1.Imaging Diagnosis of Chronic Tuberculous Pyothorax Associated with Malignant Tumors:A Report of 3 Cases

Objective To study the clinical and imaging features of chronic tuberculous pyothorax associated with malignant tumors.Methods Imaging features of three cases of chronic tuberculous pyothorax associated with malignant tumors proved by pathology were analyzed with literatures reviewed.Results On chest radiograph and CT,thickening and calcification of the parietal and visceral pleura,soft tissue masses in the lung field adjacent to the empyema were showed.The soft tissue masses were connected with the wall of empyema,the density was homogeneous(one case) or heterogeneous(2 cases).At the location which masses connected with the empyema,the calcifications were removal,decreased or discontinuous.Conclusion The imaging features of chronic tuberculous pyothorax associated with malignant tumors are of some characteristics,especially the appearances of calcification,soft mass and haziness of fat plane are more valuable for diagnosis.
2.Investigation of parasitic infection in food on market in Qingpu District of Shanghai during 2015‒2023

ObjectiveTo understand the status of parasitic infection in the food sold on market in Qingpu District of Shanghai, and to provide an evidence for the development of prevention and control strategies for parasitic infection applicable to Qingpu District. MethodsAquatic products, meat products and other foodstuffs sold on online shops, at farm product markets, supermarkets/foodstores and restaurants were sampled in Qingpu District, Shanghai, during 2015‒2023, based on the administrative division of Qingpu District. The parasitic infection in the food samples were examined using pressing method and digestion for detecting metacercariae in freshwater products and pickled products, using dissection microscopy for Anisakis larvae in seawater products, Taenia cysticercus and Trichinella encysted larvae in meat products. ResultsA total of 1 079 samples of food products were examined during 2015‒2023, with a total parasite infection rate of 13.44%. The total parasite infection rate of freshwater fish products was 3.40% (16/471), and the difference of parasite infection rates between different freshwater fish species was statistically significant (χ2=229.609, P=0.001). The total infection rate of Clonorchis sinensis was 3.18% (15/471), which had been detected in Pseudorasbora parva, Cyprinidae rhodeus, and Carassius auratus, with a positive rate of 77.78% (7/9), 50.00% (5/10) and 3.90% (3/77), respectively. Metorchis orientalis was detected in in Pseudorasbora parva, with a positive rate of 33.33% (3/9). The positive rate of Gnathostoma spinigerum (third-stage larvae) was 0.81%. Paragonimus metacercariae were not detected in the freshwater shrimps and crabs. The infection rate of seawater fish products was 26.46%. The difference of parasite infection rate in seawater fishes was statistically significant (χ2=109.181, P=0.001). A total of 53 pork and beef samples were tested, none was detected with Trichinella larva cysts, Taenia solium metacercariae, and Taenia saginata metacercariae. The total infection rate of pickled yellow mud snail products was 58.11% (43/74). Paragonimus metacercariae was not detected in any of the pickled aquatic product samples. ConclusionThere are different degrees of parasitic infection in freshwater products, seawater products and pickled aquatic products in Qingpu District of Shanghai. The risk of parasite infection from raw or undercooked foods is high. Health education on healthy dietary practices such as throughly cooked food should be strengthened for local residents.
3.Analysis of life expectancy and cause-eliminated life expectancy of the registered residents in Qingpu District, Shanghai, 2002‒2021

