2.Pharmacokinetics and bioequiavailability of Glipizide in healthy volunteers

Objective To study the pharmacokinetics and bioequiavailability of Glipizide in healthy volunteers. Methods The plasma levels of Glipizide were determined at 30 min,1,2,3,4,5,6,8,10,12,14 h by HPLC after single-dose oral cross administration of 10 mg Glipizide tablets of two brands in 18 healthy male volunteers aged 23 to 25 years old,with the interval of one week. Results Glipizide tablets of the two brands were as follows,Tmax: (2.39?0.50),(2.33?0.49) h; Cmax: (675.3?151.7),(647.1?166.7)g/L; AUC_ 0-t : (3 565.4? 733.4) ,(3 304.8?588.4) ?g?h -1 ?L -1 . Conclusion The two preparations were bioequivalent.

Objective To investigate the functional mechanism of metformin in improving the fatty liver and the treatment of the relative metabolic abnormalities of type 2 diabetes mellitus.Methods The model of type 2 diabetes mellitus complicated with fatty liver was set up by feeding high-glucose-high-fat diet and by injecting STZ.Then,the rats were made an intervention with metformin.At the end of 4 weeks and 8 weeks,the fasting blood glucose,the level of fasting serum insulin.At the end of 18 weeks the fasting blood glucose,the level of fasting serum insulin,ALT,TNF-? and the level of leptin of every group of rats were measured and the expression of TNF-? in the liver was determined.Results Metformin could largely reduce the blood glucose,TG,TC and the fatty content of the liver,improve the state of the insulin resistance and reduce the expression level of the TNF-? in the liver.Conclusion Metformin can have the treatment functions to the rats with fatty liver in the model of type 2 diabetes mellitus complicated with fatty liver.
4.Postoperative radiotherapy of internal mammary nodes in breast cancer

Internal mammary nodes are part of the first stop of mammary gland lymphatic drainage.It plays an important role in the process of occurrence and development of breast cancer.Whether irradiation internal mammary nodes or not is still a larger controversy.At present,for the early stage patients,the efficacy of internal mammary lymph node irradiation is not clear.For the patients with positive axillary lymph nodes,inboard or central quadrant lesions,postoperative radiotherapy of internal mammary nodes has a certain value.
5.TLC scanning for the determination of phillyrin in Shuanghuanglian tablet

OBJECTIVE To develop an asssay for the quantitative determination of phillyrin in Shuanghuanglian tablet.METHODS TLCS method was selected to determine the content.RESULTS The linearity was obtained over the range of 0.31~ 1.55 μg(r=0.9996).The average recovery was 96.9% with RSD=1.49%.CONCLUSION The results showed that this method is sensitive,simple,specific and accurate for the determination of tetrahydropalmatine in Shuang Huanglian tablet.
6.Remote results of radiotherapy for primary orbital lymphoma

Objective To evaluate treatment effects and to determine the prognostic factors of primary orbital lymphoma. Methods From March 1984 to October 2000, 44 primary orbital lymphoma patients were admitted into our hospital, with 27 males and 17 females. Twenty-seven patients had eyelid or conjunctiva primary, and 17 patients orbit primary. Patient's age ranged from 14 to 86 years (median, 50.5years). According to the Ann Arbor Staging System, there were 40 ⅠEA, 4 ⅡEA lesions. Cobolt-60 ?ray was given first to the whole orbit to 30-35?Gy followed by 180 kv X-rays or 9 Mev ?-beam to the focus to a total dose of 41.0-51.5?Gy (median, 46.5?Gy). Kaplan-Meier method was used to analyze the survival. Log-rank test was used to detect the difference between groups.Results Only 1 patient failed from primary site and 5 patients from distant metastasis after radiotherapy. The 5-year and 10-year cause-specific survival rate were 90.9% and 84.5%, respectively. On univariate and multivariate analyses, eyelid or conjunctiva localization and Ann Arbor stage ⅠEA were independent favorable prognostic factors for survival. The 5-year survival was much higher in eyelid or conjunctiva patients than in orbital ones (96.3% vs 70.6%, P=0.013). The survival time was longer in stage Ⅰ than in stage Ⅱ (P=0.000). Neither the RT dose nor the age or sex influenced the outcome.Conclusions Localized orbital lymphoma can be well controlled with low-dose RT alone. Patients with eyelid or conjunctival localization and stage Ⅰ have better cause-specific survival.
7.Antibiotic Resistance in Meticillin-resistant Staphylococcus and Antibiotics Usage

OBJECTIVE To investigate antibiotic resistance and antibiotics usage of meticillin-resistant Staphylococcus(MRS) and provide reference evidence in antibiotics using.METHODS Antibiotic susceptibility test was performed by biological assay system of VITEK-2.Design questionnaires to get the message of the categories,days,effects of antibacterial drug used.RESULTS Drug sensitive ratio to vancomycin,nitrofurantoin,quinupristin-dalfopristin,linezolid were all over 90.00%.The resistant ratios to some antibacterials in MRS were higher than in meticillin-sensitive Staphylococcus(MSS).The categories of antibacterials used after Staphylococcus species detection were more than before.63.01% patients′ medication was determined by antibiotic susceptibility test.Staphylococcus isolated ratio was 43.75% after antibacterial drug used.CONCLUSIONS The status of MRS is very serious.We should pay more attention to the identification and report of resistant strains and it is important to supervise Staphylococcus efficiently and medication by antibiotic susceptibility test.
8.Survival analysis of 159 patients with hepatic encephalopathy in acute-on-chronic liver failure

