1.Comparison of the efficacy of reteplase and urokinase intravenous thrombolysis in treatment of patients with acute ST segment elevation myocardial infarction

Objective To explore the curative effect of reteplase and urokinase in treatment of acute ST elevation myocardial infarction.Methods 116 patients with acute ST segment elevation myocardial infarction who were diagnosed and received treatment in our hospital from October 2013 to October 2015,were selected as the study subjects and were randomly divided into two groups with 58 cases in each group according to the single blind method. The control group was treated with urokinase thrombolytic therapy,while the patients in the observation group were treated with reteplase thrombolytic therapy.The thrombolytic recanalization rate,and adverse event incidence were compared in the two groups.Results The clinical judgement thrombosis recanalization rates in the observation group after 0.5h,1h,1.5h,2h of thrombolysis were 27.59%,60.34%,81.03% and 86.21% respectively,which were sig-nificantly higher than 15.52%,29.31%,62.07%,70.69% of the control group,the differences between the two groups were statistically significant(χ2 =3.853,3.907,3.898,3.524,all P <0.05).After 1.5 h of thrombolytic ther-apy,the results of underwent coronary angiography showed that the infarct related artery recanalization rate of the observation group was 70.69%.The complete recanalization rate was 60.34%,which was significantly higher than 55.17% and 41.37% in control group,the differences between the two groups were statistically significant(χ2 =4.032,4.153,all P <0.05).There was no significant difference between the two groups in the incidence of adverse events(χ2 =0.250,P >0.05).Conclusion Reteplase for acute ST segment elevation myocardial infarction thrombo-lytic therapy is better than urokinase,has a high patency rate and low incidence of adverse events,it is safe and worthy of clinical popularization and application.

Objective To study the effect of cortisol and dehydroepiandrosterone sulfate(DHEA S)in fetal umbilical cord blood on term labor. Methods Cortisol and DHEA S concentrations were measured By radioimmunoassay in 100 term fetal umbilical cord blood. They were divided into four groups. Group A selective cesarean section without any birth pain ( n =18),Group B cesarean section in latent phase( n =10),Group C cesarean section in active phase( n =12),Group D spontaneous vaginal deliver( n =60). Results The concentrations of fetal umbilical cord cortisol in spontaneous vaginal deliver group was gradually increased with gestational week. 4.Nursing of locally preserved skin of female patients for neurosurgical cranioctomy

Objective To summarize the experience in nursing locally preserved skin of female patients for neurosurgical cranioctomy. Method The nursing key points were given to 66 female patients undergoing local preparation of skin for cranioctomy as follous preoperative preparation, observation over incision and head nursing. Results The time for skin preparation ranged between 3 to 6 minutes. Postoperatively, no drain tube was placed. No intracranial infections occurred. The incisions in all patients were healed at the first class. Conclusion The nursing measures of good preoperative preparation, close observation of incisions and care to the head for female patients undergoing local preparation of skin for cranioctomy can enhance their satisfaction and propel development of standardized nursing.
5.Therapeutic Observation of Scraping Therapy Based on She Medicine for Occipital Neuralgia

Objective To observe the clinical efficacy of Sha-petechia releasing therapy based on She medicine in treating occipital neuralgia, for proving its effectiveness and advantage. Method Eighty patients were randomized into a treatment group and a control group, 40 cases in each group. The treatment group was intervened by Sha-petechia releasing therapy based on She medicine, while the control group was by Western medications, 10 d as a treatment course, totally for 2 courses. The clinical efficacy and change of Visual Analogue Scale (VAS) score after intervention were observed. Result The total effective rate was 92.5% in the treatment group versus 82.5% in the control group, and the difference was statistically significant (P<0.05). Conclusion Sha-petechia releasing therapy based on She medicine is an effective approach in treating occipital neuralgia.
6.Effect of aldosterone and its antagonist spironolactone on epithelial-mesenchymal transition of mormal rat kidney epithelial cells in high glucose

