1.Maximal Thymectomy by VATS for Myasthenia Gravis:Report of 27 Cases

Objective To study the feasibility and efficacy of operative method of maximal thymectomy by video-assisted thoracoscopic surgery(VATS)for myasthenia gravis(MG).Methods From August 2005 to June 2007,27 patients with MG received maximal thymectomy by VATS(VATS group).The thymus and fat tissues in the anterior and upper mediastinum were resected.Patients who received transsternal thymectomy from May 2004 to July 2005 served as a control(conventional group).Results In the VATS group,the maximal thymectomy by VATS was completed in 26 patients,one patient was converted to open surgery because of the injury to the brachiocephalic vein.Compared with the conventional group,the patients in the VATS group had fewer blood loss [(46.0?5.7)ml vs(120.0?18.8)ml,t=-19.231,P=0.000],lower level of analgesic requirement(3 vs 12,?2=7.068,P=0.008),and shorter postoperative hospital stay [(7.0?1.2)d vs(11.0?2.5)d,t=-7.379,P=0.000].There was no significant difference in the operation time,myasthenia gravis crisis,and surgical outcomes between the two groups.Conclusions Maximal thymectomy by VATS is feasible and safe for patients with MG.Patients have shorter hospitalization,less surgical trauma and pain,lower rate of complication,and quicker recovery after the treatment.Further studies are needed to investigate its long-term efficacy
2.Effect of Nrf2 signal pathway on acute hepatotoxicity induced by CCl4 in rat

Objective To determine the effect of Nuclear factor-erythroid 2-related factor 2 ( Nrf2 ) on acute hephrotoxicity induced by CCl4 in male rat.Methods 20 male Wistar rats were randomly divided into control group and CCl4 group, 10 rats in each group, another 10 male Wistar rats were transgenic rats microinjection through the carrier, obtained the Nrf2-tk gene integration and specific transgenic rats, as the CCl4 +Nrf2 integration group.The groups was given 1%polysorbate 80 for 4 days,Then the CCl4 and CCl4 +Nrf2 integration group were intraperitoneally injected with a single dose of CCl47.5 mg· kg-1 and were killed 24 h after CCl4 injection.The serum chemical parameters including asparate aminotransferase ( AST) ,alanine aminotransferase ( ALT) and lactate dehydrogenase ( LDH) were measured.Also malonaldehyde ( MDA) ,glutathione ( GSH) ,oxidized glutathione ( GSSG) levels in the liver as well as glutathione ( GSH)/oxidized glutathione ( GSSG ) ratios were detected. Histopathologic changes in the liver were examined. Results F1generation TK transgenic rats in liver and testis and other tissues and organs were not detected the transcription of Nrf2-tk, indicating that Nrf2-tk expression in tissues is specific good.Nrf2 significantly reduced serum AST, ALT and LDH levels in a dose-dependent manner.The results of MDA levels and GSH/GSSG ratios in liver and kidney showed that Nrf2 reduced CCl4-induced hepatic lipid peroxidation,and ameliorated glutathione depletion.The histopathologic results showed that Nrf2 restrained liver and kidney damage induced by CCl4 .Conclusion Nrf2 can effectively protect male rat from acute hepatotoxicity and nephrotoxicity induced by CCl4 .
3.TPA and BrdU promote CD133 expression in nasopharyngeal carcinoma cells

Objective:To explore whether tumor-inducing agent 12-O-tetradecanoyl-phorbol-13-acetate (TPA) and bromodeoxyuridine (BrdU) affect CD133 expression in nasopharyngeal carcinoma (NPC) 5-8F cells. Methods:NPC cell line 5-8F was treated with single TPA, single BrdU, or TPA plus BrdU, respectively. CD133 mRNA and protein expression levels were detected by real-time fluorogentic quantitative PCR and Western blotting, respectively. Flow cytometry was used to separate CD133-positive cells and determine their levels. Boyden chamber test was used to measure the invasion capability of the cells. Results:Compared with untreated group, CD133 mRNA levels were increased in single BrdU group and BrdU plus TPA group (P=0.037 and 0.003, respectively), and decreased in single TPA group. Western blotting indicated that the expressions of CD133 protein was increased in all the three treated groups, and FCM showed that the quantity of CD133-positive cells also increased. The invasion capability was enhanced, especially in BrdU plus TPA group. Conclusion:Both TPA and BrdU increased CD133 expression in NPC.The effects of TPA and BrdU are synergestic.

