1.Microwave ablation for the treatment of hepatic cavernous hemangiomas:its serious complications and their prevention and treatment

Objective To analyze the complications caused by microwave ablation for hepatic cavernous hemangiomas, and to discuss the prevention and treatment of the complications. Methods During the period from July 2009 to May 2011 at authors’ hospital, a total of 34 times of microwave ablation procedure were carried out in 30 patients with hepatic cavernous hemangiomas. The microwave ablation-related complications were recorded and the prognosis was evaluated. Results Serious complications occurred in 2 patients (6.7%), including acute hemolysis leading to acute renal failure (n = 1) and abdominal wall abscess (n = 1). Postoperative fever was seen in 18 cases (60%), transient hemoglobinuria in 4 cases (13.3%), nausea with vomiting in 5 cases (16.7%), marked pain at the surgical area in 8 cases (26.7%), right pleural effusion in 4 cases (13.3%), hepatic dysfunction in 24 cases (80%) and leukocytosis in 11 cases (36.7%). No treatment-related death occurred. Conclusion For the treatment of hepatic cavernous hemangiomas, microwave ablation is minimally invasive with higher safety and satisfactory effect, although this technique carries somewhat higher risks when the hemangioma is larger or the location of the hemangioma is anatomically complicated. Some of these complications can be early detected and proper management should be actively adopted in order to avoid as well as to minimize the adverse results.
2.Characteristics of acute lymphocytic leukemia immunophenotype in 111 children

Objective To study the characteristics of child acute lymphocytic leukemia (ALL) immuno-phenotype and evaluate its diagnosis value. Methods Direct immunofluorescence staining and CD45/Side Scatter (SSC) gating of flow cytometry were used for immunophenotyping in 111 cases of child ALL. The relation of mor-phology and immunology classification was analyzed. Results Three categories could be identified,including 81 ca-ses (73.0%) of B lineage ALL, 16 cases (14.4%) of T lineage ALL and 14 cases (12.6% ) of B/T lineage ALL. There were 25 cases (22.5% ) of ALL expressing myeloid-associated antigens. According to the FAB Morphology classification,59 cases (53.2%) of L1 type and 47 cases (42.3%) of L2 type were diagnosed. The two cases (1.8%) of L3 type were classified as one case of null-ALL and one case of B-ALL. One case (0.9%)of acute my-eloblastic leukemia (AML-M2a) was identified as null-ALL. The two cases that could not be diagnosed by FAB clas-sification were c-ALL. Conclusion The immunophenotyping helps to identify the character of leukemia with an im-portant value in diagnosis of child ALL.
3.A Method for Calculating Scatter Dose of Irregular Fields

Objective To find a method for calculating the scatter dose of irregular fields. Methods The original Day function was improved to vary with radiation depth, and the improved Day function was used for irregular fields to calculate scatter dose. Results The accuracy was greatly improved by making Day function depend on radiation depth, and the error was within 1.5%. Conclusion This method can be used to calculate the scatter dose of irregular fields as well as rectangular fields.
4.Short-term and long-term efficacy of eversion and patch carotid endarterectomy:a meta-analysis

