Three-Layer thought is an important proposition in Chinese traditional philosophy. This thought embodies the Chinese people's cosmology and methodology and exerts a far-reaching influence on various aspects of Chinese culture. The embodiment of Three-Layer thought in the theory and practice of acupuncture and moxibustion from naming of acupoints, principles of treatment, needling instruments, prescription of acupoints as well as needling techniques is elaborated and briefly analyzed. Thus it illustrates the comprehensive application of Three-Layer thought in acupuncture and moxibustion through the history and the significance of Chinese traditional philosophy in the science of acupuncture and moxibustion.

4.Application of SEHNFU (ginseng-aconite) injection on post-operation of liver transplantation

Objective To study the therapeutic action of SHENFU (ginseng-aconite) injection on post-operation of liver transplantation. Methods From Jan. 2004 to Mar. 2005, 30 cases underwent orthotopic liver allotransplantation were divided into two groups randomly. The patients in control group received routine transfusion, while the patients in SHENFU injection group, besides the routine transfusion, received SHENFU injection as well. Results 120 minutes after the SHENFU treatment, the mean arterial pressure (MAP) of the patients was raised from 53.21?8.52mmHg to 75.62?7.58mmHg, meanwhile the heart rate (HR) declined from 115.3?13.2/min to 90.1?7.2/min. At the same time, the contents of lactic acid, IL-6, IL-8 and TNF-? in the patients also declined from 9.82?3.96mmol/L, 89.02?43.75pg/ml, 58.24?33.61 pg/ml and 42.33?28.15pg/ml to 4.31?2.11mmol/L, 30.08?25.12pg/ml, 29.83?20.05pg/ml and 24.52?15.61pg/ml, respectively. Objective To explore the etiological factors of primary pathological duodenogastric reflux (DGR) through investigating the relationship between severity of bile refulx,the changes of surface gastric electric rhythm and gastric emptying movement in primary pathological DGR patients.Methods From January 2007 to April 2008 in Qingdao Municipal Hospital,58 cases of outpatients diagnosed as primary pathological DGR and 21 healthy individuals (control group) were collected and underwent 24-hour gastric bilirubin monitoring,gastric endoscopy,gastric electric rhythm,and gastric emptying test.The relationship between gastric electric parameters and gastric emptying,bilirubin reflux,Hp infection was analyzed.Results 1.The main frequency in fasting and postprandial of primary pathological DGR patients [(1.94±0.04) cpm vs (2.93±0.07) cpm; (2.12±0.03) cpm vs (3.35 ±0.05) cpm],the percentage of normal gastric slow wave in fasting and postprandial (74.46± 0.56 vs 85.55 ± 1.06 ; 63.97 ± 0.64 vs 86.13 ± 1.49),and fasting/postprandial power ratio (PR) (1.68±0.02 vs 2.75±0.09) were all lower than those of control group (P<0.05).The percentage of bradygastria in fasting and postprandial of DGR patients (18.04±0.36 vs 7.76±0.78;23.73±0.91 vs 8.47±0.55),the percentage of tachygastria in fasting and postprandial (8.93±0.26 vs 5.75±0.66;13.02±0.40 vs 7.66±0.27) were higher than that of control group (P<0.05).2.The main frequency of severe reflux patients in fasting and postprandial [( 1.68 ± 0.07) cpm vs (2.13 ± 0.07)cpm; (2.18±0.09) cpm vs (2.76±0.06) cpm],the percentage of normal gastric slow wave in fasting and postprandial (69.71±0.43 vs 80.35±0.68; 56.36 ±0.85 vs 72.34±0.80),fasting /postprandial PR (1.47±0.04 vs 2.02±0.04) were lower than those of mild-reflux group (P<0.05).The percentage of bradygastria in fasting and postprandial of severe reflux patients (22.94 ± 0.68 vs 13.47 ± 0.61; 29.61 ± 1.14 vs 17.55 ± 0.51) and the percentage of tachygastria in fasting and postprandial (9.94 ± 0.54 vs 7.02 ± 0.42 ; 17.04 ± 0.70 vs 10.71 ± 0.20) were higher than that of mild-reflux group (P<0.05).3.There was no significant difference of gastric electrical parameters in fasting and postprandial between Hp-positive and Hp-negative groups (P>0.05).4.The ratio of gastric emptying in DGR group was significantly lower than that of control group (37.9% vs 90.5 %,P<0.05).The gastric emptying delay in DGR group significantly increased compared with control group (60.3% vs 9.5%,P<0.05).There was no significant difference in gastric emptying delay between severe-reflux group and light-reflux group (69.0% vs 51.7%,P > 0.05).Conclusions There is dysfuntion of gastric myoelectrical activity and gastric motility in primary pathological DGR patients,which may be an important mechanism in pathological DGR.
6.Diagnostic value of porphobilin staining of gastric mucus for primary pathological duodenogastric reflux

