1.Impact of postoperative fish oil fat emulsion supplementation on monocytes function in gastrointestinal cancer patients

Objective To investigate the effect of fish oil parenteral supplementation on monocyte function in gastrointestinal cancer patients.Methods Sixty-four gastrointestinal cancer volunteers were equally randomized into study group and control group.Both groups received conventional lipid emulsion,and the study group was added with fish oil parenteral supplementation for 5 days.Blood samples were collected on the 1st,3rd,and 6th post-operative days to detect the interleukin (IL)-1β,IL-10,IL-6,tumor necrosis factor-α,and human leukocyte antigen-DR.Results After 5 days of continuous management,the IL-1β,IL-6,and tumor necrosis factor-o levels in the study group became significantly lower than those in the control group [(1 818 ± 213)pg/Lvs (2292 ±289)pg/L,P=0.027; (415 ±71)pg/Lvs (791±163)pg/L,P=0.001; (2 194± 350) pg/L vs (2 509 ± 507)pg/L,P =0.036,respectively),whereas the level of IL-10 by mononuclear leukocytes in two groups showed no significant difference.The expression of human leukocyte antigen-DR was significantly increased in both groups,but was significantly higher in the study group than in the control group [(76.6±15.2)% vs (62.3±16.8)%,P=0.029].Conclusion Fish oil helps improve postoperative monocyte function in patients with gastrointestinal tumor.
2.Expression and Clinical Significance of HER-2 and Survivin in Gastric Cancer Tissue

Objective To investigate the expression of human epidermal growth factor receptor?2(HER?2)and Survivin protein in gastric cancer tissue,and explore its relationship with clinicopathological characteristics. Methods Immunohistochemistry assay was used for detection of HER?2 and Survivin protein expression in 70 gastric cancer biopsy samples. The relationship with the clinicoathological parameters in patients with gas?tric cancer was analyzed. Results The expression of HER?2 and Survivin protein was correlated with tumor differentiation ,TNM stage and lymph node metastasis(all P<0.05),but not with sex,age,the diameter of tumor. The correlation analysis of positive rate and the semi?quantitative pro?tein expression showed that the expression of Survivin and HER?2 was significantly positively related(all P<0.01). Conclusion The positive ex?pression of HER?2 and Survivin protein in gastric cancer correlates with tumor differentiation ,TNM stage and lymph node metastasis. HER?2 ex?pression significantly correlates with Survivin at the protein level in gastric cancer tissues.
3.Expression and relationship of Beclin1 and Bci2 in invasive pituitary adenomas

Objective To detect the expression of Beclin1 and Bcl2 in invasive pituitary adenomas and to explore the relationship of Beclin1 and Bci2 in invasive pituitary adenomas and the relativity between the 2 genes.Methods 61 specimens were classified into invasive group (32 cases) and non-invasive group (29 cases) according to the comprehensive evaluation of invasive pituitary adenomas.lmmunofluorescence analysis and RT-PCR were adopted respectively to detect the protein and mRNA expressions of Beclinl and Bcl2.The difference and relativity of Beclin1 and Bcl2 expression in invasive group and non-invasive group were analyzed.Results 32 specimens of pituitary adenoma were invasive and 29 were non-invasive.Beclin1 protein and mRNA expressions were lower in the invasive group than in the non-invasive group (P <0.01 ).Bcl2 protein and mRNA expressions were higher in the invasive group than in the non-invasive group (P <0.01 ).Pearson related analysis showed that Beclin1 mRNA expression was negtively correlated with Bcl2 mRNA expression in the invasive group ( r =-0.42,P =0.028 ).Conclusions Beclinl expression is decreased in invasive pituitary adenomas.The invasiveness of pituitary adenoma is closely related to the high expression of Bcl2 protein and mRNA,and the low expression of Beclin1 protein and mRNA.The inhibition of the autophagy may lead to the enhancement of the invasiveness of pituitary adenomas and that inhibition may come from the interaction of Beclin1 and Bcl2.
4.Innovation and transformation are the soul of orthopedics: reflections on the academic thoughts of Professor Zhang Yingze

