Objective To observe the effect of acupuncture, moxibustion combined with bladder function training on the neurogenic bladder.Methods 64 patients were randomly divided into the treatment group and control group,treatment group was given acupuncture,moxibustion combined with bladder function training,while the control group was treated with simple bladder function training, then comparing the bladder function changes between two groups after a month.Results In the treatment group,the bladder capacity,residual urine volume,urinate rate were respectively ( 349.4 ±13.5 ) mL, ( 98.7 ±15.3 ) mL, 92.26% and the control group respectively ( 323.4 ± 17.8)mL,(127.8 ±17.8) mL,51.85%,the difference between the two groups were statistically significant ( t =5.48,5.60,χ2 =3.97,all P<0.05).Conclusion Acupuncture,moxibustion combined with bladder function train-ing can be effective in the treatment of neurogenic bladder,while improve the quality of patient's life.
3.Influence of noninvasive ventilator in the treatment of plasma C-reactive protein , endothelin-1 and tumor necrosis factor-α in patients with obstructive sleep apneahypopnea syndrome

Objective To investigate the effect of noninvasive ventilator therapy on serum C-reactive protein (CRP), endothelin-1 (ET-1) and tumor necrosis factor-α (TNF-α) levels in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) and its clinical significance. Methods One hundred cases of moder-ate and severe OSAHS patients were selected by the method of parallel opening. All of the patients were given health education requirement , quitting smoking and wine , low fat diet and exercise to lose weight and other con-ventional treatment. The patients were randomly divided into the treatment group of 42 cases with noninvasive ventilator treatment , 44 cases treated with conventional treatment , to observe the changes of serum CRP , ET-1 and TNF-α levels and PSG parameters after 12 weeks in two groups. Results Apnea hypopnea index (AHI), oxygen desaturation index (ODI), the lowest oxygen saturation (LSpO2), and the average oxygen saturation MSpO2 in OSAHS patients were significantly improved after treatment (P < 0.01), but the degree of improvement in the two groups after treatment was significantly higher than the control group (P < 0.01). Plasma CRP, ET-1 and TNF-α levels in the two groups after treatment were lower than before treatment (P < 0.05 or P < 0.01), but the treatment group was significantly higher than that of the control group ( P < 0 . 05 or P < 0 . 01 ) . Conclusion Noninvasive ventilator therapy in improving the OSAHS monitoring data of patients with PSG can effectively reduce the serum CRP, TNF-α, ET-1 level, reduce the body′s inflammatory reaction.

5.The value of multi-direction adjusting multi-planar reformation in displaying the ossicles

Objective To study the positions and ways of displaying the panorama of the ossicles in one plane respectively by multi-direction adjusting multi-planar reformation (MPR). Methods Fifty normal middle ears were scanned by using the HRCT isotropic parameters, and multi-direction adjusting MPR was performed to find out the basic positions, the rotating central point of reformation basic line, and the anatomic structure orientated the adjusting directions. The angles between adjusting lines and reformation baselines were measured. The axial, coronal, sagittal and multi-adjusting MPR images were graded according to the display of the ossicles. Then Ridit analyses and X2 test were performed. Results The demonstrating ratios of the panoramas of the malleus, incus, and stapes in one plane in multi-adjusting MPR images were 100% (50 middle ear) . Objective To explore the dysfunction of dendritic cells (DC) related to TGFβ reversed after blocking the TGFβ signal pathway by recombinant adenovirus vector encoding for Smad7.Methods Smad7 by recombinant adenovirus vector was transfected into dendritic cells.Expression of immunologic phenotypes was detected by FCM,and CTL activity induced by DC was compared.Results The DC modified with Smad7 still expressed high adhesiveness factor related to maturation even if existing exogenous TGFβ1,which was significant statistically compared with DC transfected with control adenoviral vector (P <0.01).Even if existing exogenous TGFβ1,the DC modified with Smad7 pulsed with soluble antigen associated with Lewis pulmonary carcinoma could still induce potent CTL activity against Lewis pulmonary carcinoma,which showed significant difference with DC-Ad-c (P <0.01).Conclusion The inhibitory effects on function of DC of TGFβ may be reversed by blocking the Smad signal of TGFβ pathway.
7.Role of cAMP-PKA signal transduction pathway in the ischemic preconditioning-induced attenuation of ischemia-reperfusion injury In isolated rat hearts

