1.Autophagy and obesity

[Summary] Autophagy is an intracellular degradation process by which the damaged organelles and macromolecules are lysosomal dependently degraded by auto‐phagocyte under the control of autophagy‐related genes. The autophagy level in hypothalamus and adipose tissue changes in obese individuals.Autophagy participates in the regulation of food intake and energy balance ,and associates with adipocyte differentiation and adipogenesis. More and more attention has been paid to the relationship between autophagy and obesity w hich may guide the new research direction of pathogenesis study and therapy of obesity.
2.Clinical Study of Guben Xiaoliu Capsule Combined with FOLFOX4 Regimen in Treating Advanced Colorect Cancer

Objective To observe effect of Guben Xiaoliu capsule combined with FOLFOX4 regimen in treating advanced colorect cancer patients. Methods 78 advanced colorect cancer patients were randomly assigned to treatment group (38 patients) and control group (40 patients). Oxaliplatin 85 mg/m2 iv infusion for 2 hours,d1. CF 200 mg/m2 iv infusion for 2 hours followed by 5-FU 400 mg/m2 iv infusion for 22 hours,d1-2. Every two weeks was a cycle. The control group was treated by FOLFOX4 regimen,while Guben Xiaoliu capsule was added in the treatment group. Patients were evaluated after 4 cycles. 3.Study on sleep disorders in patients with Parkinson' s disease by polysomnogram

Objective To observe the polysomnographic manifestation of sleep disorder and the characteristics of sleep architecture in patients with PD.Methods 42 patients with PD and 40 normal controls underwent night polysomnography.The parameters of sleep architecture and progress in two groups and video-monitoring features were analyzed.Results According to PSG recordings,the incidence of difficulties in the initiation of sleep(73.8%),fragmented sleep(59.5%),excessive daytime sleepiness (46.1%)were respectively increased in PD patients group than that in controls(all P5)、 ESS were increased(P=0.022,0.000,0.007, 0.001,0.000,respectively).SOREMPs occurred in 6 patients(14.3%)in PD group,but didn't oecure in controls.In PD group REM without atonia(RWA)was demonstrated in 36 patients(85.7%),RBD in 19 patients;in control group,however,REM in 6 and RBD in 2 separately.The statistics analysis showed the incidence of RWA(85.7%)and RBD(45.2%)in PD group was significantly higher than that in control group(P

Myogenic potentials evoked by air conducted sound (ACS), bone conducted vibration (BCV) or galvanic pulses can be recorded with surface electrodes over contracted muscles. These myogenic potentials are of vestibular origin (utricle and saccule) and so these potentials are called vestibular evoked myogenic potentials (VEMPs). Since the vestibular system has projections to many muscle systems, there are many such VEMPs. In this review, we discuss the generated origin, response pathway, waveform characteristics and clinical application of ocular vestibular-evoked myogenic potential (oVEMP).

5.Diagnostic application value of urothelial carcinoma of the urinary bladder by dual color fluorescence in-situ hybridization

Objective To evaluate the clinical application value of dual color fluorescence in-situ hybridization (FISH) in detecting urothelial carcinoma of the urinary bladder.MethodsThe probes of chromosome 3,7,17centromeres and chromosome 9p21 region (p16) were labeled by random primer method.FISH was performed on interphase nuclei of 80 urine specimens of cancer of the urinary bladder and 20 cases of healthy persons served as normal controls.Threshold value was established.The pathological diagnosis was the golden standard.Chromosome aberration was counted.The correlations between chromosome aberration and pathologic grading and staging and their sensibility of diagnosis for urothelial carcinoma of the urinary bladder were analyzed. Results The rate of numerical aberration of chromosome 3,7,17,9p21 was 47.5%(38/80),60.7% (49/80),51.3% (41/80) and 58.8% (47/80) respectively.The positive rate of the combined use of 3,7,17 and 9p21 chromosome probes for detecting urothelial carcinoma of urinary bladder was 76.3% (61/80).The aberrations had no correlation to tumor stage.The aberration of chromosome 3,7 and 17 were correlated to pathologic grade significantly (P < 0.05).ConclusionThe progression of urothelial carcinoma of the urinary bladder is related to the aberrations of chromosome.FISH is believed to be a very important method in diagnosis of urothelial carcinoma of the urinary bladder,which may have important clinical significance for the postoperative recurrence detection and prognosis.
6.Whole course cerebral protection for patients undergoing carotid endarterectomy

