1.Effects of Compound Lidocaine Cream on Postoperative Pain After Endoscopic Sinus Surgery

Objective To explore the effects of compound lidocaine cream applied to postoperative wound and nasal packing on postoperative sinus pain after endoscopic sinus surgery . Methods From April 2015 to June 2016, 100 cases of endoscopic sinus surgery were carried out in our hospital under general anesthesia .The patients were randomly divided into two groups , with 50 cases in each group .The group A was given compound lidocaine cream 4 g on the nasal surgery wound and medical inflation sponge for intranasal packing;the group B was given saline 4 g on the nasal surgery wound and medical inflation sponge for intranasal packing . The heart rate (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP) were measured at the time of extubation (T0), 1 h after extubation (T1), 4 h after extubation (T2), 10 h after extubation (T3), and 12 h after extubation (T4), respectively.The visual analogue scale (VAS) of pain after extubation was recorded . Results The HR, SBP and DBP at T0, T1, T2 and T3 in the group A were significantly lower than those in the group B (P<0.05).The HR and SBP were not significantly different at T4 between the two groups (P>0.05), while the DBP was significantly lower in the group A than that in the group B (t=-2.562, P=0.012).The VAS scores were significantly lower in the group A than those in the group B at T 0, T1, T2 and T3 (P=0.000).No statistical difference was seen in VAS scores at T4 between the two groups (t=-1.199, P=0.233). Conclusions Application of compound lidocaine cream on nasal endoscopic wound and intranasal packing can effectively relieve the pain after endoscopic sinus surgery .The method has good analgesic effects and is worthy of popularization and application .
2.Olive oil-based lipid emulsion for parenteral nutrition in patients after hepatectomy

Objective To assess the safety and efficacy of an olive oil-based lipid emulsion for parenteral nutrition in patients after hepatectomy.Methods Thirty-one postoperative patients with elective hepateetomy were randomized to receive isonitrogenous,isocalorie parenteral nutrition over 6 days after liver lobectomy(48-72hours)with either olive oil-based lipid emulsion(study group,n=15)or standard soybean oil emulsion(control group,n=16).The liver function and plasma protoins were assessed using peripheral venous blood collected before surgery,one day after surgery,and 7 days after surgery.The safety profiles of emulsion supports and postoperative rehabilitation were also assessed.Results The preoperative serum levels of total bilirubin,direct bilirubin,alanine amiotransferase,aspartate aminotransferase,alkaline phosphatase,total protein,albumin,and prealbumin were comparable between the two groups(all P>0.05).Although the Postoperative safety profile and liver function were not significantly different between two groups(all P>0.05),plasma total proteins,albumin,and prealbumin returned to the normal levels significantly faster in the study group than in control group[(57.57±9.84)g/L vs.(47.76±6.53)g/L,P=0.000;(31.29±3.11)g/L vs.(26.34±4.87)g/L,P=0.000;(0.188±0.059)g/L vs.(0.103±0.037)g/L,P=0.000]on the 7th Postoperative day,and the Postoperative hospital stay was also significantly shorter in the study group[(13.1±1.2)d vs.(15.2±1.1)d,P=0.041].The incidence of postoperative complications in study group and control group was 26.7%and 31.3%.respectively.Conclusions Treatment with the new olive oil-based lipid emulsion is weU tolerated in hepatectomy patients.It can speed up plasma proteins recovery and may shorten postoperative hospital stay,although it does not remarkably decrease the incidence of postoperative complications.
3.MRI features of spinal vertebra, spinal meninges and spinal cord tuberculosis

Objective To analyze MRI features of tuberculosis of spinal vertebra, spinal meninges and spinal cord. Methods Fifty cases with tuberculosis of spinal vertebra, spinal meninges or spinal cord confirmed by clinical and pathological evaluation were collected in this study. Findings of plain and Gd-DTPA enhanced MRI were retrospectively reviewed. Results In 40 patients with spinal vertebra tuberculosis, there were 137 involved vertebrae, 52 involved intervertebral discs, 30 peri-vertebra cold abscesses, 23 intraspinal abscesses and compression of spinal cord, 29 athological fractures, 12 posterior process of spinal columns, 2 involved appendix. There were 4 cases of spinal meninges tuberculosis, 4 of tuberculous myelitis and 2 combination of tuberculosis of spinal meninges and spinal column. Conclusion MRI, especially the contrast MRI, could accurately and early demonstrate the morphological and pathological changes of tuberculosis of spinal vertebra, spinal meninges and spinal cord.
4.Clinical significance of Doppler flow waveform of ductus venosus in screening for fetal cardiac malformation

