1.The application of testing DQE in upgrading of DR based on flat panel detector

Objective:To discuss the test methods and the application value of detective quantum efficiency (DQE) in upgrading of DR based on flat panel detector(FPD).Methods:Combining the dose of incident X-rays with the image brought by flat panel detector. According to the international electrotechnical commission IEC 62220-1 standard method, the tungsten imaging,and combined with the relationship between the FPD and incident X-ray air kerma, using DQEPro equipment and software to calculate the result of the three different types of FPD.Results: The experiment proves that the basic performance of flat panel detector is known definitely by testing the DQE. DQE can reflect the quality of imaging performance in different types of flat-panel detector.Conclusion: The upgrading of DR based on flat panel detector and the DQE testing can be a better technical basis for DR measurement performance, then it can be ensure the quality of DR upgraded in diagnostic imaging.
2.Clinical application of carbon nanoparticles labeled lymph node in cervical lymph node dissection with papillary thyroid cancer staged preoperatively as N0.

OBJECTIVE:
To investigate the clinical application of carbon nanoparticles staining in cervical lymph node dissection on clinical neck lymph nodes the negative (cN0 period) thyroid papillary thyroid carcinoma (PTC).
METHOD:
This retrospective analysis comprised 100 papillary thyroid cancer patients who met inclusion criteria,and they were randomly divided into the nano-carbon group (50 cases) and control group (50 cases). They underwent lobectomy, subtotal thyroidectomy or total thyroidectomies and were given elective central compartment neck dissection (CCND). The number of detected lymph nodes in each group was summed, and pathological examination was conducted. The number of lymph nodes (dyedor not dyed) and the location of metastatic nodes were recorded separately.
RESULT:
In the nano-carbon group the average number of eliminated lymph nodes is significantly more than that of the control group(P<0.05). The metastasis lymph nodes in the nano-carbon group was higher than that in the control group(P<0.01). Among 100 cases, 2% had temporary vocal cord palsy, and 8% had temporary hypoparat hyroidism. No case of permanent vocal cord palsy or permanent hypocalcemia was observed.
CONCLUSION
The dyed lymph nodes can be easily identified and can be used as a guide for lymphnodes dissection in papillary thyroid carcinoma operation.

4.Repeated inspection and data automatic analysis & processing for medical accelerator dosage monitoring system

The stability and accuracy of dosage monitoring system are crucial to guarantee the dosage output veracity and its curative effect,so it is essential to repeatedly inspect dosage monitoring system.But for the present inspection process,some practical problems exist such as comparative complex inspection operation,complicated and time-wasting data analysis and process,etc.This study not only makes some progress on the methods for index inspection and data analysis & process,but also designs a data analysis & process software based on delphi7.0,which makes the whole inspection process simpler and saves much time.
5.Evidence-based medicine analysis on curative effects of intravitreous injection with triamcinolone acetonide for macular edema

Objective To seek the evidence-based medicine (EBM) evidences of curative effects of intravitreous injection with triamcinolone acetonide (TA) for macular edema. Methods All articles of intravitreous injection TA for macular edema published in English or Chinese were picked up from databases of MEDLINE and CNKI and then evaluated according to EBM standard. The data in accord with research standard were selected by using excluding and including criteria, and classified according to the appraisal standard of clinical therapeutic documents. Results In the selected papers, none in gradeⅠevidence; 1 in gradeⅡevidence; 7 in grade Ⅲ evidence; 24 in grade Ⅳ evidence; and 19 in gradeⅤevidence. Forty-two papers reported that intravitreous injection with TA had significant effect for macular edema within 3 months, and the improvement of visual acuity was recorded in these papers. Regression of macular edema was recorded in 23 papers. Among 20 papers, side-effect was found in 93 eyes (31.41%) and the serious side-effect in 4 eyes (1.35%). Conclusions Intravitreous injection with TA has some curative effects for macular edema in short term, but the quality of current study has not been encouraging. There are no grade I document and lack of the study of validity in long term and essentiality and validity of retreatment. The special attention should be payed on the increasing persistency of efficacy and preventing the serious side-effects in the future investigation.
6.Development of a test phantom used for MRI’s quality control

Objective: Design a test phantom with a full function, simple structure and high accuracy which is used for MRI’s Quality Control. Methods: Through a intensive study in imaging principle of MRI and parameters which effect the image quality ,first come up with the phantom’s performance data, and then determine the technical solutions including the choosing material, processing technology, the machining precision, at last develop a trial set of MRI test phantom using the method of numerical control machining. Make a comparative experiment between this phantom and the similar imported one to analyze the consistency of the results by the two phantoms. Results: The result of all the technical parameters of this phantom is consistent with the result of the imported one. There are some extra test components which the imported phantom doesn’t contain. Some performance data like the low contrast resolution are superior to the imported phantom. Conclusion:This MRI test phantom can satisfy the requirement of the daily quality control work in hospital.
7.Diagnosis value of thyroid imaging reporting and data system in thyroid nodules.

OBJECTIVE:
To explore the diagnostic value of the proposed thyroid imaging reporting and data system (TI-RADS) classification in thyroid nodules, and to investigate interobserver variability among different observers using TI-RADS classification for ultrasound.
METHOD:
We reviewed 667 thyroid nodules conventional ultrasound image data from 532 patients who were confirmed by Pathological diagnosis. Those ultrasound images were reviewed by 4 experienced sonographers who can independently assessed the sonographic characteristics and analyzed according to the TI-RADS classification. It is to assess the diagnostic sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of the TI-RADS classification. The multirater kappa statistics were used to assess the interobserver agreement among different observers.
RESULT:
The overall sensitivity, specificity, negative predictive value (NPV) and Accuracy were 94%, 79%, 97% and 81%, respectively. Positive predictive values (PPVs) for categories 4 and 5 were 55% and 92%. There was substantial interobserver agreement for categories 3, 4B and 5 (Kappa = 0.62, 95% CI, 0.58-0.65), and was moderate agreement for category 4A (Kappa = 0.57) and 4B(Kappa = 0.60).
CONCLUSION
The diagnostic criteria of TI-RADS for differentiating between benign and malignant thyroid nodules have a high diagnostic value. There was substantial interobserver agreement in different experienced sonographers. The TI-RADS diagnostic criteria have a high diagnostic value and has great practical value in making a proper and further treatment plan.

