1.Self-management of patient controlled analgesia pump in patients in the intensive care unit

Objective To investigate the condition of self-management of patient controlled analgesia pump in patients in the ICU.Methods 82 patients in ICU were selected in July 2014 and were randomly divided into the observation group and the control group with 41 cases in each.In the observation group,the PCAP was fixed on the bedrails of patients' bed and controlled by themselves to acquire satisfactory analgesia effect.In the control group,the PCAP was operated by the nurses.Pains scores was compared between the two groups.Results The different position of PCAP showed the different scores of pain in the patients.The pain scores in the observation group (2.93±1.47) was significantly lower than that in the control group (3.76±1.36),P<0.05.Conclusions Proper position of PCAP and controlled by patients can effectively reduce postoperative pain of patients in the ICU,and then reduce their suffering and improve their comfort.

5.Effect of intermedin on renal interstitial fibrosis of unilateral ureteral obstruction rats

Objective To observe the effect of intermedin on renal interstitial fibrosis of unilateral ureteral obstruction rats. Methods Forty male Wistar rats were randomly divided into two groups:the sham-operation group (n=10) underwent the left ureteral dissection,the other 30 rats were made as unilateral ureteral obstruction models and divided into the model group (UUO),the losartan group,the IMD group (each group n=10).At the day 14,21 after operation,randomly 5 rats from each group were blooded by abdominal arotic and obstructive kidneys were taken out.The renal histopathological changes were observed by HE and Masson staining.The BUN,Scr,and hydroxyproline (Pro) of the obstructive kideys were measured by colorimetry. The expression of TGF-β1 and intermedin was observed by immunohistochemisty staining. Results Compared with the sham-operated group,the levels of BUN,Scr,Pro,TGF-β1 and IMD in the model group increased (P<0.05).Compared with the UUO group,the levels of Scr,BUN,Pro,TGF-β1 and IMD in the losartan group were lower (P<0.05).However,IMD in the IMD group was significantly up-regulated (P< 0.05),the others were down-regulated (P<0.05). Conclusion IMD can improve the renal interstitial fibrosis,and the mechanism maybe antagonizes the TGF-β1.
6.Risk factors of chronic kidney disease in elderly patients with type 2 diabetes mellitus

Objective To investigate clinical characteristics and risk factors in elderly patients with type 2 diabetes mellitus (T2DM) complicated with chronic kidney disease (CKD).Methods Two hundred and two T2DM patients aged ≥60 y admitted in Tongren Hospital from August 2009 to February 2012 were enrolled in the study,including 64 cases complicated with CKD (CKD group)and 138 cases without CKD (non-CKD group).Hemoglobin (Hgb),albumin (ALB),glycated hemoglobin A1C (HbA1 c),fasting plasma glucose(FBG),postprandial glucose (PBG),serum creatinine (Scr),blood urea nitrogen (BUN),uric acid (UA),total cholesterol (TC),triglyceride (TG),high densitylipoprotein (HDL-C),low density lipoprotein (LDL-C) were measured and ratio of urinary albumin to creatinine (UAER/CR) were calculated,ankle brachial index (ABI) and ambulatory blood pressure was examined.Results The duration of diabetes in CKD group was longer than that in non-CKD group [(13 ± 11) vs.(10 ± 7) y,t =2.35,P < 0.05].Hgb and ALB in CKD group were lower than those in non-CKD group [(12.1 ± 1.7) vs.(13.3 ±1.3) g/L,(34.4±2.6) vs.(36.8 ±3.4) g/L,t=-5.73,-4.75,allP<0.01]; UA,TG,24 h systolic blood pressure (SBP),24 h pulse pressure (PP)in CKD group were higher than those in non-CKD group [(367 ± 81) vs.(323 ± 74) μmol/L,(1.6 ± 0.9) vs.(1.3 ± 0.7) mmol/L,(133 ± 14) vs.(127±16)mmHg,(65 ±12) vs.(58 ±12)mmHg,t =3.82,2.50,2.47,3.68,all P<0.1].The UAER/CR in CKD group and non-CKD group was 86.2 mg/g (24.6-374.8 mg/g) and 6.0 mg/g (3.6-17.8 mg/g),respectively (P < 0.01) and ABI in CKD group and non-CKD group was 0.94 ± 0.18 and 1.03 ±0.18 (-2.78,P <0.05).There were no significant differences in FBG,PBG,HbA1c,TC,HDL-C andLDL-C between two groups(t =-2.02,1.47,1.48,-0.53,-1.20,-0.76,all P>0.05).Logistic regression analysis showed that ALB,TG and UAER/CR were the independent risk factors of CKD in T2DM patients(OR:0.79,3.63 and 1.04,95% CI:0.62-1.01,1.45-9.01 and 1.00-1.08).Spearman correlation analysis demonstrated that eGFR was positively associated with Hgb,ALB,ABI (r =0.38,0.33 and 0.28,all P < 0.01),negatively associated with age,BUN,Cr,UA,TG,UAER/CR,24 hSBP and PP in all diabetes patients(r =-0.32,-0.63,-0.89,-0.34,-0.18,-0.65,-0.16 and -0.248,all P <0.01).Conclusions ALB,TG,UAER/CR are the independent risk factors of chronic kidney disease in elderly patients with type 2 diabetes mellitus.
7.Determination of Lead in Air With Hydride Generation Atomic Fluorescence Spectrophotometry

