1.Study on decomposed recipes of traditional Chinese medicine

OBJECTIVE: To introduce and analyze the present status of research on decomposed recipes of traditional Chinese medicine (TCM). METHODS: The relevant literature on the study of the decomposed recipes of TCM in the past five years was collected and analyzed. RESULTS AND CONCLUSION: Studies on the decomposed recipes of TCM have been made from the whole body level to the organ, cell and molecular research level. The pharmacological research in vitro is the main way and quantitative changes of the chemical ingredients have been measured before and after composition. It is very important to combine the modern science and technology in exploring the composing mechanisms of TCM remedies.
2.Expression of CD44V6 and p53 gene mutation in human ovarian carcinoma

Purpose:To investigate p53 gene mutation, CD44V6 expression and their relationship with metastasis of ovarian carcinoma in tumor diagnosis. Methods:PCR SSCP with silver staining was used to detect the mutation of p53 gene; by using Southern blot and image analysis, the quantitative and qualitative expression of CD44V6 were also determined. Results:The positive percentage of CD44V6 expression and p53 gene mutation was not detectable in any of the normal ovarian specimens but in the benign tumors, non metastasizing and metastasizing carcinomas it was 10%, 75%, 88% and 5%, 40%, 60% respectively. The mean dark density of each band in these four groups(mentioned above) was 85.25?23.16, 817.11?126.5, 3820.14?289.43 and 10132.92?1521.20 respectively. The expression of CD44V6 of metastasizing carcinomas was higher than that of non metastasizing group. Conclusions:The expression of CD44V6 is related to tumor metastasis; the positive percentage of CD44V6 is higher than that of p53 gene mutation in the group of metastasizing and non metastasizing tumors; Compared to p53 gene mutation, CD44V6 is a better marker for tumor metastasis.
3. Clinicopathologic features of anaplastic large cell lymphoma in children

Objective: To study the clinical manifestations and histopathological and immunohistochemical features of anaplastic large cell lymphoma (ALCL) in children. Methods: We analyzed the clinical data, performed pathological examination, and tested the immunophenotypes of four ALCL children who were admitted in our hospital from 2001 to 2006. Results: The main clinical manifestations included enlarged superficial lymphnodes, rapid development disease, irregular fever, and hepatosplenomegaly. Morphological examination showed that the normal structure of lymphnode was destroyed and the large cells infiltrated into lymphnode sinuses. The tumor cells had large size with eccentric, horseshoe or kidney-shaped nucleus. Immunohistochemical analysis showed that the tumor cells positively expressed activin receptor-like kinase-1 (ALK-1), epithelial membrane antigen (EMA), and Ki-1. Conclusion: Detection of the expression of ALK-1, Ki-1, and EMA in ALCLs will provide assistance for diagnosis and differential diagnosis of ALCLs.
4.Hysteroscopic Resection with Dipolar Electrodes for Uterine Submucous Myoma:Report of 456 Cases

Objective To determine the efficacy and safety of hysteroscopic resection with dipolar electrodes for submucous myoma of the uterus. Methods By hysteroscopy combined with B-ultrasonography,totally 456 cases of submucous myoma of the uterus were confirmed in our hospital from January 1998 to December 2007. Among the patients,241 cases showed type 0,138 patients were type Ⅰ,and 77 cases were type Ⅱ. In 341 of the cases,the myoma sized 1.0 to 2.0 in diameter,86 cases from 2.1 to 4.0 in diameter,and the other 29 cases had the myoma ranged from 4.1 to 6.0 cm in diameter. Hysteroscopic resection with dipolar electrodes were performed on all the cases,among which microelectrode was used in 341 patients,and ring electrode was employed in 115 cases. Results The operation time ranged from 9 to 55 min with a mean of (32.9?16.7) min. The removed tissues weighed (30.2?8.2) g in average (ranged from 5 to 55 g). During the operation,the patients had 5 to 100 ml blood loss [mean,(48.1?12.7) ml]. No complications occurred during and after the operation. The patients were followed up for three months,during which 431 (94.5%) showed reduced menstruation and decreased menstrual blood loss,only 5 patients (1.1%,all were type Ⅱ) had residual myoma. In this series,totally 42 patients wished pregnancy,15 of them became pregnant in 24 months postoperation. After the treatment,anemia was corrected in 206 patients with the level of Hb increased to a normal range;before the operation,81 patients had menstrual pain,42 of them were relieved and 19 were improved by the operation,while the other 20 showed no changes in the symptoms. Conclusion Hysteroscopic resection with dipolar electrodes is safe and effective for uterine submucous myoma.
5.Values of Laparoscopic Surgery for Benign Gynecological Diseases in Senile Women