Objective To systematically review the short-term and long-term efficacy of eversion carotid endarterectomy (eCEA) and patch carotid endarterectomy (pCEA) for the treatment of carotid artery stenosis.Methods The published literature on eCEA and pCEA control studies in medline,PubMed,Ovid,CNKI and CBM (1970.5-2016.10) databases were retrieved by computers.Two reviewers selected literature and extracted data independently according to the inclusion and exclusion criteria.Cochrane Collaboration Network Special Software Rev Man 5.2 was used to analyze the meta-analysis of short-term and long-term outcome measures.Results A total of 1 137 articles were retrieved.Ten studies were included and analyzed (3 of them were randomized controlled trial).A total of 3 213 patients were enrolled,including surgical intervention 3 299 case/time (1 512 in the eCEA group and 1 787 in the pCEA group).The results of meta-analysis showed that:(1) the mean operative time in the pCEA group was shorter 22±8 min than that in the pCEA group.The intraoperative utilization ratio of shunt tube,eCEA was significantly lower than pCEA,they were 12.6 %(53/421) and 50.2% (357/711) respectively (OR,0.11,95%CI 0.08-0.15,P<0.01).The postoperative incidence of stroke within 30 d (OR,0.42,95%CI 0.23-0.76,P=0.004) and the incidence of stroke after 30 d in eCEA were lower than those in pCEA (OR,0.26,95%CI 0.09-0.78,P=0.02).There was significant difference.(2) eCEA reduced the incidence of restenosis at day 30 after procedure (OR,0.57,95%CI 0.38-0.86,P=0.008).Conclusion Compared with pCEA,eCEA has the advantages of reducing the operation time and lowering the utilization rate of shunt tube.At the same time,eCEA can reduce the occurrence of stroke within 30 d and 30 d after procedure,and significantly reduce the incidence of restenosis.
5.Comparison of biological properties between adult peripheral blood derived smooth muscle progenitor cells and mature vessel smooth muscle cells

BACKGROUND:Vascular smooth muscle is the main cell component of constructing vascular wall tissue structure and maintaining vascular tension.Studies have shown that smooth muscle progenitor cells obtained from peripheral blood can differentiate into vascular smooth muscle cells.OBJECTIVE:To compare smooth muscle progenitor cells dedved from human peripheral blood with mature vessel wall smooth muscle cells in cell morphology and proliferation potential.DESIGN,TIME AND SETTING:Cell culture and observation trials were performed at the Key Laboratory of Molecular Medicine in Jiangxi province from October 2008 to April 2009.MATERIALS:Peripheral blood 20mL were adopted from 6 healthy volunteers,and great saphenous vein were sourced from 6 patients with varicose vein.METHODS:Mononuclear cells were isolated from human peripheral blood and cultured in fibronectin,platelet derived growth factor-BB and EGM-2 medium.At the same time,the smooth muscle cells were obtained from great saphenous vein with the collagenase digestion method.High-glucose DMEM medium was used for the culture.MAIN OUTCOME MEASURES:Cellular morphology;Cellular immumofluorescence method and RT-PCR method were used to detect the expression of α-SMA and calponinl.MTT method was used to analyze the proliferation activity.RESULTS:The smooth muscle progenitor cells denved from human peripheral blood exhibited biological characteristics of vascular smooth musc Je cells.they were found to express α-SMA and calponin1.The proliferation activity of smooth muscle progenitor cells was also greater than vascular smooth muscle cells.CONCLUSlON:Peripheral blood mononuclear cells in the stimulus of platelet-derived growth factor can differentiate into smooth muscle progenitor cells.Compared to smooth muscle cells,smooth muscle progenitor cells proliferate better,and are not easy to age in a long time of cultivation.Smooth muscle progenitor cells are a ideal ceil source of tissue engineered vessel.
6.Improved Brisson's operation correction for concealed penis after circumcision

Objective To study the effective correction for concealed penis after being operated by circumcision.Methods A total of 16 concealed penis patients underwent circumcision were corrected by improved Brisson's operation,and the treatment effects were observed.Results Shapes of penis were recovered totally or nearly in 16 cases.Five cases appeared serious edema of the penis,but recovered from 3-6 months.The penises were all developed normal without problem of retraction or curvature in 1-2 years of follow up.Conclusion Improved Brisson's operation for concealed penis is an effective remediation for concealed penis after mistherapy by circumcision.
7.Significance of monitoring indexes of coagulation and fibrinolytic before and after delivery

