1.The expression of Survivin,PTEN in tongue squamous cell carcinoma and the correlation with vascular endothelial growth factor and its clinical significance

Objective To evaluate the expression of Survivin,PTEN in tongue squamous cell carcinoma (TSCC) and the correlation with vascular endothelial growth factor (VEGF) and its clinical significance.Methods The expression of Survivin,PTEN and VEGF of 72 TSCC samples and 15 normal tongue mucosa (NTM) samples were detected by immunohistochemical method.Results The positive expression rate of Survivin,PTEN and VEGF in TSCC were 66.7%(48/72),61.1%(44/72),70.8%(51/72)respectively,while the positive expression rate in NTM were 0,100.0% (15/15),0.There were statistical significance between the rate in TSCC and NTM respectively (P <0.01 ).The expression of Survivin and VEGF in TSCC were positively correlated with TNM stage,histological grade and lymph node metastasis (P< 0.05 or < 0.01 ),while the expression of PTEN had negative correlation with them (P< 0.05 or < 0.01 ).The expression of Survivin was positively correlated with VEGF expression in TSCC (r=0.6482,P<0.01),while the expression of PTEN was negatively correleted with VEGF expression (r =-0.4027,P <0.01).Conclusions The expression of Survivin and PTEN in TSCC are associated with tumor occurrence and development closely,and are both significantly correlated with VEGF.Joint detection of Survivin,PTEN and VEGF have important reference value in clinical diagnosis,metastasis and prognosis of TSCC.
2.Cervical Mediastinoscopy for Diagnosis of Thoracic Diseases in 281 Patients

Objective To investigate the value of cervical mediastinoscopy(CM) in the diagnosis of thoracic diseases.Methods From January 1990 to September 2008,281 patients with diseases in the chest were examined by cervical mediastinoscopy under local infiltration anesthesia or general anesthesia.Through the pretracheal space,exploration and biopsy of the neoplasms and lymph nodes around the trachea were carried out.Results Among the cases,pathological diagnosis were made in 268 patients(95.4%),including the 23 patients who were not clearly diagnosed before the operation.The accuracy of the preoperative diagnosis was 56.9%(41/72) for malignant tumors,and 87.4%(104/119) for benign lesions.Conclusions Cervical mediastinoscopy is an effective method to detect mediastinum diseases with or without other thoracic diseases,especially for simply enlarged lymph nodes in the mediastinum.For the patients with lung cancer complicated with enlarged lymph nodes in the mediastinum or those with tumors in the mediastinum,the method is also feasible.
3.A comparison of Sun’s operation and ascending aorta replacement combined with open placement of triple-branched stent graft in treatment of type A aortic dissection

Objective To evaluate results of surgical treatment for patients with acute type A aortic dissection using Sun’s operation and triple-branched stent graft. Methods According to the operation mode, thirty-three patients with type A aortic dissection were divided into Sun’s operation group (n=22) and triple branches aortic arch stent-graft placement op?eration (triple-branched) group (n=11). Preoperative examinations included cardiac ultrasound, aortic CT angiography (CTA), hepatic and renal functions and blood routine test in all patients. Intraoperative monitoring included the index about cardiopulmonary bypass and blood loss. The perioperative hepatic and renal functions and complications were also recorded. Survival and recovery rates were evaluated by follow-up between two groups of patients. Results There were six periopera?tive death in Sun’s group, and three patients died in triple-branched group. In triple-branched group, the intraoperative blood loss was significantly increased than that in Sun’s group [(3 586.4±2 926.8) mL vs. (2 630.5±1 821.2) mL, P<0.05]. Postoperative echocardiographic examinations revealed that the left ventricular size (LVEDd) and the aortic diameter were decreased while the left ventricular ejection fraction (LVEF) was elevated after surgery in Sun’s group [(50.9±6.9) mm vs. (55.0±7.5) mm,(28.2±1.6) mm vs. (48.8±11.0) mm, 0.620±0.031 vs. 0.469±0.104, P<0.05]. Whereas, only the aortic diame?ter was decreased after surgery in triple-branched group [(28.6±3.9) mm vs. (50.9±9.2) mm, P<0.05]. Kaplan-Meier surviv?al curve showed that five-year survival rate of Sun’s group was similar with that of triple-branched group (Log-rank χ2=0.095,P>0.05). At 5 year after operation, there were no significant differences in the recurrence of new aortic dissection, the incidence of cerebral infarction and mortality between Sun’s group and triple-branched group (P>0.05). Conclusion Sun’s operation can significantly decrease patient’s intraoperative blood loss and improve cardiac function. But its survival rate and long term results need advanced observation.
4.Observation and nursing of patients after minimally invasive ventriculo-atrial shunt

Objective To summarize the observation points and nursing experience after minimally invasive ventriculo-atrial shunt. Methods 45 patients after minimally invasive ventficulo-atrial shunt were given preoperative psychological care and preparations, postoperative observation of vital signs, con-sciousness, pupil, with or without intracranial hypertension, inadequate shunt or transitional symptoms, in-fectious symptoms, specific and basic care, awareness of postoperative complications and detailed guidance for discharge and follow-up jobs. Results Half month after shunt 28 cases regained consciousness, 9 cases with alleviated consciousness dysfunction, 8 cases with unchanged consciousness dysfunction, 5 cases with shunt blockage; 3 cases with over-shunt; 5 cases with inadequate shunt, 1 case with blood-borne in-fection, 1 case with shunt exposed, no intracranial infection, air embolism, complications such as endocardi-tis took place. Conclusions Adequate preoperative preparation and close postoperative observation, ef-fective specific care and basic care can increase success rate of ventriculo-atrial shunt and improve the quality of life of patients.
5.Therapeutic effect comparison between unipolar and bipolar radiofrequency ablation in treating atrial fibrillation in elderly patients

