Background Ultra-long scan depth OCT can achieve imaging of full range of human ocular anterior segment. However, the measurement of the dimension of anterior segment from the OCT image with high speed and precision is a challenge at present. The software of automatic data processing is still lack in analyzing spectral domain OCT.Objective This study was to perform the automatic biometry and data processing of human ocular anterior segment OCT image by using self-developed automatic detection software and evaluate the accuracy and repeatability of this method. Methods Twenty eyes of 10 normal subjects were included in Eye Hospital of Wenzhou Medical University from June to July 2013.The OCT image of anterior eye segments were obtained with custom-made ultra-long scan depth OCT under the informed consent.Anautomatic software algorithm was developed for the biometric measurement on these OCT images, including boundary segmentation, image registration and optical correction of OCT images. The boundary segmentation algorithm utilized the axial gradient information of OCT images and the shortest path search principal based on the dynamic programming to optimize edge finding.Central corneal thickness (CCT),anterior chamber depth (ACD),pupil diameter (PD),lens thickness (LT),radius of lens anterior curvatures (LAC) and radius of lens of posterior curvatures (LPC) were automatically and manually measured, and the validity of automatic detection algorithm was assessed by calculating the intraclass correlation coefficient (ICC) between the automatic and manual measurements, and the repeatability was validated by calculating the coefficient of repeatability (COR) between repeated measurement. This study was approved by the Ethic Committee of Wenzhou Medical University and informed consent was obtained from all subjects. Results There were no significant differences in the results of CCT,ACD,PD,LT,LAC and LPC between the automatic and manual measurements (P =0.205,0.167,0.285,0.127,0.102,0.074).The results were consistent between automatic and manual measurements (all at ICC>0.75).The repeated measurement values were consistent in CCT,ACD and LT in both automatic and manual modes (all at ICC>0.75).The reproducibilities of automatic biometry in PD and LAC (ICC =0.793,0.872;COR =2.90,5.79) were better than those of manual mode (ICC =0.631,0.579;COR =5.62,10.46);while the reproducibility of automatic biometry in LPC (ICC =0.663;COR =6.17) was lower than that of manual mode (ICC =0.794,COR =4.79).Conclusions Self-developed automatic detection software appears to be accurate and repeatable in measuring dimension of spectral domain OCT images. This automatic software algorithm can be used for the biometry and monitor of human ocular anterior segment.

Objective To find the key miRNA that relative to peritoneal fibrosis associated with peritoneal dialysis (PD) by microarray technology, and verify its expression in vitro and in vivo. Methods The peritoneal fibrosis mouse model associated with PD were established by intraperitoneal injection of lipopolysaccharide (LPS)+ 4.25% peritoneal dialysate. The expression of miRNA was detected by microarray in peritoneal tissues. The expression of miRNA profiles between fibrotic and normal peritoneal tissues was compared. The differentially expressed miRNA (miR-200a) was validated by real-time PCR in lager sample size cohorts. The expressions of miR-200a were also detected in the epithelial-mesenchymal transition (EMT) process of human peritoneal mesothelium cells. Results In mice model of PD,peritoneal tissue was markedly thickened and with a massive extracellular matrix accumulation. In contrast with control, the expression level of epithelial marker E-cadherin was significantly decreased,α-SMA,Col-Ⅰ and FN were remarkably increased (P < 0.05).By miRNA microarray analysis,miR-200a was significantly down-regulated (3.31 folds change,P < 0.05) in fibrotic peritoneal tissues. The down-regulated expression level of miR-200a was also validated by realtime PCR in larger cohorts (P < 0.05).Then,the expression level of miR-200a was detected in the EMT process of human peritoneal mesothelium cells.During the process of TGF-β1 induced EMT,miR -200a was significantly down-regulated compared with the control (P < 0.05).Conclusions Downregulated expression of miR-200a was observed both during peritoneal fibrosis and TGF-β 1 induced EMT in vivo and in vitro, suggesting that miR-200a may be involved in the peritoneum fibrosis by regulating the target genes of EMT.

Objective To investigate risk factors associated with lymph node metastasis and prognosis of rectal neuroendocrine tumor (NET). Methods Clinicopathological data of 69 patients with rectal NET in our department from April 2003 to October 2011 were retrospectively analyzed. Associations of clinicopothological factors with lymph node metastasis and prognosis were examined using univariate and multivariate analysis. Results Of the 69 patients, 9 cases had lymph node metastasis. The lymph node metastasis was significantly associated with tumor size, T stage and G grade by univariate analysis. Multivariate analysis showed that T stage was the only risk factor associated with lymph node metastasis. The overall 5-year survival rate was 90.3%. Prognosis of rectal NET was significantly associated with tumor size, T stage, N stage, M stage, TNM stage and G grade by univariate analysis. Multivariate analysis showed that M stage was significantly associated with long-term survival in rectal NET patients (P=0.000, HR=2.285, 95%CI:1.484~3.518). There was no significant difference in patients with stage Ⅰ between local and radical resection, while there were significant differences in those with stage Ⅱ or higher between the two operations (P=0.046). Conclusion T stage is associated with lymph node metastasis and both TNM stage and M stage can affect the prognosis of patients with NET, which may be used as potential predictive factors for rectal NET. Local resection should be recommended for patients with stage Ⅰ and radical resection should be recommended for patients with stage Ⅱor higher.

Objective To evaluate the feasibility of confirming syphilis reactive blood donors. Methods The serum of donors with anti-TP reaction by ELISA were confirmed by treponema pallidum particle agglutination(TPPA)and Western blotting(WB).The results of two confirmation methods that were negative, suspicious or inconsistent were followed up and compared. At the same time, the analytical index values of the screening reagent A,B and C and their combinations were e-valuated and compared using the the receiver operating characteristic curve(ROC curve)based on the results of the two confirmation methods. Results The positive rate of 223 ELISA anti-TP reactive samples(including 124 double-reagent ELISA reactive samples and 99 single-reagent ELISA reactive samples)was 57.40%confirmed by TPPA and 38.57%con-firmed by WB(89.52%vs 17.17%by TPPA and 52.42%vs 21.21%by WB for double-reagent and single-reagent ELISA reactive samples). The confirmed negative rate of TPPA was 35.43%and that of WB was 42.60%(6.45%vs 71.72%of TP-PA and 29.84%vs 58.59%of WB for double-reagent and single-reagent ELISA reactive samples).According to Kappa test, the confirmed results between the two methods were not consistent, especially for those single-regent ELISA reactive sam-ples.Thirty six cases were followed up successfully,of which 17(47.22%)confirmed changes in the test results but the changes were irregular.Based on the confirmed results of TPPA and WB,the ROC curve analysis was performed on the anti-TP screening S/CO values of double-reagent ELISA reactive samples. When combining ELISA screening reagents as A/B and A/C,the optimal S/CO values of reagent A were 1.815,5.73 and 10.205,16.165,respectively.Conclusion TPPA and WB have poor consistency in the confirmation of ELISA anti-TP reactive blood samples, and the outcome of follow-up confirmation is unclear. The S/CO threshold of ROC curve is affected by the combination of confirmatory screening reagents, and it is difficult to confirm the results of ELISA anti-TP reactive blood donors.
