2.Expression of p27, C-myc protein in lung squamous cell carcinoma and adenocarcinoma

Objective To investigate the clinical pathology characteristics of the p27 and C-myc protein expression in lung squamous cell carcinoma and adenecarcinoma as well as the correlation between the expressions of them. Method The expressions of p27 and C-mye protein in tumor tissue of 35 cases excised during pulmonectomy for lung squamous cell carcinoma and adenocarcinoma patients were examined using immunochemical method,and compared with 10 cases of normal lung tissue. Results The total positive rate of p27 protein was 17.1%(6/35 ) in 35 eases of tumor tissue, but 90.0%(9/10) in normal lung tissue, and there was significant difference (P< 0.05 ). Between high-moderate and low differentiated tissue,the positive rate had significant difference (P = 0.005). There was no significant difference of p27 protein expression in patients with different tumor size and histology types, as well as with or without lymph nodes metastasis (P > 0.05 ). The total positive rate of C-myc protein was 85.7% (30/35) in tumor tissue, but no expression existed in normal lung tissue (P < 0.05). There was significant difference of C-myc expressions between the tissue with and without lymph nodes metastasis (P =0.013),but there was no significant difference in various tumor size, differentiated degree, as well as histology type(P> 0.05). The expression of p27 and C-myc in lung squamous cell carcinoma and adenecareinoma showed negative correlation.Conclusions The expression of p27 protein is positive in normal lung tissue, but shows outstanding decrease in lung squamous cell carcinoma and adenocarcinoma tissue. Its expression correlates with the differentiated degree. The expression of C-myc is negative in normal lung tissue but shows significant increase in lung squamous cell carcinoma and adenocareinoma tissue and its expression correlates with lymph nodes metastasis. C-myc may be a valuable marker in evaluating prognosis.
3.Clinical application of alginate dressing: A comparison with traditional materials

OBJECTIVE: To explore the characteristics and clinical application of alginate dressing to provide better biological dressing for clinical wound healing. METHODS: A computer-based online search of CNKI, Medline, Wanfang, Science Direct, and Ei databases was performed for related articles published between January 1960 and October 2009, with the key words "alginate, dressing, clinical application" in Chinese and English. Studies regarding characteristics and clinical application of alginate dressing were included. Repetitive and Meta analysis were excluded. In addition, related books were manually searched. RESULTS: A total of 65 articles were collected, 17 were included, and 48 repetitive or Meta analyses were excluded. Alginate is natural polysaccharide carbohydrate extracted from sea tangle, and alginate dressing is soft non-textile fiber refined from seaweed. It contains 85% natural alginate fiber and 15% sodium tvIose. It can absorb effusion, form gelate, and exchange Na~+/Ca~(2+) with effusion. Alginate dressing has been widely used in bedsore treatment, anal fistula nursing, stoma nursing, and diabetic foot nursing. Compared with traditional dressing, infection rate of alginate dressing is lower due to good impermeability, promotion of regional vessel proliferation and blood supply, as well as moist and slightly acidic environment which benefits neutrophil function enhances disinfection. Moreover, it can be used in wound healing following abdominal region operation and abscess incision drainage.CONCLUSION: Alginate dressing provides appropriate environment for wound growth, retains active materials in diffusion, promotes active material release, benefit necrotic tissue solution and cell proliferation and differentiation, as well as epithelial cell migration. Moreover, it maintains hypoxia state of wound, stimulates newly generated capillary growth, and promotes granulation tissue growth.
4.Axillo-axillary bypass grafting for the treatment of contralateral arterial ischemia of upper limb

ObjectiveTo evaluate the efficacy of axillo-axillary bypass grafting for the treatment of contralateral arterial ischemia of upper limb. Method 53 cases underwent axillo-axillary artery PTFE graft bypass among those there were 46 cases of subclavian artery occlusion and 3 cases of subclavian artery aneurysm and 4 cases of trauma. Preoperatively 33 cases suffered from a symptom of extremity ischemia and 18 cases with subclavian artery blood steal. Result Good results were achieved in all cases without any complications. Doppler test showed the gradient of bilateral brachial pressure artery was less 10 mmHg in all cases. ConclusionAxillo-axillary artery PTFE graft bypass is an effective method for subclavian artery reconstruction with less trauma and complications. It is especially suitable for the elders with concomitant heart brain and lung diseases. It is also applicable for cases in which the subclavian arteries are not easily accessible due to artery aneurysm and artery trauma.
5.Recurrent arterial embolism of the extremities: report of 35 cases