Objective To analyze the prognostic factors for patients with hepatic encephalopathy (HE) in acute-on-chronic liver failure(ACLF).Methods A retrospective analysis was performed on 159 ACLF patients with HE.The hepatic encephalopathy was determined to baseline,the patients were divided into survivors(n =13) and nonsurvivors(n =146),The 32 factors affecting the prognosis were analyzed by Cox proportional hazard model with SPSS.Results One-month,three-month,and six-month survival rates were 20.13%,10.06% and 8.18%,respectively.Cox regression analysis of prognostic factors showed that it could the stage of hepatic encephalopathy and HRS significantly improve the survival rate of the patients with HE in acute-on-chronic liver failure.The stage of hepatic encephalopathy and HRS could significantly decrease the survival rate of the patients(x2 =18.344,11.368,all P < 0.05),elevated the levels of hepatic encephalopathy (relative risk (RR) =1.591) and HRS (RR =1.809) indicate worse prognosis with hepatic encephalopathy in acute-on-chronic failure.Conclusion The stage of hepatic encephalopathy and HRS were independent risk factor sof prognosis in acute-on-chronic liver failure.
9.Prevalence of toxoplasma, rubella virus, cytomegalovirus and herpes simplex virus infections among 3084 individuals with childbearing-age in Henan province

Objective To investigate the prevalence of toxoplasma gondii (Tox),rubella virus (RV),cytomegalovirus (CMV) and herpes simplex virus (HSV) infections (TORCH infections) among childbearing-age population in Henan province.Methods Enzyme-linked immunosorbent assay (ELISA) was applied to detect plasma TORCH IgM and IgG among 3084 childbearing-age men and women from theFirst Affiliated Hospital of Zhengzhou University during July and September,2011.The positive rates of anti-TORCH antibodies were compared among the various age and gender groups by x2 test.Results The total positive rate of anti-TORCH IgM was 5.5% (170/3084),in which the positive rate of anti-RV IgM was the highest (2.9%),followed by anti-HSV IgM (1.0%).Within positive rate of anti-TORCH IgG,anti-HSV IgG was the highest (90.4%),followed by anti-CMV IgG (89.7%),RV IgG (48.1%) and Tox IgG (0.7%).The positive rate of anti-TORCH IgM was the lowest in individuals aged > 30-40 year old.With the age increasing,the positive rates of anti-Tox IgG,anti-CMV IgG and anti-HSV IgG increased,but the positive rate of anti-RV IgG decreased.Women had higher positive rates of anti-CMV IgG and antiHSV IgG than men (x2 =83.470 and 7.026,P < 0.O1).Conclusions Current infection of TORCH exists in childbearing-age population of Henan province,and the positive rate of anti-RV IgG is low.It is recommended to screen for TORCH infection in childbearing-age men and women.
10.Clinical characteristics and diagnostic analysis of thyrotropinoma

Objective To analyze clinical charaCteristics and diagnostic experience on thyrotropinoma (TSHoma).Methods Clinical characteristics,laboratory findings,diagnostic experience,and surgical outcome were summarized from 16 cases of TSHoma in our hospital from January 2006 to September 2011.Results ( 1 )Among 16 cases ( 9 male,7 female ) aged ( 51.63 ± 13.23 ) years,14 ( 87.5 % ) cases presented with hyperthyroidism and 2 were diagnosed by physical examination. None of them had exophthalmos, acropachy, or pre-tibial myxedema.( 2 ) Serum FT4 and FT3 levels were all above normal range with serum TSH ( 4.37 ± 2.77 ) μIU/ml.There were 25% (4/16) cases with positive serum thyroglobulin antibody and/or thyroid peroxidase antibody,none with positive TSH receptor antibody.TRH stimulating test was performed in 9 cases,3 were with positive response ( peak TSH level increased by 5.45-9.20 μIU/ml compared with baseline),and 6 without response (peak TSH level increased by -0.01-3.15 μIU/ml compared with baseline).TSH was suppressed to 27.15% (5.19%-99.15 % ) of the baseline in 11 cases in which somatostatin suppression test was carried out.Prolactin and insulin-like growth factor-Ⅰ levels were increased in 2 cases.MRI performed in 15 patients showed 10 cases of microadenomas and 5 cases of pituitary adenoma.A mass in nasal cavity was found in one case,where no mass was found in the pituitary.( 3 )Surgery was made in 11 cases ( 9 patients underwent transsphenoidal operation,one craniotomy,and one underwent operation via endoscopic nasal septum ). All patients (11/11) were proved to yield pituitary adenoma pathologically.Conclusion Thyrotropinoma should be considered in hyperthyroidism with unsuppressed TSH level,and dynamic tests facilitated early diagnosis.Ectopic thyrotropinoma shoull be considered when normal pituitary morphology was shown by MRI.
