Objective To explore the influencing factor of duration of untreated psychosis(DUP) and the relationship between duration of untreated psychosis and treatment outcome in epidemiological first-episode schizophrenia. Methods 912 medical records of the first episode schizophrenia patients were recruited in the study for epidemiological survey . The general medical data, clinical diagnosis, and treatment outcome were collected. DUP was determined refer to symptom onset for schizophrenia (SOS) scale. Patients were divided into short DUP and long DUP groups according to the median values of DUP values. Results Between short DUP and long DUP groups, the difference in marriage was significant ( P < 0. 05). The unmarried patients in the short DUP group (63.7% ) was higher than the long DUP group (52.6% ), the variance was significant (λ2 =5.990, P<0.05). The divorce in the short DUP group (0.6% ) was lower than the long DUP group (4.4% ) , the variance was significant (λ2 =5.079, P<0.01).The simple schizophrenia in the short DUP group (1.5%) was lower than the long DUP group (4. 1% ) , the variance was significant (λ2 =5.868, P<0.05). The treatment outcomes between the two groups had a significant variance ( λ2 =36.093, P < 0.01) , and the Ridit analysis of treatment outcomes between the two groups showed significant difference ( u = 5.183, P<0.01). The marriage, age of onset and clinical diagnosis were significantly correlated with DUP in multiple factor Logistic regression analysis. Conclusion The marriage, age of onset and clinical diagnosis are significantly correlated with DUP in the first episode schizophrenia patients, the longer of DUP and the worse of treatment outcome.

Objective To investigate the protective effect of Qingkailing (QKL) in cardiac muscle's injury induced by endotoxin and discuss the mechanism from the inflammatory factors expression and free radical metabolism. Methods A total of 48 male ICR mice were divided into normal group, model group, low dose group and high dose group, 12 mice in each group. The QKL(9 ml/kg) was administered via gavage daily for 4 days in low dose group, the QKL (18 ml/kg) was administered via gavage daily for 4 days in high dose group, the equivalent volume of saline was administered via gavage daily for 4 days in normal group and model group. At the fifth day all groups except normal group, received intraperitoneal injection of LPS 0.2 ml (40 mg/kg), and the normal group received equivalent volume of saline. Intragastric administrated again 0.5h, 8h and 20 h after the model establishment. We took blood from hearts 1 hour after the last administration. The QR-PCR was used to detect the expression of inflammatory factors. The Elisa was used to detect IL-1β, IL-6, TNF-α, MDA, SOD and GSH-Px. Results Compared to the model group, the content of TNF-α (68.75 ± 7.73 pg/ml, 62.03 ± 16.09 pg/ml vs. 116.06 ± 21.06 pg/ml), IL-1β (110.84 ± 40.61 pg/ml, 105.51 ± 38.21 pg/ml vs. 167.53 ± 54.82 pg/ml) and IL-6 (68.78 ± 20.57 pg/ml, 59.71 ± 13.59 pg/ml vs. 108.80 ± 28.21 pg/ml) in low dose group and high dose group were significantly decreased (P<0.01). The expression of TNFα mRNA (1.42 ± 0.15, 1.30 ± 0.46 vs. 3.00 ± 0.82),IL-1β mRNA (1.20 ± 0.57, 1.01 ± 0.40 vs. 2.32 ± 1.39) and IL-6 mRNA (1.53 ± 1.10, 1.16 ± 1.09 vs. 4.12 ± 2.23) in low dose group and high dose group were significantly decreased (P<0.01 or P<0.05). The content of MDA (10.64 ± 2.91 nmol/mg, 11.36 ± 3.02 nmol/mg vs. 15.21 ± 2.31 nmol/mg) in low dose group and high dose group were significantly decreased (P<0.01). The content of SOD (282.32 ± 35.90 U/mg, 325.07 ± 34.76 U/mg vs. 249.01 ± 45.22 U/mg) and GSH-Px (48.26 ± 17.13 U/g, 49.66 ± 22.11 U/g vs. 26.47 ± 20.37 U/g) in low dose group and high dose group were significantly increased (P<0.01). Conclusions The QKL plays a protective role in myocardial injury induced by endotoxemia. Its mechanism may be associated with down-regulation of expression of inflammatory factors, reducing free radicals and improvement of antioxidation.

