Dysregulated cell cycle-mediated cell transformation and uncontrolled cell growth are some of the fundamental biolog-ical features of malignant tumors. Thus, understanding the mechanism(s) involved in the cell cycle will facilitate the discovery of the na-ture of tumor initiation and progression as well as the molecular mechanism of tumor malignancy. Consequently, biomarkers and drug targets for early diagnosis and treatment would be obtained. This study was designed to analyze in detail the key regulators of cell cycle and their importance in carcinogenesis and clinical prevention, diagnosis and treatment, and anti-cancer drug discovery. The biological impact of the dysregulated cell cycle on tumor occurrence and malignancy development is systematically described. The theoretical and practical significance of the cell cycle regulation in tumor diagnosis and therapy are also provided.

Objective To investigate the effect of fluorouracil for injection on methylene tetrahydrofolate reductase ( MTHFR ) , glutathione S-transferase ( GST) and tumor markers in patients with advanced gastric cancer.Methods Forty-eight cases with advanced gastric cancer were selected from August 2013 to August 2014 in the hospital and divided into two groups.The control group (n=24) were treated with capecitabine plan, and the experiment group ( n=24 ) were treated with fluorouracil for injection plan.MTHFR concentration, GST activity and tumor markers [ carbohydrate antigen-199(CA199), carcinoembryonic antigen (CEA), alpha fetoprotein(AFP)] levels were compared before and after treatment.ResuIts Compared with control group, MTHFR concentration of experiment group was higher (P<0.05), GST activity was lower (P<0.05), the tumor markers levels were lower (P<0.05), total effective rate of clinical symptoms was higher [16(66.67) vs.22(91.67), χ2 =4.55, P<0.05], and incidence of adverse reactions was lower [14(58.33) vs.6(25.00), χ2 =5.49, P<0.05].ConcIusion Fluorouracil for injection chemotherapy could increase the serum MTHFR concentration, reduce GST activity, and its clinical curative effect is obvious with less adverse reactions, which has important significance in treatment of patients with advanced gastric cancer.

Objective To investigate the clinical value of single-incision laparoscopic inguinal high ligation in children with recurrent inguinal hernia . Methods Clinical data of 30 pediatric recurrent inguinal hernia cases treated with single-incision laparoscopy in our hospital between January 2011 and February 2016 were analyzed retrospectively .The patients were aged 1-11 years old (mean, 6.1 ±3.3 years old).For indirect inguinal hernia, a non-absorbable suture was placed around the extra-peritoneal space of the inguinal inner ring by means of a needle with a thread hole and a hooked needle under laparoscope , and the hernial sac was highly ligated by knotting the purse string suture .If a direct inguinal hernia was identified , a similar method was used to ligate the hernial sac , and the umbilical artery fold was sutured to the Hesselbach ' s triangle in order to enhance the weak tissue . Results After surgeries, we found 28 cases were recurrent indirect inguinal hernia (4 cases with contralateral concealed hernia ), and the other 2 cases were direct inguinal hernia .The operation time ranged from 12 to 45 min (mean, 21.4 ±8.2 min).All th cases recovered with inconspicuous scars , and no recurrence or complications were found during a follow-up for 2-53 months ( mean, 23.9 ±12.5 months). Conclusions Single-incision laparoscopic inguinal high ligation is a safe and effective procedure for children with recurrent inguinal hernia .This procedure does not cause obvious scars for patients , and can diagnose and treat the occult hernia and direct hernia at the same time .

Objective To investigate plasma homocystein (Hcy) level in cardiac transplant patients.Methods Fourteen cases of heart transplant patients were recruited in this study. Plasma Hcy levels were measured by using high-performance liquid chromatography with fluorescence detection and flow-mediated dilatation in brachial arteries detected with high-resolution ultrasound in all patients, while coronary artery angiography were performed in 7 patients.Results In 2 cases of heart transplant who had single coronary artery stenosis, the plasma Hcy levels were above 12 ?mol/L. The average plasma Hcy levels in heart transplant patients were higher than in health volunteers [( 13.47? 2.78) ?mol/L vs (9.26? 3.57) ?mol/L], and flow-mediated dilatation in brachial arteries of heart transplant patients was lower than that of health volunteers [( 8.2? 3.7) % vs ( 12.5? 1.6) %]. There was a linear correlation between levels of plasma Hcy and flow-mediated dilatation (r= -0.804). Conclusion Endothelial dysfunction in the heart transplant patents might be contributed to hyperhomocysteinemia which may be potential cause of transplant coronary artery disease.

