and abandoning treatment. Conclusions Complications caused by PICC are the main reasons for abnormal extubation. Important care measures to prevent and deal with kinds of complications timely can reduce the incidence of abnormal extubation.

Objective: To investigate the effects of the cooperative actions of isoflavone, vitamin C and E on the expression of acute C reactive protein (CRP) and interleukin 6 (IL-6) in the patients with typeⅡdiabetic mellitus (DM). Method: Ninety six patients with DM were divided randomly into 4 groups, DM control group(B), and groups receiving low(C), medium (D) , and high (E) dose of the compound antioxidant of isoflavone, vitamin C and E respectively. 8.Evaluation of precise hepatectomy for primary liver cancer

Objective To evaluate the therapeutic effect and safety of precise hepatectomy for primary liver cancer.nethods A total of 103 liver cancer patients who had been admitted to Drum Tower Hospital from July 2007 to October 2009 were divided into precise hepatectomy group(n=54)and routine hepatectomy group (n=49).Peri-and postoperative conditions and the results of follow-up of patients in the 2 groups were compared.All data were analyzed via t test or Fisher exact test.Results No perioperative mortality was observed in the 2 groups.Mean blood loss was(635±608)ml in precise hepatectomy group and(929±1044)ml in routine hepatectomy group,with no significant difference between the 2 groups(t=1.722,P>0.05).Intraoperative blood transfusion volume,postoperative level of aspartate transaminase,positive rate of resection margin and incidence of postoperative complications were(222±381)ml,(158±121)U/L,2%(1/54)and 7%(4/54)in precise hepatectomy group,and were(542±785)ml,(292±347)U/L,18%(9/49)and 24%(12/49)in routine hepatectomy group,with significant difference between the 2 groups(t=2.590,2.558,P<0.05).The 1-year tumor recurrence rate and 1-year survival were 24%(8/33)and 85%(28/33)in precise hepatectomy group,42%(20/48)and 77%(37/48)in routine hepatectomy group,with no significant difference(P>0.05).Conclusion Precise hepatectomy is safe and effective in the treatment of liver caner without much injury to patients.
9.Clinical significance of regional excision of parotid pleomorphic adenoma

OBJECTIVE To investigate the feas-ibility of preserving parotid duct in regional excision of pleomorphic adenoma. METHODS 32 patients with parotid pleomorphic adenoma were treated by regional excision with the preservation of the parotid duct. RESULTS No relapse occurred during 3-6-year follow-up period. Transient facial nerve palsy occurred in only 3.1%(1/32) of patients and the drainage of the parotid duct on the surgical side was preserved. CONCLUSION Remaining function of the parotid gland function was preserved and the facial nerve was well-protected in the regional excision of pleomorphic adenomas, especially for patients with small pleomorphic adenomas.
10.Surgical treatment strategy of advanced gallbladder carcinoma: report on 17 cases

Objective To explore the optimal surgical treatment strategy of advanced gallbladder carcinoma (Nevin Ⅲ - Ⅴ ), with an aim to prolong patients' overall survival. Methods 17 patients with advanced gallbladder carcinoma were reviewed. Their preoperative diagnosis, surgical treatment, complications and survival time were studied. Results The diagnosis of advanced gallbladder carcinoma was done using different medical imaging techniques, but incorrect diagnosis still happened. There is a wide range of surgical treatment for advanced gallbladder carcinoma. Controversy still exists as whether lymph node resection should be done. In our patients, 35.3% of the 13a lymph nodes and 23. 5% of the 8 lymph nodes were positive for metastasis, which showed that lymph node resection should be carried out. Extended surgery was sometime required to ensure a R0 resection.The main complications of surgery were intraabdominal infection, bile leakage and paralytic ileus.Conclusion An aggressive surgical approach for advanced gallbladder carcinoma is required to ensure a R0 resection, which contributed to better overall survival.
