1.Relevance between relapse and course of treatment in genotype 1 chronic hepatitis C patients with slow virologic re-sponse

Objective To investigate the relevance between relapse and course of treatment in genotype 1 chronic hepatitis C (CHC)pa-tients with slow virologic response.Methods Totally 157 genotype 1 CHC inpatients or outpatients were selected from the Third People′s Hospital of Jiaozuo and the People′s Hospital of Jiaozuo from April 2010 to March 2013.The patients were treated with interferon α-1b in conjunction with ribavirin.At 6 months of the initial treatment,5 1 patients with slow virologic response were randomly divided into groups A (24 cases)and B (27 cases),which received continued treatment for another 6 and 12 months,respectively.A 1 -year follow-up of pa-tients was performed after withdrawal of treatment.Adverse reactions in patients receiving the antiviral therapy were assessed.Furthermore, group comparisons were performed on the clearance rate of hepatitis C virus (HCV)RNA and normalization rate of alanine aminotransferase (ALT)at withdrawal of treatment,as well as the CHC relapse rate and ALT normalization rate at 6 months and 1 year after withdrawal of treatment.Continuous data were compared using t test,and categorical data were compared usingχ2 test.Results The rate of adverse reac-tions in patients receiving antiviral therapy had no significant difference between groups A and B (P>0.05 ).At withdrawal of treatment, there were no significant differences between groups in terms of HCV RNA clearance rate (95.65%vs 92.59%)and ALT normalization rate (95.65% vs 88.89%)(χ2 =0.02 and 0.13,respectively,both P>0.05).At 6 months and 1 year after withdrawal of treatment,the CHC relapse rate in group B was significantly lower than that in group A (20.00% vs 50.00%,χ2 =4.69,P <0.05;36.00% vs 68.18%,χ2 =4.85,P<0.05);the ALT normalization rate in group B was nonsignificantly higher than that in group A (84.00% vs 59.09%,χ2 =3.63,P>0.05;72.00% vs 50.00%,χ2 =2.40,P>0.05).Conclusion To extend the 6 -month course of treatment can significantly reduce the relapse following slow virologic response in patients with genotype 1 CHC.
2.Comparative study on effect of radiofrequency ablation and operation for the treatment of small hepatocellular carcinoma

Objective To comparison between radiofrequency ablation introduced by tri-dimension reconstruction and ultrasound and operation for the treatment of small hepatocellular carcinoma. Methods 94 patients with small hepatocellular carcinoma from Jan 2009 to Mar 25 in Shanxi Cancer Hospital were analyzed. 45 patients were both given CT examination to rebuild tri-dimension and radiofrequency ablation treatment. 49 patients were given excision. Both groups were given CT examination to rebuild tri-dimension before the treatment. The differences in recurrence rate, overall survival and complication ratio between the two groups were compared. Results After 1, 2 and 3 years radiofrequency ablation treatment, the survival rate in radiofrequency ablation group were 95.56 % (43/45), 86.67 % (39/45), 60.00 % (27/45) respectively, comparing with 93.88%(46/49), 79.60%(39/49), 59.20%(29/49) in operation group. The difference was not statistically significant (P>0.05). There was no statistical significance in 1-year, 2-year and 3-year recurrence rates between the two groups (P> 0.05). There were statistical significances in incidence of post-treatment pains [13.33%(6/45) vs 100.00%(49/49),χ2=60.416, P<0.05] and complication (P<0.05) between the two groups. By Mar 25th, 2015, there was no needle tract implantation in the patients with radiofrequency ablation treatment. Conclusions For the small hepatocellular carcinoma with less than 3cm in diameter, introduced by CT tri-dimension image reconstruction and ultrasound, the radiofrequency ablation treatment excels in effect with fewer damages, infective complications, lower cost and can be applied many times. The overall effect is close to surgical removal, and therefore it can be used as the first line therapy for small hepatocellular carcinoma.

