1.Risk analysis of upper gastrointestinal bleeding in elderly patients with aspirin and clopidogrel co-therapy

ObjectiveTo explore the current occurrence of upper gastrointestinal bleeding in elderly patients in our hospital and its possible risk factors and treatment program.MethodsAn observational prospective analysis of medical records of 758 consecutive patients was performed.Clinical presentation,etiology and treatment were evaluated.The follow-up ended on the dates of a first occurrence of UGIB,stopping co-therapy,death or 6 months after admitted in the group.Results426 male and 332 female were admitted in the group,UGIB occurred in 48 patients.PPI,H2RA,gastric mucosa protectant plus PPI,gastric mucosa protectant plus H2RA and control were prescribed in 108,240,70,102,238 patients respectively.There were statistics difference in each group compared with control in bleeding rate.ConclusionThe occurrence of UGIB associated with A + C co-therapy in elderly was 6.3%.Co-prescription with PPI and gastric mucosa protectant was associated with a lower risk.
2.Study on the Pharmaceutical Effect of Surfactants on the Sucralfate Capsule

OBJECTIVE:To study the effect of surfactants on the pharmaceutical index of sucralfate capsule and to opti?mize its dosage used in the formula.METHODS:The sucralfate capsule was prepared with different surfactants respectively.The disintegration,dissolution and dispersibility of the preparations were determined and their stripping curves were fitted with Weibull distribution model.Different surfactants and the effects of different dosages of which on the dissolution of su?cralfates were compared.RESULT:The sodium lauryl sulphate had the biggest promotion effect on the dissolution of sucral?fate.Optimal indexes of the capsule were achieved when the addition of sodium lauryl sulphate was2%.CONCLUSION:It is advisable to adopt2%sodium lauryl sulphate as the surfactants for the sucralfate capsule.
3.X-ray expression of chest in uremia

Objective To improve the understanding of X-ray expression of chest in uremia.Methods X- ray expression of chest in 40 cases with uremia were analysed retrospectively.Results 18 eases were interstitial ede- ma of lung,5 cases alveolar edema,17 cases thoracic effusion,32 cases heart image enlargement.Conclusion Chest x-ray film is the convenient and effective method in diagnosis of the chest expression abnormity of uremia and it can guide the clinical treatment of cases with uremia.
4.The clinical effect on hemiplegic muscle spasticity with Baclofen treatment plus comprehensive rehabilitation therapy

Objective To assess the effects of Baclofen plus comprehensive rehabilitation therapy in treating patients with hemiplegic muscle spasticity after cerebral trauma and stroke. Methods Thirty-four patients were divided randomly into two groups. Seventeen patients of therapy group were treated by combining Baclofen and comprehensive rehabilitation therapy, and 17 patients of control group were treated only with comprehensive rehabi-litation therapy. The muscular tension and self-care ability between pretreatment and post-treatment in two groups were assessed by Modified Ashworth Scale and Modified Barthel Index. Through systematical isolation and purification of cellulase from Aspergillus niger using the technique of (NH_4)_2SO_4 fraction ation, gel filtration and ion exchange chromatography, we obtained seven components of endo-?-glucanase (Ⅰ-2a,Ⅰ-2b,Ⅰ-2c, Ⅰ-2d, Ⅱa, Ⅱb, Ⅲa) All of them showed homogeneity in the dise gel electrophresis.

Age-related macular degeneration (AMD) is one of the most important diseases that cause vision loss among older patients in developed countries.As the progress of population aging in China,the incidence of AMD is increasing.The activation of the complement system plays an important role in AMD pathogenesis.Complement factor Ⅰ (CFI) is an important complement regulator.In recent years,there have been numerous studies about CFI single-nucleotide polymorphisms (SNPs) and AMD.The mechanism of CFI and the association between CFI polymorphisms and AMD were reviewed in the article.
7.Correlation factor analysis of intravenous immunoglobulin with no response to treatment in Kawasaki Disease

Objective To study treatment has no risk factors associated with response to treatment with intravenous gamma globulin (IVIG)protein in children with Kawasaki disease,so as to provide basis for clinical early treatment scheme.Methods The clinical data of 187 cases of Kawasaki disease in children were retrospective ana-lyzed.Patients were divided into sensitive group (160 cases)and non sensitive group (27 cases)according to the sensitivity of IVIg treatment,differences in baseline data were compared between the two groups of children admitted to hospital.Related factors were screened by clinical manifestations and logistic regression analysis.Results In chil-dren of non sensitive group,white blood cell count (WBC),neutrophil cell count (N),hemoglobin (HB),C -reac-tive protein (CRP),erythrocyte sedimentation rate (ESR),albumin (ALB),total bilirubin (TB)and immunoglo-bulin (IgG)values were determined respectively of (18.4 ±5.4)×109 /L,(0.72 ±0.16)×109 /L,(99.2 ±11.3)g/L, (108.4 ±43.2)mg/L,(75.5 ±21.8)mm/h,(29.8 ±3.1)g/L,(21.6 ±9.3)μmol/L and (11.8 ±3.4)g/L.The data of sensitive group were (15.1 ±4.3)×109 /L,(0.62 ±0.13)×109 /L,(109.5 ±10.4)g/L,(73.5 ± 41.5)mg/L,(55.3 ±19.4)mm/h,(33.8 ±3.5)g/L,(12.9 ±8.8)μmol/L and (7.6 ±3.5)g/L.The difference between the two groups was statistically significant (t =3.548,3.570,4.701,4.018,4.915,5.578,4.713,5.791,all P <0.05).Logistic regression analysis results showed that ESR (OR =2.083),TB (OR =1.632),CRP (OR =1.592)were three main risk factors of IVIG in the treatment of children with Kawasaki disease,and high Hb (OR =0.743),Alb (OR =0.447)could reduce the risk of IVIG treatment.Conclusion In the first visit,higher WBC,N, CRP,ESRTBand IgG levels may increase the reactive risk of IVIG treatment in children with Kawasaki disease.
8.Monitoring and analysis of use of non-national immunization program vaccines in Fujian Province from 2014 to 2020

