1.Practice of Generalizing the Use of the Laboratory Electronic Order System

Objective To generalize widely the use of laboratory electronic order system in hospital,and rebuild and optimize laboratory workflow. Methods The workflows were confirmed by step-by-step and deployed synthetically materials preparation and staff training,which improved the system continuously during its working. Results All the concerned modules runs stably,which functions are improved greatly and the expected goals of design and application generalization are reached. Conclusion The functional applicability and manipulability of interface are in favor of generalizing the use of the health information system.
2.Significance of bispectral index monitoring during the patients after general anesthesia while leaving the post-anesthetic intensive care unit

Objective To discuss the significance ofbispectral index (BIS) monitoring during the patients after general anesthesia while leaving the post-anesthetic intensive care unit (PAICU). Methods Seventy-five cases after general anesthesia were selected. When they were sent out of the PAICU, recorded their BIS. Results Twenty-one cases with BIS 90 - 100,48 cases with BIS 80 - 89 in slight calm and hypnosis,5 cases with BIS 70 - 79, and 1 case with BIS 50 - 60. Conclusions Without BIS monitoring,when sent out of the PAICU,most patients after general anesthesia are gently sedated and without too much risk. But still some patients are deeply sedated. BIS monitoring provides a nice reference for the patients while leaving the PAICU.

4.Investigating the complexity of heart rate variability during anesthesia.

By use of the complexity analysis indices (approximate entropy, complexity), the heart rate variability signals obtained from 38 subjects' ECG during anesthesia were analyzed. The results showed that there is an obvious chaos change of heart rate variability during anesthesia, both the complexity and approximate entropy of heart rate variability during anesthesia are evidently less than those during consciousness (P<0.05). In this dissertation, we decompose the heart rate variability during anesthesia into 1/f part and non-fractal part, and then analyze these parts. The results reveal that the sensitivity of the complexity indices can be improved by decomposing the heart rate variability.

6.Breast tumor size assessment:comparison of conventional ultrasound and real-time ultrasonic elastography

Objective Tumor size discrepancy in measurement between real-time ultrasonic elastography and conventional uItrasound(US) was found in some breast lesions.The study was designed to investigate the value of the feature in the diagnosis of breast cancer,and its relationship with the margin features on gray-scale US.Methods Both US and real-time elastogram using a Hitachi EUB-8500 US system were performed in 308 consecutive Datients.The margin features on US and the size measured on both methods were documented.A lesion was defined as a significant size discrepancy when a larger size measured at elastogram.Sensitivity,specificity,and overall accuracy of feature to diagnosis malignancy were determined by surgical pathology as gold standard.Results There were total 166 benign,163 malignant and 3 borderline lesions.of 99 lesions with larger measurement on elastography,there were 91 malignant lesions and 8 benign lesions.By the feature of size discrepancY,the sensitivity,specificity and accuracy was achieved 55.8%,96.9%,75.7%,respectively.The size measured on elastogram was statistically larger than that on gray scale US(t=-11.0,P<0.05).The ratio of indistinct,angular and spiculated margin characteristics were significantly higher in breast cancers with larger size measurement than those with unchanged or decreased size measurement(P<0.05).Conclusions The increased size-measurement at elastography is helpful to diagnosis breast cancers.And it is more likely to be present in breast cancers with indistinct,angular or spiculated margin.
7.Level and clinical significance of plasma tissue factor in patient with non-small cell lung cancer

Objective To detect the plasma level of tissue factor (TF) in non-small cell lung cancer (NSCLC) patients,and to discuss its association with hypercoagulation,venous thromboembolism and prognosis of lung cancer.Methods Sixty-one impatients in our hospital with confirmed lung cancer were enrolled as the study group.Thirteen patients with benign pulmonary diseases and 14 healthy volunteers were selected as the control groups.Bseline and follow-up clinical data were collected from participants.Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of TF in plasma of all subjects.Results The levels of TF in plasma from NSCLC patients and participants with benign pulmonary diseases was significantly higher than that in healthy controls((550.88 ± 201.58) ng/L vs (510.77 ± 201.20) ng/L vs (178.34 ±66.73) ng/L,P <0.05).According to the plasma levels of TF,which have been detected in all subjects,the patients were divided into two groups:low level group (range from 103.73 ng/L to 476.22 ng/L) and high level group (range from 476.221 ng/L to 1003.00 ng/L).Statistical analysis showed that there was a positive correlation between plasma TF levels and TNM stages in NSCLC patients (P =0.026).Patient with metastasis had a higher plasma TF level than other patients (P =0.020).The log-rank test revealed that there was no significant difference in survival between the high level group and low level group (x2 =0.145,P =0.704).Multivariate Cox proportional hazards regression analysis indicated that plasma TF levels did not predicted for death(RR =1.001,95%CI0.998-1.004,P=0.452).Conclusion The plasma TF level in NSCLC patients was correlated with TNM stages;it had no significant relationship with hypercoagulation state and survival rate in NSCLC patients.Limitations should be aware of while evaluating the clinical course and prediction of prognosis of NSCLC patients using plasma TF levels.

Objective To investigate the effect of growth hormone replacement therapy on life quality of patients with growth hormone deficiency after sellar tumor surgery.Methods The data of 13 patients undergoing sellar tumor resection and suffering postoperative hypopituitarism from Jun.2009 to Dec.2010.were collected.Each of them was given daily 0.5 units growth hormone (about 0.17 mg) as replacement therapy.The life quality of patients at different time points was evaluated with QoL-GDHA scale.SPSS 17.0 software was used for statistical analysis.Results 1,3,and 6 months after treatment,QoL-GDHA score of the patients was 3.91± 2.29,3.38 + 2.43,and 3.23 + 2.28 repectively,significantly lower than before ( 8.69 ± 2.66).The difference had statistical significance (P <0.05 ).Conclusion Growth hormone replacement therapy can significantly improve the life quality of patients with hypopituitarism after sellar tumor surgery.
10.A study on the LIS and HIS integration.

The development of hospital business demands the integration of information systems. After studying the relative technologies for system connection,system integration and different modes of connecting system, we put forward a protocol to integrate the existing laboratory information system( LIS) and hospital information system(HIS). It introduces the multi-tier distributed technical methods for improving the client/server applications; it ascertains the units and transactions participating in workflow integration by referencing the IHE( Integrating the Healthcare Enterprises) Laboratory Technology Framework; it implements information exchange by Health Level 7(HL7) standards; and it can conserve previous investment and avoid some problems of security, maintenance and realtime process without the risk of massively updating system.
