1.Predicting the Development Trend of Health Expenditure Based on Methods of System Dynamics Simulation

Objective:To explore the usage of system dynamics analyzing the influencing factors of total health expenditure(THE),forecast the trends of total health expenditure based on the method of System Dynamics and put forward relative countermeasures.Methods:According to the literature and expert consultation,the population,elderly,GDP,government health expenditures,drug costs and the number of health workers per thousand population were the important factors for THE.These factors were incorporated into system dynamics model for total health expenditure and conducted the simulation.Results:Based on the examination,the simulation results highly corresponded to historical data (2002-2014) and the official forecast (2015-2020),which showed that the model had good stability and reliability.The simulation results showed that steadily increasing total health expenditure would reach 7 457.12 billion yuan in 2025.Conclusion:The system dynamics approach had superior characteristics to other projection methods in terms of stability and reliability.In order to control the irrational growth of total health expenditure,it needed to increase the prevention investment,decrease the incidence of chronic diseases;sustainably improve the reform of public hospitals,implement the basic drug system;change the income-oriented performance appraisal to realize the labor value of medical staffs.
2.The effects of exercise on the expression of synapotophysin around hemotomas In rats with intracerebral hemorrhage

Objective To investigate the effects of exercise on the expression of synapotophysin around hemotomas after intracerebral hemorrhage (ICH). Methods Ninety-five male Sprague-Dawley rats weighing 270 to 300 g were divided into 3 groups: a trial group (ICH-induction and exercise, n = 20), a control group ( ICH-in-duction only, n =20) and a sham-operation group (no ICH and no exercise, n = 20). The rats' brains were re-moved at 7, 14, 21, and 28 days after the operation. The other 35 rats were divided into 7 groups (6 h, 12 h, 24 h, 48 h, 72 h after ICH, no ICH and normal ). The activation of synapotophysin was measured by immunohis-tochemical techniques. The rats in the trial group begin cage-running exercise at 72 h after the operation. The oth-ers lived in standard cages. Results Synapotophysin-positive cells were seen in the tissues around the hematomas and in the cortex. There was no synapotophysin expression in the centres of the hematomas. The number of synapo-tophysin-positive cells in ICH group was significantly less than in the sham-operation and normal groups from 6 h to 24 h after the operation. There was then a gradual increase again from 48 h onwards until the 28th day after the op-eration. The trial group had the largest increases in expression. There was a significant difference compared with the control group. Compared with the sham operated group, the trial and control groups showed very significant differences. Conclusion The results suggest that synapotophysin is involved in neuron plasticity after ICH, but exercise training (cage-running) can accelerate the expression of synapotophysin, thereby improving functional re-covery.
3.Hypoxia-inducible factor-1α expression in the collagen induced arthritis model and its significance

Objective To study the hypoxia-inducible factor-1α (HIF-1α) expression in the col-lagen induced arthritis(CIA) model and its role in rheumatoid arthritis (RA) pathogenesis. Methods We established CIA model, and taked CIA model of the ankle joint to have them HE staining and HIF-1α immu-nohistochemistry staining, then the HIF-1α expression in the tissues of RA was observed. Results The ex-pression of CIA synovial membrane of rat and the lower synodal both was expressed in HIF-1α. The positive expression of HIF-1α reached the highest volume in the first 21 days. Then, with the disease progression, the expression gradually decreased, which was in significantly positive relation with the synovial pathology score, synovial hyperplasia and angingenesis rating score, however, the expression was no positively correla-ted with the inflammatory infiltration. Conclusion The expression of HIF-1α in RA tissue is positively re-lated with inflammation severity, It indicates that HIF-1α is closely related to the occurrence and develop-ment of RA.
4.Expression of collagen subtype I mRNA during autogenous tendon formation induced by grafted human hair keratin artificial tendon