Professor Zhang Yingze, an academician of Chinese Engineering Academy, has dedicated himself to clinical medicine, scientific research and education in orthopedics for over 4 decades. In recent years, Professor Zhang, together with his research team, has made marvelous breakthroughs in various aspects of traumatic orthopedics which he is trying to put into practical transformation. The significant innovations of Professor Zhang and his colleagues include (1) establishment of an epidemiologic system for Chinese traumatic orthopedics, (2) proposal of new types of fractures, e.g., spiral tibial shaft fracture associated with posterior malleolar fracture, (3) introduction of fibular osteotomy for knee osteoarthritis and (4) new diagnosis and treatment approaches for pelvic and acetabular fractures. This paper is to discuss the academic thoughts of Professor Zhang, hoping to inspire young Chinese traumatic orthopedists to devote themselves to scientific innovations.

Expert Consensus on the Application of Chinese Patent Medicine for Acute Upper Respiratory Tract Infection was established under the joint sponsorship of Specialty Committee of Emergency of World Federation of Chinese Medicine Societies, Emergency Physician Branch of Chinese Medical Doctor Association, Emergency Medicine Professional Committee of Chinese Association of Integrative Medicine and Chinese Emergency Medical Parternerships. In the consensus, the Chinese patent medicines for treatment of acute upper respiratory tract infection (AURI) were summarized and analyzed, and after the expert writers had discussed the contents of the consensus together, they decided to formulate the experts' consensus related to the AURI, expecting to provide a reference to the clinical treatment of this disease.
6.Combined predictive value of the risk factors influencing the short-term prognosis of sepsis

Objective:To explore the risk factors influencing the short-term mortality of patients with sepsis in intensive care unit (ICU) and the combined value of predicting prognosis.Methods:A retrospective analysis was performed on 104 patients with sepsis admitted to emergency ICU of Jiangsu Provincial Hospital of Traditional Chinese Medicine from January 2018 to August 2019. Multiple general information containing gender, age, past history as well as complications and sequential organ failure assessment (SOFA) score, mean arterial pressure (MAP), blood routine examination, hepatic and renal function, coagulation indicators and procalcitonin (PCT) were collected within 24 hours of admission. Patients were divided into death group and survival group according to the 28-day outcome. Univariate and multivariate Logistic regression analysis were used to find the effective factors influencing the prognosis of sepsis. Receiver operating characteristic (ROC) curve was drawn to evaluate the value of related indexes in predicting the prognosis of sepsis. Correlation between parameters that might be relevant to disease severity and SOFA score was evaluated by Pearson or Spearman correlation analysis.Results:104 patients were enrolled for final analysis, of whom 60 patients survived, while the others died with a 28-day mortality of 42.3%. ① Univariate analysis results: the incidence of acute kidney injury (AKI), SOFA score, serum creatinine (SCr), D-dimer, activated partial thromboplastin time (APTT), international normalized ratio (INR) and PCT in the death group were significantly higher than those in the survival group [incidence of AKI: 70.5% (31/44) vs. 36.7% (22/60), SOFA score: 11.0 (8.0, 13.0) vs. 8.0 (6.2, 10.0), SCr (μmol/L): 108.8 (65.5, 235.6) vs. 75.1 (55.1, 109.5), D-dimer (mg/L): 4.1 (1.6, 11.6) vs. 2.1 (1.2, 4.3), APTT (s): 42.6 (37.7, 55.7) vs. 40.3 (35.9, 44.7), INR: 1.3 (1.2, 1.5) vs. 1.2 (1.1, 1.4), PCT (μg/L): 3.1 (0.4, 39.9) vs. 0.3 (0.1, 3.4), all P < 0.05]. ② Multivariate Logistic regression analysis results: all indicators of univariate analysis were included in the multivariate Logistic regression model considering interaction between each variable. Multivariate Logistic regression analysis was repeated based on conditional backward method. Age, SOFA score, MAP, neutrophil (NEU), lymphocyte (LYM) and APTT were automatically selected by SPSS software to build the predicting model. Analysis results showed that SOFA score, NEU and LYM were independent risk factors for the short-term prognosis of sepsis [SOFA score: odds ratio ( OR) = 1.22, 95% confidence interval (95% CI) was 1.04-1.44, P = 0.02; NEU: OR = 1.14, 95% CI was 1.03-1.26, P = 0.01; LYM: OR = 0.79, 95% CI was 0.66-0.95, P = 0.01]. ③ ROC curve analysis results: the above six-variable prediction model had the optimal fitting degree defaulted by SPSS. ROC curve showed that the combination of age [area under ROC curve (AUC) = 0.60], SOFA score (AUC = 0.71), MAP (AUC = 0.53), NEU (AUC = 0.59), LYM (AUC = 0.54) and APTT (AUC = 0.61) had better sensitivity (79.5%) and specificity (65.0%) as well as the maximal AUC (AUC = 0.75), which suggested that combined prediction had higher diagnostic value in predicting the short-term prognosis of sepsis.④ Correlation analysis showed that NEU, D-dimer, prothrombin time (PT), APTT, INR and PCT were positively correlated with SOFA score ( r values were 0.26, 0.28, 0.21, 0.22, 0.10, 0.38, respectively, all P < 0.05). Conclusions:SOFA score, NEU and LYM were independent risk factors for the short-term prognosis of sepsis. The combination of age, SOFA score, MAP, NEU, LYM and APTT were more accurate than any single factor in predicting the short-term prognosis of sepsis and had higher diagnostic value. NEU, D-dimer, PT, APTT, INR and PCT were correlated with SOFA score.
7. Efficacy and safety of CTD and PCD regimens in treatment of patients with newly diagnosed multiple myeloma

