1.Advances on preventing femoral neck shortening after internal fixation of femoral neck fracture

Femoral head necrosis and nonunion is a common complication after surgery of femoral neck fracture.Recently,femoral neck shortening have caused attentions which can impact the function of hip.Incidence,mearsurement,influence factor,prevention and treatment of femoral neck shortening have been retrospective reviewed.Amount of femoral neck shortening with sliding compression system is significantly more than length of stability system,which is result from its own mechanism.However,effcacy of length of stability system still need a large scale of sample.It uses radiography to diagnose shortening.The present measurement methods include displacement of femoral head,xyz-axis measurement,the length of exposed screw,the collapse of femoral head.However,position,the angle of screen and the defect of the method would increase deviation.A measurement with simple,wide application and accuracy need to continue to explore.Apart from patients' condition,shortening also affected by the pattern and quality of surgery.Overall,older women,displacement fracture,unsatisfactory reduction will increase the risk and shrinkage of shortening.In addition,the pattern of the internal fixation becomes the research focus in the field of orthopedic clinical research.Including the transformation of sliding compression system device and design of novel device.How to combine them together organically could be a future development direction.Meanwhile,there are still in exploratory stage to decide whether biological combined with internal fixation can prevent shortening.Intertrochanteric osteotomy can achieve union and correct the limblength discrepancy,However,it did not restore the length of femoral neck.So,it is unknown that whether the method can solve the slight shortening which have impacted the hip function.
2.Internal quality control method for routine biochemistry analysis

Objective To study the valid correct method for reagent selection, calibration of analyzers, and internal quality control protocol. Methods Reagents were selected by comparing with BRENGER Mannheim(BM)reagent used in Hitachi 7170A. Analyzers were calibrated once a week. One internal quality control chart was set up for three analyzers. Specimens were analyzed by the three machines respectively. Results Except for amylase, all the results obtained from the three analyzers were not significant. Conclusion The recommended quality control protocol is useful to obtain comparable results from different automatic biochemistry analyzers.
3.Zero-profile interboby fixation system for multi-level cervical spondylotic myelopathy

BACKGROUND: Anterior cervical decompression and fusion has been widely used in the treatment of multi-level cervical spondylotic myelopathy, but accompanied with lots of complications.OBJECTIVE: To investigate the efficacy of zero-profile interboby fixation system for multi-level cervical spondylotic myelopathy.METHODS: Seventy-one patients with multi-level cervical spondylotic myelopathy were randomly divided into two groups, and the patients in group A accepted zero-profile interboby fixation system, and group B accepted cage interboby fixation system. The Japanese Orthopaedic Association score, fusion rate, as well as the incidence of dysphagia and esophageal fistula were detected to compare the efficacy between two groups.RESULTS AND CONCLUSION: (1) All cases were followed-up for 3-34 months, average of 17.5 months. The excellent and good rate at the last follow-up showed no significant difference between two groups. (2) The final fusion rate did not differ significantly between two groups, but the fusion rate in the group A was significantly higher than that in the group B at 6 and 9 months postoperatively (P < 0.05). (3) There was one patient with mild dysphagia in the group A (3%), three mild, five medium, and two severe dysphagia in the group B (29%), which showed significant difference between two groups (P < 0.05). No internal fixation loosening occurred in the group A, but three cases in the group B. The blood loss, operation time and radiology times in the group A were significantly lower than those in the group B (P < 0.05). (4) These results suggest that the effect of these two surgical methods in promoting functional recovery of spinal cord and final fusion rate show no significant differences; however, the zero-profile interboby fixation system exhibits better postoperative stability and interim fusion rate, with lower incidence of dysphagia.
4.The treatment of cystic lymphangioma with ultrasound-guided intralesion injection of Bleomycin

The inhibitors of programmed death 1 (PD-1)/PD-1 ligand (PD-L1) become a highlight in tumor immunotherapy and provide a new direction for immune-targeted therapy for cancer. More and more studies have confirmed that PD-1/PD-L1 inhibitors could improve effects and be well-tolerated. Early clinical trials of PD-1 inhibitors have shown significant clinical efficacy in various solid tumours. The unique role of the PD-1 pathway in lymphoma has been found gradually. This article summarized the advancement of PD-1 in B-cell lymphoma therapy.
6.Surgical management for avulsion fracture of calcaneal tuberosity