Objective To investigate the significance of detection of blood coagulation and fibrinolytic indexes before and after delivery.Methods 212 cases in the postpartum hemorrhage group(bleeding amount ≥500 mL)and 235 cases in the normal deliver-y group were retrospectively analyzed.The prothrombin time(PT),international normalized ratio(PT-INR),activated partial throm-boplastin time(APTT),thrombin time(TT),fibrinogen(FIB),D-dimer(DD),blood platelets(PLT)and hemoglobin(HB)were de-tected before delivery and at 72 h after delivery.85 women with the physical examination were selected as the healthy group.Results Before delivery,PT/PT-INR,APTT,TT,PLT and HB in the normal delivery group were lower than those in the healthy group, while FIB and DD were higher than those in the healthy group;PT/PT-INR,APTT,TT and DD in the postpartum hemorrhage group were higher than those in the normal delivery group,while FIB and PLT were lower than those in the normal delivery group. At 72 h after delivery,there was no statistically significant differences in PT/PT-INR,APTT,TT and DD between the normal de-livery group and the healthy group(P >0.05),but FIB was higher than that in the healthy group;PT/PT-INR,APTT,TT and DD in the postpartum hemorrhage group were higher than those in the normal delivery group,while FIB,PLT and HB were lower than those in the normal delivery group.Conclusion Dynamically monitoring the change of the blood coagulation and fibrinolysis indexes before and after delivery has important significance in preventing postpartum hemorrhage and thrombosis.
9.Effect of Butylphthalide on Autophagy of SH-SY5Y Cells Induced by l-methyl-4-phenyl-pyridiniumion

Objective To observe the effects of butylphthalide on the expression of autophagy-related protein and mRNA in l-meth-yl-4-phenyl-pyridiniumion (MPP+)-induced SH-SY5Y cells, and to explore the protective effect and possible mechanism of butylphthalide to the cell model of Parkinson's disease. Methods The SH-SY5Y cells were divided into control group (A), MPP+group (B), rapamycin pre-treated+MPP+group (C) and Butylphthalide pretreated+MPP+group (D). The relative viability of SH-SY5Y cells induced by MPP+was measured with MTT assay, the morphology of SH-SY5Y cells was observed. The expression of microtubule associated protein 1 light chain 3 (LC3)-II/I and Beclin 1 protein was detected by Western blotting. And the expression of LC3-II/I and Beclin 1 mRNA were assayed by re-al-time quantitative reverse transcription-PCR (RT-PCR). Results The viability rates of cells were significantly lower in group B than in group A (t=20.270, P<0.001), and were significantly higher in groups C and D than in group B (t>8.770, P<0.001), however, there was no significantly difference between groups C and D (t=2.270, P=0.064). The expression of LC3-II/I and Beclin 1 was higher in group B than in group A (t>6.647, P<0.01), and was higher in groups C and D than in group B (t>3.630, P<0.01), however, there was no significantly differ-ence between groups C and D (t<2.238, P≥0.05). Conclusion Butylphthalide could prevent the injury of SH-SY5Y cells induced by MPP+, which may affect Parkinson's disease by inducing autophagy.
10.Study on the relationship between plasma homocysteine and ischemic stroke in young and middle-aged adults

Objective To observe the relationship between plasma level of homocysteine(HCY) and ischemic stroke in young and middle-aged adults,explore the clinical signficance concerning the occurrence and development of ischemic stroke in young and middle-aged adults.Methods The plasma homocysteine level of 132 young and middle-aged adults patients with ischemic and 86 control peoples were measured by means of enzymatic cycling assay.Results The plasma homocysteine level in ischemic stroke was higher than that in control group( P < 0.01 ).The higher the plasma homocysteine level,the larger infarcted focus in acute ischemic stroke group of young and middleaged adults.The plasma homocysteine level and the infarcted focus was positively linearly correlated.Conclusion Hyperhomocysteinemia is a risk factor of ischemic stroke among young and middle-aged adults.The higher the plasma homocysteine level,the larger infarcted focus in acute ischemic stroke group.The plasma homocysteine level can reflect the size of the infarcted focus and the degree of disease,and the plasma homocysteine level should serve as a kind of regular examination or as an index intervention can be taken,all of which are great importance to prevent ischemic stroke and reduce its morbilily.
