1.Analysis of Nerve Excitability Test and Stapedial Reflex on the Prognosis of Facial Nerve Paralysis

Objective To evaluate the clinical application of nerve excitability test and stapedial reflex on the prognosis of facial nerve paralysis. Methods The threshold of excitability of the branches of facial nerves in both sides and stapedial reflex were tested in 50 patients.Results 34 patients out of 42 with differences of nerve excited threshold less than 3.5 mA showed complete recovery (81%),while only 2 patients out of 8 with the differences of nerve excited threshokd more than 3.5 mA showed recovery (25%), 32 patients out of 36 with positive response of stapedial reflex showed recovery (88.9%), but only 3 patients out of 14 recovered (21.4%) in non-response group.Conclusion The difference of nerve excited threshold of both sides less than 3.5 mA and positive response of stapedial refles showed a better prognosis, suggesting no severe injury to facial nerve and both nerve excitability test and stapedial reflex were useful chinical parameters to predict the prognosis of facial paralysis.

Epigenetics is a branch of conventional genetics and is an emerging research field in biomedicine.Epigenetic factors regulate gene function and phenotype by the modulation of DNA methylation,histone modification,non-coding RNA such as microRNA expression instead of changing the DNA sequence.Therefore,it is thought to be a discipline associated with life science.Epigenetics is opening new means for understanding ocular development and ocular diseases related to aging and environment,and relevant study is increasingly emphasized because epigenetic inheritance is considered to be a reversible process.Like other ocular tissues,the growth,differentiation,senescence and epithelial-mesenchymal transition (EMT) of lens epithelial cells (LECs) are modulated by epigenetic inheritance.Epigenetic inheritance participates in the regulation of gene phenotype,chromatin structure,non-coding RNAs,and cellular differentiation and potential influent in lens diseases such as cataracts.Therefore it may provide new insight of the pathogenesis of cataracts and development of novel treatment options by vigorously carrying lens research on epigenetics.Domestic ophthalmologists should keep watchful eye to these studies.
5.Effects of Vitamin D on growth and immune function in neonatal mice

Objective To study the effects of Vitamin D (VitD) supplementation on growth and immune function in neonatal mice.Methods A total of 120 mice were divided into four groups (30 mice in each group) according to dose of VitD.The high-dose group,medium-dose group and low-dose group was given 3.44,1.72,0.86 IU VitD drops,respectively.The control group was not treated with VitD drops.Rat body weight,level of peripheral blood 25-(OH)D3 were observed.The cellular immune function (determined by using delayed hypersensitivity reaction experiment),humoral immune function (assessed by antibody producing cells counts and HC50 determination) and mononuclear-macrophage phagocytic function (assessed by mouse peritoneal macrophage phagocytosis of chicken red blood cells test and carbon clearance test) were detected.The flow cytometry assay was carried out to differentiate T lymphocyte subsets.Results With the increase of dose of VitD,levels of peripheral blood 25-(OH)D3 and calcium ion were gradually increased,there were statistically significant differences when compared with the control group(P<0.05);the body weight and body length were gradually increased,while no statistically significant difference was found among the groups treated with VitD(P>0.05).Compared to the control group,the toes swelling,phagocytic percentage,number of antibody producing cells,serum soluble HC50 in the high-dose group and medium-dose group were increased significantly,while carbon clearance test phagocytic index were decreased significantly (P<0.05).With the increase of dose of VitD,the number of CD4+,CD8+ T lymphocytes and the CD4+/CD8+ ratio were gradually increased.Conclusion VitD could promote the growth and development of offspring mice,and enhance the immune function of the body.
6.Application of PDCA cycles in the normal limb position of stroke patients with hemiplegia