Objective To investigate the clinical significance of Doppler flow waveform of ductus venosus (DV) in screening of fetal cardiac malformation.Methods Color Doppler ultrasonography was used to study DV flow in 240 normal fetuses and 20 fetuses with cardiac malformation that were screened out from the high-risk fetuses with congenital heart disease (CHD).Results ① The normal Doppler flow waveform of the DV was a characteristic triphasic pattern with forward flow during cardiac cycle,reflecting ventricular systole (S),ventricular diastole (D) and atrial systole (A).② In the group of 20 fetuses with cardiac malformation,eight had reverse flow and nine had decreased or absent flow during atrial contraction,the indices of the DV in fetuses with cardiac malformation were higher than those in gestational age-matched normal fetuses ( P
5.Diagnosis of fetal truncus artery by echocardiography

Objective To investigate the clinical value of fetal echocardiography in prenatal diagnosis of fetal truncus artery. Methods Retrospective analysis was made in 10 cases with fetal truncus artery for their fetal echocardiography and follow-up findings. Echocardiographic characteristics was summed up in various fetal heart views. Results Truncus artery was confirmed by autopsy in 7 fetuses,and 3 cases were correspondingly verified by postnatal echocardiography. Echocardiographic characteristic images of fetal truncus artery was larger ventricular septal defect, only 1 great artery arising from heart, and pulmonary artery arising from truncus artery. Conclusions Echocardiography has important value in accurate diagnosis of fetal truncus artery and caution should be paid in differential diagnosis with other fetal complicated congenital heart diseases.
6.Study on Relationship Between mRNA of Urokinase-Type Plasminogen Activator and Breast Cancer, Lymph Nodes Metastasis

Objective To investigate the relationship between the expression of urokinase-type plasminogen activator (uPA) mRNA and breast cancer, lymph node metastasis. Methods Sixty patients with breast tumor were selected randomly and the expression of uPA mRNA was detected with RT-PCR. The patients were divided into benign group (18 cases) and malignant group (42 cases) which included 22 cases with lymph node metastasis and 20 cases without lymph node metastasis. The relationship between uPA mRNA expression and breast cancer, lymph node metastasis was analyzed. Results Among these 18 benign tumors, low expression of uPA mRNA was found in 2 cases and the others were negative. While in 42 cases of malignant tumor, uPA mRNA were positive in 22 cases of lymph node metastasis, 16 of which were high expression, 5 of which were moderate expression, and 1 was low expression. uPA mRNA were positive in 18 of 20 cases of nonmetastatic lymph node, 1 of which was high expression, 5 of which were moderate expression and 12 of which were low expression, the other 2 were negative expression. 7.Effects of aerobic exercise on blood glucose and insulin levels in chronic intermittent hypoxic rats and mechanism of oxidative stress

Objective:To investigate the effects of aerobic exercise on blood glucose and insulin levels in chronic intermittent hypoxic(CIH)rats and the underlying mechanisms, and to provide new insights for the prevention and treatment of diabetes caused by CIH.Methods:SD rats were randomly divided into the blank control group, CIH control group and CIH exercise group.After adaptive feeding, a rat model of CIH was established.The CIH exercise group received non-weight bearing exercise training through swimming.After 4 weeks, all rats were sacrificed and levels of total antioxidant capacity(T-AOC), reactive oxygen species(ROS), malondialdehyde(MDA), fasting blood glucose(FPG)and fasting insulin(FINS)were measured.Results:Compared with the blank control group and CIH control group, levels of ROS, MDA, FPG, T-AOC, FINS and FPG were significantly different in the CIH exercise group( F=4.60, 5.03, 4.87, 4.52 and 6.42, P=0.021, 0.015, 0.017, 0.022 and 0.006). Compared with the blank control group, levels of ROS, MDA, FPG and FINS increased and levels of T-AOC declined in the CIH control and exercise groups(all P<0.05). Compared with the CIH control group, levels of ROS, MDA, FPG and FINS decreased and T-AOC levels increased in the CIH exercise group(all P<0.05). Conclusions:CIH increases blood glucose and insulin levels by activating the oxidative stress response.Aerobic exercise can reduce the impact of oxidative stress on blood glucose and insulin levels.
8.Effects of age on left ventricular twist-displacement loop assessed by velocity vector imaging

