1.Clinical analysis of 4 cases of amyotrophic lateral sclerosis concomitant with Sj(o)gren' s syndrome

Objective To describe the clinical characteristics of 4 patients with concomitant amyotrophic lateral sclerosis (ALS) and Sj(o)gren' s syndrome.Methods The clinical features,laboratory findings,and electrophysioiogical manifestations etc.of these patients were analyzed.Results Four female patients all developed progressively aggregated weakness in their limb/limbs at the fifth to sixth decades of their lives.According to their neurologic findings,3 met the criteria of clinical probable ALS,while the other was diagnosed with probable laboratory-supported ALS.All these patients showed diffused patterns of neurogenic changes in electromyography tests.Motor symptoms of 2 patients transiently improved after immunomodulation treatments.Conclusion Careful screening of the Sj(o)gren' s syndrome symptoms in patients with ALS is necessary so as to deal with the treatable concomitant disease in time.
2.Comparative Identification of Ajuga decumbens Thunb. from Pogostemon cablin(Blanco.) Benth

Objective To find out evidences for the identification of Ajuga decumbens Thunb.(Jinchuangxiaocao) and Pogostemon cablin (Blanco.) Benth.(Guanghuoxiang) . Methods Fresh stems and leaves of the two species were collected and used as the samples. We used stereoscopy to observe and compare the macroscopic appearance of the original plants, and used light microscope to observe the stem cross-section, petiole cross-section, leaf epiderm and powder of the two species for microscopic identification. Results The results of macroscopic character identification showed that the petiole of Jinchuangxiaocao had narrow wing, while that of Guanghuoxiang had no narrow wing; the angles between leaf main vein and lateral vein of Jinchuangxiaocao were less than 45°, while the angles of Guanghuoxiang were more than 45°. In respect of microscopic identification, Jinchuangxiaocao had the glandular scales with multicellular head, and Guanghuoxiang had the glandular scales with one-celled head; the intercellular space and crystals were found in the cortex of Guanghuoxiang stems and mesophyll, while Jinchuangxiaocao has no such structures. Jinchuangxiaocao had diacytic and anomocytic stomata, and Guanghuoxiang only had diacytic stomata. Conclusion The appearance and microscopic differences can be used for the identification of Jinchuangxiaocao from Guanghuoxiang.

5.Clinical study on plasma orexin-A levels in patients with chronic obstructive pulmonary disease.

Objective To explore the change of plasma orexin-A levels in patients with chronic obstructive pulmonary disease(COPD).Methods Lung function test and blood gas analysis were performed in 15 obese COPD patients,20 COPD patients without obesity,20 obese controls and 20 normal healthy adults(control group).In both obese COPD group and obese group,the body mass index(BMI)was higher than 25 and there was no significant difference in BMI.The plasma was deproteinized by chromatography,and the plasma orexin-A levels were determined by radioimmunoassay.Results The plasma orexin-A levels in the obese COPD group[(8.82?1.90)ng/L]and the non-obese COPD group[(8.69?1.84)ng/L]were significantly higher than those in the obese group[(7.18?1.45)ng/L(P 0.05).Plasma orexin-A levels in patients with COPD correlated negatively with partial pressure of oxygen(PaO_2)(r = -0.527 ,P
6.To intensify our understanding about management of severe burn infection.

Nowadays, it is necessary to emphasize the three basic inseparable elements in the treatment of severe burn infection, which are systemic care, burn wound care, and rational use of antimicrobials topically or systematically. Systemic care has been shifted from simple nutritional support to maintaining the systemic homeostasis, including balancing immune-inflammatory response, and protecting organs from dysfunction. Some work focused on regulating systemic immune response in the initial phase and the balance of inflammatory response after occurrence of severe burn infection have been reported. These results at least broaden our thinking to recognize that treatment should not only destroy microbes, but also balance the response of the body. Escharectomy in earlier phase has been a consensus. Currently, we turn our vision into how to use "damage control surgery (DCS)" concept in management of severe burn. DCS in burn care includes the evaluation of perioperative situation more accurate to make a more appropriate surgical decision. Meanwhile, an overall strategy should be established to confront the rapidly increasing drug resistance of the pathogens. The release of endotoxin after use of antimicrobials, which has been studied widely, should be explored further.

