1.Diagnostic Value of Spiral CT Scanning and CT Virtual Endoscopy for Gastric Carcinoma

Objective To evaluate spiral CT scanning and CT virtual endoscopy(CTVE) in the clinical diagnosis of gastric carcinoma.Methods Spiral CT scan was performed in 28 patients with pathologically-proved gastric cancer . The CT scan parameters were : collimating width 5 mm , Pitch 1 . The volumetric data were acquired using a spiral CT scanner and transferred to computer workstation . CTVE was performed by navigator smooth function . The results were compared with the findings of fiberoptic endoscopy and/or surgery .Results Spiral CT scan could demonstrate the location ,size,extension of lesions and the relation with organs around .In 28 patients,CT findings included omentum involvement(n=7),pancreatic ivolvement (n=4),hepatic involvement (n=3),lymph node metastasis(n=3). CTVE could clearly demonstrate the three-dimensional image of normal wall stomach and the lesions , size and extension of lesions . The appearances of the inner lesions and normal wall of stomach on CTVE were similar to those on fiberoptic endoscopy and/or surgery . The accuracy in detecting lesion by CTVE was 92.8% in 28 patients . Conclusion CTVE combination with spiral CT scan is of great value in clinical diagnosis of gastric carcinoma .
2.Diagnostic Value of Spiral CT and 3D Reconstruction in Pulmonary Tuberculosis

Objective To evaluate the diagnostic value of spiral CT and all kinds of 3D reconstruction in pulmonary tuberculosis .Methods 145 cases of pulmonary tuberculosis confirmed by clinical study and histopathology were analyzed retrospectively. All cases weregiven to helical scan and different cases were given to different 3D reconstruction (CTVE, MPR and CPR) .Results Spiral CT (HRCTespecially) could find many signs of pulmonary tuberculosis, such as centrilobular distribution nodules, tree-in-bud, cavitations,tuberculous pneumonia, miliary nodules, branchogenic dissemination, endobranchial tuberculosis, lymph node enlargement and calcification. The accuracy of spiral CT in the diagnosis of pulmonary tuberculosis was 96.6%, The accuracy of spiral CT with 3Dreconstruction in the diagnosis of pulmonary tuberculosis was 99.31%.Conclusion Pulmonary tuberculosis can be diagnosed accurately by spiral CT with all kinds of 3D reconstruction.
3.The CT imaging features of pneumonic type bronchioloalveolar carcinoma and the diagnostic value of CT-guided percutaneous puncture biopsy

Objective To investigate the CT imaging features of pneumonic type Bronchio-loalveolar carcinoma(BAC) and the value of CT-guided percutaneous puncture biopsy in pneumonic type BAC.Methods Twenty-five cases of suspected pneumonic type BAC were biopsied with CT-guided percutaneous puncture.GE 64-slices plain and post contrast CT scans were performed in all patients.Two radiologists finished imaging determination.Site of puncture was selected in suspicious areas of the tumor.The negative pressure aspiration needle were used to be biopsy in the ideal location after vertically into the needle and again performed CT scanning were performed to observe the complications such as hemorrhage,pneumothorax.Specimens were fixed by formalin.Results Twenty-five samples were successfully got from 25 patients.Pathology data showed that 14 cases were pneumonia with BAC,6 cases with pneumonia,2 cases with offungus infection,and 3 cases with caseous pneumonia.Of 14 cases (71.4%) with pneumonia type BAC,CT images of 10 cases showed air bronchogram of lung tissue,5 cases(35.7%) with alveolar gas cavity or cavity of lung tissue,5 cases(35.7%) with ground glass opacity and multiple nodules.Enhanced CT scanning the area of consolidation showed that 11 cases(78.6%) were with mixed low density area angiography and the mean peak time was 90 s,9 cases (64.2%) were with the time density curve of speed up and slow down type.Conclusion The method of CT guided lung biopsy combined with typical imaging findings can enhance early diagnosis rate of pneumonia type BAC.Meanwhile CT feature of pneumonia type BAC shows honeycomb air cavity,void,angiographic sign,dead branches syndrome,multiple peripheral acinar nodules and ground glass density.Enhanced scan area of consolidation shows visible angiogran sign,slow drop type of time density curve of a fast rise.
4.Feasibility of Double Low Dose Technology in Multi-slice Head and Neck CT Angiography Without Adaptive Statistical Iterative Reconstruction