9.Effects of Qingkailing on gene expression and free radical metabolism in mice with endotoxemia

Objective To investigate the protective effect of Qingkailing (QKL) in cardiac muscle's injury induced by endotoxin and discuss the mechanism from the inflammatory factors expression and free radical metabolism. Methods A total of 48 male ICR mice were divided into normal group, model group, low dose group and high dose group, 12 mice in each group. The QKL(9 ml/kg) was administered via gavage daily for 4 days in low dose group, the QKL (18 ml/kg) was administered via gavage daily for 4 days in high dose group, the equivalent volume of saline was administered via gavage daily for 4 days in normal group and model group. At the fifth day all groups except normal group, received intraperitoneal injection of LPS 0.2 ml (40 mg/kg), and the normal group received equivalent volume of saline. Intragastric administrated again 0.5h, 8h and 20 h after the model establishment. We took blood from hearts 1 hour after the last administration. The QR-PCR was used to detect the expression of inflammatory factors. The Elisa was used to detect IL-1β, IL-6, TNF-α, MDA, SOD and GSH-Px. Results Compared to the model group, the content of TNF-α (68.75 ± 7.73 pg/ml, 62.03 ± 16.09 pg/ml vs. 116.06 ± 21.06 pg/ml), IL-1β (110.84 ± 40.61 pg/ml, 105.51 ± 38.21 pg/ml vs. 167.53 ± 54.82 pg/ml) and IL-6 (68.78 ± 20.57 pg/ml, 59.71 ± 13.59 pg/ml vs. 108.80 ± 28.21 pg/ml) in low dose group and high dose group were significantly decreased (P<0.01). The expression of TNFα mRNA (1.42 ± 0.15, 1.30 ± 0.46 vs. 3.00 ± 0.82),IL-1β mRNA (1.20 ± 0.57, 1.01 ± 0.40 vs. 2.32 ± 1.39) and IL-6 mRNA (1.53 ± 1.10, 1.16 ± 1.09 vs. 4.12 ± 2.23) in low dose group and high dose group were significantly decreased (P<0.01 or P<0.05). The content of MDA (10.64 ± 2.91 nmol/mg, 11.36 ± 3.02 nmol/mg vs. 15.21 ± 2.31 nmol/mg) in low dose group and high dose group were significantly decreased (P<0.01). The content of SOD (282.32 ± 35.90 U/mg, 325.07 ± 34.76 U/mg vs. 249.01 ± 45.22 U/mg) and GSH-Px (48.26 ± 17.13 U/g, 49.66 ± 22.11 U/g vs. 26.47 ± 20.37 U/g) in low dose group and high dose group were significantly increased (P<0.01). Conclusions The QKL plays a protective role in myocardial injury induced by endotoxemia. Its mechanism may be associated with down-regulation of expression of inflammatory factors, reducing free radicals and improvement of antioxidation.
10.Treatment of talar cartilage injury and bone cyst with modified biplane Chevron osteotomy and autogenous osteochondral transplantation

Objective:To evaluate the clinical efficacy of the modified biplane Chevron osteotomy and autogenous osteochondral transplantation for the treatment of talar cartilage injury with bone cyst.Methods:From February 2016 to February 2019, 26 patients with talar cartilage injury and bone cyst were treated at Department of Orthopaedics, The First Affiliated Hospital to Guangxi Medical University. They were 16 males and 10 females, aged from 22 to 50 years (average, 36.2 years). According to the Hepple classification, there were 5 cases of type Ⅳ and 21 cases of type Ⅴ. The extent and range of talar cartilage injury were evaluated by arthroscopy, the modified biplane Chevron osteotomy of medial malleolus was performed to expose cartilage defects medial to the talus, unstable cartilage was removed thoroughly, sclerotic wall of the bone cyst was freshly treated, and an osteochondral column taken from the non-weight-bearing area of the ipsilateral femoral medial condyle was implanted into the injured area of talar cartilage. The clinical efficacy was evaluated by comparing the ankle-hindfoot scores of American Orthopedic Foot and Ankle Society (AOFAS), Karlsson ankle scores, visual analogue scale (VAS) and Lysholm ankle scores between preoperation and one year post-operation.Results:All the 26 patients were followed up for an average of 20.6 months (from 12 to 30 months). Follow-up did not observe any postoperative complications like incision infection, cyst recurrence or malunion, or any obvious pain or movement limitation at the donor knee joint. The AOFAS ankle-hindfoot scores were significantly increased from preoperative 64.3±3.9 to 89.5±5.1 one year postoperation, the Karlsson scores were significantly increased from preoperative 60.5±5.5 to 85.2±6.9 one year postoperation, and the VAS scores were significantly decreased from preoperative 6.2±1.1 to 1.8±0.9 one year post-operation (all P<0.05). The Lysholm ankle scores before and after operation were 94.7±1.9 and 94.1±1.8, respectively, showing no significant difference ( P>0.05). Conclusion:In the treatment of talar osteochondral injury and bone cyst, the modified biplane Chevron osteotomy of medial malleolus and autogenous transplantation of osteochondral column can effectively relieve ankle pain and improve ankle function, leading to satisfactory clinical efficacy.