Objective To determine the effect of aldosterone and its antagonist, spironolactone on epithelial-mesenchymal transition (EMT) of normal rat kidney epithelial cells (NRK-52E) in a high glucose milieu,and to explore the mechanism of renoprotection in diabetic nephropathy (DN ) in rats involving aldosterone and spironolacton. Methods NRK-52E cells were simultaneously cultured in the serum-free Dulbecco's modification of Eagle's medium Dulbecco (DMEM) for 12 hours. Then the low glucose (LG) group was cultured in LG (1000 mg/L) DMEM:The high glucose (HG) group was cultured in high glucose (4 500 mg/L) DMEM. The aldosterone (Aldo) groups were cultured in high glucose DMEM with the addition of 10,50 and 100 nmol/L aldosterone respectively. The SP group was cultured in high glucose (4 500 mg/L) DMEM plus 10~(-7)mol/L spironolactone. Immunohistochemistry, RT-PCR and Western blot were used to detect E-cadherin and α smooth muscle actin(α-SMA) mRNA expression. Results RT-PCR showed that compared with the LG Group, E-cadherin mRNA expression in the HG group was significantly lower, and α-SMA mRNA expression was significantly increased(P<0.05). E-cadherin mRNA expression in the 50 nmol/L Aldo group and 100 nmol/L Aldo group was significantly lower than that in the HG group, while the expression of α-SMA mRNA was significantly increased in the HG group(P<0.05), with a dose-dependent relationship with aldosterone(r=-0.70,P<0.05;r=0.67, P<0.05). E-cadherin mRNA in the SP group was significantly higher,while α-SMA mRNA expression was lower than that in the HG group(P<0.01). Immunohistochemistry and Western blot showed that compared with the LG group, E-cadherin protein expression was significantly reduced, and α-SMA expression was significantly increased in the HG group(P<0.01). In the 10 nmol/L Aldo, 50 nmol/L Aldo, and the 100 nmol/L Aldo groups, E-cadherin protein expression was significantly lower, and α-SMA protein expression was significantly higher than that in the HG group(P<0.05), with a dose-dependent relationship with aldosterone(r=-0.83,P<0.05;r=0.81, P<0.05). In the SP group, E-cadherin protein expression was higher, and α-SMA protein level was lower than that in the HG group(P<0.05). Conclusion Aldosterone can promote EMT of tubular epithelial cells in a high sugar milieu, leading to renal interstitial fibrosis in Diabetic nephropathy. Spironolactone can inhibit high glucose-induced renal tubular epithelial cells EMT, which may be an important mechanism for the inhibition of renal interstitial fibrosis.

Aim To develop the in vitro and in vivo models induced by shrimp tropomyosin( ST) and mono-clonal tropomyosin-specific murine IgE antibody ( anti-ST-IgE mAb) . Methods ST was purified from Metap-enaeusensis by an isoelectric precipitation method. The anti-ST-IgE mAb was obtained from hybridomas. After RBL-2 H3 cells were sensitized with anti-ST-IgE mAb and challenged with ST,β-hexosaminidase release was determined. Passive systemic anaphylaxis ( PSA ) was induced in mice and the rectal temperature was recor-ded after ST challenge within 30 min by a thermal probe. Results A significant increase ofβ-hexosamin-idase was observed in sensitized cells after ST chal-lenge. The average temperature drop after ST challenge was 1. 44℃ in PSA mice within 30 min. Conclusion The in vitro and in vivo models induced by ST and anti-ST-IgE mAb are established as an improvement of pres-ent models of type Ⅰ allergy.

