Chinese Journal of Medical Imaging 2010;(1):43-46
Purpose To explore the value of muti-planar reconstruction (MPR) with 64-slices spiral CT in diagnosing solitary bronchioloalveolar carcinoma (BAC).Materials and Methods The CT manifestations of pathologically-proved BAC were analyzed. MPR images were obtained with 3D reconstruction software, and the findings on the MPR images were compared with primary axial images. Results For all 30 cases of BAC, MPR images were significantly superior to conventional axial images in the aspect of demonstrating ground glass opacity, deep lobulation, halo sign, spiculation, pleural indentation, vascular convergence sign and air-bronchogram. Meanwhile, the false vacuole sign was reduced on MPR images. Conclusions MPR with spiral CT, easily performed, can show the morphology of BAC more precisely than routine axial images, which could benefit the diagnosis of solitary BAC.

BACKGROUND:The treatment of pulmonary hypertension secondary to congenital heart disease has been a hot topic in the clinical research on cardiac surgery. Although traditional drugs for reducing pulmonary hypertension have excelent effects, there are some defaults, such as difficult monitoring and rebounding phenomenon after drug withdrawal. The traditional heart dacron graft is prone to cause complications, such as deformation, thrombosis, embolism, hemolysis and infection. OBJECTIVE:To investigate the effect of glutaraldehyde-fixed bovine pericardium patch and aerosolized iloprost in patients with pulmonary hypertension secondary to congenital heart disease. METHODS:Ninety patients with pulmonary hypertension due to congenital heart disease underwent a surgery. Glutaraldehyde-treated bovine pericardium patch were used to repair cardiac septal defect, and then aerosolized iloprost was applied after operation, administered for 3 days according to 30 ng/min/kg, every 4 hours in the first 12 hours of a day, and every 6 hours in the resting 12 hours. Mean arterial pressure, mean pulmonary arterial pressure, systemic vascular resistance index, pulmonary vascular resistance index were recorded before inhalation, immediately after inhalation, and 30 minutes after inhalation. The pericardium-associated complications, and cardiac function were also observed at folow-ups. RESULTS AND CONCLUSION: The involved 90 cases were detected by echocardiography. The results showed that, al the flaps were closed, there was no shunting or echo discontinuation of atrial septum. The heart contraction function was normal. No pericardium-associated complications were found. There was no significant difference in the mean arterial pressure and systemic vascular resistance index in al patients at different time points. The mean pulmonary arterial pressure and pulmonary vascular resistance index immediately after inhalation were significantly lower than that before inhalation (P < 0.01). The decrease was also significant 30 minutes after inhalation (P < 0.05). The intervention of glutaraldehyde-fixed bovine pericardium patch and aerosolized iloprost is safe and effective to treat patients with pulmonary hypertension secondary to congenital heart disease.

Objective To investigate the association between arterial stiffness of the common carotid artery(CCA)and insulin resistance in hemodialysis patients. Methods Arterial stiffness index β of CCA was evaluated by an ultrasonic phase-lock Echo-tracking system in 80stable non-diabetic hemodialysis patients.Insulin resistance was detected by the homeostasis model assessment method(HOMA-IR).Plasma hemoglobin, serum albumin, total cholesterol, high density lipoprotein, low density lipoprotein,triglyceride,lipoprotein(a),ApoA1,ApoB,CRP,calcium,phosphorus and creatinine were determined by standard methods. Results The stiffness index β was 11.41±4.13 in patients with previous cardiovascular disease(CVD)and 9.75±3.63 in those without CVD(P<0.05).The stiffness index β was positively correlated with HOMA-IR(r=0.321,P<0.01),as well as with age(r=0.376,P<0.01),pulse pressure(r=0.267,P<0.05),and duration of hemodialysis(r=0.219,P<0.05).In stepwise multiple regression analysis,HOMA-IR(β=0.228,P<0.05)and age(β=0.308,P<0.01)were identified as significant independent variables for stiffness index β of CCA. Conclusions Insulin resistance is associated with aaefial stiffness in nondiabetic hemodialysis patients. The increased arterial stiffness may be the link between insulin resistance and cardiovascular morbidity as well as mortality in hemodialysis patients.

