1.The use of Propofol senile patients undergoing minor surgery

Objective To discuss the feasibility and safety of propofol used in senile patients undergoing minor surgery including gastroscopy and colonoscopy.Methods 40 patients aged 80-88 years old were randomly divided into two groups with 20 cases for each group.Group P received only propofol by intravenous injection in a speed of 1ml/s till eyelash response disappeared.In patients of group F fentanyl was first given at 0.5?g/kg,and 1 minute later,the patients were given propofol by intravenous injection in a same speed as in group P if there was no discomfort until eyelash response disappeared.MAP,HR,SpO2 and RR were determined before induction of anesthesia,beginning of operation,and 1st,3rd and 5th minute after operation.The onset time of anesthesia,total dosage of propofol used,time of palinesthesia and follow-up results were recorded.Results MAP was significantly decreased in both groups at the beginning of operation,and more stable in group F at the 3rd and 5th minute after operation.There were significant differences between two groups in amount of propofol used,and the dosage in group F was less than that used in group P(P
2.Population pharmacokinetics of remifentanil in adult patients undergoing elective major abdominal sur-gery

Objective The aim of this study was to explore possible pharmacokinetic factors and develop a population pharmacokinetic model for remifentanil in adult patients.Methods Eleven healthy patients,undergoing elective major abdominal surgery,aged 25 to 86 years,received random-ly remifentanil 0.3μg·kg-1 ·min-1 (group R3),or 0.6μg·kg-1 ·min-1 (group R6).Frequent ar-terial blood samples were drawn according to predetermined time and assayed for remifentanil concen-tration.Nonlinear mixed-effects modeling (NONMEM)was used to evaluate the time courses of the measured concentrations.The covariates include age,bodyweight (WT),gender,lean body mass (LBM),body mass index (BMI)and body surface area (BSA).Results The pharmacokinetic data of remifentanil were well described using a three-compartment linear model with first-order elimination from the central compartment.Forward analysis showed that age,height and body mass index (BMI) does not affect the pharmacokinetic parameters,which are contrast with body weight,lean body mass (LBM),body surface area (BSA)and gender;further analysis demonstrated only a significant effect of body weight on remifentanil systemic clearance (CL)and volume of the central compartment (V). For typical 60 years patients,PK parameters were:V1 =7.64 L,V2 =4.81 L,V3 =4.34 L,CL1 =2.74 L/min,CL2 = 0.738 L/min,CL3 = 0.0905 L/min.Conclusion The pharmacokinetics of remifentanil is consistent with its rapid elimination by blood and tissue esterase in Chinese patients. The systemic clearance and volume of distribution of central compartment increases with body weight in the population and the range of covariates studied,which suggests that a patient with greater body weight needs a greater initial dose and maintenance infusion rate higher to obtain a stable plasma con-centrations and clinical effects.
3.Application value of MR diffusion weighted imaging of apparent diffusion coefficient in diagnosis of breast nodular lesions

