1.Analysis on the main pathogenic bacteria and risk factors of 891 cases in hospital with urinary tract infections

Objective To investigate the distribution and drug resistance of major pathogens for urinary tract infections and related risk factors of hospitalized patients for prevention and treatment of urinary tract infections and to guide the reasonable application of antibiotics.Methods From January 2007 to December 2013 ,the risk factors and pathogen resistance of hospitalized patients with urinary system infection in the First People' s Hospital of Shangqiu were analyzed.Results Totally 891 strains were isolated, in which 638 (71.6%) were Gram-negative strains, 218 (24.5%) were Gram-positive strains, and 35 (3.9%) were Candida.Among Gram-negative strains, Escherichia coli was the most prevalent species, accounting for 43.5%, followed by Enterococcus spp(22.1%), major strains were Enterococcus faecalis and Enterococcus faecium.About 62.1% (553/891) of the strains were isolated from female patients, 37.9% (338/891) from male patients.The prevalence of ESBLs-producing strains in Escherichia coli, Klebsiella spp and P.Mirabilis were 59.0% (526/891), 57.8% (515/891) and 15.1% (135/891).ESBLs-producing Enterobacteriaceae strains were more resistant than non-ESBLs-producing strains in terms of antibiotic resistance rate.The ESBLs-producing Enterobacteriaceae strains were highly susceptible to carbapenems.The average prevalence of Pseudomonas aeruginosa was 4.0%(36/891).Six strains were identified as multi-drug resistance of Pseudomonas aeruginosa were respectively, and 3 were carbapenem-resistant strains.Enterococcus spp were completely sensitive to linezolid, vancomycin and teicoplanin.Among Candida strains,the major strains Candida albicans was completely sensitive to amphotericin B, amphotericin and nystatin.The percentage of Candida albicans resistant to itraconazole,voriconazole and fluconazole was 13.0% (116/891), 17.4% (155/891) and 39.1% (348/891).Clinical urinary tract infections related to sex, age, basic diseases, invasive operation and unreasonable application of antibiotics significantly (P<0.05).Conclusion Escherichia coli is the leading clinical isolates strains account for most urinary tract infections in the First People' s Hospital of Shangqiu, followed by Enterococcus spp.The prevalence of Candida albicans is the major strains among Candida strains.Should be pay attention to the susceptible population and apply anbiotics reasonably.

Fibroblast growth factor receptor ( FGFR) is one of the molecules involved in tumor formation , and plays an impor-tant role in cell proliferation , angiogenesis and migration .In this article, we review the role of FGFR signal pathway in breast cancer , the correlation with the risk and prognosis , the function of FGFRs as potential therapeutic targets for breast cancer and discuss various treatment strategies of targeted FGFR 1 in clinical trials .
3.Expressions of TFPI-2 and MMP-9 in esophageal squamous cell carcinoma tissue and their relationship with vasculogenic mimicry

Objective To investigate the expressions of tissue factor pathway inhibitor-2 (TFPI-2 ) and matrix metalloproteinase-9 (MMP-9)in esophageal squamous cell carcinoma(ESCC)tissue and their relationship with vasculogenic mimicry (VM).Methods 162 cases of esophageal squamous cell carcinoma tissues were collected. CD34/periodic acid-schiff double staining was performed to observe the distribution of VM in ESCC tissue,and immunohistochemical staining was used to detect the expressions of TFPI-2 and MMP-9 in ESCC tissue. The relationship between VM and the clinicopathologic parameters of ESCC, the expressions of TFPI-2 and MMP-9 were analyzed.Results The positive rate of VM in ESCC tissue was 20.37%.The positive rate of VM in poorly differentiated group(40.38%)was significantly higher than those in moderately differentiated group(11.76%)and well differentiated group (7.14%)(χ2 = 20.915, P < 0.01 ). The positive rate of VM in TNM Ⅰ-Ⅱ(11.59%)group was significantly lower than that in TNM Ⅲ group(26.88%)(χ2=5.707,P=0.017).The positive rate of TFPI-2 in VM(+)group(33.34%)was significantly higher than that in VM(-)group(6.45%) (χ2=4.582,P=0.032)in poorly differentiated ESCC.The positive rate of MMP-9 in VM(+)group(78.79%) was significantly higher than that in VM(-)group(44.96%)(χ2=12.05,P=0.001).The expression level of TFPI-2 in poorly differentiated group was positively correlated with VM (r= 0.166,P= 0.032 ), and the expression level of MMP-9 was positively correlated with the VM(r=0.183,P=0.018).The five-year survival rate in VM (-) group was significant higher than that in VM (+) group (χ2 = 22.84, P < 0.001 ). Conclusion VM exists in ESCC tissue,especially in poorly differentiated and advanced ESCC tissue.VM is related to poor prognostis of ESCC.TFPI-2 and MMP-9 might involve in the formation of VM in ESCC.
4.Application of evaluation for the uncertainty of measurement in clinical laboratories