Objective To compare clinical effect of unipolar versus bipolar radiofrequency ablation in treating atrial fibrillation (AF) in elderly patients and discuss the correlated risk factors.Methods From October 2008 to December 2013,a total of 50 elderly patients with organic heart disease and AF underwent radiofrequency ablation surgery.All patients were divided into 2 groups of unipolar (group A,n=20) and bipolar (group B,n=30 patients) radiofrequency ablations.The variations of electrocardiogram (ECG) and ultrasonic cardiogram (UCG) in patients were collected and the complications and the NYHA class of the patients were recorded at 3,6 and 12 months after surgery.The total 43 elderly patients were divided into sinus group and non-sinus group according to their ECG at 12 months after surgery.Results The recovery rates of sinus rhythm at 3,6 and 12 months after surgery were 73.7%(14/19),66.7%(12/18) and 61.1% (11/18)respectively in group A and 82.8%(24/29),85.2%(23/27)and 88.0%(22/25) respectively in group B.Two groups of the recovery rate of sinus rhythm had a statistically significant difference between the two groups (P<0.05) at 12 months after surgery.The complication rate in group A was higher than in group B [55.0% (11 cases) vs.26.7% (8 cases),P<0.05].Preoperative history of AF and left atrial diameter were the influencing factors for the sinus rhythm recovery rates (P<0.05).Conclusions Bipolar radiofrequency ablation for AF is safe and has less complications and high success rate of restoring sinus rhythm.The patients with long preoperative history of AF and left atrium distension have a low success rate of restoring sinus rhythm with a high recurrence rate.
7.Neurotoxicity preventive effect of oxaliplatin with lipoic acid plus sodium potassium magnesium calcium and glucose injection

A total of 218 patients on chemotherapeutic regimens containing oxaliplatin were randomly divided into experimental (n =120) and control (n =98) groups.The experimental group received an intravenous infusion of lipoic acid plus sodium potassium magnesium calcium and glucose injection.The control group had only normal saline.Overall incidence of neurotoxicity and toxicity grade of peripheral nerve were observed after 4,8 and 12 cycles.Those with neurotoxic symptoms were followed up for 1 year.No significantly statistical difference existed in the incidence of peripheral neurotoxicity after 4,8 cycles (P >0.05).After 12 cycles,31 patients in the experimental group had an onset of neurotoxicity of grade3 (n=8,6.7%) &grade4 (n=0) versus21 cases of grade3 (n=21,21.4%) and grade4 (n=5,5.1%) in the control group.Statistically significant differences existed between grades 3 and 4 neurotoxicity (P <0.05).After 1 year of follow-up,the incidence of grade 1 of neurotoxicity was 2.5% (n =3) in the experimental group versus 23.7% (n =9) in the control group.And the inter-group difference was statistically significant (P < 0.05).Lipoic acid plus sodium potassium magnesium,calcium and glucose injection can effectively prevent the occurrences of acute and chronic peripheral neurotoxicity associated with oxaliplatin.
8.Effects of trichosanthin on apoptosis and cytoskeleton microtubule structure reconfiguration in lung cancer A549 cells

Objective:This study aimed to explore the effects of trichosanthin on the proliferative inhibition and apoptosis induc-tion in human lung carcinoma A549 cells. Methods:A549 cells were treated with various concentrations of TCS. The inhibitory effects in proliferation were detected by the MTT method. The microfilament changes were observed by transmission electron microscopy. Apoptosis rate and cell cycle were determined by flow cytometry. Results:A549 cells treated with TCS presented apoptotic changes and decreased cell activity. When the concentration increased and time was prolonged, the inhibition rate increased correspondingly. Conclusion:Pharmacological doses of TCS inhibited the proliferation and differentiation in lung carcinoma A549 cells and affected the function in A549 cells by changes in the cytoskeleton.
9. Protective effect of autologous platelet-rich plasma in elderly patients with type A aortic dissection undergoing total arch replacement surgery

Objective:
To assess the therapeutic effect and safety of autologous platelet-rich plasma(aPRP)in elderly patients undergoing total arch replacement surgery.
Methods:
A total of 44 elderly patients aged 60 years and over undergoing total arch replacement surgery in our hospital from August 2015 to August 2018 were enrolled.Forty-four patients were divided into the control group without aPRP application(n=23)and the aPRP application group(n=21). Patients were followed up postoperatively, and preoperative and intraoperative data, postoperative mortality, the incidence of continuous renal replacement therapy(CRRT)and neurological complications were collected and statistically analyzed.
Results:
Compared with the control group, the aPRP application group showed the transfusion volume of allogeneic red blood cell and fresh frozen plasma were significantly decreased in aPRP group[(69.1±138.3)ml
10.Prediction model for the risk of postoperative death in patients with acute type A aortic dissection

Objective:Using different machine learning methods to construct and screen the best prediction model for predicting the risk of death within 30 days after surgery in patients with acute type A aortic dissection.Methods:Five hundred and twenty-one patients with acute type A aortic dissection who underwent surgery between 2015 and 2022 were included, after collecting their perioperative date and screening them, 329 patients were retained. two different groups of predictor variables were generated by using Lasso regression and principal component analysis, after that, logistic regression, support vector machine algorithm, random forest algorithm, gradient boosting algorithm, and super learning algorithm were used to develop prediction models for the risk of death within 30 days after surgery. Finally, we compare the models and select the best one. Results:The AUC values for all models rangrd from 0.791-0.959. The model using Lasso regression to determine the predictor variables and built by the super learning algorithm had the best prediction with an AUC value of 0.959. Conclusion:The super learning algorithm better than other algorithms in predicting death within 30 days after acute type A aortic dissection.