Objective To study the diagnostic value of porphobilin staining of gastric mucus for primary pathologic duodenogastric reflux (DGR). Methods A total of 58 DGR patients diagnosed from January, 2007 to April, 2008 were recruited to the study as DGR group, and 21 healthy volunteers as control.All subjects underwent 24-hour intragastric bilirubin monitor and gastroscopy. Bilirubin absorption value of 0. 25 and median reflux time of 23.60% were taken as thresholds to differentiate low reflux group ( reflux time < 23.60% ) and high reflux group (reflux time ≥23.60% ). Porphobilin staining of gastric mucosa was quantitatively analyzed. Results Deposition of porphobilin in mucosa of gastric antrum, gastric angle and gastric body in primary pathologi DGR group was significantly higher than those in healthy group (P <0. 05 ). The occurrence of atrophic and intestinal metaplasia of gastric antrum in high reflux group was significantly higher than that of low reflux group (P < 0. 05). Deposition of porphobilin in mucosa of gastric antrum, gastric angle and gastric body in high reflux group was significantly higher than that of low reflux group (P < 0. 05 ). The New Sydney system pathological scores of gastric antrum and angle of high reflux group was higher than that of low reflux group ( P < 0. 05 ). The deposition of porphobilin in mucosa of gastric antrum and gastric angle was positively correlated with New Sydney system pathological scores in primary pathological DGR group (r=0.59, P=0.041 andr=0.73, P=0.038). Conclusion Porphobilin staining of mucosa in gastric antrum can reflect the severity of bile reflux, and is positively correlated with the extent of gastric mucosal lesion, which may be helpful in diagnosis of primary pathological DGR.
7.Effect and mechanism of polydatin on diabetic myocardial hypertrophy in mice.

To observe the preventive effect of polydatin on diabetic myocardial hypertrophy in mice and discuss its and mechanism. The diabetic model was induced with low dose STZ (40 mg x kg(-1) x d(-1) x 5 d, ip) for five days in mice. The myocardial hypertrophy was determined by hypertrophy indexes (LVHI, left ventricular/right ventricle and septum), left ventricular/body weight (LV/BW), the histological examination and the mRNA expression of atrial natriuretic factor(ANF). The fast blood glucose(FBG), serum insulin and plasma hemoglobin A1c ( HbA1c) levels were detected, and then HOMA insulin resistance index ( HOMA. IR) was calculated. The mRNA and protein expressions were measured by qRT-PCR and western blotting, respectively. According to the results, the FBG of the model group exceeded 11.1 mmol x L(-1), with notable decrease in BW and significant increase in insulin, HbA1c and HOME. IR, suggesting the successful establishment and stability of the diabetic model. The increases in LVHI, LV/BW, cell surface and ANF mRNA indicated a myocardial hypertrophy in diabetic mice. Meanwhile, the model group showed decrease in mRNA and protein expressions of PPARβ and significant increase in NF-κB p65, COX-2 and iNOS expressions. After the preventation with PD (50, 100 mg x kg(-1) x d(-1)), diabetic mice showed increase in BW, reduction in the levels of FBG, insulin and HbA1 c, relief in insulin resistance and significant recovery in hypertrophy indexes, indicating PD has the protective effect in diabetic myocardial hypertrophy. Meanwhile, PD up-regulated the expression of PPARβ, inhibited the expressions of NF-κB p65, COX-2 and iNOS, demonstrating that PD's protective effect may be related to the activation of PPARβ and the inhibition of NF-κB, COX-2 and iNOS signaling pathways.

8.Effect of the chosen incision on corneal astigmatism after implantable collamer lens surgery

AIM: To evaluate the effect of the chosen incision on corneal astigmatism after implantable collamer lens ( ICL) surgery.
METHODS:The study included 195 eyes of 102 patients, and all eyes were randomly divided into two groups: the chosen incision group ( Group A, 97 eyes) and temporal corneal incision ( Group B, 98 eyes ) . Before the operation, and 1wk, 1mo and 3mo after the operation, each patient was examined with corneal topography to observe the changes of corneal astigmatism.
RESULTS: Preoperative corneal astigmatisms were (1-26±0. 35) D in group A and (1. 28±0. 38) D in group B, thus there was no statistically significant difference ( P>0-05). One week postoperatively, the astigmatism were (0.93±0.29)D in group A and (1.32±0.33)D in group B. One month postoperatively, the astigmatism were (0. 85±0.16)D in group A and (1.27±0.18)D in group B. Three months postoperatively, the astigmatism were ( 0. 80±0-13)D in group A and (1. 25±0. 20) D in group B. The differences between the two groups were statistically significant (P<0. 01).
CONCLUSION: The chosen incision can reduce postoperative astigmatism to a certain extent after ICL surgery.
9.The expression of resistin in adipose tissues of patients with polycystic ovary syndrome and insulin resistance.

The relationship between the expression of resistin in polycystic ovary syndrome (PCOS) and insulin resistance was investigated. The plasma resistin concentrations in 35 patients with PCOS and 40 controls were measured by ELISA. Luteinizing hormone (LH), follicle-stimulating hormone (FSH), and fasting insulin (FIN) were tested by radioimmunoassay. Insulin resistance index (HOMA-IR) was calculated. Fasting plasma glucose (FPG) was determined by oxidase test. Western blot and reverse transcriptase PCR (RT-PCR) methods were used to detect the expression of resistin in adipose tissues. The levels of plasma resistin, LH, LH/FSH and FIN and HOMA-IR in patients with PCOS were significantly higher than those in control group (all P<0.05). Plasma resistin was correlated positively with FPG, FIN, HOMA-IR, LH and LH/FSH (r=0.56, 0.60, 0.65, 0.48, and 0.42 respectively). Resistin protein and mRNA expression levels in patients with PCOS were significantly higher than those in normal tissues (all P<0.01). It was concluded that resistin might be involved in the pathogenesis of insulin resistance of PCOS.
10.Risk factors influencing the prognosis of critical ill patients with ARDS

ObjectiveTo observe the risk factors influencing the prognosis of critical ill patients with acute respiratory distress syndrome (ARDS).MethodsThe clinical data of 67 critical ill patients with ARDS from 2003 to 2004 were reviewed retrospectively.ResultsThe acute physiology and chronic health evaluation (APACHEⅡ) scores and age were all differed significantly between survivors and non-survivors groups (P<0.05). The earlier of respiratory supported, the lowest of mortality (P<0.05).Conclusion APACHEⅡ scores, age and the time of starting respiratory support are main risk factors for mortality of critical ill patients with ARDS.
