3.Theaflavin plus bone marrow mesenchymal stem cell transplantation in treatment of steroid-associated femoral head necrosis in rabbits

60%(lot number 20040828, Jiahui, Shanghai, China).METHODS:Rabbit autologous BMSCs were isolated and cultured in vitro.At the third passage, BMSCs at 2?1010/L were combined on 3 cm?2 cm gelatin sponge.Each gelatin sponge contained 60 ?L cell suspension, placed in a cell incubator for 4 hours for further use.In each group, rabbit models of steroid-associated femoral head necrosis were established by liquid nitrogen frozen method.After natural rewarming, a hole was drilled.Rabbits in the model control group were left intact.Rabbits in the decompression group were implanted with gelatin sponge.Rabbits of the BMSC transplantation and theaflavin + BMSC transplantation groups were implanted with gelatin sponge with rabbit autologous BMSCs, and perfused with 250 mg theaflavin three time every day till death.MAIN OUTCOME MEASURES:The following parameters were measured:general conditions of animals;signal changes in the femoral head necrosis region were observed in the hip by magnetic resonance imaging scanning.Histology of femoral head samples was observed.Results of scanning electron microscope observation.RESULTS:After 8 weeks, rabbits became more active than before treatment, and walking way became normal gradually in the BMSC transplantation, theaflavin + BMSC transplantation groups.Four weeks after model induction, magnetic resonance imaging signal region of all groups had no obvious changes, but 8 weeks later, the necrosis signal region of the model control group magnified while it reduced obviously in the decompression group.Low signal region range became slightly small in the cell transplantation group.The low signal region range became significantly small in the theaflavin + BMSC transplantation group, and bone density signal was nearly normal.Compared with other three groups, positive number of empty lacunae significantly reduced in the theaflavin + BMSC transplantation group at 4 and 8 weeks following model induction(P
4.Determination of the percentage of blood chlormezanone by GC/MS for clinical application on acute poisoning patients

Objective To develop a rapid and sensitive method for the determination of the percentage of blood chlormezanone by gas chromatograpy-mass spectrometry(GC/MS)for the clinical application on acute poisoning patients.Methods Chlormezanone in blood was extracted and separated with acetic ether.The acetic ether extractives from the blood were warmed with water at 80℃ and blown to dry with nitrogen gas.The dry extractives were then dissolved in 100?l of ethanol for GC/MS.To set up the linear equation for the determination of blood chlormezanone,draw the standard curve,calculate the extractive yield,detection limit and repeatability of chlormezanone.The GC was equipped with a 30m length,0.25mm I.D.,0.25?m film thickness HP-5MS(5% phenyl-methylpolysiloxane).Helium was used as the carrier gas at a constant flow rate of 1.0mL/min.The 1?l samples was injected GC/MS in split mode of 10∶1.The temperature program:160℃ for 2min,10℃/min up to 280℃ and hold for 10min.The injector,MS quadrupole rods,ion source and transfer line were kept at 250℃,150℃,230℃ and 280℃,respectively.The EI electron impact energy was 70eV.Results The extractive yield with acetic ether for blood chlormezanone was 82.1%,RSD=6.6%,implying that and acetic ether is an ideal extraction solvent.The calibration curve was y=13 852x+140 588,r=0.998 2.The detection limit was 4?g/L(SCAN m/z 30-280)and 0.1?g/L(SIM m/z 98,152,154).The RSD of precision in one day was 2.6%.The RSD of the precision between days was 4.9%.Conclusions The extraction and analysis method is suitable to the diagnosis of the acute poisoning patient of chlormezanone.
5.Diagnostic Value of Peptidylarginine Deaminase-4 in Patient with Rheumatoid Arthritis