5.IDENTIFICATION OF PI3K/Akt PATHWAY RELATED GENES IN VASCULAR SMOOTH MUSCLE CELLS SUBJECTED TO CYCLIC STRAIN

Objective To screen out the PI3K/Akt pathway related genes in vascular smooth muscle cells (VSMCs) subjected to cyclic strain. Method VSMCs of rat aorta were subjected to cyclic strain (10 %, 1 Hz) by using a FX-4000T system. Phosphorylation of Akt was examined by Western blotting. Suppression subtractive hybridization (SSH) method was employed to analyze the differently expressed cDNA sequence between the strained VSMCs pretreated with and without wortmannin, a specific inhibitor of PI3K. These fragments were ligated with T vectors, screened through the blue-white screening system to establish cDNA library, and was sequenced and analyzed in GenBank with BLASTN search.Result Level of Akt phosphorylation of VSMCs was significantly enhanced by the mechanical strain compared with the control. Ten different expressed sequence tags (EST) were gained after sequencing 30 clones randomly selected from 54 white clones. There may be six genes related with the mechanical strain and cell signal PI3K/Akt, such as High mobility group box 1 (HMGB1), Neural precursor cells expressed (Nedd4a), Cyclin-dependent kinase 5 (CDK5), Histone deacetylase 3 (HDAC3), Ubiquitin-like 1 (Uble1a) and Heterogeneous nuclear ribonucleoprotein H1 (hnRNP). Conclusion SSH is an effective and reliable approach to screen out the genes related with the mechanical strain and PI3K/Akt pathway in VSMCs.
6.Efficacy of intraperitoneal sustained-release chemotherapy with 5-FU on mice bearing H22 ascitic tumor

Objective:To evaluate the inhibitory effect of intraperitoneal sustained-release chemotherapy with 5-FU on the growth of H22 ascitic tumor in mice.Methods: Mouse H22 ascitic tumor model was established by intraperitoneal injection of(0.2 ml) H22 ascitic cells(4?10~(6)cells) and the animals were subsequently divided into 4 groups randomly, namely,the saline control group(received saline),peritoneal chemotherapy group(received common 5-FU),sustained-release chemotherapy group(received sustained-release 5-FU),and negative control group(received control sustained-release agent).The survival times of the mice were recorded in all groups.The apoptosis rates of H22 ascitic cells were analyzed with flow cytometry 9 and 12 days after injection of H22 cells and the proliferation index was calculated.Electron microscope was used to observe H22 cells 12 days after peritoneal injection.Results: The average survival time of peritoneal chemotherapy group([13.7?1.7] d) was significantly shorter than that of sustained-release chemotherapy group([15.3?2.0]d)(P

Objective To discuss the method of implant for alveolar bone deficiency.Methods 37 cases of bone deficiency were chosen to use maxillary sinus augmentation,localized management of sinus floor,autologous jaw chips transplantation,alveolar bone distraction.Results All of the implants osseointegration except 1 implant loosed because of improper prosthesis.Conclusion All of the methods above provide can good ways for wider indication of implantation.
8.Clinical Study of Shuangyi Qushi Tongluo Capsules in Treating Knee Osteoarthritis

Objective To evaluate the efficacy and safety of Shuangyi Qushi Tongluo Capsules (SQTC) for the treatment of knee osteoarthritis (KOA).Methods A multi-center,randomized,parallel-controlled trial was carried out in 240 cases of KOA patients.The patients were divided into trial group (N =120) and control group (N =120),which was given SQTC,Xianlinggubao capsules respectively for 8 weeks.Before treatment,and 2,4 and 8 weeks after treatment,we recorded the scores of clinical symptoms and traditional Chinese medical syndromes,Western Ontario and McMaster Universities Arthritis Index (WOMAC) scores and knee function scores.After treatment,the clinical efficacy and adverse reaction were observed.Results (1) After treatment for 8 weeks,the clinical efficacy of the trial group was superior to that of the control group (P < 0.05).(2) The trial group had better effect on improving the visual analogue scale (VAS) scores of knee pain,VAS scores of limitation of motion and time for morning stiffness than the control group after treatment for 8 weeks(P < 0.05 or P < 0.01).(3) After treatment for 4 and 8 weeks,the trial group had better effect on improving scores of traditional Chinese medical syndromes,WOMAC scores and Japanese Orthopedic Association(JOA) scores than the control group (P < 0.05 or P < 0.01).(4) Before and after treatment,the results of blood,urine and stool routine examination,liver and kidney function,and electrocardiography showed no significant changes.Conclusion SQTC are effective and safe in treating KOA,and can start an effect shortly.
9.Comparison of diagnostic values of Clusterin, CXCL13, Podoplanin, CD21 and CD35 in foll-cular dendritic cell sarcoma