Objective:To investigate the expression of AQP4 during the development of presyrinx state of experimental syringomyelia in rabbits.Methods:The experimental syringomyelia models of rabbits were established by intra-cisternal injection of Kaolin.The expression of AQP4,AQP4mRNA and the water content of upper cervical spinal cord were measured with immunohistochemistry,Western blot,RT-PCR and dry-wet measurement on days 1,3,7,14,and 21 after operation,respectively.Results:Compared with animals of control group,the water content increased in those of Kaolin group from the 1st day(68.35%?0.70%),reached its peak on the 7th day(72.92%?0.86%),lasted to the 14th day(72.58%?0.55%),and then began to drop on the 21st day(70.03%?0.77%),while AQP-4 immunoreactive expression decreased on the 3st day[integral optical density(IOD)320.5?44.2],reached its minimum on the 7th day(IOD 258.7?26.5),lasted to the 14th day,and recovered partially on the 21th day approximately(IOD 321.5?46.1).RT-PCR found the decreasing of AQP4 mRNA coincided well with that of AQP4 immunoreactive expression in presyrinx state.The linear regression analysis indicated that expression of AQP4 and its mRNA in cervical cord had a negtive correlation with the change of spinal water content(r=-0.769,P
4.Quality Analysis of Cortex Moudan from Different Parts of Anhui Province

Objective To analyze the quality of Cortex Moudan which grew for four years from main planting parts in Anhui province.Methods According to Pharmacopoeias of People Republic of China(2005 edition),the water content,the total ash content,ash content of acid infusibility,the content of the thing soak out and paeonol content were measured to investigate the quality of Cortex Moudan.Results Paeonol content of Cortex Moudan collected from main planting parts in Anhui province was from 0.98% to 1.77%.Conclusion The quality of Cortex Moudan from Nanling is the best of all.
5.Study on testis gene chip of mouse model with syndrome of deficiency of kidney yang

Objective: To inquire into the testis gene change of mouse model with kidney-yang deficiency induced by overfatigue and excessive sexual life, the modern science mechanism of syndrome of kidney-yang deficiency and kidney controlling reproduction can be revealed on the gene level. Methods: The model of kidney-yang deficiency was established by each male mouse with six female mice keeping in the same cage, and all the male mice were forced to swim for 30-40 minutes everyday lasting for four weeks. The testis genes of mice in control group and model group were detected with Mouse OneArrayTM Whole Genome DNA microarray. The differentially expressed genes were screened under the condition of the relative fluorescence intensity ratio of the two groups ≥2 and ≤0.5 and further classified according to gene function by using Molecule Annotation System (MAS) created by CapitalBio Corp. Beijing, China. Results: The mouse model with kidney-yang deficiency was established successfully by the method of overfatigue and excessive sexual life. The scatter plot of gene expression profiles of comparing control group with model group was drawn. Differentially expressed genes were screened, including 2425 up-regulated genes and 3080 down-regulated genes. Among the first one hundred up-regulated genes, 41 genes were known and among the first one hundred down-regulated genes, 62 genes were known. These genes were mainly related to the cellular structure/function, material/energy metabolism, signal transduction/transmission, inflammation/immunization, cell apoptosis, transcription/translation,proliferation/differentiation and cell cycle. Conclusion: The testis gene of kidney-yang deficiency mouse had a wide change. The essence of kidney-yang deficiency syndrome and the theory of the kidney originating and controlling life reproduction were explained on the gene level.
6.Superoxide dismutase participates in p38 MAPK-mediated neuroprotection of limb ischemic preconditioning in global brain ischemic rats

AIM To explore the role of superoxide dismutase (SOD) in the p38 mitogen-activated protein kinase (MAPK) mediated brain ischemic tolerance induced by limb ischemic preconditioning (LIP). METHODS The Wistar rats with permanent occlusion of the bilateral vertebral arteries were subjected to occlude the bilateral femoral arteries for 10 min, 3 times, at an interval of 10 min to get the LIP, then global brain ischemia was induced immediately by occluding the bilateral common carotid arteries for 8 min. SB 203580 (100 μmol·L-1, in a volume of 25 μL), an inhibitor of p38 MAPK, was intraventricularly injected 30 min before LIP in SB 203580+LIP+brain ischemia group. Xanthinoxidase and thiomalonylurea methods were used to determine SOD activity and malondialdehyde (MDA) content of the hippocampus, respectively. Thionin staining was used for observing histological changes of the hippocampus. RESULTS LIP significantly prevented the decrease of SOD activity, the increase of MDA content and the delayed neuronal death in the CA1 hippocampus induced by the brain ischemia. SB 203580 pretreatment evidently blocked the protective effect of LIP against the delayed neuronal death and the modulation on SOD activity and MDA content. CONCLUSIONSOD may play an important role served as a downstream molecule of p38 MAPK in the induction of brain ischemic tolerance by LIP.
7.The Multi-imaging Diagnostic Values of Aortic Diverticulum With the Comparison of Clinical Application