Objective To monitor and analyze the use of non-national immunization program(nNIP) vaccines in Fujian Province from 2014 to 2020,and provide a reference for improving the management of nNIP vaccination service.Methods The nNIP vaccination data from 2014 to 2020 were extracted from the National Immunization Program Information System(NIPIS),and analyzed by descriptive epidemiological method.Results From 2014 to 2020,a total of 28 835 834 doses of nNIP vaccines were administered in Fujian Province,with an average of 1 015 doses per 10 000 population,and the varieties used increased from 22 to 28.The annual administered doses of nNIP vaccine,the proportion in total doses of all vaccines and the number of vaccinations per capita basically increased year by year.The most widely used varieties mainly include varicella attenuated live vaccine(VarV),hepatitis B(HepB) vaccine,rabies vaccine(RabV) for human use,haemophilus influenza type b(Hib) polysaccharide conjugate vaccine,influenza vaccine(InfV) and enterovirus 71(EV71)inactivated vaccine.The three vaccines with the highest replacement rates were hepatitis A vaccine,group A meningococcal vaccine and group A and C meningococcal vaccine,with the replacement rates of 22.42%,10.32% and 9.89%respectively.Conclusion There are more and more types and doses of nNIP vaccines used in Fujian Province,so it is necessary to further strengthen monitoring and management and improve vaccination service.
9.Comparison of long-term effects between procedure for prolapse and hemorrhoids and Milligan Morgan hemorrhoidectomy in the treatment of Ⅲ and Ⅳ degree of hemorrhoids

Objective To compare the long-term effects between procedurefor prolapse and hemorrhoids(PPH) and Milligan-Morgan hemorrhoidectomy( MMH ) in the treatment of Ⅲ and Ⅳ degree of hemorrhoids. Method The clinical data of 43 cases with Ⅲ and Ⅳ degree hemorrhoids received PPH (PPH group)and 71 cases received MMH (MMH group) were analyzed retrospectively from October 2001 to July 2003. Results Late bleeding in 5 cases of PPH group and no one of MMH group (P <0.05).Hemorrhoidal recurrence prolapsed in 9 cases of PPH group and 5 cases of MMH group (P < 0.05). The rectal neck pressure preoperative,after 6 months, 1 year and 3 years were (122.0 ± 11.3), (90.0 ± 10.4),(103.0 ± 13.8) and (113.0 ± 11.2) cm H2O (1 cm H2O =0.098 kPa) of PPH group,after 6 months was decreased obviously than preoperative (P<0.05),and they were ( 126.0 ± 13.5), (91.0 ± 12.4), (78.0 ±9.8) and (81.0 ± 7.5 ) cm H2O of MMH group,after 6 months, 1 year and 3 years were decreased obviously than preoperative (P < 0.05). Conclusion PPH is safety and effect at short time,but it brings more late bleeding and anal pain,and its rate of prospective hemorrhoidal recurrence is more higher than MMH.
10.Clinical manifestations of childhood chaotic atrial tachycardia

Objective To study the clinical characteristics of children with chaotic atrial tachycardia (CAT),and to improve its clinical diagnosis and treatment.Methods Clinical data,follow-up,treatment and prognosis concerning 33 patients with CAT from Sep.2001 to Oct.2013 in Beijing Children's Hospital were analyzed.Results The 33 patients included 23 boys and 10 girls with ages ranging from 3 days to 13 months,5 months on the average.CAT was conformed by electrocardiogram in all patients:multifocal atrial tachycardia defined by 3 distinct P'-waveforms,irregular P'R,RR and P'-P' intervals,and isoelectric baseline between P'-waves.Combined with symptoms of atrial flutter,atrial fibrillation,atrial premature beats,atrial tachycardia,and interior conduction,differences were detected by 24-hour holter electrocardiography.Echocardiographic features indicated mild heart enlargement in 19 cases,medium enlargement in 4 cases,severe enlargement in 3 cases,atrial septal defect in 5 cases,ventricular septal defect in 3 cases,atrial septal defect coupled with patent ductus arteriosus in 1 case,congenital total anomalous pulmonary venous cormection in 1 case,and patent foramen ovale closure in 12 cases.Twenty-three patients had heart failure.Digoxine,amiodarone and Betaloc were administered to the patients for maintaining their heart functions.No antiarrhythmic drug therapy was used in 2 patients.Twenty-six of the patients (78.8%) had normal electrocardiographic sinus rhythm within 3 to 18 months after their discharge from hospital.Three of the patients (9.1%) still had CAT symptoms in their eletroccardiogram,but their heart rates were kept under control during the 12-month follow-up clinical visits.Of 26 children with cardiac enlargement,echocardiograms in 21 cases (80.8%) returned to normal within 1-3 months after their electrocardiograms returned to normal,and for the other 5 cases (19.2%),the recovery took 6-12 months.One patient died and 3 patients did not participate in the follow-up visits.Conclusions CAT is often found in newborns and infants.Its diagnosis relies on electrocardiography and 24-hour holter electrocardiography.Treatment with Digoxine,and/or β-receptor block,and amiodarone according to with the heart functions of patients can restore sinus rhythm,and achieve good prognostic results.