Objective To investigate the diagnostic value of peptidylarginine deaminase-4 (PADI-4)detected in patients with rheumatoid arthritis (RA).Methods The levels of PADI-4,anti-CCP antibodies,AKA and APF were detected by ELISA in serum samples from 58 patients with RA,40 other rheumatic disease and 30 healthy individuals.The indicators were evaluate by application of ROC curve analysis,analysis of variance and correlation.Results PADI-4 serum level(2.653±2.719 U/L) of patients with RA was significantly higher than other rheumatic diseases group (0.872 ± 0.292 U/L)and the healthy group (0.793±0.243 U/L),the difference was statistically significant (t=22.732,35.371,P<0.01).Receiver operator curve analysis (ROC)showed an opitium cut off level for PADI-4 at 1.284 U/L,the sensitivity of PADI-4 was 62.1%.The specificity was 91.4% in RA.There was no significant difference between the positive rate of PADI-4 (62.1%)and APF (50%),AKA (56.9%)(χ2=0.322,P=0.570;χ2=1.715,P=0.190),and there was asignificant difference between PADI-4 and anti CCP antibody (χ2=4.161,P=0.041);a positive correlation between PADI-4 and APF,AKA (r=0.652,0.666, P<0.01),PADI-4 and between anti CCP antibody showed no correlation (r=0.122,P=0.357).Conclusion PADI-4 was significantly increased in serum of RA patients in part,PADI-4 has good sensitivity and specificity of RA,a new diagnostic markers might become independent of antibody against CCP in RA.
6.Effect of Yuzhenxifeng Decoction on the brain ferritin in PD mouse models

Objective To study the effect of Yuzhenxifeng Decoction on the brain ferritin in PD mouse models .Methods All ex‐perimental animals were devided into 4 groups:the control group ,the model group ,the positive drug group and TCM group .Prepare PD model mice with MPTP ,then use the immunohistochemical staining technique to observe the change of expression of Fn .Results the results showed that compared with the model group ,movement coordination disorder of rats in TCM group were relieved ;the Fn level of the model group on 6th ,13th ,20th were higher than control group(P<0 .05) ,and were 2 .21 times ,1 .15 times and 0 .36 times higher respectively .Compared with model group mice ,expression of Fn were enhanced in the treatment group .Conclusion Yuzhenxifeng Decoction can improve the expression of Fn in the brain ,which provide the basis for further study on mechanism of the treatment of Parkinson′s disease .
7.Determination of sinomenine and chelidonine in Tong’an Injection by HPLC

AIM:To determine the contents of sinomenine and chelidonine in Tong’an Injection(Caulis sinomenii, Chelidonium majus Linn, etc). METHODS: HPLC was used. The conditions included the gradient elution with methanol-0.1% triethylamine. The detection wavelength was set at 280 nm. RESULTS: The calibration curve was linear in the range of 162-1 620 ?g for the sinomenine and the range of 35-350 ?g for chelidonine, respectively. The average recovery for sinomenine was 99.56% and the relative standard deviation was 0.41%(n=5). The average recovery for chelidonine was 99.46% and the relative standard deviation was 0.62% (n=5). CONCLUSION: The method is simple, rapid and specialty. It can be used for the determination of sinomenine and chelidonine in Tong’an Injection.
8.Analysis of the Application Effects of PDCA Cycle Theory in ADR Monitoring and Management

OBJECTIVE:To explore the effects of PDCA(plan,do,check,action)cycle theory in the ADR monitoring of our hospital. METHODS:The problems of ADR monitoring in our hospital in 2012 were analyzed and intervened by PDCA cycle. The effects of PDCA cycle were evaluated 1 year later. RESULTS:After implementation of PDCA cycle,we had improved the manage-ment system,brought ADR monitoring into the appraisal of clinical pharmacists,established the networking platform of ADR report-ing,strengthened the training of physicians and nurses,the publicty of ADR information and ADR monitoring of important medi-cine. The number of ADR reports increased by 18.37% in 2013,the proportion of new and serious ADR reports obviously in-creased(increasing from 22.28% to 38.42%);in terms of ADR reporting source,the ADR reporting in wards obviously increased (increasing from 8.16% to 43.84%). The proportion of ADR induced by antibiotics or TCM preparation decreased significantly(re-spetively decreasing from 54.52% to 43.84% and 23.03% to 13.30%);in term of ADR clinical manifestations,the proportion of skin and its appendants involved decreased significantly (decreasing from 53.64% to 39.41%). CONCLUSIONS:The application of PDCA cycle obviously improves the management of ADR monitoring in our hospital.
9.Quantification of neomangiferin in rat plasma by liquid chromatography-tandem mass spectrometry and its application to bioavailability study$

