Objective To study the fatal risk factors to the patients with sepsis.Methods The data of 49 patients with sepsis were analyzed,who were consecutively admitted to intensive care unit(ICU)of Peking Union Medical College Hospital within six months.Patients' general states,clinical information,human leucocyte antigen-DR(HLA-DR)in blood monocytes and plasma level of diamine oxidase(DAO)were recorded.Simple logistic regression was done to analyze all the risk factors,and statistically significant variables were selected and used in multivariate and unconditioned Logistic regression analysis.The primary end point was hospital mortality.Results The subjects involved in present study were consisted of 49 patients with sepsis admitted to ICU of the Peking Union Medical College Hospital from Aug.2003 to Jan.2004,averagely aged 60.8?16.9 years;including 32 males(65.3%)and 17 females(34.7%);with APACHEⅡscores 18.6?8.0 and SOFA scores 5.9?3.8.Twenty-one out of the 49 patients(42.9%)were from non-operation department,and the other 28 patients(57.1%)were from operation department.ICU duration was 14.4?12.8 days;eighteen of the 49 septic patients died during hospitalization,with the 28-days mortality of 36.7%.Positive blood cultures(OR=5.237,95%CI 1.041~26.34,P=0.045),decreased platelet count and blood HLA-DR(OR=0.943,95%CI 0.901~0.98,P=0.01),and increased plasma levels of DAO(OR=2.235,95%CI 1.101~4.53,P=0.026)were obviously associated with mortality.The positive blood cultures,decreased blood HLA-DR and increased plasma levels of DAO were independent fatal risk factors for the patients with sepsis.Conclusions The positive blood cultures,decreased blood HLA-DR and increased plasma levels of DAO are high risk for the patients with sepsis at admission.

?AIM: To investigate the corneal high order aberrations in adults with myopia and to study its correlations with myopic degree and keratometry.?METHODS: A total of 63 patients ( 126 eyes ) from the ophthalmology department of our hospital were included in this study during May 2014 to June 2015. Visual acuity, intraocular pressure ( IOP ) , diopter, slit lamp examination, central corneal thickness ( CCT ) , funduscopic examination, corneal topography and corneal total higher order aberrations ( THOAs ) were recorded. Right eye was selected. According to the spherical equivalence ( SE) , 63 eyes were divided into two groups, the mild to moderate myopia group (M-M group, SE<-6. 00D) and the high myopia group (H group, SE≥-6. 00D). The correlations of corneal THOAs with diopter, keratometry, IOP and CCT were analyzed. Kolmogorov-Smirnov Test was used for normality test. Independent-samples t test was used for comparison between two groups. Pearson correlation analysis was for comparison between two continuous variables.?RESULTS: Z44 was found a statistic difference between the M-M group and H group (t=2. 51,P=0. 02). Z53was negatively correlated with Ks(r=-0.4, P=0. 04) in H group.?CONCLUSION: The THOAs parameters are influenced by diopter and keratometry. TCA is negatively correlated with SE and Sph in patients with high myopia, and spherical aberration is positively correlated with keratometry.

Methods SGC7901 cells were treated with 10nmol/L,30nmol/L and 60nmol/L of wortmannin,a specific inhibitor of AKT,for different time periods. Cell viability was estimated by MTT assay. Objective To investigate the role of p38 mitogen-activated protein kinase (MAPK) signal transduction pathway in lipopolysaccharide (LPS)-induced acute lung injury (ALI).Methods Sixty male SD rats weighing 180-230 g were randomly divided into 4 groups: control group (group C, n = 6), ALI group ( n = 24),p38MAPK specific inhibitor SB203580 + ALI group (group SB + ALI, n = 24), SB203580 group (group SB,n =6). LPS 5 mg/kg was injected intravenously via tail vein in group ALI and SB + ALI, while the equal volume of normal saline was given instead in group C. Group SB + ALI received iv injection of SB203580 10 mg/kg via tail vein 30 min before LPS administration. Group SB received injection of SB203580. The rats were sacrificed at 1, 3 and6 h agter LPS administration (T1-3) in group ALI and SB + ALI (8 rats at each time point) andat 1 h after administration in C and SB groups. The lungs were immediately removed for microscopic examination and determination of phosphorylated p38MAPK (p-p38MAPK) expression, the concentration of protein in bronchoalveolar lavage fluid (BALF) and apoptotic index (AI). Another 32 rats were selected and randomly divided into 2 groups for survival study: ALI group and SB + ALI group ( n = 16 each), and then they were treated as mentioned above and observed for 48 h. Results The concentration of protein in BALF, AI and p-p38MAPK expression were significantly increased in group ALI and SB + ALI compared with group C, while decreased in group SB + ALI compared with group ALI ( P ＜ 0. 05 ). LPS-induced pulmonary histological changes were significantly attenuated in group SB + ALI compared with group ALI. The survival rate was significantly decreased in group ALI compgred with group SB + ALI ( P ＜ 0.01 ). Conclusion p38 MAPK signal transduction pathway is involved in LPS-induced ALI, which may be related to the apoptosis in the cells in the lung.

