1.Enhancement of baicalin percutaneous penetration by azone and oleic acid

Objective To study the effect of azone,oleic acid,and their combination on the trans-dermal obsorption of baicalin.Methods Improved Franz-cells and HPLC were used for the transdermal study.The effect of azone,oleic acid,and their combination on the transdermal absorption of baicalin was studied using uniform design.Results The 24 h penetration amount of baicalin was obtained as 466.11 ?g/cm~2 and 77.34 ?g/cm~2 when the azone concentration was 10% or the oleic acid concentration was 14%,respectively.When using a combination of azone and oleic acid in the ratio of 10%∶0.5%,the 24 h penetration amount of baicalin was 641.88 ?g/cm~2 and the enhancement ratio of penetration was 146.69.Conclusion Both azone and oleic acid could improve the skin penetration amount of baicalin.Combinated application of azone and oleic acid could further enhance baicalin transdermal delivery rate.The maximum penetration amount of baicalin is obtained by using a combination of azone and oleic acid in the ratio of 10%∶0.5%.
2.Maturity-onset diabetes of the young 11 caused by B lymphocyte kinase mutation in a family

The clinical data of a patient with maturity-onset diabetes of the young 11 (MODY11) caused by B lymphocyte kinase( BLK) gene mutations in Children′s Hospital Affiliated to Nanjing Medical University in August 2018 were retrospectively analyzed.The diabetes mellitus epidemiologic investigation was carried out on the patient′s family.The 13-year-old boy was diagnosed with type 1 diabetes at the local hospital 6 months ago.Physical examination showed that he was 65.5 kg in weight, 169.2 cm in height and 22.9 kg/m 2 in the body mass index.He was overweight without acanthosis nigricans.Laboratory measurements revealed fasting blood glucose 11.79 mmol/L, fasting insulin 18.05 mmol/L, and fasting C-peptide 1.12 mmol/L.The glycosylated hemoglobin was 12.0%, while the islet antibodies were all negative.Among 4 consecutive generations of this family, 11 members presented with diabetes, 8 cases of who were treated with insulin and 3 cases with oral hypoglycemic drugs.The whole exome sequencing identified a heterozygous mutation in exon 9 of BLK (c.809C>T) in this patient and his mother.This mutation caused the amino acid change p. T270M (threonine>methionine). Many cases of MODY are misdiagnosed as either type 1 diabetes or type 2 diabetes.MODY11 is rare, mostly characterized by overweight or obesity, insufficient serum insulin secretion and commonly insulin dependence.
3.Activated Carbon Enrichment Combined with Pyrolysis Zeeman Atomic Absorption Spectroscopy for Determination of Trace Amounts of Mercury in Water

Abstract A method for determination of trace mercury in water was established. The trace mercury in water was adsorbed quantitatively by activated carbon, and then determined by electrical pyrolysis atomic absorption spectrometry. In comparison with the detection methods of total mercury in water at present, the method avoids the steps of digestion, reduces the mercury pollution and the loss of the mercury, and is simple in operation. The effects of particle size of activated carbon, acid treatment method, acid medium and enrichment time on the enrichment efficiency were investigated. The effect of the pyrolysis temperature and the interfering ions on the determination results was investigated. Three standard addition procedures including activated carbon blank addition, solution blank addition and environmental water samples addition were studied. Regression correlation coefficients of three standard curves drawn by the three methods reached 0 . 9999 . The slope of the three standard curves had no difference by statistical test, indicating that the determination of mercury in environmental water samples under the experiment conditions was not interfered by the coexistent elements, which showed that the activated carbon blank addition method could be directly used for preparing standard curve of the method. The water samples containing 5 ng/L and 50 ng/L mercury were determined by the method, and the relative standard deviation were 7. 2% and 4. 2% (n=11), respectively, with a detection limit of 1. 2 ng/L. The recovery experiment was carried out after adding 10 ng/L mercury to the surface water and tap water samples, and the recoveries were between 92. 0% and 103. 0%. Analysis results were compared with ICP-MS as control and the deviation of the two methods were between 2 . 9% and 3 . 4%, indicating that the method was accurate and reliable, and had good precision.
4.Experimental study of the influence of expanded capsule on biomechanics of autograftedcostal cartilage.

OBJECTIVETo explore the effect of the expanded capsule on biomechanics of the transplanted autologous costal cartilage.
METHODSTen New Zealand white rabbits aged 3 months were involved and four 15 ml tissue expanders were implanted symmetrically on the back of each rabbit. After 1 month, expanded capsules were removed (experimental group) on the left side and were reserved (control group) on the right side. Meanwhile, the 6th and 7th costal cartilage without the perichondrium were taken out and each cartilage was divided into two pieces and put under the expanded capsule symmetrically. The cartilage at the upper sides and lower sides were taken out at 4 weeks and 8 weeks later, respectively. All the cartilages were sculptured for the demand of the machine text. The tension and compression property of the cartilages were assessed by the material testing machine (Instron 5967) and the results were analyzed statistically.
RESULTS8 weeks lateral, the average ultimate tension strength was (4. 954 + 2. 8) MPa in experimental group and (2.939 + 0.842) MPa in control group. The average compressive strength was (58.74 ± 13.77) MPa in experimental group, and (47.61 ± 11.41) MPa in control group. The biomechanics property of the cartilages in the experimental group is better than the control group, and the differences were statistically significant.
CONCLUSIONSRemoving the expanded capsule is benefical to maintain the biomechanics of autologous costal cartilage.