Purpose To evaluate the diagnostic values of Clusterin, CXCL13, Podoplanin (D2-40), CD21 and CD35 in follcular den-dritic cell sarcoma. Methods The expression levels of 10 cases of follcular dendritic cell sarcoma ( FDCS) and 12 types of FDCS mimics (83 cases in total) were investigated by immunohistochemical methods, the latter including solitary fibrous tumor, leiomyosar-coma, gastrointestinal stromal tumor and others. The diagnostic validities of the five biomarkers were compared. Results ( 1 ) The positive rates of Clusterin, CXCL13, D2-40, CD21 and CD35 in the FDCS group were 100%, 70%, 60%, 90% and 80%, respec-tively, the corresponding rates in the control group were 30%, 4%, 11%, 2% and 0 in turn. (2) The five biomarkers could be cate-gorized into 3 groups, according to their diagnostic values in FDCS. The first group included CD21 and CD35, which had much higher sensitivities (90%, 80%), specificities (100%, 98%) and accuracies (98%, 96%), compared with the other biomarkers. The second group included CXCL13 and D2-40, which had a relatively lower sensitivities (70%, 60%), specificities (96%, 89%) and accuracies (94%, 86%), compared with CD21 and CD35. The third group included Clusterin, which had the highest sensitivity (100%), while the specificity (70%)and accuracy (73%) were inferior to the first and second groups. (3) The diagnostic values of CD21 and CD35 combination were 100%, 98%, 98%, 83% and 100%, respectively. Conclusions (1) CD21 and CD35 are the most valuable biomarkers for FDCS. The combined diagnostic effect of the two markers is generally superior to that of single marker. (2) Clusterin has the highest sensitivity for FDCS. However, the frequent expression in FDCS mimickers restricts its diagnostic values for FDCS. (3) CXCL13 and D2-40 may be used as a marker for FDCS, but are not recommended as the first selection based on their diagnostic performance. (4) On the reasons that FDCS mimickers may not uncommonly express Clusterin and D2-40, and rarely show positivity for CD21 or CXCL13, more attention should be paid to the phenomenon to avoid misdiagnosis.
10.Expression of the Four Mismatch Repair Genes Protein of Patients with Colorectal Cancer and Its Clinical Significance

Objective:To analyze the expression of the four mismatch repair genes protein (hMLH1,hMSH2,hMSH6 and hPMS2) of patients with colorectal cancer and its clinical significance.Methods:177 cases of patients with colorectal caner in the Third Affiliated Hospital of Guangzhou Medical University from January 2013 to December 2015 were randomly selected.Tested the expression of the hMLH1,hMSH2,hMSH6 and hPMS2 by immunohistochemistry,the relationship between protein expression and clinical parameters was analyzed.Results:Among 177 cases ofcolorectal cancer tissue,the deletion rate ofhMLH1 protein was 6.2% (11/177),the deletion rate of hMLH2 protein was 4.0%(7/177),the deletion rate of hMSH6 protein was 1.7%(3/177),the deletion rate of hPMS2 protein was 8.0%(14/177),the sum of the four values accounted for 19.8%(35/177) of all cases of colorectal cancer.The loss of expression of the four mismatch repair genes protein were correlated to tumor location (P<0.05),besides,the loss of expression of the hMLH1 and hPMS2 protein were correlated to degree of tumor differentiation (P<0.05),he loss of expression of the hMSH6 protein were correlated to depth of tumor invasion(P<0.05);But the loss was not correlated to age,sexes,lymph node metastasis and distant metastasis(P>0.05).Conclusion:The expression of loss phenomenon with mismatch repair protein appears in part of colorectal cancer,the loss phenomenon with mis match repair protein were correlated to tumor location and degree of tumor differentiation.Mutations of four genes in hMLH1,hPMS2,hMSH6 and bMSH2,to provide a reference value for the clinical judgment of prognosis and to develop a treatment plan.