Objective To study safety and values of laparoscopic surgery in the management of gynecological diseases in senile women.Methods Clinical data between 27 cases of laparoscope surgery(Laparoscopic Group) and 25 cases of open surgery(Open Group) from January 2001 to December 2006 were retrospectively compared.Results The operation time in the Laparoscopic Group(20.0?7.9) min was significantly shorter than that in the Open Group(44.0?7.2) min(t=-11.419,P=0.000);the blood loss in the Laparoscopic Group(21.9?20.0)ml was distinctively less than that in the Open Group(62.6?29.4)ml(t=-5.875,P=0.000);the postoperative pyrexia in the Laparoscopic Group(3 cases) was significantly less than that in the Open Group(12 cases)(?2=8.606,P=0.001);the time to first flatus in the Laparoscope Group(13.9?2.9)h was significantly shorter than that in the Open Group(23.4?4.3)h(t=-9.404,P=0.000);hospital stay in the Laparoscope Group(7.6?0.9) d was significantly shorter than that in the Open Group(10.2?1.2) d(t=-8.882,P=0.000).Conclusions On the basis of intensive treatment of preoperative complications and perioperative monitoring,laparoscopic surgery is an ideal procedure for benign gynecological diseases in senile women.
6.The clinical effect of hydromorphone combined with ropivacaine in abdominal transverse plane block after laparoscopic myomectomy and its effect on stress response

Objective:To investigate the clinical effect of hydromorphone combined with ropivacaine and its effect on stress response after laparoscopic myomectomy.Methods:From March 2017 to March 2020 in Shangyu Hospital of Traditional Chinese Medicine, Zhejiang Province, 100 patients who were performed laparoscopic hysteromyoma were randomly divided into observation group (50 cases) and control group (50 cases). Both groups were treated with plane block of transverse abdominal muscle after anesthesia induction. The control group was injected with ropivacaine, and the observation group was injected with hydromorphone and ropivacaine. The changes of hemodynamics at the time of entering operation room (T 0), 1 h after operation (T 1) and after surgery (T 2) were compared between two groups. The visual analogue scale (VAS) score at postoperative 3 h, 12 h and 24 h, Ramsay sedation score during operation, and serum stress level at preoperative and postoperative 24 h were compared between two groups. Results:The levels of heart rate, systolic and diastolic blood pressure between the two groups at different times had no significant differences ( P>0.05). The VAS scores of the observation group at postoperative 3 h, 12 h and 24 h were lower than those in control group [(3.05 ± 0.32) scores vs. (3.42 ± 0.39) scores, (2.16 ± 0.38) scores vs. (2.73 ± 0.43) scores, (1.43 ± 0.29) scores vs. (2.28 ± 0.35) scores], and there were significant differences ( t = 5.186, 7.024, 13.223, P<0.05). The scores of Ramsay sedation score in observation group was higher than that in control group [(3.79 ± 0.45) scores vs. (2.54 ± 0.39) scores], and there was significant difference ( t = 14.843, P<0.05). The levels of noradrenaline, interleukin-6, and cortisol in observation group at postoperative 24 h were lower than those in control group [(254.18 ± 14.35) pmol/L vs. (328.73 ± 18.69) pmol/L, (26.89 ± 4.10) ng/L vs. (35.27 ± 5.46) ng/L, (214.30 ± 21.28) pmol/L vs. (296.53 ± 32.19) pmol/L], and there were significant differences ( t = 22.371, 8.678, 15.068, P<0.05). Conclusions:The combination of hydromorphone and ropivacaine in abdominal transverse plane block after laparoscopic myomectomy has good analgesic and sedative effects and can reduce stress response.
7.Effect of Early Rehabilitation Nursing on Secondary Disturbance in Hemiplegics after Stroke

ObjectiveTo study the effect of early rehabilitation nursing on secondary disturbance in hemiplegics after stroke.Methods140 patients were randomly divided into two groups , rehabilitation nursing group (70 cases) and control group (70 cases). Patients in the rehabilitation nursing group were given clinical treatment and regularly convalescent nursing, while those in control group were given clinical treatment and unguided self-training. Evaluation was performed in pre-treatment and 12 weeks post-treatment respectively.ResultsSecondary disturbance rate in the rehabilitation nursing group was significant lower than that in the control group (P<0.01). The motor scores and ADL scores measured by Barthel Index in both groups raised after 12 weeks treatment, but the scores in the early rehabilitation nursing group were obviously superior to those in the control group (P<0.01).ConclusionEarly rehabilitation nursing for hemiplegics afte sroke may prevent or lessen the secondary disturbance, as well as obviously improve motor function of the limbs and ADL.

As the diagnosis and treatment levels increased, the cure rate and lifetime of the patients with breast cancer improved. In China,surgery and drug treatment received more attention, but the overall rehabilitation was ignored. This article summarized the status of breast cancer patients's rehabilitation in lymphoedema, upper limb dysfunction, psychological problem, sexual dysfunction, childbirth trouble,dystrophy, follow-up visit and family-society support, and so on.