Objective To establish a method to determine lead in air. Methods The filter film used to collect the lead in air was digested with acid followed by adding mixed solution of potassium ferricyanide,oxalic acid and hydrochloric acid, then double ways atomic flurescence spectrophotometry was employed to determine lead. Results The optimal concentration of hydrochloric acid, potassium ferricyanide and oxalic acid were 0.12 mol/L, 8 g/L and 0.4 g/L respectively. The detection limit of lead was 1 ?g/L, the relative standard deviation was 2.5%. The recovery rates ranged from 90% to 95%, the linar range was 10-80 ?g/L. Conclusion This method was simple, rapid, accurate and sensitive. It was suitable for determining lead in air.
8.Imaging of intracranial neuronal and mixed neuronal-glial tumours

Objective To investigate the characteristic clinical, imaging, and pathologic findings of intracranial neuronal and mixed neuronal-glial tumours. Methods The imaging findings of surgery and pathobiology proved intracranial neuronal and mixed neuronal-glial tumours in 14 cases (7 male and 7 female, ranging in age from 6-56 years; mean age 33.8 years) were retrospectively analyzed. Results Eight gangliogliomas were located in the frontal lobe (4 cases), temporal lobe (1 case),fronto- temporal lobe(2 cases), and pons (1 case). They appeared as iso-or low density on CT, iso-or low signal intensity on T 1WI, and high signal intensity on T 2WI on MR imaging. Two central neurocytomas were located in the supratentorial ventricles. Four desmoplastic gangliogliomas were seen as cystic masses, appearing as low signal intensity on T 1WI and high signal intensity on T 2WI Conclusion Intracranial neuronal and mixed neuronal-glial tumours had imaging characteristics. Combined with clinical history, it was possible to make a tendency preoperative diagnosis using CT or MR.

10.Expressions of EphB4 and HIF-1? in human gastric carcinoma and their biological implication

Objective To investigate the expressions of EphB4 receptor and hypoxia inducible factor 1? (HIF-1?) protein in human primary gastric carcinoma, and to study the biological implication of their expression. Methods The expressions of EphB4 receptor and HIF-1? protein were detected in 74 specimens of human primary gastric carcinoma (gastric carcinoma group) and in 13 normal gastric mucosa tissues (control group) by immunohistochemistry. Correlations between the expression of these proteins, and clinical and pathological features were respectively analyzed. Result High expressions of EphB4 receptor and HIF-1? protein were found to be 62.2% (46/74) and 52.7% (39/74) respectively in gastric carcinoma group, while the expressions of EphB4 receptor and HIF-1? protein in control group were 15.38% (2/13) and 0% (0/13), respectively.