Objective:To assess effect of age on the characteristic of letf ventricular (LV) twist-displacement loop in health volunteers by velocity vector imaging (VVI) and to provide a new method for LV function evaluation in clinic.
Methods:Atfer obtaining basal and apical LV short-axis images in 98 healthy volunteers (18-75 years old) by 2-dimensional echocardiography, we use VVI sotfware to analysis LV twist motion and radial displacement at each plane off-line. hTe peak LV twist (Ptw), the peak untwist velocity (PutwV), the proportion of untwist in isovolumetric relaxation period (Iutw%) and LV radial displacement (Dis) were measured and calculated. Then we constructed LV twist-displacement loop and compared the characteristic of them among different groups.
Results:Ptw increased gradually with the increase in age. The biggest PutwV was in the group of 30-60 years old. Iutw%increased gradually before 60 years old, then decreased atfer that. Dis was not obviously different among the three groups. hTe characteristic of LV twist-displacement loop was like the configuration of 8. There was a linear relation between twist and displacement during systole, and the slope increased gradually with the increase in age. During early diastole, the relatively small radial expanding displacement displayed with untwisting, resulting in a much steeper twist-displacement relationship curve occurred in each group, which was getting smooth gradually when the radial expanding displacement increased during mid to late diastole.
Conclusions:VVI can be used to effectively and noninvasively assess LV twist-displacement loop with change in age and provide important information for LV function. hTe effect of age must take into account when evaluate the LV function by the twist-displacement loop.
9.Effect of mfn2 on mitochondrial function in steatosis hepatocytes

AIM: To investigate the effect of mfn2 on mitochondrial function in steatosis hepatocytes. METHODS: Plasmid pEGFP-mfn2 was transfected into hepatocyte strain L02 by Lipofectamine 2000 in vitro, then the steatosis model of hepatocytes was establish by oleic acid induction. RT-PCR was used to evaluate mRNA expression and Western blotting was use to detect the protein expression. ATP level was determined by firefly luciferase bioluminescent. ROS production was measured by fluorescence probe DCFH-DA. Chondrosome transmembrane potential of L02 was observed by labeling of JC-1 and FCM. RESULTS: The stable expression of ectogenesis mitofusin2 in L02 cells was confirmed by RT-PCR and Western blotting. In the model of oleic acids induced lipid formation, Mfn2 obviously inhibited the descent of chondrosome transmembrane potential and ATP level, and increased ROS production in L02 cells. CONCLUSION: Up-regulated expression of mfn2 attenuates mitochondria dysfunction caused by oleic acids induced lipid formation.
10.Relationship between daytime sleepiness and oxygen desaturation in the male elderly with obstructive sleep apnea/hyponea syndrome

Objective To analyze the relationship between daytime sleepiness and oxygen desaturation in the male eldcrly with obstructive sleep apnea /hyponea syndrome (OSAHS).Methods 147 male elderly (aged 60-80 years) with OSAHS in our sleep laboratory from March 2008 to February 2011 were divided into daytime sleepiness and daytime non-sleepiness groups. Epworth sleeping scale (ESS) and polysomnography (PSG) were employed to detect the degree of sleepiness.Oxygen desaturation were evaluated by oxygen desaturation index (ODI),average and minimum blood oxygen,time percentage of 90% oxygen desaturation (CT90). Resulls There were significant differences between the two groups in body mass index (BMI) (t=4.157),ODI (t=-10.063),average (t=5.800) and minimum (U=916) blood oxygen,CT90(U=887)and apnca/hypopneaindex (AHI) (t =-11.361) (all P<0.001).ESS scores were related with desaturation parameters of ODI,average and minimum blood oxygen and CT90 (r=0.683,-0.450,-0.583,0.507,all P<0.001).ODI was correlated with average and minimum blood oxygen and CT00 (r=-0.628,-0.763,0.689,all P<0.001).Among parameters of oxygen desaturation,only ODI had influence on ESS scores (odds ratio=1.11,95%CI:1.07 1.15),The sensitivity and specificity predicting daytime sleepiness at ODI ≥23 times/h were about 79.2% and 84.3%,respectively.Conclusions Daytime sleepiness is associated with oxygen desaturaion and may be predicted by ODI through which average and minimum blood oxygen and CT00 are related with daytime sleepiness in the male elderly with OSAHS.