Objective:
To observe the efficacy and safety of CTD (cyclophosphamide, thalidomide, dexamethasone) and PCD (bortezomib, cyclophosphamide, dexamethasone) regimens in treatment of patients with newly diagnosed multiple myeloma (NDMM) .
Methods:
A retrospective analysis was carried out on 88 cases of NDMM patients admitted to our hospital from July 2013 to January 2016, including 49 cases in CTD group and 39 cases in PCD group. The outcomes of two different regimens were analyzed, including response, prognosis, and adverse events.
Results:
The total overall remission rates (ORR, better than PR) of CTD and PCD were 65.3% (32/49) and 84.6% (33/39) , while very good partial response (VGPR) were 30.6% (15/49) and 53.8% (21/39) , and differences were statistically significant (

OBJECTIVE:
To detect chromosomal aberrations by using cytoplasmic light chain immunofluorescence with fluorescence in situ hybridization (cIg-FISH), and to explore the correlation of del(17p13) with clinical characteristics, drug response and prognosis among patients with newly diagnosed multiple myeloma (NDMM).
METHODS:
Clinical data of 198 cases of NDMM was collected. cIg-FISH and a specific probe (TP53) were used to detect karyotypic abnormalities in bone marrow samples derived from the patients. Correlation between karyotypic abnormalities and clinical data was analyzed.
RESULTS:
Nineteen of the 198 patients (9.6%) were found to have a karyotype involving del(17p13). The overall survival (OS) and progression-free survival (PFS) for patients with or without del(17p13) was significantly different (P<0.01). No significant difference was found in OS and PFS between patients carrying a del(17p13) on bortezomib and non-bortezomib regimen (OS: P = 0.873; PFS: P = 0.610).
CONCLUSION
cIg-FISH is a simple and convenient method for the detection of karyotypic anomalies in multiple myeloma. Del(17p13) is an indicator for poor prognosis for multiple myeloma patients. Bortezomib cannot improve the survival disadvantage of del(17p13).
9.Textual quantitative analysis of rural-oriented tuition-waived medical student training policy in China from the perspective of policy instruments