Neomangiferin, a natural C-glucosyl xanthone, has recently received a great deal of attention due to its multiple biological activities. In this study, a rapid and sensitive ultra-high performance liquid chroma-tography tandem mass spectrometry (UHPLC–MS/MS) method for the quantification of neomangiferin in rat plasma was developed. Using chloramphenicol as an internal standard (IS), plasma samples were subjected to a direct protein precipitation process using methanol (containing 0.05% formic acid). Quan-tification was performed by multiple reactions monitoring (MRM) method, with the transitions of the parent ions to the product ions of m/z 583.1-330.9 for NG and m/z 321.1-151.9 for IS. The assay was shown to be linear over the range of 0.2–400 ng/mL, with a lower limit of quantification of 0.2 ng/mL. Mean recovery of neomangiferin in plasma was in the range of 97.76%–101.94%. Relative standard deviations (RSDs) of intra-day and inter-day precision were both o 10%. The accuracy of the method ranged from 94.20%to 108.72%. This method was successfully applied to pharmacokinetic study of neomangiferin after intravenous (2 mg/kg) and intragastric (10 mg/kg) administration for the first time. The oral absolute bioavailability of neomangiferin was estimated to be 0.53%7 0.08%with an elimination half-life (t1/2) value of 2.74 7 0.92 h, indicating its poor absorption and/or strong metabolism in vivo.
10.Collapsed femoral head measured by X-ray and CT before hip replacement:study protocol for a single-center, open-label and diagnostic trial

BACKGROUND: To determine the non-or collapsed osteonecrosis of the femoral head (ONFH) and degree of collapse, can help surgeons design reasonable treatment strategy for patients with ONFH. In China, articular cartilage collapse less than or greater than 2 mm, is a basis for choosing palliative surgery or hip replacement. However, the measurement of collapsed ONFH before replacement has not yet been confirmed. OBJECTIVE: To predict the collapse of femoral head before replacement by CT and X-ray examinations, and to compare with the intraoperative excised femoral head specimens.METHODS: This was a single-center, open-label and diagnostic trial, which was finished at the Third Hospital of Hebei Medical University, Shijiazhuang, Hebei Province, China. Eighty-six patients with unilateral ONFH undergoing total hip replacement were recruited, and allocated into three groups, such as X-ray, CT and specimen groups. The X-ray group underwent X-ray examination at anteroposterior pelvic (bilateral hips) at 1 week before surgery, the CT group received CT scan at bilateral hips at 1 week before surgery, and the specimen group was subjected to the removal of affected femoral head, and then directly measured using ruler and vernier caliper. Primary outcome was the sensitivity of thesethree methods for measuring collapsed femoral head. Secondary outcomes were the specificity, positive predictive value,negative predictive value, positive likelihood ratio, and negative likelihood ratio of these three methods for the diagnosisof collapsed femoral head and the diagnostic accuracy; the diagnostic consistence of CT and X-ray examinations; the height of the lateral, middle and external femoral head measured by three methods. The study was approved by the Ethics Committee of the Third Hospital of Hebei Medical University, China, and will be performed in accordance with the guidelines of the Declaration of Helsinki, formulated by the World Medical Association. Written informed consent will be obtained from all participants prior to the trial. DISCUSSION: In this study, the height of femoral head was measured and analyzed in all-dimensional and multi-anglemanner based on intraoperative removed specimens, so as to make an accurate prediction for collapsed femoral head, thus providing imaging reference for surgeons prior to choosing an appropriate operative scheme