BACKGROUND: Numerous experiments and clinical practice show that human hair keratin artificial tendon induces the organism to form autogenous tendon. The process of autogenous tendon formation mainly involves the synthesis, secretion and package of collagen subtype I.OBJECTIVE: To explore the role of collagen subtype I mRNA expression in autologous tendon formation after human hair keratin artificial tendon implantation.DESIGN: A completely randomized controlled experiment based on the experimental animals.SETTING: Department of Histology and Embryology and Department of Biochemistry and Molecular Biology of Southern Medical University.MATERIALS: The experiment was conducted in the Experimental Animal Center of the First Military Medical University of Chinese PLA from May 2003 to September 2004. Totally 33 New Zealand rabbits of either gender,weighing 2.0 to 2. 5 kg, were provided by the center. The animals were randomly divided into experiment 3, 6, 9, 12, 16 and 20 weeks groups, negative control 9 and 20 weeks groups and normal control group. Among them,experiment 3 and 6 weeks group and normal control group had 3 rabbits in each and the other groups had 4 rabbits. Human hair keratin artificial tendons were normal human hair treated by a series of biochemical methods and were supplied by the Department of Biochemistry & Molecular Biology of the university. The human hair keratin artificial tendons were divided into three groups with different degradation rates, namely, fast(F), medium(B) and slow(Z). The tendons were made up of the fast, medium and slow degradation groups mixed at the ratio of 4: 3: 3.off by 1.0- 2.0 cm, human hair keratin artificial tendon was grafted by end-to-end anastomosis with both ends of the broken tendon before sewing control group, no artificial tendon was implanted although the animals ungroup was normal rabbits' tendon. Sampling was carried out at 3, 6, 9, 12, 16and 20 weeks after human hair keratin artificial tendon implantation in experiment groups, and at 9 and 20 weeks after operation in negative control group, respectively. The expression of collagen subtype I mRNA was detected at weeks 3, 6, 9, 12, 16 and 20 after grafting using reverse transcription polymerase chain reaction technique.MAIN OUTCOME MEASURES: The ratio of collagen subtype I mRNA to Glyceraldehyde-3-phosphate dehydrogenase(GADPH) mRNA in normal tendon and autogenous tendon induced by human hair keratin artificial tendon at all time points was calculated, and significance test between all these paired groups were performed.collagen subtype I mRNA/GADPH mRNA expression was 0.96 ±0.02 in expression of collagen subtype I mRNA/GADPH mRNA in autogenous tendon induced by human hair keratin artificial tendon in experiment group appeared at week 3, increased rapidly at week 3 to 6, peaked at week 6, and remained stable at week 9 to 20. The expression at week 6 was significantly higher in experiment group than in normal control group( F = 6. 254, P < 0.05); the expression at other weeks was also significantly higher in experiment group than in normal control group( F= 1. 258 - 1. 987, P > 0.05).CONCLUSION: The activation, proliferation and secretion of collagen protein as well as the synthesis of collagen subtype I by tenocytes may be responsible for autologous tendon formation after human hair keratin artificial tendon implantation.
5.Validation of Microbial Limit Test for Compound Benzocaine Gel

Objective: To establish a microbial limit test method for compound benzocaine gel.Methods: According to the general principles of Chinese Pharmacopoeia (2015 edition), method applicable experiments were performed respectively for the routine method, neutralization method and dilution & neutralization method.Using the recovery ratio of test bacteria as the index, the medium of dilution & neutralization method was adopted in the total aerobic microbial count and total combined yeasts and molds count, and the neutralization method was used for the control bacteria detection.The neutralizing agents were polysorbate 80 and lecithin.Results: The method of dilution & neutralization could eliminate the bacteriostasis of the drug, and the recovery ratio of each test bacteria was within the range of 0.5-2.0, which was in line with the requirements of Chinese Pharmacopoeia.Escherichia coli, Pseudomonas aeruginosa and Staphylococcus aureus could be detected out in the positive control bacterial test,and bacteria were not detected out in the negative test.Conclusion: The method can be used for the microbial limit test for compound benzocaine gel.

7.Prediction of secondary structure and B-cell epitope of human IL-37

Objective:To predict the secondary structure and B-cell epitope of human IL-37.Methods:Based on IL-37b ami-no acid sequence, the secondary structure was predicted by SOPMA; hydrophilicity, flexibility, accessibility index were predicted by software of ProScale, Bcepred, respectively.Combined the results according to these methods , the B cell epitopes of IL-37b were pre-dicted.Results: The second structure of IL-37b contained extended strand (31.65%), random coil (52.75%), alpha helix (8.26%), beta turn (7.34%) and the most possible epitopes of IL-37b were located in or adjacent to amino acid 21-27, 34-75, 175-192 , 213-215 .Conclusion:These results will be helpful for the estimate of the epitopes and provide a theory basis for developing mon -oclonal antibodies against human IL-37.
8.Clinicopathological analysis of 23 cases of NK/T-cell lymphoma of nasal type.

