1.Probing the timing of treatment of neonatal dacryocystitis

Objective To find earliest or the best time for children with neonatal dacryocystitis acceptable treatment of lacrimal to shorten the course and improve treatment Methods Different time points after birth accepted probing treatment efficacy in children with neonatal dacryocystitis were retrospectively analyzed. Results Probing technique was effective for newborn infant with dacryocystitis. Symptoms recovered in 47 eyes,and 26 eyes were significantly improved with only a simple washing,there are 33 eyes need to wash and rinse probe or the expansion of the sclera again,and it was more common in the disease course of 4 ~6 months. Once probing technique treatment in the group( within 3 days) were effective in 23/34(67. 7% ) ,more than it in the group(more than 4 days) with effect of 24/72(33. 3%), One probing technique treatment in the group(within 10 days) were effect of 12/53(22.6%). Conclusion Press and simple washing techniques ineffective in children with neonatal dacryocystitis, was depended on operator proficiency, and time of lacrimal passage sounding could really advance the treatment to patients within 10 days after birth.
2.A Comparison of Laparoscopic-assisted pull-through with Open Approach for Hirschsprung's disease in infants and children

Objective To compare the therapeutic results of laparoscopic-assisted endorectal colon pull-through with radical Swenson's technique for Hirschsprung's disease Methods 39 children who had undergone Swenson pull-through were retrospectively reviewed.The patients were divided into 2 groups:open Swenson(n=20),laparoscopic procdeures(n=19). Use of laparoscopic equipment,operating time,hospital day,intraoperative blood loss,cost of hospital and postoperative complications were chosen as itews for comparisiom. Results The patienst in open Swenson passed stool and flatus in (78?28) hours and those in laparoscopic procedures did in (27?7) hours(t=2.87, P0.05),no major complications were found Conclusions The therapeutic results of laparoscopic-assisted coloc pull-through are similar to those of open and this procedure is economical.It appears to reduce postperative intestinal adhesion and postoperative recovery time dramatically, has smaller incision and renders itself more acceptable by the parents.
3.Self-choice right among patients visiting psychology department of TCM hospital

ObjectiveTo study the self-choice right of the patients who visit psychology department in TCM hospitals and provide a reference for further TCM psychological treatment application.Methods Self-choice and visiting condition inquiry enacted by ourselves was used in the survey.It is an unregistered and on-sight investigation.The inquiries were distributed by doctor.Patients were instructed by their doctors in the process.Results64.35% of patients in psychology department of TCM hospitals select treatment methods by themselves.TCM is the first choice among 66.96% of patients in psychology department of TCM hospitals.44.35% of patients visiting psychology department refuse western medicine due to their side effects.6.09% of patients in psychology department of TCM hospital know a little about psychological treatment; however,some of them believe in psychological treatment.ConclusionThe research finds out the main methods of self-choice right among patients visiting psychology department of TCM hospital.It proposes that self-choice right should be respected,at the same time,doctors are responsible to guide patients to understand the etiology of diseases.These efforts will contribute to harmony relationship between doctors and patients and facilitate their recovery and rehabilitation.
4. Recent updates on drug-resistance mechanisms in Mycobacterium tuberculosis and the role of PE/PPE proteins in drug resistance

Tuberculosis (TB), an infectious disease caused by Mycobacterium tuberculosis (M. tb) and mainly spread by airborne transmission, remains a global health problem. After the wide clinical utilization of antibiotics in treatment of TB, the drug resistance has become a major threat to global TB control and this threat has been heightened by the emergence and propagation of multi-drug resistant tuberculosis (MDR-TB), extensively drug-resistant tuberculosis (XDR-TB) and totally drug-resistant tuberculosis (TDR-TB). The research in the mode-of-action mechanisms of anti-TB drugs and the drug resistance mechanisms of M. tb will facilitate the novel anti-TB drugs development, the optimization of current anti-TB drugs and the development of new drug resistance diagnosis technologies. This review provides a comprehensive overview of mode-of-action mechanisms of new anti-TB drugs and mechanisms of resistance to these drugs in M. tb, with particular attention to the role of newly-identified mycobacterial outer-membrane channel proteins (PE/PPE proteins) in TB drug resistance.
5.Expression of survivin protein and its relation with the expression of Bcl-2 and Bax in the trophoblasts of human normal placenta

