Objective To study the influence of RNA interference Caspase-3 gene on apoptosis of transformed human embryonic kidney 293 cells induced by cadmium in vitro. Methods Transformed human embryonic kidney 293 cell were treated with siRNA for 36 h. The treated cells were incubated with 40 ?mol/L CdCl2 for 12-24 h and the cells viability were measured with methyl thiazolyl tetrazolium(MTT) assay. In addition,the expression of caspase-3 gene in 293 cells was detected by the methods of reverse transcription polymerase chain reaction (RT-PCR) and Western-blot analysis,and the occurrence of apoptosis was determined by flowcytometry with Annexin-V and propidium iodide (PI) double labeling method. Objective To analyze the mechanism of imipenem resistance in Pseudomonas aeruginosa clinical isolates. Methods? Antibiotic?resistance?was?analyzed?using?VITEK32?system.?Metallo?β-lactamase?activity?was?determined?by?double-disc?synergy?test.?Amp?C?β-lactamase?activity?was?determined?by?Kirby-Bauer?disc?method.?OprD?protein?was?analyzed?by?sodium?dodecyl sulphate-polyacrylamide gel electrophoresis. PCR was performed to amplify gene oprD. The amplified products were subject?to?sequencing?analysis.?The?phylogenetic?relationship?was?determined?using?random?amplified?polymorphic?DNA?(RAPD)?method. Results Membrane protein OprD was analyzed in 7 clinical isolates of imipenem-intermediate P. aeruginosa. Two strains were devoid of OprD proteins and the corresponding oprD genes were found disrupted by the insertion element ISRP10 in the coding regions. Five strains had OprD proteins with different sizes. Sequence analysis showed that the peptides ranged from 427 to 443 amino acids. Multiple amino acid substitutions and / or deletions were found within the Loop 1 through Loop 8 of the OprD secondary structures. Conclusions ISRP10 inactivation and amino acid substitutions in oprD gene confer imipenem resistance in the clinical isolates of P. aeruginosa.

Objective To investigate the protective effect of GSH on lung injury in rats with severe acute pancreatitis(SAP) and investigate the mechanism of severe acute pancreatitis associated lung injury. Methods Fifty-four Spragwe Dawley rats were randomly divided into three groups: control group, model(SAP)group and herbal group. Rats in model group and herbal group were induced pancreatitis by retrograde injection of 5% sodium taurocholate into the pancreatobiliary duct. Animals were killed at 3h, 6h, 12h after injection. The serum amylase was examined at 3h, 6h, 12h after injection. Pulmonary tissue fluid content was measured.The expression of pulmonary TNF-α mRNA and LIF mRNA was detected by semi-quantitative reverse transcription polymerase chain reaction (RT-PCR). Results The serum level of amylase was lowered significantly in herbal group in comparision with that in model group (P<0.05) and pulmonary tissue fluid content in herbal group was reduced compared with model group (P<0.05). Similarly, the level of pulmonary TNF-α mRNA and LIF mRNA expressions was both significantly reduced in herbal group compared with model group (P<0.05). LIF mRNA was not examined in normal control group. Conclusion Treatment with GSH can prevent lung injury of rats with SAP.

Objective To investigate the diagnosis and treatment of bilateral thyroid carcinoma(BTC).Methods Clinical data of 68 cases with BTC from July 2001 to July 2008 were analyzed retrospectively.Results All patients underwent surgical treatment.22 cases received total thyroidectomy and bilateral functional neck dissection,30 cases received total thyroidectomy and unilateral functional neck dissection,14 cases received total thyroidectomy and lymph node dissection of Ⅵ area,2 cases received palliative resection.12 cases received proper radioactive 131I therapy after operation.Postoperative complications occured in 16 cases,including 5 cases with transient recurrent nerve paralysis,1 case with permanent injury of recurrent laryngeal nerves received trachea incision,1 case with tracheomalacia and asphyxia,6 cases with convulsion caused by hypocalcemia,1 case with permanent hypoparathyroidism,2 cases with chylous fistula(one was cured by saturing and ligaturing and the other by conservative treatment).Low echo solid or cystic solid nodules in thyroid were found by preoperative ultrasonic scan in all cases,in which 38(55.88%)cases were associated with gravel calcified lesion in thyroid nodules.The diagnosis was comfirmed by needle aspiration biopsy(NAB)in 42 cases preoperatively and 64 cases(94.12%)by frozen section intraoperatively.63(92.64%)cases were followed up postoperatively for a period from 6 months to 5 years,all 5 cases with recurrence and/or metastasis had tumor-free survival after reoperation except for 1 case with undifferentiated cancer died on the 6th month after operation.Conclusions Total thyroidectomy and lymph node dissection is essential to the treatment of BTC.Gravel calcified lesion revealed by preoperative high frequency ultrasonagraphy is an important characteristic for preoperative diagnosis of thyroid cancer.