ObjectiveTo estimate the applications of ADC value and rADC value in the diagnosis of nodular lesions of breasts.Methods Fifty-two cases with 66 nodular lesions of breasts confirmed by histopathology underwent diffusion-weighted magnetic resonance imaging.Three b values (0,800 and 1000 s/mm2) were applied.The mean ADC values of the breast nodules,the ADC values of ipsilateral breast( rADC1 )and ADC values of contralateral breast (rADC2 )were respectively measured.The independent-samples t-test and chi-square test were used for statistical analyses.ResultsOf the 52 patients,there were 18 patients with infiltrating ductal carcinoma and 34 patients with fibroadenoma.50 patients with 64 lesions were examined by DWI.( 1 ) at b = 800 s/mm2,the mean ADC values of malignant nodules [ ( 1.01 ±0.09) × 10-3 mm2/s],rADC800-1 (0.52 ±0.07)and rADC800-2 (0.51 ±0.06) were lower than that of the benign nodules [ ADC value = ( 1.54 ± 0.28 ) × 10 -3 mm2/s,t = 8.217,P < 0.01 ; rADC800-1 =0.77 ±0.15,t =9.339,P<0.01 ; rADC800-2 =0.76 ±0.14,t = 10.394,P <0.01 ].The one-side upper limits of 95% medical reference value of mean values of infiltrating ductal carcinoma were adopted as the threshold point to distinguish the malignant from the benign.The threshold value of breast malignant nodule ADC,the rADC800-1 and rADC800-2 were respectively 1.05 × 10-3 mm2/s,0.55 and 0.53.The sensitivities of the three methods were 75.0%,65.0% and 60.0% ; the specificities were 100.0%,95.7% and 97.8% ;the positive predictive values were respectively 100.0%,86.7% and 92.3% ; the negative predictive values were 90.2%,86.3% and 84.9%; the diagnosis accordance rates were respectively 92.4%,86.4% and 86.4%.( 2 ) at b = 1000 s/mm2,the mean ADC values of malignant nodules [ ( 0.93 ± 0.08 ) ×10-3 mm2/s],rADC1000-1 (0.53 ±0.09) and rADC1000-2 (0.52 ±0.07) were also lower than that of the benign nodules[ ADC value= (1.45 ±0.28) ×10-3 mm2/s,t=11.844,P<0.01; rADC1000-1 =0.75 ±0.16,t=5.820,P < 0.01 ; rADC1000-2 = 0.74 ± 0.15,t = 8.082,P < 0.01 ].The threshold value points breast malignant nodule ADC,the rADC1000-1 and rADC1000-2 were respectively 0.97 × 10-3 mm2/s,0.58,0.55.The sensitivities were all 70.0% ; the specificities were respectively 100.0%,95.7% and 93.5% ;the positive predictive values were 100.0%,87.5% and 82.4% ; the negative predictive values were 88.5%,88.0% and 87.8% ; the diagnosis accordance rates were 90.9%,87.9% and 86.5% respectively.There were no significant differences in specificities and the diagnosis accordance rates ( x2 = 1.232,2.263 ; P =0.942,0.812 ).Conclusions ADC value and rADC value are both important parameters of MRI in differentiating benign and malignant breast diseases.The study indicated that ADC value ( at b =800 s/mm2) was the most valuable parameter.
4.Study of apparent diffusion coefficient value in breasts of different ages and different menstrual phases

Objective To analyze the differences of ADC values in breasts of women of different ages and different menstrual phases, so as to direct the choice of the examination time of MR DWI. MethodsThe breasts of 65 healthy volunteers were scanned with the routine MRI plain scan and DWI in the menstrual, proliferative and secretary phases. DWI was conducted with single shot echo planar imaging technique and b value were 0, 1000 s/mm2. The women were divided into three groups: Group 1(aged 20 to 29 years, 21 cases), Group 2 (aged 30 to 39, 21cases), and Group 3 (aged 40 to 49, 23 cases). The ADC values of all 130 breasts at nipple level in the different phases were measured. The ADC values in the three age groups and in the different menstrual phases were compared using ANOVA. Results The mean ADC values of Group 1 were (2.14±0.14) ×10-3, (2.03±0.18) ×10-3and (2.10±0.19)×10-3mm2/s for left breast, and (2.08±0.17) ×10-3, (2.02±0.16) ×10-3and (2.09±0.17) ×10-3mm2/s for right breast in the menstrual, proliferative and secretary phases. They were slightly higher than Group 3, which were (2.02±0.27) ×10-3, (1.97±0.25) ×10-3and (2.03±0.22)×10-3 mm2/s for left breast and (1.99±0.29) ×10-3, (1.93±0.26) ×10-3and (2.03±0.28)×10-3 mm2/s for right breast. The mean ADC values of Group 2 [left breast: (1.94±0.25) ×10-3, (1.91±0.21) ×10-3and (1.97±0.21)×10-3 mm2/s ; right breast: (1.97±0.26)×10-3, (1.89±0.25)×10-3and (1.96±0.22)×10-3 mm2/s) were the lowest among the three age groups. There were significant differences in different menstrual phases (F= 23.600, P<0.01), but no statistical difference was found among the three age groups or between left breasts and right breasts (F= 1.683, 2.248;P>0.05).Conclusions The mean ADC values of breasts decrease markedly in the proliferative phase.The effects of the menstrual cycle on the breast ADC values should be considered in the evaluation of breast diseases with DWI.
5.Establishment of a stable AR42J cell line expressing EGFP LC3

Objective To establish a stable AR42J cell line expressing EGFP LC3.Methods The EGFP LC3 overexpressed Lentivirus was constructed and transfected into pancreatic acinar cells (AR42J) of rats.The rats with Lentiviral EGFP transfection were treated as negative control.The transfection efficiency was detected by inverted fluorescence microscope and flow cytometry.The EGFP LC3 protein expression in the stable cell lines were analyzed by Western blot.The cells were treated with thapsigargin to establish endoplasmic reticulum stress model,and the LC3,PERK protein expressions were detected by Western blot.Results The transfection efficiency of Lentiviral EGFP LC3 of AR42J cell was > 85%,which could achieve stable passage.The expression of LC3 mRNA of AR42J cells transfected with Lentiviral EGFP LC3 was 9.14 ±0.32 folds higher than that of negative control,which had no expression of LC3 protein,only EGFP expression.However,compared with non-transfection group,the LC3 mRNA expression in EGFP group was not significantly different.Conclusions A pancreatic acinar cell line (AR42J) of rat stably expressing EGFP LC3 protein is successfully constructed.And it may provide a new model for further research of the relationship between acute pancreatitis and autophagy.
6.Molecular mechanism of Golgi protein 73 in inflammation