Purpose To explore the feasibility of double low dose technology in multi-slice head and neck CT angiography, so to reduce contrast agent as well as reduce radiation dose. In that way, the hazards of radiation and the risk of contrast induced nephropathy would be reduced. Materials and Methods Fifty-one patients took double low proposal were recruited as double low group. Another 51 patients who took conventional proposal were recruited as conventional group. The scanning parameters of double low group were 100 kV, 300 mA, 0.7 ml/kg iodinated contrast agent and which of conventional group were 120 kV, 350 mA, 1.0 ml/kg iodinated contrast agent. The CT value of the vessels and image noise were measured. Signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), subjective image quality score, radiation dose and iodine load between the two groups were compared. Results The volume CT dose index (CTDIvol) of the double low group and the conventional group were 18.00 mGy and 33.86 mGy, respectively. And the effective dose were 3.85 mSv/(mGy · cm) and 7.25 mSv/(mGy · cm), respectively. The iodine loads were 224 mgI/kg and 320 mgI/kg, respectively. SNR and CNR at the aortic arch were higher in double low group than in conventional group, but which was not statistic significant (t=-1.572 and -1.783, P>0.05). The SNR and CNR of bilateral carotid arteries, internal carotid arteries, and middle cerebral arteries had no statistic significance between the two groups (t=0.341-1.739, P>0.05). The subjective image quality scores of double low group and conventional group were (3.69±0.47) scores and (3.70±0.46) scores, which showed no statistic difference (Z= - 0.213, P>0.05). Conclusion Approving images of multi-slice head and neck CT angiography can be obtained by using double low dose technology without adaptive statistical iterative reconstruction.
5.Clinical significance of volume of epicardial adipose tissue and calcification score on evaluating the degree of coronary artery stenosis

Objective To investigate the comparison of volume of epicardium adipose tissue and calcification score on evaluating the degree of coronary artery stenosis(CHD).Methods Two hundred and ten cases with suspected coronary artery disease scanned by the 64 multi slice CT (MSCT) were selected as our subjects.One hundred and forty-five cases were diagnosed non coronary heart disease by with < 50% coronary artery stenosis by coronary angiography and served as non-CHD group.Other 55 cases were diagnosed as CHD with coronary artery stenosis ≥ 50% and served as CHD group.The volume of epicardium adipose tissue and calcification score of all cases were calculated with Siemens definition workstation.Results The volume of epicardium adpose tissue and calcification score in patients of CHD group were (117.3 ± 57.6) cm3 and (413.3 ± 670.5) %,significant different from that of non-CHD group ((100.0 ± 46.5) cm3,(36.2 ± 118.5).The differences were statistically significant (t =2.316,6.564; P < 0.05,P < 0.01).ROC curves showed that the sensitivity and specificity of calcification score were higher than the methods of volume of epicardium adipose tissue in terms of the diagnosis of stenosis and coronary artery disease.Conclusion Calcification score is proved to be a better method of predicting the coronary artery stenosis.
6.Correlation of epicardial adipose tissue volume with coronary plaques in patients with a coronary artery calcium score of zero

Objective To evaluate the correlation of epicardial adipose tissue volume (EATV)with coronary plaques in patients with a coronary artery calcium score of zero.Methods 183 patients with a coronary artery calcium score of zero were selected.They were divided into plaque group and control group according to the findings of CT coronary angiography.Independent t test was used to analyze the difference of EATV between two groups.Results ①EATV was significant higher in plaque group than that in control group (P <0.05).②EATV was non-significant higher in plaque group than that in control group for female individuals (P >0.05), while it was significant higher in plaque group than that in control group for male individuals (P <0.05).③EATV was significant higher in plaque group than that in control group for the individuals with age< 50 years (P <0.05 ),meanwhile it was significant higher in plaque group than that in control group in age≥50 years(P <0.05).Conclusion EATV is correlated with coronary plaques in male patients with a coronary artery calcium score of zero while there is no correlation with female patients.EATV is correlated with coronary plaques in different age patients with a coronary artery calcium score of zero.
