1.Value of combined determination of tumor markers in the diagnosis of senile patients with colorectal cancer

Objective To evaluate the value of serum tumor markers in diagnosis of over 80 years old patients with colorectal cancer.Methods Serum CA199,CA242 and CEA of 68 cases senile patients with colorectal cancer and 35 cases senile patients with benign large intestinal diseases were detected by ELISA.Results Serum levels of CA199,CA242 and CEA in patients with colorectal cancer were (37.95 ± 15.95 ) U/ml,( 50.08 ± 24.20 ) U/ml,( 22.34 ± 17.35 ) μg/L respectively and were significantly higher than that in patients with benign large intestinal diseases CA199 ( 10.73 ± 6.27 ) U/ml,CA242 ( 8.00 ± 3.52 ) U/ml,CEA(2.32 ± 1.01 )μg/L.In addition,the level of these three markers was increased with the progress of colorectal cancer and the sensitivity of diagnosis was related with the stages of colorectal cancer.Parallel combined testing methods increased the diagnostic sensitivity to 83.6%,and serial combined testing increased the diagnostic specificity to 88.7 %.Conclusions Serum tumor markers showed certain values in the diagnosis of the senile patients with colorectal carcinoma.Furthermore,the combined determination of tumor markers is helpful for early diagnosis of colorectal cancer and increases the sensitivity and specificity in diagnosing of colorectal carcinoma.
2.Determination of Elements Seleninum in Se Semen Cassiae by Graphite Atomic Absorption Spectrophotometer

Objective To explore the determination of elements selenium content in Se Semen Cassiae. Method Selenium content in Se Semen Cassiae was measured by graphite atomic absorption spectrophotometer. Result and Conclusion An appropriate extraction method and digesting condition were screened out. The determined results were accurate when the digested sample was dissolved and metered volume with 1% HNO3 which containing 0.1% Tween 80.

4.Nursing care and epidemiological analyse of elderly burn patients

Objective To explore the epidemiological character of elderly burn patients and to provide the experience of prevention and treatment.Methods The retrospective study method was used,and totals of 531 elderly patients with burn from January 2002 to December 2010 in two hospitals were selected.Cases information such as demographics,clinical diagnosis,treatments,causes of injury,mortality rate and causes of death since admission and so on was recorded and analysed,then,the epidemiological character was summarized and the prevention and treatment were proposed.Results Among 531 patients with burn,the average age was 72.86,the rate of burn in home was 72.5% and in accident was 16.70%.The clinical cure rate was 82.11%and the mortality rate was 2.33%.The proportion of the elderly burn patients account for the total admitted patients in the same period was 5.13% in 2002 and 5.64% in 2010,and the average age of elderly burn patients was higher from 72.53 years old in 2002 to 73.58 years old in 2010.Conclusions The proportion of the elderly burn patients account for total admitted patients in the same period was higher as well as the average age of patients from 2002 to 2010.We should pay more attention to the prevention and education for the burn in home as the main cause of elderly burn patients is the home burn.

8.Salvage therapy by hemicortical excision and reconstruction for low-grade malignant bone tumour

Objective: To treat low-grade limb malignant tumors with hemicortical excision and reconstruction of the defect with recycled pasteurized autogratf. Methods:From January 2005 to January 2011, 10 patients [age (27.0±6.5) years] suffering from low-grade limb malignant tumors were treated with hemicortical excision and reconstruction of the defect with recycled pasteurized autogratf:7 were histopathologically diagnosed as parosteal osteosarcoma, and 3 as peripheral chondrosarcoma. According to the Enneking staging system, 8 were in Stage IA and 2 were in IB. Results:All the patients were followed-up for (50±23) months. All the patients had bony union at the last follow-up. The length of tumor bone resection was (9.5±2.7) cm, operation time was (150±45) min, intraoperative blood loss was (1000±350) mL, and drainage volume was (450±200) mL. hTe only fracture in the host bone during the operation was treated by internal ifxation. Local recurrence occurred in 1 patient was treated by a wide operative margin and reconstruction with a prosthesis. There was no distal metastasis. A ccording to the Enneking system, 9 patients had excellent results and 1 had good results. Conclusion:Hemicortical excision for low-grade limb malignant tumors is safe and effective.
9.Clinical value of contrast-enhanced ultrasonography and serum markers in assessing portal hypertension in patients with decompensated liver cirrhosis

Objective To analyze the possibility of assessing portal hypertension in patients with decompensated liver cirrhosis via contrast-enhanced ultrasonography and common serum markers.Methods Fifty-four patients with decompensated liver cirrhosis were divided into two groups according to hepatic venous pressure gradient (HVPG):HVPG<12 mmHg (1 mmHg=0.133 kPa) and HVPG ≥12 mmHg.The non-invasive index of routine blood test,liver function,coagulation function,Child-pugh score (CPS) and the results of contrast-enhanced ultrasound such as hepatic artery arrival time (HAAT),hepatic vein arrival time (HVAT)、portal vein arrival time (PVAT),hepatic artery to henatic vein arrive transmit time (HA-HVTT) and portal vein to hepatic vein arrive transmit time (PV-HVTT) were assessed by univariate analysis and multivariate Logistic regression analysis,and then were used to generate a diagnostic model.The receiver operating characteristic curve was also used for analysis.Results The non-invasive model is Y =-0.217 × PV-HVTT + 1.526 × CPS-7.097.When the area under ROC curve (AUROC) was ≥0.857 and the best cutoff value was ≥0.631,and the sensitivity and specificity in judging HVPG≥ 12 mmHg were 87.5 % and 78.6%,respectively.Conclusions The model composed of PV-HVTT and CPS could be used to assess portal hypertension.
10.Value of Single Photon Emission Computed Tomography Thyroid Imaging for Diagnosis of Congenital Hypothyroidism

Objective To explore the causes of congenital hypothyroidism by single photon emission computed tomography(SPECT) thyroid imaging.Methods Nineteen cases with congenital hypothyroidism were collected,and SPECT thyroid imaging was performed 15 minutes late after injected 99m TcO_ 4- 18.5-37.0 MBq.Results Five cases were found with normal glands,1 case with enlarged thyroid, 2 cases with enlarged thyroid and increased radiation,4 cases with ectopic thyroid,4 with a hypogenetic thyroid,and the last 3 cases showed no thyroid imaging.Conclusions The causes of congenital hypothyroidism can be found by SPECT thyroid imaging,which is important to the diagnosis of congenital hypothyroidism combined with detection of T_3,T_4,TSH.
