1.Clinical analysis of ultrasound-guided catheter drainage and vacuum-assisted Mammotome system in treatment of lactational breast abscess

Objective To investigate the clinical application value of ultrasound-guided catheter drainage and vacuum-assisted Mammotome system in the treatment of lactational breast abscess. Methods The clinical data of 46 patients with lactational breast abscesses were analyzed retrospectively. All patients were divided into control group (28 patients) and treatment group (18 patients). Homemade irrigation and drainage tube was used in control group,and vacuum-assisted Mammotome system was used in treatment group. The curative effect of 2 groups was compared. Results All patients were cured. The mean healing time in treatment group was (9.5 ± 1.3) d,in control group was (12.5 ± 2.2) d, and there was significant difference (P < 0.01). The rate of breast-feeding in control group was 57.1% (16/28), in treatment group was 13/18,and there was no significant difference (χ2=1.069, P>0.05). All patients recovered well without recurrence. Conclusions Ultrasound-guided catheter drainage should be employed as first line treatment of breast abscess with diameter greater than the 3 cm. If patient′s economic conditions are good and have high requirement on the beauty outcome, vacuum-assisted Mammotome system can be applied as treatment option.
2.Effect of propofol on gastric intramucosal pH after myocardial ischemia-reperf usion in rabbits

Objective To investigate the changes in gastric intramucosal pH (pHi) and the effect of propofol on microcirculatory perfusion after myocardial ischemia-reperfusion injury in rabbits. Methods Twenty healthy adult rabbits of both sexes, weighing 2.0-2.7kg were randomly divided into two groups: A control group (n=10) and B propofol group (n = 10) . The animals were anesthetized with 2% sodium pentothal iv. Anesthesia was maintained with intermittent iv boluses of fentanyl and vecuronium. The animals were tracheotomized and mechanically ventilated during fluid and propofol infusion. PaCO2 was maintained at 35-40 mm Hg. Right internal jugular vein was cannulated for fluid and propofol infusion. Left carotid artery was cannulated for BP and HR monitoring and blood sampling. TRIP tonometry catheter (14F) was placed in the stomach. Lactated Ringer's solution was infused at 6-8 ml-kg-1 h-1 during experiment. In group B propofol was infused at 5mg-kg-1-h-1 when BP and HR were stabilized for 10 min, chest was opened and heart exposed. Left anterior descending artery (LAD) was tied for 60 min and then released for reperfusion. Hemodynamics and pHi were measured before myocardial ischemia (T0) , 60 min after myocardial ischemia (T1), 60 min (T2), 90min (T3) and 180min (T4) after reperfusion was started. Results There was no significant difference in BP and HR from T0 to T4 between the two groups. pHi decreased significantly after myocardial ischemia-reperfusion injury in both groups. pHi was significantly lower at T3 in propofol group than that in control group (P
3.Kaino-sinus syndrome: a case report.

Kaino-sinus syndrome is one new complication of endoscopic sinus surgery. The patient had an operation of endoscopic surgery, because he was suffered from an illness of frontal nasal inverted papilloma again. After operation he was suffering pain in left epicanthal folds and frontal part, and had dry scab in nasal cavity when he breathed cold air. The symptoms could relieve by blocking prenaris. We found he had a big aperture of frontal sinus and dry scab in nasal endoscopic examination. His symptoms disappeared after cleaning and washing accessory nasal cavity by salt water.

4.Value of enhancement scan and angiography of dual-source computed tomography in purulent meningitis

Objective To evaluate the clinical application of enhancement scan and angiography of dual-source computed tomography (CT) in purulent meningitis.Methods The clinical data,imaging data of enhancement scan and angiography of dual-source CT of 8 cases with purulent meningitis were analyzed retrospectively.Results There was linear or bead-like high intensity enhancement of different degree around the cistern,groove or the cerebral vessels in 6 cases,especially around the basal cistern and ambient cistern; a ring-like enhancement was shown in the temporal lobe in 1 case; fusiform change of the cerebral arteries was shown in angiography of dual-source CT in 2 cases.Conclusion Enhancement scan and angiography of dual-source CT is valuable for early diagnosis of purulent meningitis and improve the diagnosis rate.
5.Clinical and imaging features of jugular foramen tumors

OBJECTIVE To study the clinical and radiological characteristics of jugular foramen tumors.METHODS The clinical data of 32 patients with tumors of jugular foramen region treated from May 2006 to December 2009 were retrospectively reviewed.All patients were referred to systematic imaging examination of the temporal bone,such as CT(X-ray computed tomography) and MRI(magnetic resonance).Twenty two patients were further examined by angiography and embo1ized 24 hours before operation.Thirty one patients underwent different operations via different approaches, while one patient with glomus jugular tumor didn't receive operation because of fever caused by sigmoid sinus thrombophlebitis.According to pathologic examination, 21 cases were glomus jugular tumors, 10 cranial nerve neurilemmomas.RESULTS In the 32 cases, the major initial clinical symptoms of jugular foramen tumors were tinnitus,hearing loss and facial palsy.Bone erosion and the "salt and pepper" sign were the characteristic appearance of glomus jugular tumor.Neurilemmoma showed compressive bone change, cystic area and moderate enhancement on CT.CONCLUSION The combination of CT and MRI examination is benefit for both diagnosis and the choice of surgical plan.
6.Modified Duhamel operation on seven cases of adult Hirschsprung's disease