Objective To investigate the role of cAMP-PKA signal transduction pathway in the ischemie pleeonditioning-induced attenuation of ischemia.1eperfusion(ITR)injury in isohted rat hearts.Methods Fifty healthy adult SD rats weighing 300-350 g were anesthetized with intraperitoneal(IP)pentobarbital 50 meg/kg.Their he.arts were excised and perfused in a Langendorff apparatus with 37℃ oxygenated(95%O2-5%CO2)modified K-H solution at a constant pressure of 70 cm H2O,and randomly divided into 5 groups(n=10 each):group Ⅰ I/R;group Ⅱ ischemic preconditioning(IPC);groupⅢH89(PKA inhibitor);group Ⅳ PDTC(NF-κB inhibitor)and group Ⅴ db-cAMP.The experiment started after 10 min stabilization.The isolated hearts were tinct perfuzed for 30 min.followed by 60 min ischemia and 30 min leperfusion in group I/R(Ⅰ).Group IPC(Ⅱ)w88 subjected to 3 episodes of 5 min ischemia at 5 min intervals before I/R.Group Ⅲ and Ⅳ received 5 min perfnsion withH89 10 μmol/L and PDTC 100 μmol/L 3 times at 5 min intervals respectively before I/R.Group Ⅴ was peffnsed with db-cAMP 200 μmol/L for 30 min before I/R.Left ventricular developed pressure(LVDP)and ± dp/dtu were measured at stabilization period and 10, 20, 30 rain of reperfusion. Coronary flow (CF) was measured at stabilization period and 30 min of reperfusion and activities of LDH and creafinc kinase (CK) in the coronary effluent were determined. The myocardial specimens were obtained at 30 rain of reperfusion for determination of NF-κB-DNA binding activity (by EMSA) and expression of TNF-κ mBNA (by RT-PCR ) and p-CBEB (Ser133) (by Western blot). Results Compared with 1/R group, NF-κB-DNA binding activity and TNF-α mRNA expression were significandy decreased, ± dp/dt and CF were significandy increased, CK and LHD activities in the coronary effluent were significantly decreased in group IPC and db-cAMP (group Ⅱ , Ⅴ ) and p-CREB (Ser133) expression was significantly increased in group IPC, PDTC and db-cAMP (group Ⅱ , Ⅳ,Ⅴ ). Compared with IPC group, NF-κB-DNA binding activity and TNF-α mBNA expression were significantly increased, ± dp/dtmax, LVDP and CF were significantly decreased, CK and LDH activities were significantly increased in group H89 and PDTC (group Ⅲ, Ⅳ ) and p-CREB (Ser133) expression was significantly decreased in group H89(group Ⅲ ). Conclusion lschemic preconditioning can attenuate I/R injury in isolated hearts by inhibition of NF-κB-DNA binding activity via cAMP-PKA signal transduction pathway which reduces gene transcription of inflammatory cytokine.

OBJECTIVE: Interventional occlusion of intracristal ventricular septal defects (IVSD) is a node point. Reports concerning angiography features and successful occlusion of IVSD using domestic occluder are rare. The aim of this paper is to investigate the angiography features of IVSD and the experience of interventional occlusion with domestic occluder. METHODS: Totally 46 cases of IVSD were diagnosed by color Doppler echocardiography that they had interventional indications.According to the short axis view of aortic, they were divided into 4 groups: A (11:30-12:00), B (12:00-12:30), C (12:30-13:00) and D (13:00-13:30). Different groups were performed with adequate angle angiography to show ventricular septal defect, as well as to decide strategy, which to choose occluder and occluded methods.RESULTS: All 46 patients were occluded successful. Technical success rate was 100%, without related complications. A group included 10 cases, B group 13 cases, C group 16 cases and D group 7 cases. The left anterior oblique angle of IVSD left ventricular angiography was greater than other type of VSD, and increased gradually with the changes in the defect location (Group A to D). According to the defect size and different groups, the defects were successfully occluded with symmetric,decentered, zere-decentered type occluder. Modified pigtail catheter was good value in practice occlusion of IVSD.CONCLUSIONS: Left anterior oblique angle in IVSD left ventricular angiography should be bigger. Domestic occluder is safe,effective, with less complication and cost, which can be the first choose for patients with LVSD.
10.Effects of oxygen free radicals and captopril on endogenous NOS inhibitor in human vascular endothelial cells

AIM: To observe the effects of oxygen free radical (OFR) and captopril on the level of asymmetric NG, NG-dimethyl-L-arginine (ADMA) in human vascular endothelial cells (HUVECs).METHODS: HUVECs of 3-6 th passage, cultured with modified Jaffes' method, were used in the experiment and divided into three groups: (1)Cells cultured with equivalence of DMEM medium as control; (2)OFR intervention groups, OFR at concentrations of 0 01 mmol/L, or 0 1 mmol/L, respectively, were added to the cell culture; (3)Drug intervention groups: the cell culture was treated with 0 1 mmol/L of OFR combined with 50 mg/L or 100 mg/L of captopril, respectively. Concentrations of ADMA, L-arginine, nitric oxide(NO), endothelin(ET) and the activity of angiotensin-converting enzyme(ACE) in conditioned medium were measured after 24 h exposure. RESULTS: Concentrations of ADMA, ET and the activity of ACE were increased, while the amount of NO decreased in OFR intervention groups compared with control group. After treatment with captopril, ADMA, ET concentrations and the activity of ACE were decreased, while the amount of NO increased, but the level of L-arginine had no obvious change. CONCLUSIONS: OFR induces endothelial dysfunction through increasing ADMA concentration, while captopril relieves endothelial dysfunction induced by ox-LDL through decreasing ADMA concentration.