ObjectiveTo analyze the trends in life expectancy and cause-eliminated life expectancy of the registered residents in Qingpu District, Shanghai from 2002 to 2021, and to identify the major diseases causing life expectancy loss in the area, so as to provide a theoretical basis for strengthening disease prevention and interventions and optimizing health resources allocation. MethodsUsing the International Classification of Diseases (ICD)-10 classification of death causes, average life expectancy and cause-eliminated life expectancy were calculated with the abbreviated life table and the cause-eliminated life expectancy table. A trend forecast of life expectancy from 2022 to 2029 was made. ResultsFrom 2002 to 2021, the life expectancy of Qingpu District residents showed an increasing trend, with an average annual growth of 0.32 years (AAPC=0.39%, P<0.001). Among the seven major causes of death, the cause-eliminated expected life expectancy for circulatory system diseases showed the most significant upward trend (AAPC=0.53%, P<0.001), rising from the third leading cause of death in 2002 to the first in 2021. The life expectancy loss rate due to circulatory system diseases increased from 3.79% to 7.97%. Respiratory system diseases showed the largest decline, decreasing from the first leading cause of death in 2002 to the fourth in 2021, with the life expectancy loss rate decreasing from 6.83% to 0.99%. ConclusionLife expectancy of the registered residents in Qingpu District has reached a leading level in China. Future efforts should focus on effectively increasing the life expectancy of male residents, strengthening the comprehensive prevention and treatment of chronic diseases such as cerebrovascular diseases, diabetes, and malignant tumors, and improving health promotion policies for residents to achieve a steady long-term increase in regional life expectancy.
4.Influencing factors of job burnout among medical staff in public hospitals of grade Ⅱ and lower-level in a suburban area of Shanghai City

Objective To investigate the prevalence and influencing factors of job burnout among medical staff in secondary and lower-level public hospitals in a suburban area of Shanghai City. Methods A total of 1 959 in-service medical staff from 15 grade Ⅱ and lower-level public hospitals were selected as the research subjects using cluster sampling method. The Maslach Burnout Inventory-Human Service Survey was used to investigate their job burnout situation. Results The median and the 25th and 75th percentiles of job burnout scores for the research subjects were 2.7 (2.1, 3.1) points, and the detection rate of job burnout was 56.6% (1 109/1 959). The detection rate of mild, moderate, and severe job burnout was 26.7%, 18.5%, and 11.4%, respectively. The results of multiple linear regression analysis showed that medical staff aged 20-<31、31-<41、41-<51 had a higher risk of job burnout than those the age of ≥51 (P<0.01), unmarried medical staff had a higher risk of job burnout than married medical staff (P<0.05), intermediate professional title medical staff had a higher risk of job burnout than senior professional title medical staff (P<0.05), medical staff in grade Ⅱ hospitals had a higher risk of job burnout than those in gradeⅠ hospitals (P<0.01), and medical and nursing staff had a higher risk of job burnout than pharmaceutical and technical staff (P<0.01), after excluding confounding factors such as gender, professional title, educational level, and management position. Conclusion Job burnout is prominent among medical staff in grade Ⅱ and lower-level public hospitals in suburban Shanghai City. Age, marital status, professional title, hospital level, and job type are the independent influencing factors of their job burnout.
5.Analysis of the clinical effect of percutaneous pedicle screw fixation combined with transpedicular bone grafting in the treatment of thoracolumbar fracture.

OBJECTIVE:
To investigate the clinical efficacy of percutaneous screw fixation combined with minimally invasive transpedicular bone grafting and non-bone grafting in the treatment of thoracolumbar fractures.
METHODS:
From Janury 2021 to June 2022, 40 patients with thoracolumbar fracture were divided into the experimental group and the control group. There were 26 patients in the experimental group, including 21 males and 5 females with an aberage age of (47.3±12.3) years old, who underwent percutaneous pedicle screw fixation combined with transpedicular autogenous bone grafting. In the control group, 14 patients received percutaneous pedicle screw fixation only. including 7 makes and 7 females with an average age of (50.2±11.2) years old. The operative time, intraoperative blood loss, anterior height ratio of injured vertebrae, Cobb angle, visual analogue score (VAS), MacNab scores, loosening or broken of the implants. were compared and analyzed.
RESULTS:
There was no significant difference in operation time, intraoperative blood loss, VAS and anterior height ratio of injured vertebrae between the two groups. Compared with the preoperative results, VAS and anterior height ratio of injured vertebrae were improved statistically(P<0.05). For Cobb angle of injured vertebra, there was no significant difference between the two groups before surgery (P=0.766). While at 1 week, 3 months and 12 months after surgery, there were statistically differences between the two groups (P values were 0.042, 0.007 and 0.039, respectively). The Cobb angle of injured vertebrae one year after operation was statistically decreased in both groups compared with that before surgery (P<0.001). One year after surgery, the excellent and good rate of Macnab scores was 96.15% in the experimental group and 92.86% in the control group, and there was no statistical differences between the two groups (P=0.648). There was one patient in the control group suffering superficial wound infection on the third day, which was cured by dressing change and anti-infection treatment. There were no postoperative screw loosening and broken in both groups.
CONCLUSION
The two surgical methods have the advantages of less trauma, less pain and quicker recovery, which can restore the height of the injured vertebra, reconstruct the spinal sequence and reduce the fracture of the vertebral body. Transpedicular autogenous bone grafting can increase the stability of the fractured vertebra and maintain the height of the vertebra better after surgery, thus reducing the possibility of complications such as kyphosis, screw loosening and broken.