Objective To study the expression of Survivin protein and its relation with the expression of Bcl-2, Bax in the tropho- blasts of human normal placenta. Methods The normal placental tissues (8 -9week ,18-23 week and 37-40week) were fixed, embed- ded, sectioned, and Survivin, Bcl-2, and Bax in the trophoblasts were detected with immunnohistochemistry. Result As the gestational age advanced , the staining intensity of Survivin in the trophoblasts was significantly decreased from the first trimester group to the second trimes- ter group to the term group (PU value: 11.74±0.8,9.95±0.43,8. 83 ~ O. 67, respectively, P <0.01 ), while the staining intensity of Bcl-2 and Bax in trophoblast was significantly increased (PU value of Bcl-2 : 4.33±0.60, 5.00±0.75,6.87±0.45, respectively, P<0.01 and PU value of Bax: 9.82±1.12,16.00±1.05,27.48±2.10, respectively, P <0.01 ). Expression of Survivin in trophoblasts has no rela- tionship with the expression of Bcl-2 and Bax (P>0.05 ). Conclusion Survivin may take part in the development of human normal pla- centa through the way of suppressing the apoptesis in trophoblasts. Expression of Survivin in trophoblasts has no relationship with the expres- sion of Bcl-2 and Bax, which indicate that they regulate apoptesis of trophoblasts via different biological pathways.
6.Characteristics of magnetic resonance diffusion weighted imaging in rabbit models of cerebral ischemia-reperfusion injury established by using thread blocking method

BACKGROUND: A middle cerebral artery occlusion and reperfusion(MCAO/R) model in rats with suture has been widely used in the researches of acute focal ischemic cerebral infarction, while the model in rabbits by the same method is relatively rare. Magnetic resonance diffusion weighted imaging (MR DWI) has been paid close attention recently for its sharp sensitivity of cerebral ischemia.OBJECTIVE: To establish rabbit models of MCAO/R by intraluminal thread, and study the characteristics of MR DWI after cerebral ischemia and reperfusion.DESIGN: Random controlled animal experiment.SETTING: Institute of Cerebrovascular Diseases, Affiliated Hospital of Qingdao University Medical College.MATERIALS: The experiment was accomplished at the Key Laboratory of Brain Diseases Prevention and Cure of Shandong Province from March to June in 2005. A total of 103 adult healthy New Zealand rabbits of either sex, 10-12 weeks old and 1.8-3.3 kg weight were provided by the Experimental Animal Center of Shandong Agricultural Academy (SCX20040013).They were bred at quiet, sanitary and dry conditions.METHODS: Animal groups: 103 rabbits were divided randomly into group A (n=53) and group B (n=50). The rabbits in group A were treated with suture of 0.51-0.55 mm as the diameter of thread, while group B was reassigned into B1 (0.46-0.50 mm), B2 (0.51-0.55 mm) and B3 (0.56-0.60 mm).The successful MCAO/R models in 57 cases were randomly divided into permanent ischemia group (n=30, ischemia 1, 3, 6, 12, 24 andl 48 hours, 5ones at each time point) and ischemic reperfusion group (n=27, reperfusion 0, 2 and 5 hours, 5 ones at each time point; reperfusion 11, 23 and 47hours, 4 ones at each time point). Another 10 rabbits receiving sham operations were regarded as contrasts for permanent ischemia group and ischemia reperfusion group, with 5 ones in each.MAIN OUTCOME MEASURES: The changes of hyperintensity area on DWI and apparent diffusion coefficient (ADC) were measured in permanent ischemia group and ischemic reperfusion group.RESULTS: The data of 57 successful model rabbits were involved in the result analysis.①The successful rate in group A (26 cases, 49.1%) was significantly lower than that in group B (31 cases, 62.0%).②In ischemia group:The hyperintensity area on DWI with declined ADC appeared at ischemia 1 hour. The hyperintensity areas on DWI at different times increased gradually from ischemia 1 hour and unchanged within 24 hours. The mean ADC at different times declined at first and then gradually increased.③In reperfusion group: Comparing with ischemia 1 hour, the hyperintensity area on DWI reduced while ADC increased at reperfusion 2 hours and 5 hours, and enlarged with ADC high at reperfusion 11 hours, then continued to enlarge with ADC reduced significantly at 23 hours and 47 hours.CONCLUSION: The diameter of thread tip and the inserting distance of thread are main factors for establishing successful MCAO/R models. The hyperintensity area on DWI and the decreasing ADC after acute cerebral ischemia can be improved by early reperfusion, but the secondary decreasing ADC may be induced by continuously reperfusion.

