Objective To evaluate the multi-slice spiral CT (MSCT) features of crush pulmonary parenchymal injuries in the massive Wenchuan earthquake. Methods One hundred and seventeen patients with crush pulmonary parenchymal injuries in the massive Wenchuan earthquake were reviewed, the location, extent and MSCT appearances of the injuries and associated thoracic injuries were evaluated. The MSCT appearances of the injuries on 1 d, 2--5 d and 6--14 d were compared using Chi-square test. Results One hundred and thirteen patients had pulmonary contusion and bilaterally in 70 cases, 7 had pulmonary laceration. A total of two hundreds and eighty-three lung lobes were involved and 3 or more lobes involved in 47 patients. Ill-defined patchy opacity was detected in 75 patients, associated ground-glass opacity (GGO) in 24 cases. Lung laceration presented as pneumatocele in 4 cases and demarcated hematoma in 3 cases. Coexisted rib fractures were detected in 98 patients and flail chests were in 29 patients. Forty-three patients had coexisted fractures of scapula, clavicle and (or) sternum. Ninety-nine patients had coexisted pleural injuries. Conclusions Crush pulmonary parenchymal injuries in the Wenchuan earthquake are severe. The bilateral, multifocal and multiple lobes involved injuries with a high incidence of coexisted other severe thoracic injuries are the noticeable features.

AIM and METHODS: To determine the role of different K +-channels in attenuation of vasoreactivity of intrapulminary artery rings induced by chronic hypoxia. RESULTS: ①Acute hypoxia-induced pulmonary vasoconstriction(HPV) could be significantly attenuated by chronic hypobaric hypoxia for 15 days and for 30 days. ②HPV could be significantly potentiated by ATP-sensitive K +-channel (K ATP ) blocker or Ca 2+ -activated K +-channel (K Ca )blocker, and the potentiated scope in chronic hypoxic group was much higher than that observed in control group. ③ Delayed rectifier K +-channel (K DR )blocker showed no effect on HPV in both control group and chronic hypoxic group.CONCLUSIONS: Both K ATP and K Ca play an important modulating role in HPV and it's potentiation may be a critical mechanism for the attenuated vasoreactivity to acute hypoxia; following chronic hypobaric hypoxia.

Objective To explore the mechanisms and effects of adoptive immunotherapy with ovarian cancer vaccine modified by GM-CSF gene which was used after immunologic reconstitution during lymphopenia induced by chemotherapy. Methods Lymphopenia was induced by chemotherapy with cyclophosphamide. The immune reconstituted model was built in rats. The tumor vaccine draining lymph nodes were harvested after the ovarian cancer cells NUTU-19 modified by GM-CSF gene were injected. The effector T cells (T_E) were got after being stimulated and amplified.Enzyme-linked immunosorbent assay was used to detect the level of interleukin (IL)-2 and IL-4 secreted by T_E.Intracellular cytokine staining was used to determine frequency of tumor-specific T_E.Fluorescence-activated cell sorting (FACS) was used to detect the special cytotoxicity ofT_E killing target cells. The survival period of rats bearing pre-established abdominal ovariam carcinoma after being adoptively transferred byT_E.was observed. Results Compared with those in control group, the significant higher levels IL-2[(65.7±4.0) pg/ml]and lower levels IL-4 [(277±49) pg/ml]were observed in chemotherapy-immune recunstitution-vaccine immunization group. The amount of CD_4~+ T cells secreting interferon-γ (13.0±2.1)% were also significantly increased. The rate of the special cytotoxicity of killing T cells (86.5±1.1) % was markedly improved. The survival period of rats (110±16) days was increased in chemotherapy-immune reconstitution-vaccine immunization group. Conclusions The combined immunotherapy of chemotherapy-immune reconstitution-tumor vaccine immunotherapy may increase the frequency and function of specific tumor T_E.The specific cytotoxicity is increased and the weak reaction of T_E to tumor is improved, which showed that this therapy can enhance immune reaction.

Background and objective Pre-operative assessment of mitral valve(MV)anatomy is essential to surgical design in patientsundergoing MV repair.Although 2-dimensional(2D)echocardiography provides precise information regarding MV anatomy,RT-3DTEE could increase the understanding of MV apparatus and individual scallop identification. We aimed to investigate the value of RT-3DTEE in MV repair. Methods RT-3DTEE was performed in six patients with mitral valve prolapse(MVP) by using Philips IE33with X7-2t probe.Preoperative RT-3DTEE studies were compared with surgical findings in patients undergoing surgical mitral valverepair, and quantitative evaluation was performed by QLab 6.0 software before and after surgicalmitralvalve repair. Results RT-3DTEE could display dynamic morphology of MV,the location of prolapse, and spatial relation to the surrounding tissue. It couldprovide surgical views of the valves and the valvular apparatus. These resuIts were consistent with surgical findings. The quantitativeevahuation before and after surgical MV repair indicated that anterolateral to posteromedial diameter of annalus.anterior to posteriordiameter of annulus, perimeter of annullus.,and area of annalus in projectionplane were significantlv smaller after operation comparedwith those before operation(P<0.05).The length of posterior leaflet,,the area of anterior and posterior leaflet, the maximal prolapseheight, the volume of leaflet prolapse and the length of coaptation in projection planewere significantly reduced after operation(P<0.05).Conclusion RT-3DTEE is a unique new medality for rapid and accurate evaluation of mitral valve prolapse and miwal valverepair.