Objective: To investigate the multi-imaging diagnostic values, especially MSCT technology in patients with congenital aortic diverticulum with its clinical application. Methods: The MSCT ifndings in 12 patients with congenital aortic diverticulum were retrospectively analyzed. Results: There were 9 patients with right aortic arch and 1 with left aortic arch, all of them having coexisted aberrant subclavian artery which initially dilated like aneurysm by diverticulum changing (Kommerell diverticulum), and there was 1 patient with incomplete double aortic arch with atresia of left arch combining retro-esophageal aortic diverticulum (RAD) and 1 patient with ducts diverticulum. Echocardiogram only made the suggestive diagnosis of speeding up blood lfow or right aortic arch in 4 patients. While MSCT accurately displayed the diverticulum for the location, morphology and with or without other complications. The post-eroanterior chest radiograph indicated “double aortic node” as the special sign in 8 patients. The echocardiogram, X-ray and MSCT for correctly diagnosing the aortic diverticulum were as 0, 72.7% and 100% respectively. Conclusion: MSCT is a rather ideal non-invasive diagnosing method for aortic diverticulum, meanwhile X-ray could also make suggestive diagnosis; if MSCT and X-ray joint with echocardiogram examination may provide the effective supplement for valve structure and hemodynamics condition in relevant patients.
8.Limb ischemic preconditioning attenuates neuronal apoptosis in CA1 hippocampus and brain edema evoked by brain ischemia via activating mitogen-activated protein kinase p38 in rats

AIM To observe whether limb ischemic preconditioning (LIP) could attenuate pyramidal neuronal apoptosis of the CA1 hippocampus and brain edema evoked by brain ischemia in rats. METHODSSeventy-two rats whose bilateral vertebral arteries occluded permanently were randomly assigned into 6 groups: sham, LIP(bilateral femoral arteries were clamped for 10 min, 3 times, in a 10-min interval), brain ischemic insult, LIP+brain ischemic insult, DMSO+LIP+brain ischemic insult and SB 203580+LIP+brain ischemic insult groups. Assays for neuronal apoptosis were performed using TUNEL staining. The percentage of wet over dry tissue weight of the brain was measured by weighing method. RESULTS There were almost no TUNEL-positive cells in the CA1 hippocampus in either sham or LIP group. Clear TUNEL-positive pyramidal neurons of the CA1 hippocampus and increase in brain water content were detected in rats subjected to brain ischemic insult. But the number of TUNEL-positive cells and the increase in brain water content were significantly decreased in LIP+brain ischemic insult group compared with that in brain ischemic insult group, indicated that LIP prevented the occurrence of apoptosis of pyramidal neurons of the CA1 hippocampus and brain edema induced by brain ischemic insult. Pretreatment with SB 203580, an inhibitor of mitogen activated protein kinase p38(p38 MAPK), significantly increased the number of TUNEL-positive cells and brain water in SB 203580+LIP+brain ischemic insult group compared with that in DMSO+LIP+brain ischemic insult group, indicated that SB 203580 blocked the protection of LIP against neuronal apoptosis in the CA1 hippocampus and brain edema. CONCLUSION LIP could attenuate pyramidal neurons apoptosis of the CA1 hippocampus and brain edema evoked by brain ischemia, which maybe related to the activation of p38 MAPK.

Objective To analyze the reason of hemoptysis after the bidirectional Glenn shunt procedure on complex congenital heart disease.Methods The feature of imaging data of 24 patients (1 6 males,8 females;age ranges:3-27 years;14 cases of single ventricle,3 cases of tricuspid Atresia,4 cases of pulmonary Atresia,3 cases of double Outlet Right Ventricle)after the bi-directional Glenn shunt procedure were retrospectively studied.Results Six patients had various degrees of hemoptysis (50 - 300 mL)after surgery,except for one case which has bronchiectasis on the left inferior lobe.The common feature of the other 5 cases present as plaque ground glass opacity and pulmonary arteriovenous fistula located at the corresponding subpleural field.After different therapy (three cases were performed endovascular management,2 cases were under expectant treatment),the lesion disappeared or obviously smaller which was clear evidence for the existence and bleeding of fistula.The occurrence of this disease in the present study was nearly 20.8%,which were accord with references.Conclusion Pulmonary arteriovenous fistula should be considered when crypto-genic hemoptysis happened after bi-directional Glenn shunt with complex congenital heart disease which exclude tuberculosis,bron-chiectasis or rupture and bleeding of MAPCAs.Multiple sliced CT angiography can be used to as the first line examination and sup-ply acute evidence for clinic therapy in time.