9.In vitro study of folate receptor-targeted and paclitaxel-loaded ultrasound contrast agent

Objective To prepare the folate receptor-targeted and paclitaxel-loaded ultrasound contrast agent (folate-poly(lactide-co-glycolide)-paclitaxel,FOL-PLGA-PTX) and to investigate its targeting and imaging performance in vitro.Methods Paclitaxel-loaded PLGA-COOH micmcapsules with a core of liquid perfluorocarbon (PLGA-PTX) were prepared using single emulsion technique and then conjugated with folate by carbodiimide method.The size,surface potential,entrapment efficiency and drug loading efficiency were measured by Malvern laser detector and HPLC.The connectivity condition of PLGA-PTX with folate and the binding rate of fluorescent antibody were detected by immunofluorescence staining and flow cytometry.The targeting performance of FOL-PLGA-PTX was checked after co-incubated with human SKOV3cell lines in vitro and compared with that of non-targeted group and free folic acid intervention group.In vitro experiments were performed to explore the effects of FOL-PLGA-PTX on the enhancement of ultrasound imaging after irradiation by high intensity focused ultrasound (HIFU).Two-sample t test and one-way analysis of variance were used to analyze data.Results The average diameter of FOL-PLGA-PTX was (244.43 ±13.32) nm,with the drug entrapment efficiency of (86.23 ± 1.23)% and loading amount of (8.62±0.12)%.The binding rate of folate was as high as (98.49± 1.28)%.The connection rate of FOL-PLGAPTX on SKOV3 cells was higher than that of non-targeted group ((84.32±4.25) % vs (16.45±2.89) %; F289.45,t=10.654,P<0.01) and the free folic acid intervention group ((36.33±3.23)%; t=8.923,P<0.01).During in vitro ultrasound imaging,the average grey scale of FOL-PLGA-PTX before HIFU irradiation was significantly lower than that after HIFU irradiation (39.32±3.64 vs 126.44±7.15 ; t =4.829,P<0.01).Conclusion FOL-PLGA-PTX has been prepared successfully,with high entrapment efficiency and much drug loading,which can target to SKOV3 cells specifically and effectively in vitro,and enhance the ultrasound imaging greatly after HIFU irradiation.
10.Using the PDCA cycle to improve the rehabilitation of postoperative patients with anal fistula

Objective To evaluate the PDCA cycle on the rehabilitation of postoperative patients with anal fistula, aiming at improving the recovery of patients and preventing recurrence. Methods 144 postoperative patients of anal fistula were randomly divided into the observation group and the control group with 72 cases in each group. The observation group received intervention based upon the PDCA cy-cle, and the control group was given the traditional oral propaganda. The recovery time, recurrence rate, mastering of education content and self-control ability were compared in the two groups using t test and χ2 test. Results The mastering level of education content and the self-management skills in the observation group were superior to those of the control group. After 3 to 24 months of follow-up, the observation group showed shorter recovery time and less recurrence rate than that of the control group. Conclusions Nurs-ing intervention based upon PDCA cycle can strengthen the patients' mastering of related knowledge, pro-mote the recovery, reduce recurrence rate and improve the patients' quality of life.