Objective:To investigate the surgical treatment of adult Hirschsprung's disease (AHD). Methods: A retrospective clinical analysis was proceeded in 7 cases of AHD treated in our hospital from January 2003 to May 2008. There were 3 males and 4 females with an age ranged from 18 to 53 years,including three cases with general segment type,two cases with short segment type,one case with long segment type and one case with whole segment type. All cases were received the modified Duhamel operation. Results: All cases had satisfactory outcome without serious complications,such as soiling, blind pouch syndrome, fecal incontinence and sexual disorder in male patients. Pelvic hydrops occurred in one case and anastomotic inflammation occurred in another case,and both were cured by intensive therapy. Conclusion: The Modified Duhamel Operation is an effective and safe procedure for AHD. Through this procedure the postoperative recurrence rate is reduced, as well the sexual function and the defecate function are remained .
7.Application of acoustically evoked stapedius reflex threshold via cochlear implant for maximum comfortable level determination

OBJECTIVE To investigate the feasibility of determining the maximum comfortabl level by using the acoustically evoked stapedius reflex threshold via cochlear implant. METHODS Eleve cases of Nucleus C124 cochlear implant recipients operated in our hospital were selected(8-39 years old, who can decide TIC value by themselves with the postoperative time ranging from device initiation to 1 year after operation),of those four cases were diagnosed as bilateral large vestibular aqueduct syndrome,1 case as common cavity dysplasia,.neural response telemetry of these five patients with inner ear dysplasia were very high or barely measurable.Real-MCL level and acoustically evoked stapedius reflex threshold under activated condition were measured respectively. The stimulating sound was 110dB of white noise.RESULTS The two group values of ASRT and real-MCL were found to be consistant .There is no significant difference statistically (P>0.05). The thresholds of ASR increase with the increasing of real-MCL at the beginning of device initiation. CONCLUSION The acoustically evoked stapedius reflex threshold may reflect (ASRT) the maximum comfortable stimulation level under activated conditions in patients with cochlear implants.it is simple, reliable and may be used to determine the MCL of child cochlear implanted patients.
9.Immunohistochemical analysis of primary renal cell carcinomas and paired bone metastases

Objective To compare the histopathologic and immunohistochemical differences between primary renal cell carcinomas and paired bone metastases in order to discuss the significance in the selection of standard targeted therapies.Methods The clinical data of 19 patients who underwent nephrectomy and resection of bone metastases successively from January 2003 to September 2013 were analysed retrospectively.The paraffin-embedded surgical samples of all the patients were obtained for histopathologic and immunohistochemical analysis.The differences of Fuhrman grades,expression of Ki-67,CD34,vascular endothelial growth factor receptor 2 (VEGFR2),epidermal growth factor receptor (EGFR) and CXC subfamily receptor 4 (CXCR4) were compared between primary renal cell carcinomas and their paired bone metastases.Microvessel density (MVD) was evaluated by the CD34 immunostaining.Results The Fuhrman grades of samples from bone metastases were higher than that of primary tumors (36.8%,7/19) (P=0.008).The Ki-67 label index was (4.00±3.96)% in primary tumors and (7.90±7.38)% in bone metastases (P=0.033).The microvessel density (MVD) was 58.13±22.90 in primary tumors and 46.71±25.40 in the bone metastases (P=0.026).The immunohistochemistry scores of VEGFR2 were 4.68±1.20 in primary tumors and 4.05±1.58 in bone metastases (P=0.014).The immunohistochemistry scores of EGFR were 5.89±1.05 in primary tumors and 5.47± 1.12 in bone metastases (P=0.134).The immunohistochemistryscores of CXCR4 in cytomembrane and cytoplasm were 1.74±1.97 in primary tumors and 2.16± 1.64 in bone metastases (P=0.414).The inununohistochemistry scores of CXCR4 in cell nucleus were 2.52±2.09 in primary tumors and 3.42±1.95 in bone metastases (P=0.009).Conclusions The Fuhrman grades and the expression of Ki-67 and CXCR4 in cell nucleus were higher in bone metastases than that in the primary renal cell carcinomas.The MVD and the expression of VEGFR2 were lower in bone metastases than that in the primary tumors.The above alternations may contribute to the poor prognosis of bone metastasis and the poor result of angiosuppressive therapy.
10.Effects of Long Term Injection of Sodium Salicylate on the ABR and Expression of GAD67 in Rat Inferior Colliculus

Objective To observe the effects of long term injection sodium salicylate on the auditory brain-stem response(ABR)and expression of glutamic acid decarboxylase -67(GAD67) in rat inferior colliculus .Methods Eighteen healthy Wistar rats were randomly divided into three groups :the sodium salicylate group (intramuscular injection of 10% sodium salicylate ,175 mg/kg ,twice daliy for 28 days) ,the saline group (intramuscular injection with saline on same does at the same time) ,the control group (without any treatment) .The rats received ABR after modeling ,then were decapitated and inferior colliculus tissues were stripped .Western blot was used to study the dif-ferent expression of GAD67 protein levels in the three groups .Results Compared with the saline group and control group ,ABR thresholds of the sodium salicylate group were significantly elevated and latency of wave Ⅲ was aslo sig-nificantly prolonged(P<0 .01) ,while there was no significant difference between the saline group and the control group(P>0 .05) .The inferior colliculus GAD67 protein expression level of sodium salicylate group was significantly higher than the saline group and control group(P<0 .01) ,while there was no significant difference between the saline group and the control group(P>0 .05) .Conclusion Long term injection of sodium salicylate can cause a change in the inferior colliculus of GAD67 protein expression and the up regulation of GAD67 expression may occur as a com-pensatory response to increase inhibiting effect .The change of GAD67 protein expression is likely as a compensatory and regulatory mechanisms for sodium salicylate ototoxicity .