The uncertainty of measurement is an important parameter for clincial laboratories. According to the definition, the uncertainty of measurement is a parameter, associated with the result of a measurement, that characterizers the dispersion of the values that could reasonably be attributed to the measurand. So we can use the results of Internal Quality Control to evaluate the uncertainty of measurement in clinical laboratories. The standard uncertainty is equal to the standard deviation.
5.Effect of Motor Imagery on Unilateral Spatial Neglect in Stroke Patients

Objective To observe the effect of motor imagery on unilateral spatial neglect (USN) in stroke patients. Methods 40 stroke patients with USN were divided into treatment group (n=20) and control group (n=20). Both groups received routine physical therapy training,while the treatment group received motor imagery in addition. All the patients were assessed with Chinese Behavioral Inattention Test (CBIT), Mini-mental State Examination (MMSE), Fugl-Meyer assessment (FMA) and modified Barthel index (MBI) before and 8 weeks after treatment. Results The scores of CBIT, MMSE, FMA and MBI were better in the treatment group than in the control group (P<0.01).Conclusion Motor imagery combined with routine physical treatment can improve the function of stroke patients with USN and their activities of daily living.
6.Distribution characteristics of blood culture pathogens

Objective To investigate distribution characteristics of blood culture pathogens,and provid a basis for clinical prevention and control in bloodstream infections.Methods The data of the patients with positive blood culture and the nonduplicate strains were retrospectively analyzed with WHONET 5.6 software in the First People's Hospital of Shangqiu from January 2011 to December 2014.Results The total number of positive strains of blood culture was 1 306 from 25 397 blood cultures,and the positive rates were 5.1%,of which gram positive cocci and gram negative organisms accounted for 64.8% (846/1 306) and 31.2% (408/1 306),respectively.Candida accounted for 4.0% (52/1 306).Gram positive bacteria were mainly Coagulase negative Staphylococcus (CNS) of 564 (43.2%) strains,Staphylococcus aureus of 96 (7.4%) strains,Enterococcus faecium of 48 (3.7%) strains and Streptococcus pneumoniae of 31 (2.4%) strains.The isolated rate of Enterococcus faecium was more than Enterococcus faecalis 20 strains (1.5%).The isolated rate of Escherichia coli,Klebsiella pneumoniae,Acinetobacter Bauman and Pseudomonas aeruginosa were 12.5% (163 strains),6.2%(81 strains),2.0%(26 strains) and 1.8%(23 strains),respectively.Conclusion The isolated rate of Enterococcus faecium more than Enterococcus faecalis in blood culture,the main pathogens are gram positive cocci in children group.Isolates of gram positive bacteria in the proportion of infants,children and adults with blood culture were 85.2%,87.0%,46.5%.There were 264 strains bacteria isolated in infants and young children's intensive care unit,accounting for 67.3% of all bacteria isolated from infants and young children.There were 122 strains bacteria isolated in pediatric intensive care unit,accounting for 56.7% of all bacteria isolated from childen.There were 255 strains bacteria isolated in adult intensive care unit,accounting for 36.5% of all bacteria isolated from adult.Severe basic diseases were the vulnerable groups of bloodstream infections.
7.Characters of HIV-1-specific CD8 T-cell responses in peripheral circulation of chronically HIV-1-infected children undergoing highly active antiretroviral therapy