ObjectiveTo improve the therapies for recurrent arterial embolism of the extremities. MethodsFrom Dec. 1984 to Dec. 1997, 35 patients of recurrent embolism of the extremities were treated,results were compared to that of 248 patients with first onset.ResultsThe recurrent cases accounted for 12.4% of the disease.One year recurrence developed in 63% of the 35 cases.32 cases suffered from second arterial embolism and 3 cases had third onset with a total of 42 limbs.The rates of cure,mortality and amputation were respectively 54.3%,22.9%, and 2.9% in the recurrent group,compared to 72.6%,10.1%,and 14.9% in the control group.There were statistically significant differences of cure,mortality and amputation rates between the two groups(? 2test, P
6.Diagnosis and treatment of acute peripheral artery injury

Objectives To study the diagnosis, treatment and prognosis of acute peripheral artery injury, and ways to improve the treatment. Methods Clinical data of 211 cases of acute peripheral arterial injuries admitted between January 1988 and June 2002 in Anzhen hospital were analyzed retrospectively. Of the 211 cases,73 cases were with blunt injuries, 99 penetrating injuries, 37 iatrogenic injuries, 2 gunshot injuries. Management procedures included arterial primary or patch repair in 28 cases, ligation alone in 17, end-to-end anastomosis in 41, interposition reverse saphenous vein graft in 37, interposition prosthetic graft in 29, thrombectomy in 14, excision of pseudoanurysm in 36, interventional thrombolytise in 2, ultrosonic-guided packing in 2, conservative management in 2. 7.Treatment of Takayasu's arteritis with thoracoabdominal aorta stenosis by percutaneous transluminal angioplasty(PTA)

Objective To summarize our experience in percutaneous transluminal angioplasty(PTA) in the management of thoracoabdominal aortic stenosis of Takayasu's arteritis. Methods From 1987~2001, 12 patients of Takayasu's arteritis with thoracoabdominal aortic stenosis underwent PTA. Aortography and pressure of stenotic segment of arota before and after angioplasty were used to evaluate efficacy. Color Doppler ultrasound examination and Doppler segmental pressure measurement were used in follow-up. Results Success was achieved in all cases without mortality and serious complications except for 1 case of acute thrombosis of iliofemoral artery. The residual stenosis were less than 50% in all cases and the pressure gradient was 5 0mmHg~10 5 mmHg with average pressure being 7 2 mmHg after angioplasty ,although they were 25mmHg~46 mmHg averaging of 37 7 mmHg before angioplasty. All cases were followed up for 9(12~72) months and the result was good without lumen restenosis and obstruction. Conclusions PTA is the first choice for Takayasu's arteritis of thoracoabdominal aortic stenosis of local size and proved to be safe, effectiveand economical. It is also effective for long size cases. It is not necessary to dilate aorta stenosis to normal size, and reducing pressure gradient effectively is enough.

10.An observation on preprocedural white blood cell count and long-term patency of stent implanted in subclavian artery

Objective To study relationship between preprocedural white blood cell count(WBC) and long-term patency of stent implanted in the subelavian artery.Methods Data of 113 cases with stents implanted in the atherosclerotic SUbclavian arteries in Anzhen Hospital,Beijing during January 2005 to January 2008 were analyzed.Student t-test was used to compare the difierence in WBC count between those with patency and those with stenosis after the procedure and chi-square test was used to compare the difference in stent pateney between those with higher and lower WBC counts.Results There Was significant difference in WBC counts between 102 cases with patency and 11 cases with stenosis of the stents after implantation(t=-5.051.P<0.001).Percentage of patency ofthe stents in 36 cases witll higher WBC count(>7.195×109/L)was significantly higher than that in 77 cases with lower WBC count(≤7.195× 109/L)before the procedure(P=0.001).Among 113 cases with stents implanted,109 cases(96.5%) could keep their stents patent one year after implantation,and 60 cases(89.6%)could keep stents patent three yeats after implantation.Conclusions In addition to those traditional factors associated with long-term patency of stents after their implantation,inereased WBC count alSO associated with stenosis or occlusion of the stents after the procedure.