OBJECTIVE:
To improve the understanding of nasal NK/T-cell lymphoma by analyzing its phenotypic and clinicopathological features.
METHOD:
Twenty-three cases of nasal NK/T-cell lymphoma diagnosed between 2003 and 2007 in the department of pathology of Guilin Medical College were included in the study. The expression level of TIA-1, CD56, CD3, CD20, CK and EBV markers was determined by immunohistochemistry. The results were correlated with clinicopathological features.
RESULT:
69.9% (16/23) of the nasal NK/T-cell lymphoma occurred in the nasal cavity. All the 23 cases displayed necrosis, ulceration and nose bleeding. 39.1% (9/23) showed angiodestructive growth pattern. 21.74% (5/23) were accompanied by squamous cell carcinoma-like epitheliomatous hyperplasia. All the cases were positive for TIA-1 and CD3. 95.7% (22/23) of the cases were positive for CD56, while 21.7% (5/23) were weakly positive for EBV. None of the cases was positive for either CD20 or CK.
CONCLUSION
Nasal NK/T-cell lymphoma is characterized by multiple clinicopathological features. Attention is needed to differentiate the tumor from inflammatory lesions and low grade squamous cell carcinoma. Understanding of various morphological and phenotypic features (i.e. expression of TIA-1, CD56 and CD3, and lack of CD20 and CK) is the key for the diagnosis of nasal NK/T-cell lymphoma.

9. Effect of HPV detection and cervical cell basis detection on early diagnosis of cervical precancerous lesions

Objective:
To analyze the effect of human papillomavirus (HPV) detection and cervical cell basal fluid on the early diagnosis of cervical precancerous lesions.
Methods:
From January 2016 to January 2017, 223 patients in the Maternal and Child Health Care Hospital of Yongkang who received gynecological cervical cancer precancerous lesions screening were selected.All patients into the outpatient screening categories were given cervical liquid-based cytology (TCT) examination and HPV detection, 61 patients with test results of double positive received colposcopy cervical biopsy histopathological examination, the final diagnosis of 49 patients with epithelial cell changes.The accuracy and sensitivity of different detection methods were compared.
Results:
The detection rates of high risk HPV infection in cervicitis, CINⅠ, CINⅡ-CINⅢ and cervical cancer patients were 16.22%, 39.29%, 71.43% and 100.00% in 223 cases of this study.The total positive rate of TCT was 28.57%.The total positive rate was 44.44% in HPV group and 75.51% in combination test.The positive rate of combined detection was higher than TCT (χ2=21.63,

By engineering the subsite +1 of cyclodextrin glycosyltransferase (CGTase) from Paenibacillus macerans, we improved its maltodextrin specificity for 2-O-D-glucopyranosyl-L-ascorbic acid (AA-2G) synthesis. Specifically, we conducted site-saturation mutagenesis on Leu194, Ala230, and His233 in subsite +1 separately and gained 3 mutants L194N (leucine --> asparagine), A230D (alanine --> aspartic acid), and H233E (histidine --> glutamic acid) produced higher AA-2G yield than the wild-type and the other mutant CGTases. Therefore, the 3 mutants L194N, A230D, and H233E were further used to construct the double and triple mutations. Among the 7 obtained combinational mutants, the triple mutant L194N/A230D/H233E produced the highest AA-2G titer of 1.95 g/L, which was increased by 62.5% compared with that produced by the wild-type CGTase. Then, we modeled the reaction kinetics of all the mutants and found a substrate inhibition by high titer of L-AA for the mutants. The optimal temperature, pH, and reaction time of all the mutants were also determined. The structure modeling indicated that the enhanced maltodextrin specificity may be related with the changes of hydrogen bonding interactions between the side chain of residue at the three positions (194, 230 and 233) and the substrate sugars.