Objective To explore the application effect of PDCA cycles on the normal limb position of stroke patients with hemiplegia. Methods The stroke patients with hemiplegia (128 cases) were selected as study subjects. Patients(62 cases)during January to September 2014 were set as the control group,and received routine nursing care. Patients(66 cases)during October 2014 to July 2015 were set as the experimental group,and used PDCA cycles management on the normal limb position additionally. The application effect of PDCA cycles on the normal limb position was evaluated through comparing two groups with qualification rates of normal limb position and incidence of complications. Results The qualification rates of normal limb position in the control group was 38.71%(24/62), which was higher than that of the control group, which was 75.76% (50/66) (χ2=16.504, P<0.01). The complications occurred in the control group were strephenopodia (11 cases), foot drop (16 cases), dislocation of shoulder (9 cases), omodynia (27 cases) and myospasm (34 cases), and they were 3 cases, 7 cases, 2 cases, 15 cases and 18 cases in the experimental group respectively. The incidence of complications was lower than those of the control group (χ2=4.001-8.961, P < 0.05), and the difference was statistically significant between two groups. Conclusions PDCA cycles management could improve the qualification rates of normal limb position and reduce the incidence of complications, which was beneficial to the recovery of limb function.
7.Clinical analysis of 74 cases of bloodstream infections caused by multidrug-resist-ant Acinetobacter

Objective To study the clinical characteristics,antimicrobial restistance of bloodstream infections (bacteremia) caused by multidrug-resistant Acinetobacter and analyze the outcomes of antibacterial therapy.Methods The clinical data were reviewed retrospectively for 74 patients with bloodstream infection caused by multidrug-resistant Acinetobacter who were trea-ted in HuaShan hospital from January 2005 to December 2011 .Results During the 6-year period,74 patients were diagnosed with multidrug-resistant Acinetobacter bacteremia,73 of which were nosocomial infections.The remaining one was community-acquired. Primary bloodstream infection accounted for 51 .4% (38/74),and secondary infection 48.6% (36/74), mainly secondary to pulmonary infections (23.0%,17/74). Solid tumor was the most common underlying disease (24.3%,18/74).Prior corticosteroid therapy,indwelling deep venous catheter,surgery and invasive procedures were predisposing factors of bacteremia. Acinetobacter-related bloodstream infections were associated with higher white blood cell count,increased neutrophil percentage,higher APACHE II score and lower serum albumin level.The bloodstream infection was caused by Acinetobacter baumannii in 65 pa-tients,Acinetobacter lwoffi in 7 patients,both Acinetobacter baumannii and Acinetobacter junii in one patient.The all-cause mortality rate was 27.0% (20/74).In vitro susceptibility testing showed that 20.0% (15/75 )of the Acinetobacter isolates were resistant to cefoperazone-sulbactam,which was the lowest among all the antibiotics tested.About 40.0% to 42.7% of the isolates were resistant to carbapenems.The outcome was related to the antimicrobial restistance.Carbapenem non-suscepti-ble Acinetobacter was associated with poorer outcome compared with carbapenem-susceptible Acinetobacter (mortality 46.9%vs 11 .9%,P <0.05 ).Cefoperazone-sulbactam non-susceptible Acinetobacter was also associated with poorer outcome com-pared with cefoperazone-sulbactam susceptible Acinetobacter (mortality 40.0% vs 18.2%,P <0.05).Of the 32 patients who had infections with carbapenem-non-susceptible Acinetobacter,20 received sulbactam-containing antimicrobial agent.The mor-tality of these 20 patients was 20.0% (4/20),significantly lower than that of the 12 patients who did not receive sulbactam-containing antimicrobial agent (66.7%).Conclusions Majority of the bloodstream infections caused by multidrug-resistant Acinetobacter are nosocomial infections.Surgical operation and serious condition may predispose the patients to develop Acine-tobacter bacteremia.Acinetobacter isolates are highly resistant to commonly used antibiotics.The Acinetobacter isolates not susceptible to carbapenem or cefoperazone-sulbactam are associated with poorer outcome and higher mortality.More attention should be paid to prevention and control of Acinetobacter-related nosocomial infections.

Objective To detect the intervened effect of the compound of Polygonum caspidatum to the NAFLD rat models and analyze its mechanism at the level of gene expression.Methods The Wistar rats were fed with high fatty chow to induce NAFLD and intervened with the compound of Polygonum caspidatum.The mRNA relative level of leptin,adiponectin,tumor necrosis factor ?,resistin and uncoupling protein 2 in the adipose tissue of the intervenient and control rats was detected with RT- qPCR method.The data differences between two groups were analyzed in t-test.Results Comparing to the control group,the relative level of leptin mRNA in the intervenient group was significantly increased(P0.05).At the same time,the resistin and UCP2 mRNA didn’t been detected in all samples of these two groups.Conclusion Metformin hydrychloride can adjust the adipose metabolism in liver and improve the steatosis and inflammatory reaction in liver cell.