6.Effects and mechanisms of curcumin on apoptosis of retinal ganglion cells in chronic ocular hypertension rats

AIM:To investigate the effects and mechanisms of curcumin on apoptosis of retinal ganglion cells(RGCs)in chronic ocular hypertension rats.METHODS:A total of 21 Spraque-Dawley(SD)rats were randomly divided into 3 groups with 7 rats in each group. The rat models of chronic ocular hypertension were established by cauterization of the superior scleral veins in the high intraocular pressure model group and the curcumin treatment group, and the sham operation group only cut the conjunctiva without the cauterization of the superior scleral veins; the rats in the curcumin treatment group were intragastrically treated with curcumin at a dose of 4mL/kg, and the rats in the sham operation group and the high intraocular pressure model group were treated with pure water at a dose of 4mL/kg for 3wk. After 3wk, HE staining was used to observe the morphological and pathological changes of retina, the number of RGCs and the thickness of ganglion cell layer(GCL)in each group of rats; TUNEL staining was used to observe the apoptosis of RGCs and retinal cells in each group of rats; the expression levels of glutamate-cysteine ligase modifier subunit(GCLM)and heme oxygenase-1(HO-1)in the retina of each group of rats were detected by real-time fluorescence quantitative PCR, immunohistochemical staining and Western blot.RESULTS:Compared with the sham operation group, the retinal morphology of rats in the high intraocular pressure model group and the curcumin treatment group was disorganized, the number of RGCs was reduced, the GCL was thinner, the apoptosis rate of RGCs and retinal cells increased, and the expression levels of GCLM and HO-1 increased. Compared with the high intraocular pressure model group, the retinal morphology of rats in the curcumin treatment group was basically normal, the number of RGCs increased, the GCL thickened, the apoptosis rate of RGCs and retinal cells decreased, and the expression levels of GCLM and HO-1 increased.CONCLUSION:Curcumin can inhibit the apoptosis of RGCs in the rat model of chronic ocular hypertension by up-regulating the expression of antioxidant genes GCLM and HO-1.
7.Characteristics and trend analysis of injury causes of death in Qingpu District of Shanghai,2002‒2020

ObjectiveTo analyze the characteristics and changing trend of injury cause of mortality of residents in Qingpu District from 2002 to 2020, and to provide scientific reference for formulating regional prevention and control measures. MethodsThe injury mortality data of the registered residents in Qingpu District from 2002 to 2020 were collected. The indicators such as crude mortality rate, standardized mortality rate, and the ranking of causes of death were calculated. ResultsFrom 2002 to 2020, the average annual crude mortality rate was 50.27/100 000, the age-standardized mortality rate based on the world standard population(ASRW) was 30.08/100 000, and the age-standardized mortality rate based on the 2010 Chinese census(ASMRC) was 35.58/100 000. The average annual crude mortality rate of males was higher than that of females [Z=54.402, Mantel-Hanszel χ2=1 742.509, P<0.01). The overall injury mortality rate showed a downward trend with an average annual percent change(AAPC)of -4.07% (95%CI: -5.23%‒-2.90%), P<0.001]. The top four causes of injury death were transportation accident, indeliberate fall, drowning, and suicide. The leading causes of death in 0‒ years old, 15‒ years old and ≥65 years old were drowning, transportation accident and indeliberate fall, respectively. The ASRW of transportation accident, drowning and suicide all showed a decreasing trend, and the AAPC were -8.22% (95%CI: -10.16%‒-6.24%), -6.99% (95%CI: -9.68%‒-4.22%) and -6.21% (95%CI: -9.38%‒-2.94%), respectively. ConclusionThe injury death rate of residents in Qingpu District shows a decreasing trend, and the distribution characteristics of injury death are different among different genders and age groups. Corresponding prevention and control strategies should be adopted for different populations.
8.Stroke incidence of the household population inShanghai's Qingpu District in 2017 - 2022