According to the designed specific primers of gene fragment based on the Salvia miltiorrhiza transcriptome data, with the method of reverse transcription polymerase chain reaction (RT-PCR), this study cloned full-length cDNA sequence of 1-hydroxy-2-methyl-2-(E)-butenyl-4-diphosphate synthase gene from Salvia miltiorrhiza bge.f.alba, this sequence is named as SmHDS and its GenBank registration number is KJ746807. SmHDS, 2 529 bp long, contains an ORF of 2 229 bp, encodes 742 amino acids, including 5' UTR 170 bp and 3' UTR 130 bp. Using bioinformatics software, having made a homology analysis of the obtained sequence, we can have a conclusion that SmHDS have a close genetic relationship with HDS of Salvia miltiorrhiza. Analysis result of prokaryotic expression revealed that in Escherichia coli, SmHDS expressed target proteins which in size are comparable with the protein predicted. Meanwhile, the 4 factors which can influence the protein expression were optimized, the 4 factors are inducing temperature, inducing time, IPTG concentrations and density of inducing host bacterium (A600). The optimal expression conditions of SmHDS were 30 degrees C until the A600 is 0.6, and add IPTG to a final concentration of 0.2 mmol x L(-1), and the induction time of 20 h. It provides theoretical basis for the further study of the function of 1-hydroxy-2-methyl-2-(E)-butenyl-4-diphosphate synthase in the biosynthesis of tanshinone compounds.

The swollen root of Rehmannia glutinosa is used as one kind of important Chinese traditional medicine. The root of R. glutinosa usually swelled in rotational cropping but not in continuous cropping. The rhizosphere microorganisms of R. glutinosa under different farming condition were thought related to that. In this study, the endophytic fungi in the root of R. glutinosa growing in various soil conditions were isolated for the study of the relationship between the microorganisms and the root enlargement of their host plants. The dominant endophytes, Verticillium spp., Fusarium oxysporum, F. redolens and Ceratobasidium spp. were identified by morphological observation and 18S rDNA and ITS sequence analysis. The preliminary investigation showed that the excessive growth of Verticillium and Fusarium genus fungi is unfavorable for the R. glutinosa root swelling, but Ceratobasidium fungi has no effects on the root enlargement.

Objective To develop and validate a mortality risk prediction model for patients infected with avian influenza A H 7N9 virus. Methods A stratified and random sampling method was adopted for selection of subjects .A total of 102 patients infected with avian influenza A H7N9 virus, who were admitted to the designated hospitals in Zhejiang Province during March 2013 and March 2015, were enrolled.Standard questionnaires were used to collect data about demographic , epidemiologic and clinical characteristics , and the data were retrospectively reviewed . Univariate analysis and stepwise logistic regression analysis were used to identify the mortality risk factors of patients infected with avian influenza A H7N9 virus, and nomogram was applied to develop the risk prediction model .The accuracy of the prediction model was assessed using Concordance index (C-index) and receiver operating characteristic (ROC) curve. Results Stepwise multiple logistic regression analysis showed that age ≥60 years (χ2 =3.98, OR=2.99, 95%CI:1.05-9.21, P<0.05), increased initial neutrophil count (χ2 =6.66,OR=5.06, 95%CI:1.56-18.83, P<0.05), C-reactive protein≥120mg/L (χ2 =8.63, OR=5.15, 95%CI:1.79-16.31, P<0. 01), poor hand hygiene (χ2 =6.83, OR =10.29, 95%CI:2.18-81.49, P <0.01) and 5 days of incubation period or shorter (χ2 =7.23, OR=4.75, 95%CI:1.59-15.80, P<0.01) were independent risk factors for mortality of patients .Based on the above study , a risk prediction model of nomogram was developed.Poor hand hygiene (grade A, 100.0 points) ranked on the top of all risk factors, followed by C-reactive protein≥120 mg/L (grade B, 76.5 points), increased initial neutrophil count (grade C, 70.5 points), 5 days of incubation period or shorter (grade D, 62.0 points) and age ≥60 years (grade E, 51.0 points). The C-index and the area under the curve were 0.833 and 0.817 for the nomogram model , respectively;and the nomogram model fitted well .Conclusion Nomogram model can effectively predict and estimate the risk of death for patients infected with avian influenza A H 7N9 virus.

This paper firstly introduces the concept, method and current research of molecular phylogeography and then discusses its application in the study of geoherbs. The relativity between three genetic differentiation patterns of plant inferred by molecular phylogeography (i.e. allopatric fragmentation, restricted gene flow with isolation by distance and range expansion) and the formation of genuine character is analysed. Molecular authentication of geoherbs based on molecular phylogeography has the advantage of former molecular identification at technology and knowing genetic differentiation of geoherbs. Using molecular phylogeography for study on changing history of geoherbs habitat is also explicated. The problem of germplasm degeneration in cultural geoherbs could be effectively resolved by molecular phylogeography method. The application of molecular phylogeography in these subjects opens up prospects for study on geoherbs by using the principle and method of molecular phylogeography.

Chinese Journal of Hospital Administration 2018;34(9):705-709
Based on an analysis of the compensation policy and its problems of the primary medical and health institutions, this paper put forward the corresponding reform framework in Zhejiang province. It proposed to change the compensation policy of " predefining revenue and expenditure, subsidizing its gap after performance appraisal" into " a mixed system of special subsidy and pay for performance". Related policies were also described including governmental functions, special subsidies for input-based payments, classified payments for output services, precautions against financial risks, etc. This paper also suggested that we focus on dealing with the six pairs of balance, such as that between subsidy for the supply and demand sides, and that between internal market and external market.