Objective To study the effect of Golgi protein 73(GP73) on inflammation, and to reveal the effect of GP73 on tumorigenesis and metastasis.Methods The transcriptional activity of NF-κB and the expression of IL-1β, IL-6 and TNF-αwith GP73 overexpression or knockdown were detected to illuminate the role of GP73 in inflammation.According to the TCGA database, the correlation between the transcriptional activity of GP73 and the expression of NF-κB, IL-1β, IL-6 and TNF-αwas analyzed to determine the role of GP73 in tumor inflammation.Results Correlative analysis showed that there was a positive correlation between the expression of GP73 with NF-κB, IL-1β, IL-6 and TNF-α.The transcriptional activity of NF-κB was upregulated by GP73 overexpression, but downregulated by GP73 knockdown.The expression of IL-1β, IL-6 and TNF-αwas upregulated by GP73 overexpression.Ammonium pyrrolidinedithiocarbamate ( PDTC ) was in-volved in inflammation reaction induced by GP73.Conclusion GP73 is possibly involved in inflammation and promotes tu-morigenesis and metastasis.
7.The Clinical Value of Combined Detection of PCT and IL-6 in the Differential Diagnosis Septic and Non-septic SIRS in ICU

Objective:To explore the value of combined detection of PCT and IL-6 in differential diagnosis SIRSin ICU patients.Methods:100 patients with ICU admitted to our hospital from 2013 to 2016 were choosen,including 61 cases with non septic SIRS and 39 cases with sepsis,and 50 healthy persons over the same period were selected as control,and they were divided into non-septic group,sepsis group and control group.The levels of serum PCT and IL-6 were detected by electrochemiluminescence assay,and took PCT of 2 g/L and IL-6 of 50 ng/L for the critical value to identify non infectious SIRS and sepsis,to evaluate the clinical diagnostic value of combined detection.Results:The maximum values of PCT and IL-6in the non-septic group respectively were 0.91 ± 0.54 μg/L and 62.77± 11.75 ng/L,in the septic group respectively were 24.49± 5.00 μg/L and 1542.69± 361.66 ng/L,in the control group respectively were 0.08± 0.06 tμg/L and 3.68± 1.11 ng/L,the maximum values of PCT and IL-6 in the non-sepsis group and the sepsis group were significantly higher than control group (P<0.05).Compared with the non-septic group,the maximum valuesin sepsis group were significantly increased (P<0.05).The proportions of PCT > 2 g/L and IL-6 < 50ng/L in the non-septic group respectively were 21.31% and 65.57%,in the septic group respectively were 92.31% and 87.18%,the proportions of PCT>2 g/L,IL-6<50 ng/L in the sepsis groupwere significantly higher chan those in the non-septic group (P<0.05).The positive predictive values,sensitivity and specificity of PCT were higher than IL-6,the positive value,specificity of combined detection was higher than IL-6 and PCT,while the sensitivity of combined detection was higher than IL-6,P<0.05.Conclusions:Combined detection of PCT and IL-6 is helpful for differential diagnosis of sepsis and non-septic SIRS.
8.Basic leucine zipper ATF-like transcription factor involvement in regulating the immune pathogenesis of tuberculosis