Objective:To develop murine models of Th2 response induced by shrimp tropomyosin (ST). Methods:Mice were sensitized with ST for 6 weeks. The serum antigen-special IgE (sIgE),total IgE and sIgG level,Th1/Th2 cytokines production were measured by ELISA. The basophil activation in mice was measured by flow cytometry. Results:The intraperitoneal sensitization with ST for 6 weeks induced significant increase of serum sIgE,total IgE and sIgG (sIgG1,sIgG2a and sIgG2b) level in mice. Th2 cell response was induced and cytokines (IL-4,IL-5,IL-10 and IL-13) production increased in splenocytes stimulated by ST,while Th1 cytokine (IFN-γ) production decreased. As the markers of basophil activation,CD200R and CD41 expression also increased in response to ST. Conclusion:The Th2 response is dominant in ST-induced anaphylaxis in mice.
9.Determination of Nitrate in Seawater with Valve-free Continuous Flow Analysis

A valve-free continuous flow method and instrument were established,with only a multi-channel pump for delivering the sample and reagent,and without any injection or solenoid valves and sample loop for selecting and adding the sample or reagent.Nitrate was reduced to nitrite with Cu-Cd reductant column,and then detected with spectrophotometric detector.The proposed method was suitable for determination of nitrate at normal level in most of estuary and coastal seawaters.With the optimum parameters,the linear range and detection limit were 5-180 μmol/L and 0.27 μmol/L,respectively.The samples of 10 and 80 μmol/L nitrate were continually measured for 11 times,and the relative standard deviations were 1.4% and 1.3%,respectively.The recovery of real samples at different salinity ranged between 99.4% and 106.1%.There was no significant difference in the analytical results between the proposed method and the flow injection analysis (FIA).In comparison with FIA,the method and instrument were less cost and easy to operate,and was suitable to be applied in general laboratories and field for continuous monitoring.The method was successfully used to measure the nitrate in seawater samples in Xiamen's Western Harbor and monitor nitrate in Jiulongjiang estuary.
10.Effect of local aldosterone on renal epithelial-mesenchymal transition in diabetic nephropathy rats

Objective To explore the effect of aldosterone on renal epithelialmesenchymal transition in streptozocin(STZ)-induced diabetic nephropathy rats. Methods Wistar rats were intraperitoneally injected with STZ(60 mg/kg)for the preparation of diabetic model.After 4 weeks,the rats with urinary protein>30 mg/d were regarded as successful diabetic nephropathy(n=16),and were randomly divided into diabetic nephropathy(DN group,n=8)and spironolactone group(SP group,n=8).Then eight healthy rats were selected randomly as control group(N group,n=8).SP group rats were treated with spironolactone 40 mg·kg-1·d-1,and N group and DN group rats were given equal water.After 8 weeks,rats were sacrificed to collect urine,blood plasma,kidney tissue for detection of 24 h urinary protein,creatinine and renal pathological changes.Aldosterone concentration in plasma and kidney tissue was detected by mdioimmunoassay;E-cadherin,α-SMA protein expression by immunohistochemistry,Western blotting; E-cadherin,α-SMA mRNA expression by RT-PCR. Results Compared with N group,serum creatinine, urinary protein excretion in the DN rats were significantly higher (P<0.01,respectively), E-cadhefin protein and mRNA were significantly reduced (P<0.01, respectively),α-SMA protein and mRNA expression was up-regulated (P<0.01, respectively). Aldosterone level of kidney tissue in DN rats was increased obviously [(24.71±5.30) ng/g vs (16.38±2.85) ng/g, P<0.01], which was positively correlated with urinary protein excretion, serum creatinine and α-SMA protein (r=0.737, 0.574, 0.688, P<0.01, respectively), and negatively correlated with E-cadherin protein (r=-0.659, P<0.O1). While no significant difference was found in serum aldosterone among three groups. Compared with DN rats, urinary protein excretion, serum creatinine were reduced (P<0.01, respectively), E-cadherin protein and mRNA were increased (P<0.01, respectively), α-SMA protein and mRNA expression were decreased (P <0.01, respectively) in SP group rats.Conclusions Local aldosterone involves in renal epithelial-mesenchymal transition in diabetic nephropathy rat. Spironolactone can block the effect of aldosterone and play a role in renal protection.