Objective To explore the effect and mechanism of fluvastatin preventing and curing restenosis. Methods 48 rabbits were randomly divided into 3 groups. In control group, rabbits were given basic food. In balloon injury group, rabbits were given basic food and balloon injury of general artery on right neck. In fluvastatin balloon group, rabbits were given basic food,ballon injury of general artery and 10mg?kg -1 ?d -1 fluvastatin. The expression of MMP-9 and TIMP-2 of mRNA was detected at 3, 7, 14 and 30 days after injury respectively by method of in site hybridization. Results There was little expression of MMP-9 and TIMP-2 mRNA in control group, and the expression of MMP-9 and TIMP-2 mRNA in middle membrane of blood vessels began at 3 days after blood injury, and reached maximum at 7 days after injury. There was a little expression of MMP-9 and TIMP mRNA in inner membrane of blood vessels at 14 and 30 days after injury, and MMP-9 expression significantly decreased after the fluvastatin interference(P

Objective To investigate the feasibility of laparoscopy assisted Meckel’s diverticulum resection in children. Methods Meckel’s diverticulum resection was performed under laparoscope in 12 children from October 2000 to April 2002.Two 3 mm incisions were made on the McBurney point and the midpoint between the umbilicus and the pubis,respectively.Three 3 mm trocar were introduced.When the diverticulum was found,the incision was prolonged to 2 cm long along the inferior border of the umbilicus.The lesion was removed externally and the intestine was anastomosed. Results All operations were completed smoothly under laparoscope. The intraoperative blood loss was 2～3 ml and the operation time was 1 2～2 5 h.The diverticulum was 1 cm?2 cm～3 cm?4 cm in size.The patients were dismissed 5～7 days after surgery,without obvious scar formation. Conclusions Laparoscopic Meckel’s diverticulum resection is feasible, with the advantages of the minimal invasion, rapid recovery and unobvious scars.

Objective To discuss the blood substitutes during the perioperative period for hemophilic children with surgical disease as well as the coping strategies to its postoperative complications.Methods A retrospective study between 2003 and 2013 from one our centre identified a total of 41 perations performed in haemophiliac patients.33 patients were diagnosed with haemophilia A,among whom 10 were severe cases,13 moderate cases and 8 mild cases.8 patients were diagnosed with haemophilia B,among whom 2 were severe cases and 6 were mild cases.Different kinds of operation were required for each case.Results According to the monitoring of the coagulation tests PT and APTT before and after the operation respectively,after the use of the blood substitutes such as FV Ⅲ,PPSB and fresh frozen plasma,38 patients underwent surgical treatment successfully and had full recovery without any surgical complications,1 patient was dead,1 suffered intraperitoneal hemorrhage and 1 had delayed wound healing.Conclusion Full preparation and thorough plan before the operations,combined with appropriate blood substitutes can effectively reduce postoperative bleeding for hemophilic children.

Objective To assess the efficacy and safety of 308-nm excimer laser combined with topical tacrolimus 0.03％ cream for the treatment of childhood vitiligo on the face or neck.Methods Sixty-eight children aged ＜ or =14 years with vitiligo on the face or neck were enrolled in this study,and divided into two groups based on lesional sites,i.e.,skin group with lesions on the forehead,cheek,lower mandible,neck or in the periortic region and mucosa group with lesions in the perioral or periorbital region.All the patients received 308-nm excimer laser irradiation once or twice weekly and topical tacrolimus 0.03％ cream twice daily.Twenty sessions of irradiation served as a treatment course.Efficacy was evaluated at the end of the treatment course.The effects of erythema reaction and cumulative irradiation dose on efficacy were assessed.Results After 20 times of laser therapy,an excellent response was achieved in 78.4％ of the patients in the skin group,and 54.8％ in the mucosa group (P ＜ 0.05).During the treatment,the average frequency of erythema reaction lasting 48 hours or longer was significantly lower in the skin group than in the mucosa group (5.84 vs.9.12,P ＜ 0.01).After 10 and 20 times of laser therapy,the cumulative radiation dose was 4215 mJ/cm2 and 10 453 mJ/cm2 in the skin group,respectively,compared to 3364 mJ/cm2 and 7430 mJ/cm2 in the mucosa group,respectively (both P ＜ 0.01).Conclusions The 308-nm excimer laser combined with topical tacrolimus cream is effective and safe for the treatment of childhood vitiligo on the face or neck.Vitiligo on the forehead,cheek,lower mandible,neck and in the periortic region appears to respond better to this therapy with a weaker erythema reaction than that in the perioral or periorbital region.

Objective To observe the curative effect of narrow-band ultraviolet(NB-UVB)on stable vitiligo with autologous epidermis grafting.Methods A total of 82 patients with stable vitiligo were recruited and randomly divided into a control group and a treatment group.The patients in the treatment group were pretreated with NB-UVB before autologous epidermis grafting,while those in the control group were not.The pretreatment with NB-UVB was implemented twice a week for at least 4 weeks before the autologous epidermis grafting. A 3-month follow-up observation was conducted with regard to the effectiveness. Results Pretreatment with NB-UVB could significantly increase the survival rate of skin grafts and rate of repigment of leukoplakia,when compared with autologous epidermises transplantation alone.The rate of complete repigment raised significantly(P＜0.05),and so did the curative effect for vulgaris generalized vitiligo(P＜0.029).It was found that the curative effect on vitiligo was closely related to the frequency of NB-UVB radiation,location of leukoplakia,color of skin,and the time of repigment appearance. ConclusiOn Pretreatment with NB-UVB can significantly improve curative effect of autologous epidermis grafting on vitiligo.