Objective:To investigate the principle and measures of prevention and treatment on pancreatic fistula due to pancreatic injury.Method:The clinical data of 131 pancreatic injury patients were analyzed retrospectively.Operation and combined therapy during perioperative were performed.Results:Among 35 cases(26.7%,35/131) with pancreatic fistula,3 cases shaped internal fistula showed by visualization and cured spontaneously after operations,9 cases with pancreatic pseudocyst after operations were treated by Roux-en-Y pancreatic cystojejunostomy,2 cases(5.7%,2/35) died of MODS and abdominal infections.The rest were recovered.The follow-up from 3 months to 5 years showed that there were no cases died of complications related to pancreatic fistula.Conclusions:Operation combined with multiple therapies was effective in the prevention and treatment of pancreatic fistula.

Objective To investigate the clinical application of pancreatic head resection with the second portion duodenectomy (PHRSPD). MethodsThe clinical data of 4 PHRSPD cases were analyzed retrospectively. ResultsThere were 3 benign and 1 low-grade malignant periampullary tumors. The mean surgical time was 417 min, mean blood transfusion was 533 ml, mean hospital stay was 28.6 days. Delayed gastric emptying was encounted in one case. There was no hospital morality or major surgical complications. All cases were alive in the follow-up ranging from 6 to 36 months. There was no newly developed diabetes mellitus or severe gastroenteral symptoms. No reccurrence was found in the case with low-grade malignant duodenal tumor after 26 months. ConclusionsPHRSPD is an efficient way to treat benign and low-grade periampullary neoplasm and accompanying massive bleeding episodes.

Objective To investigate the effect of platelet-activating factor acetylhydrolase(PAF-AH) on(severe) acute pancreatitis(SAP).Methods Forty-five male Wistar rats were randomly divided into three groups(n=15 in each group),including group A(sham operation group),group B(SAP group) and group C(PAF-AH treated group).PAF-AH(5mg/kg) was infused through the dorsal artery of penis after 1 hour and 24 hours of establishing the model in group C.Forty-eight hours after animal models were established,the levels of maleic dialdehyde(MDA),thromboxane B2(TXB_2),prostaglandin I2(PGI_2),TXB_2/PGI_2,(platelet)-activating factor(PAF),endotoxin,tumor necrosis factor(TNF-?) in serum,and the activity of(myeloperoxidase)(MPO) in lung tissue were examined in each group.The pancreas was scored on the basis of pathological changes.Results Compared with group B,the levels of MDA,TXB_2,TXB_2/PGI_2,PAF,(endotoxin),TNF-? in serum,and MPO activity in lung tissue and the pathological scores of pancreas were(significantly) lower in group C(P0.05).Conclusions PAF plays a critical role during the course of SAP.(Administration) of PAF-AH can hydrolyze PAF,improve pancreatic microcirculatory dysfunction,suppress(systemic) inflammatory reaction,relieve pancreatitis-associated intestinal tract injury and lung injury and(markedly) ameliorate the state of SAP.

Objective To evaluate the correlation between regional blood perfusion,metabolism and angiogenesis in breast cancer.Methods The PET/CT functional imaging technique was applied to quantitatively detect the central and marginal blood perfusion parameters including blood flow(BF),blood volume(BV)and permeability of surface(Ps),and metabolism index of standard uptake value(SUV)of the tumor in 33 breast cancer patients.The expression of CD31.CD105 and VEGF in paraffin section of breast cancer were detected by immunohistochemical method,then MVD(CD31)and MVD(CD105)were obtained.The relationship between the regional blood perfusion and metabolism and MVD(CD3 1),MVD(CD105)and the expression of VEGF were analyzed. Results There was significant correlation of MVD (CD31)with BF of marginal region(P<0.05).There were significant correlations of MVD(CD105)with BF.PS and SUV(P<0.05). Conclusions Regional blood perfusion,metabolism is correlated with angiogenesis in breast cancer tissue.PETT/CT regional blood perfusion and metabolic imaging iS a noninvasive method which can be used to estimate angiogenesis status clinically in breast cancer.