Objective To characterize the properties of human immunodeficiency virus-1-(HIV-1)specific cytotoxic T lymphocyte(CTL)responses,and to analyze the factors which may affect CTL responses before and after treatment with highly active antiretroviral therapy(HAART)in children with chronic HIV-1 infection.Methods A total of 24 HIV-1-infected children were studied.Circulating CD4 T cell counts and HIV-1 RNA levels were routinely measured.Peripheral HIV-1-specific CTL frequency and the specific interferon-?-producing lymphocytes were determined by tetramer staining and enzyme-linked immunospot(ELISPOT)assay,respectively.Results It was found that HIV-1-specific CTL responses could be detected in vitro in more than 80% of Chinese children with HIV-1 infection.Circulating CTL frequency and IFN-?-producing lymphocyte counts were significantly reduced in HAART suppressed HIV-1 infected children in comparison with HAART failure children and HAART na?ve children.The duration of virus suppression after HAART was found to be inversely correlated with CTL frequency.Conclusion HIV-1-specific CTL response to virus was warranted by antigenic stimulation.The present work may elucidate the immune mechanisms influencing the outcome of HIV-1 carriers with treatment of HAART.
8.Study on DNA extracted from cigarette butts

Objective To study the DNA extraction and its typing in cigarette butts. Method Chelex 100 extraction method was used to extract DNA of 170 cigarette butts, STR loci were typed after PCR amplification by Profiler Plus kit. Results 21 cigarette butts from one volunteer have no results, the others were all obtained typing results. The samples which were added a little tobacco have no results. Sometimes DNA in sponge contacted with mouth was typed. The small fragment loci were typed in cigarette butts which had been reserved for six months. Conclusion Cigarette butt can be typed by DNA analysis, which is useful in forensic identification.
9.Effect of Community-based Rehabilitation Stroke Unit on Functional Recovery of Stroke Patients

Objective To observe the effect of community-based rehabilitation stroke unit on the function of stroke patients. Methods 94 patients with stroke were divided into intervention group (n=47) and control group (n=47). The control group accepted routine community home care, while the intervention group accepted community-based rehabilitation stroke unit care. They were assessed with modified Barthel index (MBI), simple Fugl-Meyer assessment (FMA) and Social Disability Screening Schedule (SDSS) before and after intervention. Results There was not significant difference between the groups in scores of MBI, FMA and SDSS (P>0.05) before intervention. All the scores improved significantly in both groups (P<0.01) 3 months after intervention, and improved more in the intervention group than in the control group (P<0.05). Conclusion Community-based rehabilitation stroke unit can improve the functional recovery of stroke patients.
10.Effect of arsenic trioxide on growth of a heterologous graft model for human breast cancer in nude mice

Objective To study the inhibitory effect of arsenic trioxide(AS2O3)on the tumor growth of breast cancer line MDA-MB-435s implanted subcutaneously in nude mice and its mechanism.Methods BALB/C-nu/nu nude mice were subcutaneously injected with MDA-MB-435s breast cancer cells that are ER-positive,and treated with intraperitoneal injection of AS2O3 and DDP in different concentrations.The implanted tumor was weighed,and tumor inhibition rates were calculated.The expression of PTEN and Caspase-7 induced by AS2 03 were examined by immunohistochemical method.Results The growth of implanted tumor was markedly inhibited with DDP,low dose and high dose AS2O3,and the inhibitory rates were 48.68%,32.80%,66.67%respectively.The immunohisto chemical staining showed that the number of PTEN and Caspase-7 protein increased markedly(P<0.05).Conclu sions AS2O3 inhibits the growth of human breast cancer cell implanted tumor.The molecular mechanism of AS2O3 on induction of apoptosis of breast cancer cells may be throush increasing the expression of PrrEN and Caspase-7(P <0.05).