Objective To evalute perioperative whole course cerebral protection for patients undergoing carotid endarterectomy (CEA) Methods From 2002 to 2004, 20 patients received CEA in our hospital using whole course perioperative cerebral protection Preoperatively carotid angiography was performed in only one patient, while Doppler ultrasonography and CTA were applied to all patients Measures of intraoperative cerebral protection included general anaesthesia, selective shunting, and transcranial Doppler monitoring (TCD), followed by postoperative delayed removal of tracheal cannula, brain ice bag, maintaining blood pressure and appropriate use of diuretics under the guidance of TCD Results Shunt was used in 7 cases (35%), 15 patients (85%) underwent patch angioplasty There were no postoperative cranial nerves related complications Mortality was 0% Conclusion Perioperative whole course cerebral protection could effectively decrease postoperative cranial nerves related complications in patients undergoing carotid endarterectomy

8.Mechanism and clinical progress of molecular targeted cancer therapy.

Molecular target-based cancer therapy is playing a more and more important role in cancer therapy because of its high specificity, good tolerance and so on. There are different kinds of molecular targeted drugs such as monoclonal antibodies and small molecular kinase inhibitors, and more than 50 drugs have been approved since 1997. When the first monoclonal antibody, rituximab, was on the market. The development of molecular target-based cancer therapeutics has become the main approach. Based on this, we summarized the drugs approved by FDA and introduced their mechanism of actions and clinical applications. In order to incorporate most molecular targeted drugs and describe clearly various characteristics, we divided them into four categories: drugs related to EGFR, drugs related to antiangiogenesis, drugs related to specific antigen and other targeted drugs. The purpose of this review is to provide a current status of this field and discover the main problems in the molecular targeted therapy.

9.Construction of a general piggyBac transposon inducible cell immortalization vector and verification of its basic properties.

In order to construct generally efficient cell immortalization vector, pTP-hTERT, we modified the traditional piggyBac (PB) transposon using artificial synthesis, PCR and enzyme digestion. The modified vector contained the necessary transposon elements, a PB transposase expression cassette, a co-expression selectable element and a human telomerase reverse transcriptase (hTERT) expression cassette. The co-expression selectable element had two markers, enhanced green fluorescent protein (EGFP) gene and puromycin-resistance (Puro) gene, linked by porcine teschovirus-1 2A peptide (P2A). To validate the functionality of vector elements, we transfected pTP-hTERT into HEK293 cell, selected the positive cell clones and then conducted RT-PCR, Western blotting (WB) and Tail-PCR, methylene blue staining and statistic analysis on selected cells. The results of sequencing and cell culture show that the pTP-hTERT was constructed successfully and the positive cell could be selected by puromycin. The WB results, P2A cutting EGFP and Puro fusion protein with high efficiency, reflected the selectable element worked. The sequencing result of Tail-PCR confirmed the vector integrated into the genome through transposition. The results of methylene blue staining and statistic analysis indicated the clone of positive cells triggered by pTP-hTERT significantly increased (P < 0.01) compared with control group. The construction of pTP-hTERT provides an efficient tool for establishing immortalized cell lines and a demonstration for building other eukaryotic plasmids.

10.Influence about complications of gastrointestinal tract among ICU patients by three methods of nasal feeding pump

Objective To discuss the influence about three kinds of nasal feeding methods on ICU critical patients with gastrointestinal complications, thus providing guidance for ICU patients to choose reasonable way of nasal feeding pump injection. Methods 120 ICU patients were randomly assigned into three groups, which is the continuous 6-hour and intermittent 2-hour every three time pump group and continuous 24-hours pump group and intermittent 24-hour(continuous 2-hour and intermittent 1-hour every eight time units)pump group ,each group has 40 cases. To observe the the influence about three different way of nasogastric on ICU critical patients with gastrointestinal complications. Results The incidence of diarrhea and constipation among the three groups was not statistically significant (P>0.05) . The gastric retention rate of the continuous 24-hour pump group was significantly higher than that of the continuous 6-hour and intermittent 2-hour pump group and the intermittent 24-hour pump group, the differences were statistically significant (P < 0.05). There was no statistically significant difference between the continuous 6-hour and intermittent 2-hour pump group and the intermittent 24-hour pump group, (P > 0.05). Conclusions The continuous 24-hour pump group in gastric retention was slightly worse than the other two pump way. Compared with ICU class three a week and feeding times , the continuous 6-hour and intermittent 2-hour pump method is more suitable for ICU patients.