Objective To understand the characteristics and temporal trends of stroke incidence in the household population of Shanghai's Qingpu District and to provide a basis for the development of comprehensive prevention and control strategies. Methods The stroke case database for Qingpu District from 2017-2022 was obtained from the Shanghai Stroke and Acute Myocardial Infarction Registry and Reporting Information System. The average age of onset, incidence rate, standardised incidence rate, and constitutive ratio were calculated. Independent samples t-tests were used for comparisons between groups, 2-tests and 2-trend tests for comparisons of rates, and the Joinpoint regression model for calculating the annual percentage change (APC) to analyse the temporal trend of rates. Results Between 2017 and 2022, the average age of stroke onset in the household population of Shanghai's Qingpu District was 73.69±11.60 years. The average annual incidence rate was 556.62/100 000, with an average annual standardised incidence rate of 333.76/100000. There was an increasing trend in the incidence and standardised incidence of stroke in males (APC=7.06%, t=3.44, P=0.03, APC=5.32%, t=3.04, P=0.04). The incidence of stroke increases with age, with cases mainly concentrated in those aged 65 years and above, accounting for 79.47%. Ischemic stroke dominates the stroke typology, accounting for 91.08% of cases, while the incidence of hemorrhagic stroke shows an increasing trend (APC=4.64%, t=4.59, P=0.01). Conclusion The occurrence of stroke in the general population of Shanghai’s Qingpu District is concerning. The study indicates that males, individuals aged 65 years and above, and ischaemic stroke are significant factors that require attention for stroke prevention and control.
9.Exposure assessment of fluoride in drinking water in Qingpu District, Shanghai from 2016 to 2021

ObjectiveThe monitoring of fluoride in drinking water in Qingpu District was conducted to evaluate the external exposure of residents through drinking water, and to provide data support for the continuous improvement of drinking water quality in the area and the health level of residents. MethodsFrom 2016 to 2021, the pipe network water was collected from 42 monitoring points in Qingpu District, every month, and the fluoride concentration in the pipe network water was detected by ion chromatography. ResultsThe fluctuation range of fluoride concentration in drinking water in Qingpu District from 2016 to 2021 was between 0.10 and 0.96 mg·L-1. The fluoride in drinking water did not exceed the standard, which was lower than the lower limit of the WHO recommended range. The median fluoride concentration in drinking water in the Qingcaosha water source area was 0.20 mg·L-1, and the median fluoride concentration in drinking water in the Dianshan lake water source area was 0.44 mg·L-1, and the difference between the two was statistically significant (Z=20.260, P<0.001). The fluoride concentration in drinking water had a decreasing trend year by year, especially in the water supply area of Dianshan lake water system. The fluoride concentration in drinking water in Qingcaosha water source water supply area decreased relatively gently, from 0.25 mg·L-1 in 2017 to 0.20 mg·L-1 gradually. According to the "Chinese Residents' Dietary Nutrient Reference Intake 2013 Edition", the requirement of 1.5 mg·d-1 of suitable fluoride intake (AI) for adults. About 50% of the residents in the water supply area of Dianshan lake would consume less than0.75 mg·d-1, fluoride through drinking water, which was lower than 1/2 of the AI value. The fluoride content in the water supply area of Qingcaosha water source was relatively low, and 75% of the residents ingested fluoride through drinking water was lower than 0.37 mg·d-1, which was lower than the 1/4 of the AI value. ConclusionThe concentration of fluoride in drinking water in Qingpu District is lower than the sanitary standard of drinking water in China,and the risk of excessive fluoride exposure is very low. However, the fluoride concentration in drinking water has a decreasing trend year by year, and intervention measures should be used rationally to reduce the health risks caused by insufficient fluoride intake.