Objective To discuss the operative methods and cnrative effect of calcaneal tuberosity fracture.Methods The data of 15 patients with calcaneal tuberosity fracture was retrospectively analyzed who received treatment between January 2008 and June 2011.There were 9 males and 6 female,with the age ranging from 31 to 68 years (average,51.4 years).All the patients had unilateral acute injury,with the left foot in 7 cases and the right foot in 8 cases.According to the Beavis classification,there were 3 cases in type Ⅰ and 12 cases in type Ⅱ.All the cases in type Ⅰ and 10 cases in type Ⅱ were treated with open reduction and screw fixation.The other 2 cases in type Ⅱ with larger fragment involving a portion of the subtalar joint were treated with plate and screw fixation.The effect of the treatment was assessed according to the ankle and hindfoot score system of American Orthopaedic Foot and Ankle Society (AOFAS) after the operation.Results Ten patients were followed up for 12 to 36 months (average,20 months).The healing time in these patients ranged from 8 to 25 weeks (average,12 weeks).The postoperative score ranged from 47 to 100 points (average,91.1 points).There were 7 cases which received excellent effect,2 cases,good,and 1 case,poor.The rate of excellent and good was 90%.Necrosis of skin and soft tissue and exposure of the plate happened to one patient,who eventually healed after 3 weeks by debridement with plate preserved and peroneal artery perforator flap transplantation.Loss of reduction happened to another patient,who was treated with the revision surgery by open reduction and screw fixation again.Conclusion To patients with obvious Fracture displacement whose soft tissues are irritated severely.Emergency open reduction and internal fixation operation should be offered to prevent the necrosis of the flaps as far as possible.To patients with small fractures,it is advisable to choose open reduction and large diameter screw fixation,while plate and screw fixation may be better for patients with larger tuberosity fragment,especially when the fracture line extends to the facet of the subtalar joint.
7.Investigation of clinical value for MRI and arthroscope in diagnosis of an-terior cruciate ligament

Objective To investigate clinical value of MRI and arthroscope in diagnosis of anterior cruciate ligament, and to compare diagnosis accuracy and provide a reference for clinical diagnosis. Methods A total of 40 suspected pa-tients with anterior cruciate ligament were selected from July 2011 to July 2014, MRI were used in all patients before surgeries for diagnosing, and arthroscope was used during surgeries, results were compared between MRI and arthro-scope application. Results 10 cases of 44 suspected ACL knees were detected normal under arthroscope, the other 34 cases were detected injured, partial tear 11 cases while complete tear 23 cases;Under MRI 11 cases were detected nor-mal, the other 33 cases were detected injured, partial tear 13 cases while complete tear 20 cases. The sensitivity of MRI was 91.18%(31/34), the specificity rate was 72.73%(8/11), false positive rate was 4.55%(2/44), false negative rate was 6.82%(3/44), accuracy rate was 88.64% (39/44) (Kappa=0.762, P=0.001). Conclusion MRI is an accurate way to diagnose anterior cruciate ligament, particularly for absent ACL and complete tear, for partial tear ACL patients MRI should combine with other diagnosis to decrease false results.
8.Earlier assessing death toll after disastrous earthquake

Objective To explore a method for earlier evaluating death toll based on a function relationship ( an increasing hour-increasing death index ( K value) followed with time (T) changing after catastrophic earthquake. Methods Information data of 10 typical occurrences of catastrophic earthquake obtained from China International Search and Rescue Team (CISAR) were analyzed. Total deaths were estimated according to the simulation function made by hour-increasing death index (K value) followed with time (T) changing. Expected value of the simulation function was assessed by statistical software SPSS version 17.0 to establish the model of simulation function.Results The length of time (T) to reach K maximum (Kmmax) was ( 12.94 ± 8.18) h and then the K value was gradually decreased. Kmax was symmetrically scattered within 2 T time.Estimated death toll (W) within 2 T was obtained from calculating the integration summation of the function to get a formula as W =∫∞ kf(t) dt.This Estimated death toll(W) numbers was correlated approximately with the death toll (M) from authoritative report (P < 0.01 ).According to the regression analysis of model simulation curve,the predictive function of death toll within 2 T was M =W1.23 ×0.194.A determinant coefficient of this power function R2 was 0.88. Conclusions Earlier estimating death toll within about 12 h should be carried out by using the data of real-time information report system.

10.Fasting Blood Glucose, Cholesterol, and Risk of Primary Liver Cancer: The Kailuan Study

Purpose:
The influence of fasting blood glucose (FBG) and cholesterolemia primary liver cancer (PLC) in china was analyzed via a large prospective cohort study based on a community population, and the combined effects between them were investigated.
Materials and Methods:
Overall, 98,936 staff from the Kailuan Group who participated in and finished physical examinations between 2006 and 2007 were included in the cohort study. Their medical information was collected and they were followed up after examination. The correlations of serum FBG or TC with PLC were analyzed. Then, we categorized all staff into four groups: normal FBG/ non-hypocholesterolemia, normal FBG/hypocholesterolemia, elevated FBGon-hypocholesterolemia, elevated FBG/hypocholesterolemia and normal FBG/ non-hypocholesterolemia was used as a control group. The combined effects of elevated FBG and hypocholesterolemia with PLC were analyzed using the Age-scale Cox proportional hazard regression model.
Results:
During 1,134,843.68 person*years follow up, a total of 388 PLC cases occured. We found the elevated FBG and hypocholesterolemia increases the risk for PLC, respectively. Compared with the non-hypocholesterolemiaormal FBG group, the risk of PLC was significantly increased in the non-hypocholesterolemia/elevated FBG group (HR=1.19,95%CI 0.88–1.62) and hypocholesterolemiaormal FBG group (HR=1.53,95%CI 1.19–1.97), and in the hypocholesterolemia/elevated FBG group (HR=3.16 95%CI2.13-4.69). And, a significant interaction effect was found of FBG and TC on PLC. All results were independent from the influence of liver disease.
Conclusion
Elevated serum FBG and hypocholesterolemia are risk factors for PLC, especially when combined. Thus, for the prevention and treatment of PLC, serum FBG and TC levels should be investigated.