Objective To explore the role of basic leucine zipper ATF-like transcription factor (BATF) in active tuberculosis, and to provide clues for diagnosis and therapy of tuberculosis.Methods Sixteen patients with active tuberculosis (ATB), ten cases of latent tuberculosis infection (LTBI) and fourteen healthy controls (HC) were included in this study.Flow cytometry was applied to detect the expressions of BATF and programmed death-1 (PD-1) in T lymphocytes, and the changes of BATF by blockade of PD-1/PD-L pathway using specific blocking antibody antiPD-1, antiPD-L1 and antiPD-L2.The expressions of BATF were compared using Mann-Whitney U test.And the relation of BATF and PD-1 was analyzed using Pearson correlation analysis.Results The CD4+ T lymphocytes expressing BATF accounted for 5.16% (2.96%,8.71%) of CD4+ T lymphocytes in ATB group, which was higher than 1.05% (0.40%,1.27%) in LTBI group and 0.71%(0.43%,1.21%) in HC group, and the difference were statistically significant (U value were 6.5 and 9.0, respectively, both P<0.01).The CD8+ T lymphocytes expressing BATF accounted for 4.10% (2.27%,8.17%) of CD8+ T lymphocytes in ATB group, which was higher than 0.55% (0.34%,1.18%) in LTBI group and 0.84% (0.41%,1.29%) in HC group, and the difference were statistically significant (U value were 5.0 and 8.0, respectively, both P<0.01).Furthermore, the percentage of BATF+ PD-1+ CD4+ T lymphocytes in the peripheral blood of ATB was significantly higher than those in the peripheral blood of LTBI and HC, the difference were statistically significant (Uvalue were 16.0 and 14.5, respectively, both P<0.01), and the percentage of BATF+ PD-1+ CD8+ T lymphocytes in the peripheral blood of ATB was significantly higher than those in the peripheral blood of LTBI and HC, the difference were statistically significant (Uvalue were 10.0 and 16.5, respectively, both P<0.01).In addition, there was a positive correlation between the percentage of BATF+ T cells and PD-1+ T cells, both in CD4+ T cells (r=0.676,P=0.016) and CD8+ T cells (r=0.610,P=0.035).The expressions of BATF both in CD4+ T cells and CD8+ T cells were decreased followed by blockade of PD-1/PD-L pathway (P<0.05).Conclusions BATF may be involved in the regulation of immune pathogenesis of tuberculosis.In order to provide a theory for anti-tuberculosis immunotherapy fargeting BATF, further research need to be proceeded.
9.Prevention of wrong absorption of pancreatic pseudocyst using guided transmural drainage under endoscopic ultrasonography

Objective To discuss how to preven wrong absorption of pancreatic pseudocyst using guided transmural drainage under endoscopic ultrasonography.Methods Retrospectively analyzed the clinical data of patients with pancreatic pseudocysts underwent operations of Endoscopic ultrasonography guided transmural drainage.Results 16 cases of pancreatic pseudocysts were finished with guided transmural drainage under endoscopic ultrasonography using needling to place bypass grafting with successful rate 100.0% of guided transmural drainage by needling. After needling, 2 cased happened regurgitation which led to wrongly absorbed, rate of occurrence is 12.5%. Generally, pancreatic pseudocysts of 16 cases disappeared completely with cure rate 100.0%. From above, stents were pulled out by endoscope in 3 cases while stents were removed voluntarily in another 13 cases.Conclusion Dorsal elevated position, detailed operation and esophageal annular tubes can effectively prevent wrong absorption of guided transmural drainage under endoscopic ultrasonography of pancreatic pseudocysts.
10.Analysis of the correlation between intelligence and social mood for children with expressive language disorder

ObjectiveTo explore the characteristics and correlation of intelligence development and socioemotional development in 1~3 years old children with expressive language disorder,and provide a basis for early intervention.Methods125 cases with expressive language disorder(language disorder group) and 126 normal children (normal group) are evaluated in two parts (intelligence and social mood) with 0~6 years old neuropsychological development diagnosis scale and Chinese version of urban infant-toddler social and emotional assessment.Results (1)The scores in language disorder group were lower than normal group in the area of free movements (81.60±10.40 vs 89.62±7.94),adaptability (81.48±10.95 vs 91.25±8.89),language (67.46±7.20 vs 89.13±8.24),social behavior (76.61±9.73 vs 90.12±8.13) and total developmental quotient (80.17±6.39 vs 91.15±6.05) (P<0.05).(2) There were significant differences between language disorder group and normal group in the area of externalizing behavior(56.28±10.15 vs 53.57±7.91),deregulation (56.45±10.61 vs 51.11±9.32) and capabilities(46.79±9.08 vs 51.25±7.47) (P<0.05).The boys' scores in internalizing behavior were lower than the girls' (49.19±10.76 vs 54.71±9.90) (P<0.05).(3) The scores of gross motor had positive correlation with externalizing behavior (r=0.220,P<0.05).The scores of language had negative correlation with externalizing behavior and deregulation(r=-0.650,P<0.05;r=-0.470,P<0.05).The scores of social behavior had negative correlation with externalizing behavior (r=-0.208,P<0.05).There was also a negative correlation between total development and deregulation (r=-0.184,P<0.05).(4) Multiple stepwise linear regression results showed that the externalizing behavior domain and mothers' education could predict 44.1% of variances in the language area.ConclusionThe children with expressive language disorder not only show backward in level of language development,but also in the development of other areas,and also bad social behavior and emotional problems.Individualized parenting interventions should be adopted to promote these children's intelligent development in an all-round way.
