1.Biomechanical changes of spine after thoracolumbar fracture and dislocation treated with pedicle screw internal fixation

BACKGROUND:Anterior decompression, bone graft fusion, plate and screw fixation are commonly used in clinic for the treatment of thoracolumbar burst or comminuted fractures. The posterior surgery including decompression laminectomy and pedicle screw internal fixation.
OBJECTIVE:To explore the biomechanical changes of spine after thoracolumbar fracture and dislocation treated with pedicle screw internal fixation.
METHODS:Forty-six patients with thoracolumbar fracture and dislocation were treated with pedicle screw internal fixation, and then the recovery of spinal cord injury and fracture reduction were observed after internal fixation, immediately after internal fixation and 1-year fol ow-up.
RESULTS AND CONCLUSION:Al the 46 cases were fol owed-up. Frankel function classification assessment was used to evaluate the spinal cord injury grade. There was no significant difference in the percentage of A grade between periods (P>0.05), and there were significant differences in the percentage of B-E grades when compared between immediately postoperative period, 1-year fol ow-up period and preoperative period (P<0.05), but there was no significant difference when compared between immediately postoperative period and 1-year-fol ow period (P>0.05). The anterior height percentage, posterior height percentage and Cobb angle indicators were used to assess the fracture reduction, and the results showed there was no significant difference in posterior height between periods (P>0.05), and there was significant difference in Cobb angle when compared between immediately postoperative period, 1-year fol ow-up period and preoperative period (P<0.05), but there was no significant difference between immediately postoperative period and 1-year fol ow-up period (P>0.05). The results indicate that pedicle screw internal fixation system for the treatment of thoracolumbar spinal fracture and dislocation is conducive to the fracture reduction and functional recovery.
2.Influences of astragulas(黄芪) on cerebroischemiareperfusion injury in gerbil

Objective:To study the influences of astragulas(黄芪) on cerebroischemiareperf usion injury in gerbils.Methods:The cerebral tissue delayed neuron death(DND) models of gerbil were established by means of ligating bilateral carotid arteries for 15 minutes then reperfusing for 48 hours and divided into shamoperation(SO) group,ischemiareperfusion(IR) group and astragulas injection (AI,黄芪注射液) group.The activities of Na +K+ATPase and nitric oxide synthase(NOS) and the changes in contents of nitric oxide(NO) and excitat ory amino acid(EAA) in cerebral tissue were compared in different groups.Results:(1)In comparison with SO group the activities of Na+K+ATPase in IR and AI group were obviously decreased furthermore the decreases in IR group were mo re significant than those in AI group(all P<0.01).(2) The changes in co nten ts of lactic acid(LD) in cerebral tissue in all the groups had not statistically significance.(3)In comparison with SO group the activitis of NOS and the conten ts of NO in IR and AI group were reduced but the reductions in IR group were muc h more significant (P<0.05).(4)The contents of glutamic acid(Glu) in IR and AI group were higher than those in SO group and the increases in IR group w .05),meanwhile they in IR group were higher than those in AI group but the di fference was not significant.(6)Among all the groups the contents of γam inobutyric a cid (γGABA) and glycine(Gly) had not significant differences (all P>0.05).Conclusions:The mechanisms of astragulas on anticerebral ischemiareperfusion inj ury may be related to its preventing the reductions of activities of Na+K +ATPase,NO S,and the decreases in synthetic volume of NO as well as its heightening the con tents of EAA after ischemia of brain tissue.

5. One-stage operation for bilateral posterior communicating artery aneurysms via unilateral pterional approach: An efficacy analysis

Objective: To investigate the feasibility and operative effect of the one-stage microsurgery for clipping of bilateral posterior communicating artery aneurysms (BpcoAA). Methods: The clinical data of 28 patients with BpcoAA were analyzed retrospectively. All patients underwent craniotomy and microsurgical clipping of BPcoAA via unilateral pterional approach. The patients were followed up for 6 months to 3 years after microsurgery. The head 3 D-CTA of the patients and their general conditions were reexamined. The Glasgow outcome scale (COS) scores were used to assess the prognosis. Results: Circled digit oneGood preoperative 3D-CTA showed 28 cases with 56 aneurysms, the aneurysms were clipped completely in 24 cases, the contralateral aneurysms were not clipped completely in 3 cases, the aneurysm was not clipped completely on the approach side in 1 case, and none of the patients died. The aneurysms ruptured in 8 cases on the approach sides during the microsurgery. Circled digit twoAfter microsurgery, 7 cases had hydrocephalus, and 5 had vasospasm on the approach sides and 3 on the contralateral sides, 3 suffered pulmonary infection, and 2 had oculomotor nerve injury on the contralateral sides. Circled digit threeThe mean follow-up time of the patients was 1.7 years. The GOS scores: 5 points in 10 cases, 4 points in 7 cases, 3 points in 9 cases, and 2 points in 2 cases. There were no aneurysm recurrence and new aneurysm formation. The patients whose aneurysms were not clipped completely had no recurrence of bleeding. Conclusion: According to 3D-CTA examination, using unilateral pterional approach, aiming at the location of the optic chiasm and the contralateral aneurysm pointing, the one-stage microsurgery for clipping of BpcoAA is safe and feasible.