8.Application of Speckle Tracking Imaging in the Evaluation of Chronic Heart Failure

OBJECTIVE:Taking metoprolol succinate sustained-release tablets as an example,to explore the value of speckle tracking imaging in therapeutic efficacy evaluation of CHF drugs. METHODS:68 CHF patients who had never underwent regular treatment were given Metoprolol succinate sustained-release tablets and anti-heart failure agents for 3 months. NYHA classification, resting heart rate(RHR),6 min walk distance(6MWD)and conventional echocardiographic measurements were tested before and 1 and 3 months after treatment. Dynamic image were collected in the apical 2-chamber view,apical 4-chamber view and left ven-tricular long axis plane for speckle tracking imaging offline analysis. The left ventricular global longitudinal strain (LVGLS) were measured. After 3 months of treatment,CHF patients were divided into standard group(group A,RHR 55-60 times/min)and non-standard group (group B,RHR>60 times/min) according to resting heart rate. The improvement of above indicators were com-pared between 2 groups,and that of LVGLS was also compared. RESULTS:After 1 month of treatment,RHR,LVEDd,LVEF and LVGLS of group A were improved significantly,compared with group B,with statistical significance (P<0.05). After 3 months of treatment,there was statistical significance in above indicators between 2 groups(P<0.05). In group A,RHR,6MWD and LVGLS improved significantly after 1 month of treatment,there was statistical significance,compared with before treatment (P<0.05);there was statistical significance in above indicators after 3 months of treatment,compared with before treatment and after 1 month of treatment (P<0.05). In group B,RHR and 6MWD improved significantly after one month of treatment;there was statistical significance,compared with before treatment(P<0.05);there was statistical significance in significant improvement of each indicator between after 3 months of treatment and before treatment (P<0.05);only there were statistical significance in RHR and NYHA grading,compared with after one month of treatment(P<0.05). CONCLUSIONS:Speckle tracking imaging can early reflect the left ventricular function improvement of CHF patients after anti-heart failure treatment,especially in patients with RHR control standard earlier and more significant. Speckle tracking had good clinical application value in the follow-up treatment of heart failure as routine evaluation index.
9.Analysis of death cause in rats with spinal cord injury

Objective To investigate the causes for death in rats after spinal cord injury.Methods A total of 120 adult Wister rats were selected for the study. The animal model with acute spinal injury at T10 was established by using Allen' s combat (25 g · cm). The dissection analysis was performed in death rats. Results Of all, 25 patients died, with mortality rate of 21%. Of death rats, five rats were died before awakening, with no abnormal anatomy; 12 rats died within three days after injury and three died of injuries 3-7days injury. Anatomy found pulmonary bleeding and edema, even hematocele bladder in some rats. There were three rats died within 1-2 weeks, one died of injury only after 2-3 weeks, with lung infection and urinary tract infection. There was no death after three weeks. Conclusions The early causes for death of rats with spinal cord injury is mainly due to lung congestion and pulmonary edema, whereas the leading cause of late death of rats is pulmonary and urinary tract infection.
10.Clinical analysis on 624 elderly patients with acute kidney injury

Objective To investigate the pathogens,clinical characteristic and therapeutic method of acute kidney injury(AKI) Methods The morbidity,composition of pathogeny,staging and prognosis of 624 cases with AKI recruited by our department from January 1999 to December 2009,to evaluate the impacts of possible predisposing factors.Results The mortality rate of AKI was 19.6%(122/624),The mortality in the staging of exhaustion,damage,risk were 37.8%,33.4% and 6.7%,respectively.Of the 624 patients there were 21 cases (3.4%) discharged due to exacerbation,29 cases (4.7%) maintained the status quo,and 452 cases (72.4%) cured or improved;Among 138 patients with blood purification there vere 116 cascs(84.1 %)cured or improved and 22 cases (90.9%) with long term blood purification cured or improved.The pathogenyof the non-elderly group (<60 years old)were original diseases and collapse,and the pathogeny of the elderly group (≥60 years old)were original diseases.Conclusions AKI is serious with high incidence and mortality in hospitalized patients.The efficacy of long-term blood purification is better than common treatment.Early prevention,early diagnosis and active intervention will help to increase survival rate of AKI aud improve prognosis.