Objective To investigate the effect of combined application of mannitol and monosialoganglioside on neurological function and its efficacy in brain edema after intracerebral hemorrhage. Methods 92 cases of cerebral edema in patients with cerebral hemorrhage in our hospital, were randomly divided into groups, each group of 46 cases, the control group on the basis of the treatment of mannitol (125mL per times, once per 8h) treatment, the study group on the basic of control group received monosialogangliosides (20mg per times, once daily), 10d for a course of treatment, determination of serum indexes, neurological functions were recorded. Results The effective rate of control group was 71.74%, which was lower than 91.30% of the study group, there was significant difference (P < 0.05); compared with control group after treatment , in study group the intracranial pressure, intracranial hematoma, edema decreased, the urine volume increased, National Institutes of Health Stroke Scale (NIHSS) and quality of life score decreased, interleukin (IL-1β), high sensitive C reaction protein (Hs-CRP), IL-6 and IL-8 levels decreased, Na+-K+-ATP enzyme increased, the ratio of cerebrospinal fluid albumin and serum albumin, matrix metalloproteinase (MMP-2) and MMP-9 decreased, there were significant differences (P < 0.05). Conclusion The effect of the combined application of mannitol and monosialoganglioside on cerebral edema after cerebral hemorrhage is exact and could improve neurologic function.

Background High myopia is seriously harmful to visual function.To explore the mechanism of high myopia is significant for the prevention and treatment.Complement proteins C1q and C3 have emerged as critical mediators of synaptic refinement and plasticity,however,their effects in retina on myopia remain elusive.Objective This study was to investigate the expressions of complement factors C1q and C3 in the retina with negative lensdefocused myopia in guinea pigs.Methods The use and care of animals complied with the application of Peking Union Medical College Hospital Iaboratory Animal Welfare Ethics Committee.Twelve 3-day-old pigmented guinea pigs were assigned randomly to the lens-defocused group and the normal control group.The left eyes were covered by -10 D PMMA lens for 4 weeks as the defocused eye group,and the right eyes were covered using 0 D PMMA lens in the same way as the fellow control eye group.The right eyes of the normal guinea pigs were used as the normal control eye group.The refractive diopter of guinea pigs was examined by retinoscopy.The animals were sacrificed at the fourth week and the expressions of complement C1q and C3 in the retinas of guinea pigs were detected by Western blot.Results The diptors were (-1.21±0.71)D,(+2.46±0.75)D and (+1.75±0.50)Din the defocused eye group,normal eye group and the fellow eye group at the fourth week,showing a significant difference among the groups (F=51.55,P=0.69),and diopters were insignificantly different between the normal eye group and fellow eye group (q =2.62,P=0.08).However,the diopters of the defocused eye group were significantly higher than those of the fellow eye group (q =10.92,P＜0.01).The expressions of C1q and C3 proteins in the retinas were significantly different among the defocused eye group,fellow eye group and normal eye group (C1 q:F=8.810,P =0.003;C3:F =14.490,P＜0.001),and expression levels of C1q and C3 proteins in the defocused eye group were significant higher than those of the fellow eye group (C1q:q=4.14,P=0.01 ;C3:q=4.71,P=0.005) ;while no significant differences were found between the fellow eye group and the normal eye group (C1q:q =1.61,P =0.27; C3:q =2.82,P =0.070).Conclusions The expression levels of C1q and C3 up-regulate in the retinas with lens-defocused myopic animal model.Excessive complement activation in retinas may be involved in the development of myopia.
