1.Clinical efficacy of esomeprazole, mosapride, magnesium plus aluminum in reflux esophagitis

Objective To investigate clinical effect of esomeprazole, mosapride, magnesium plus aluminum in patients with reflux esophagitis. Methods 98 patients with reflux esophagitis who were treated in our hospital from October 2014 to October 2016 were selected and randomly divided into control group and observation group, with 49 cases in each group.In the control group, patients were treated with esomeprazole and mosapride.In the observation group, patients were given esomeprazole, mosapride and magnesium plus aluminum.The levels of serum gastrin, serum motilin and 24-hour pH in the esophagus were compared before and after treatment .Results After treatment,The levels of serum gastrin and serum motilin in the observation group were significantly higher than those in the control group, the difference was statistically significant (P<0.05), while the total time percentage of pH<4 at standing position, total time percentage of pH <4 at supine position, total reflux time percentage of pH <4, and the duration of continuous reflux≥five mins in the observation group were significantly lower than those in the control group, the difference was statistically significant (P<0.05). Conclusion Esomeprazole, mosapride and magnesium plus aluminum have significant clinical effect in patients with reflux esophagitis, and can effectively reduce 24-hour pH in the esophagus, decrease the number of reflux, increase serum gastrin and serum motilin levels.
2.To explore the clinical curative effect of Huangkui capsule combined with valsartan in the treatment of diabetic nephropathy

Objective To study the clinical effect of Huangkui capsule combined with valsartan in the treatment of diabetic nephropathy.Methods50 patients with diabetic nephropathy treated in our hospital from April 2014 to October 2016 were selected as the research object in this study.They were randomly divided into the control group and the experimental group (n=two), with each group having a total of 25 patients.The control group was treated with valsartan treatment, patients in the experimental group of Huangkui capsule combined with valsartan treatment, comparative analysis of the experimental group and the control group of patients the treatment effect and total cholesterol and urinary albumin excretion rate.ResultsBefore treatment, there was no significant difference between the experimental group and the control group in terms of triglyceride, total cholesterol and urinary albumin excretion rate, and there was no statistical significance.After treatment, the urinary protein excretion rate and cholesterol and triglyceride levels in the experimental group were significantly lower than those in the control group, with statistical difference (P<0.05).In the control group of 25 patients, the total effective number of cases in the case of 20 cases, the effective rate was 80.0%.In the control group of 25 patients, the total effective number of cases in the case of 24 cases, the effective rate was 96.0%.As a result, the treatment effect of the patients in the control group was significantly lower than that in the experimental group, with statistical difference (P<0.05).There were no serious complications in diabetic nephropathy patients in the experimental group and the control group.ConclusionHuangkui capsule combined with valsartan in the treatment of diabetic nephropathy can improve the treatment efficiency to a great extent, high safety, no significant adverse reactions, improve the quality of life of patients, the maximum extent to reduce the pain of patients, with further clinical promotion and application significance.
3.Therapeutic Efficacy of Modified Taohong Siwu Decoction Combined with Limited Internal Fixation and External Fixation on Tibial Plateau Fracture

Objective To explore the therapeutic efficacy of modified Taohong Siwu Decoctio n(TSD) combined with limited internal fixation and external fixation for the treatment of tibial plateau fracture of Schatzker type Ⅴand Ⅵ. M ethods A total of 31 cases of tibial plateau fractures of Schatzker type Ⅴ and Ⅵ treated with TSD orally combined with limited internal fixation and external fixation were enrolled into the study. Follow-up was carried out for the evaluation of therapeutic effect and postoperative complications. Results All of the cases received follow-up for 6 months to 3 years. According to Merchant scoring criteria, the therapeutic effect was excellent for 15 cases, good for 12 cases, acceptable for 3 cases and inferior for one case, with the excellent and good rate being 87.1%. One case had pin tract infection, and then was cured after symptomatic treatment. Conclusion For tibial plateau fracture of Schatzker typeⅤandⅥwith unsatisfactory skin state, we can perform limited internal fixation and external fixation to take the place of strong steel plate fixation with extensive exposure for surgical treatment, and then give postoperative oral use of TSD, which will be a safe and effective therapy for the fracture by promoting the relief of the clinical symptoms and signs of early fracture.
4.Diagnosis and treatment of spontaneous carotid artery dissection

ObjectiveTo discuss the diagnosis and treatment of spontaneous carotid artery dissection (SCAD).Methods Clinical diagnosis and treatment data of 9 patients with SCAD were analyzed retrospectively.ResultsVascular ultrasound showed stenoses or occlusions of carotid artery.Angiography showed “flame-shaped ” sign in totally occluded proximal section and free intimal flap in stenoses section.One patient with total occlusion of bilateral internal carotid artery and 6 patients with total occlusion of unilateral internal carotid artery were treated with antiplatelet therapy.Two patients with incomplete occlusion of the carotid were operated with stent implantation in carotid arteries,and the clinical effect of both operations was satisfactory.ConclusionAngiography remains the gold standard in diagnosing SCAD,and stent implantation is an effective treatment of this disease.
5.A Proteomic Method For Core Needle Biopsy Sample Characterization

Proteomic analysis of core needle biopsy (CNB) sample from patient populations is critical to our understanding of human disease,but has been hindered by its particular small size.Here,we present a method for the proteomic analysis of CNB sample based on the two dimensional electrophoresis.Proteins were extracted directly from 3 rat liver CNB specimens and a human prostate CNB sample.respectively.24 cm Immobiline DryStrip (pH 3-10NL) and 12.5% SDS-PAGE were introduced to separate the proteins.Interesting spots were analyzed by MALDI TOF/TOF mass spectrometry after tryptic digestion.With this method,consistent electrophoretic patterns of more than 2 500 protein spots were reproducibly obtained after silver staining,from rat liver CNB specimens.Qualitatively and quantitatively reproducible results also yield when the method was applied to a human prostate CNB sample.57 stochastically selected protein spots were analyzed by MALDI TOF/TOF moss spectrometry.and were identified with high confidence including faint ones.This simple and reproducible approach raises the opportunity of defining key molecular events of human disease pathologies.
6.A case of cerebral hemorrhage in patients with methylmalonic aciduria with hyperhomocystinuria

A case of late-onset methylmalonic aciduria combined with hyperhomocysteinemia (cblC type) is reported. The main manifestations were the reduction of intelligence,the instability of walking,and the inability to take care of oneself,with secondary cerebral hemorrhage. The effect of treatment was good. MMACHC gene mutation detection showed exon1 deletion, indicating that delExon1 is one of the causes of late onset methylmalonic aciduria, cblC type.