Objective:To analyze the use of policy tools for rural-oriented tuition-waived medical student training policy and to provide relevant suggestions for the continuous promotion of the policy.Methods:With "rural-oriented tuition-waived medical students" as the key word, the policy texts were collected and screened from government portals. Using ROTHWELL disaggregated method to build the rural order directional medical students training policy analysis framework, applying Excel 2019 software for classification and coding of policy texts.Results:A total of 13 rural-oriented medical student training policy texts were screened and obtained. The X dimension of the policy analysis framework for rural order-oriented medical student training included three policy tools, namely, supply, environment and demand, and the Y dimension included three policy objectives, namely, available, usable, and retained. In X dimension, environmental policy tools were most frequently used. In Y dimension, the "retained" target had the highest frequency of use.Conclusion:There were differences in the frequency of using policy tools for targeted medical student cultivation in different policies. The frequency of using environmental tools is higher, which highlighted the attention of the state to medical and health services. The internal structure of policy tools is unbalanced, so the configuration of supply-oriented policy tools should be optimized, and the construction of demand-oriented policy tools should be emphasized. It is suggested to continuously optimize the combination of policy tools, improve the compatibility between policy tools and rural order-oriented medical student training, and pay attention to the sustainability of policy tools.
10.The role of Ki-67 in the prognostic evaluation of multiple myeloma

Objective:To investigate the expression level of Ki-67 in the bone marrow biopsy of newly diagnosed MM patients, and its relationship with clinical efficacy and prognosis.Methods:Bone marrow pathological samples of 124 newly diagnosed MM patients in Jiangsu Province Hospital from January 2012 to June 2017 were collected. The expression level of Ki-67 in myeloma cells was detected by using immunohistochemistry. X-tile software was applied to find a cutoff of Ki-67. The patients were divided into the high Ki-67 expression group and the low Ki-67 expression group, and the clinical characteristics, therapeutic efficacy and survival of both groups were compared. Chi-square test or Fisher's exact test was used to analyze the counting data. Kaplan-Meier method was applied to make survival anlaysis. Cox regression model was used for univariate prognostic analysis and multivariate prognostic analysis.Results:A total of 124 newly diagnosed MM patients were enrolled with median follow-up of 36 months. The proportion of the positive myeloma cells in abnormal plasmocytes was used to quantize the expression level of Ki-67. Using a cutoff of 20%, these cases could be divided into two groups; the proportion of positive cells was lower than 20% (the low Ki-67 expression group) and the proportion of positive cells was 20% or above (the high Ki-67 expression group). There were 27 cases (21.7%) in the high expression group and 97 cases (78.2%) in the low expression group. There were no statistically significant differences in the clinical characteristics and treatment regimens (all P > 0.05). The overall remission rate (ORR) of patients in the high Ki-67 expression group was lower than that of patients in the low Ki-67 expression group [59.3% (16/27) vs. 83.5% (81/97)], and the difference was statistically significant (χ 2 = 7.290, P = 0.007). The percentage of patients who achieved very good partial remission (VGPR) and complete remission in the high Ki-67 expression group was lower than that of those in the low Ki-67 expression group [33.3% (9/27) vs. 66.0% (64/97)], and the difference was statistically significant (χ 2 = 9.297, P = 0.002). There were statistically significant differences in the median progression free survival (PFS) time (12.0 months vs. 31.0 months, P < 0.01) and 3-year PFS rate (10% vs. 37%, P = 0.002). The median overall survival (OS) time was 39.0 months and 56.5 months in the high and low Ki-67 expression groups, respectively ( P = 0.003). The multivariate analysis showed that high Ki-67 expression was an independent affecting factor for PFS ( HR = 3.592, 95% CI 1.921-6.719, P < 0.01) and OS ( HR = 3.511, 95% CI 1.537-8.022, P = 0.003). Conclusions:High expression of Ki-67 is an independent poor prognostic factor affecting therapeutic effect and survival for newly diagnosed MM patients.