Objectives Research into effect on PBL model in prosthodontics education.Methods The PBL model was performed in education of a part of prosthodontics,and education effect was evaluated by table analysis.Results PBL model is proved to be better than the multimedia education model in the training of students'self-study ability,learning initiative,problem-analyzing and problem-solving competence,and the team spirit etc.Conclution Comparaed with the multimedia education model,PBL model is of significance in some respects.

9.Nonlinear predictability analysis of diabetic autonomic neuropathy

BACKGROUND: Researches on the diagnosis and detection methods of diabetic autonomic neuropathy(DAN) is very weak at present.OBJECTIVE: To get the data about the degree of autonomic neuropathy by using the nonlinear heart rate variability(HRV) so as to provide clinical evidence for the early diagnosis of DAN.DESIGN: A single sample study.SETTINGS: Computer department in a university; Staff room of medical physics of department of biomedical engineering in a university.PARTICIPANTS: The experiment was completed in the Staff Room of Medical Physics, Department of Biomedical Engineering in the First Military Medical University of Chinese PLA from March to May 2002. Thirty-four diabetic patients, including 22 with DAN complications, were selected from the inpatients of Department of Endocrinology in the Affiliated Nanfang Hospital of the First Military Medical University of PLA, and other 40 normal subjects, who were from the faculty in the First Military Medical University of Chinese PLA and students in the Department of Physical Education, Chinese PLA Institute of Physical Education, were taken as the controls. The R wave to R wave intervals(RRI) time series is sampled and corrected to 512 points for analysis.INTERVENTIONS: Based on the virtual instrumental workbench-LabVIEW, 74 standard adjacent RRI signals were selected from 34 diabetic patients and 40 normal cases, and then analyzed with the method of nonlinear predictability.MAIN OUTCOME MEASURES: Collective and analytical results of the 74 standard adjacent RRI signals.RESULTS: There was no significant difference between the diabetic patients without DAN and the controls by the analysis with normalized mean square error(NMSE) NMSE ( P = 0. 075 ), but the significant difference between the DAN patients and the controls( P = 0. 001) . While significant analysis between the diabetic patients without DAN complications and the controls was significant by the analysis of mean absolute error(MAE) (P = 0. 007 and P = 0. 000), there was also significant difference between the diabetic patients with and without DAN ( P = 0. 001 ).CONCLUSION: Different impaired degree of autonomic nerve system in diabetic patients can result in nonlinear reduction in HRV analysis, but nonlinear predictability analysis can prove the nonlinear model of HRV,. and can also provide important state information of sympathetic and parasympathetic systems so that it can provide reliable evidence for the earlier diagnosis and the prognosis evaluation of DAN.
10.Protective effects of Chaiqin Chengqi Decoction on isolated pancreatic acinar cells in acute pancreatitis rats and the mechanisms

OBJECTIVE: To investigate the protective effects of Chaiqin Chengqi Decoction (CQCQD), a compound traditional Chinese herbal medicine, on acinar cells and the changes of intracellular calcium concentration ([Ca(2+)]i) in acute pancreatitis (AP) rats. METHODS: CQCQ serum (CQCQS) was prepared by intragastric administration of CQCQD in SD rats, and AP was induced in SD rats by biliary-pancreatic duct ligation method. Pancreatic acinar cells were isolated by collagenase digestion and co-incubated with the drug serum containing CQCQD in vitro, then cell viability was determined by methyl thiazolyl tetrazolium (MTT) method and intracellular fluorescent intensity (FI) was observed and analyzed to investigate the changes of intracellular calcium concentration ([Ca(2+)]i). RESULTS: Both 5% and 10% CQCQS could improve the viability of pancreatic acinar cells in AP rats (P<0.05), and 10% CQCQS was more effective than 5% CQCQS (P<0.05). [Ca(2+)]i was elevated in AP rats (P<0.05), while the level of [Ca(2+)]i elevation was reduced after CQCQS treatment (P<0.05). CONCLUSION: CQCQD has a protective effect on pancreatic acinar cells of AP rats, which may be associated with its inhibition effect on intracellular calcium overload.