Objective:To establish and evaluate a new real-time quality control method that can identify the random errors by using the backpropagation neural network (BPNN) algorithm and taking blood glucose test as an example. Methods:A total of 219 000 blood glucose results measured by Siemens advia 2 400 analytical system from January 2019 to July 2020 and derived from Laboratory Information System of Beijing Chaoyang Hospital Laboratory Department was regarded as the unbiased data of our study. Six deviations with different sizes were introduced to generate the corresponding biased data. With each biased data, BPNN and MovSD algorithms were used and tested, and then evaluated by traceability method and clinical method. Results:For BPNN algorithm, the block size was pre-set to 10 and the false-positive rate in all biases was within 0.1%. For MovSD, however, the optimal block size and exclusive limit were 150 and 10% separately and its false-positive rate in all biases was 0.38%, which was 0.28% higher than BPNN. Especially, for the least two error factors of 0.5 and 1, all the random errors were not detected by MovSD; for the error factor larger than 1, random errors could be detected by MovSD but the MNPed was higher than that of BPNN under all deviations. The difference was up to 91.67 times. 460 000 reference data were produced by traceability procedure. The uncertainty of BPNN algorithm evaluated by these reference data was only 0.078%.Conclusion:A real-time quality control method based on BPNN algorithm was successfully established to identify random errors in analytical phase, which was more efficient than MovSD method and provided a new idea and method for the identification of random errors in clinical practice.

Objective:To provide a feasible method for the evaluation of cardiac function based on cardiac gated 4DCT, the radiomics technology combined with enhanced ECG gated 4DCT images were used to quantitatively analyze the changes of left ventricular CT radiomics characteristics in cardiac cycle. Methods:The enhanced ECG 4DCT images of 14 patients were reconstructed at intervals of 5% of cardiac cycle. The left ventricular muscle (LVM) and the contrast agent well filled area of left ventricular were delineated with a 13 mm diameter sphere (Cardiac Region of Interest, cardiac ROI) in a single phase. 3Dslicer software was used to extract 92 features of all the sketches, analyze the distribution of CT values on the cardiac ROI and LVM, and preliminarily screen the stable features based on the cardiac ROI (one-way ANOVA). The stable features were used to further screen LVM (one-way ANOVA) to get the difference features. Wilcoxon rank sum test was used to analyze the change of characteristics with heartbeat in the heartbeat cycle. Results:In the heartbeat cycle the mean CT values of cardiac cavity ROI in cardiac cavity changed less than that in LVM, with the change rates of 9.23% and 17.88%, respectively. There were 36 stable features with no significant difference in cardiac cavity ROI ( P>0.05). 20 of them were statistically significant ( F=1.641-6.206, P<0.05), and the average change rate was 98.63%, such as median (-103.96%) and mean (123.67%) of the first order matrix, gray level non uniformity (99.81%) of GLDM matrix and other changes reached more than 99%. The differences between the maximum and minimum values in different cardiac cycles were statistically significant ( Z=-3.921--3.173, P<0.05). Conclusions:With the combination of radiomics and enhanced ECG 4DCT image, the microscopic changes of CT image features in the cardiac cycle can be amplifed. A new method for the assessment of left ventricular function changes was provided. The features such as median, mean may have more application potential.

Heat shock protein A9 (HSPA9), a member of the heat shock protein family, is a putative receptor for Tembusu virus (TMUV). By using Western blot and co-immunoprecipitation assays, E protein domains I and II were identified as the functional domains that facilitate HSPA9 binding. Twenty-five overlapping peptides covering domain I and domain II sequences were synthesized and analyzed by using an HSPA9 binding assay. Two peptides showed the capability of binding to HSPA9. Dot blot assay of truncated peptides indicated that amino acid residues 19 to 22 and 245 to 252 of E protein constitute the minimal motifs required for TMUV binding to HSPA9. Importantly, peptides harboring those two minimal motifs could effectively inhibit TMUV infection. Our results provide insight into TMUV-receptor interaction, thereby creating opportunities for elucidating the mechanism of TMUV entry.