5.Correlation of zearalenone pollution with precocious puberty girls

Objective To explore the relationship of zearalenone (ZEA) and precocious puberty in girls.Methods The peripheral serums from 71 cases of precocious puberty girls and 50 cases of healthy girls were collected respectively and concentrations of ZEA were detected by high performance liquid chromatography.Bone age,body mass index (BMI),volume of uterus and ovaries,concentrations of estradiol (E2),luteinizing hormone (LH) and folliclestimulating hormone (FSH) were detected,and the residence of each subject was recorded as well.Results (1) In 71 patients,52 patients were diagnosed as idiopathic central precocious puberty,others were diagnosed as premature thelarche.(2) In 71 patients,serum ZEA was detected from 51 patients,and undetected in 20 patients.(3)Concentration of ZEA in precocious puberty girls [(318 ±34) ng/L]was significantly increased than that in healthy girls [(143 ± 35) ng/L,P =0.002],but no distinct difference existed between ICPP group and PT group(P =0.326).(4)Compared with uterus volume of ZEA in the undetected patients (1.975 ±0.150) cm3,the uterus volume of ZEA detected patients (2.972 ±0.180) cm3,which was significantly enlarged,there was significant difference (P =0.01) ; Percentage of overweight girls in ZEA detected patients (31.4%,16/51 cases) was lower than which in ZEA undetected patients (65.0%,13/20 cases,P =0.01) ; Although there was no statistical differences in the breast diameter,bone age,value of E2,LH and FSH between ZEA detected patients and undetected patients (all P > 0.05),but the increased tendency in ZEA detected patients existed.(5) ZEA in serum was detected in 53.3% (16/30 cases) patients living in the cities,and the rate was obviously lower than 82.9% of the patients (34/41 cases) living in the countryside,there was significant difference (P =0.007).Conclusions ZEA is correlated with precocious puberty in girls.ZEA pollution might be one of reasons for precocious puberty occurrence.

7.Comparison of an in-house tuberculosis-specific IFN-γ release assays with T-SPOT TB in latent tuberculosis infection diagnosis among HIV-infected individuals

Objective To evaluate the diagnostic value of two tuberculosis-specific IFN-γ release assays in latent tuberculosis infection among HIV-infected individuals. Methods The levels of tuberculosis antigen-specific IFN-γin 102 HIV patients from AIDS Outpatient Clinic of Shenzhen Third People's Hospital were detected by in-house tuberculosis-specific IFN-γ ELISpot assay and commercial T-SPOT TB kit, and tuberculin skin test (TST) were done at the same time. There were 66 males and 36 females,and the average age was 35. Results Seventeen HIV infected patients were positive in both IFN-γ ELISpot and T-SPOT TB methods, the sensitivity, specificity positive predictive value(PPV), negative predictive value(NPV) and compliance rates of ELISpot were 94. 4% ,94. 0% ,77. 3% ,98. 8% and 94. 1% ,respectively. Three patients were positive in both IFN-γELISpot and T-SPOT TB methods, the sensitivity, specificity, PPV, NPV and compliance rates of TST were 16. 7%, 98. 8%, 75.0%, 84. 7% and 84. 3%, respectively. The average number of spots using three kinds of antigen ESAT-6, Pool A,Pool B obtained were 26. 89 ±5. 77,18. 96 ±4. 75 and 14. 51 ± 3.77, respectively. Only ESAT-6 and Pool B have a statistically significant difference (H=7.557,P = 0.022 9), no significant difference was shown between other groups. There was no significant difference between the positive rate and the CD4+ T cellls number(x2 =0. 860 8 ,P =0. 650 2) ,as the same as the T-SPOT TB (x2 = 1. 396 4, P = 0. 497 5 ). Conclusions The performance of this in-house tuberculosis-specific IFN-γ ELISPot assay was comparable to T-SPOT assay in diagnosis of latent tuberculosis infection, and the sensitivity and specificity of both these two assays were all much higher than TST. They canbe recommended in diagnosing latent tuberculosis infection in HIV infected patients.

Objective To explore CD59 expression on CD4+T cells in HIV infected patients and its relationship with apoptosis.Methods 12 HIV infected patients and 10 healthy donors were performed in this study.The PBMC(peripheral blood monocyte)were collected and cell surface cytokine were stained,and then were evaluated with the BD FACSCanto flow cytometry.The expression of CD59 on T lymphocyte subsets were analyzed by FACSDiva software,and the apoptosis rate of CD59+CD4+T cells and CD59-CD4+T cells in every group was analyzed respectively,then the results were compared between groups.Results Compared with healthy donor,the expression of CD59 on T cells in HIV infected patients was significantly hisher(t=5.198,P<0.01),and the apoptosis rate of CD59+CD4+T cells had significantly higher(t=5.968,P<0.01).The apoptosis rate of CD59-CD4+T cells was no difference between two groups (t=0.1353,P=0.8577).Condnsion HIV infection increase CD59 expression on CD4+T cells,and CD59+CD4+T cells were prone to apoptosis.
9.Research on preparation and relevant performance test for new-type acellular dermal matrix

Objective To prepare a new-type acellular dermal matrix (ADM) and research on its relevant performance,which would provide theoretical evidence for clinical application.Methods Skin of Bama suckling pig was taken as resource of skin,and technologies of physics,chemistry and biology were selected to prepare new-type ADM.To detect the external structure,physical and chemical property as well as biological property of the prepared new-type ADM,hematoxylin-eosin (HE) staining observation,scanning electron microscope observation,amino acid analysis,material porosity and hydrophilicity test,tensile strength and in vitro degradation experiment,cytotoxicity test,and animal experiment have been conducted.Results New-type ADM cells have been thoroughly removed and dermal matrix remains intact with collagen content of 95.55%,connective three-dimensional pore structure,(85.03 ± O.99) % of porosity,(24.56 ± 0.57) ° of contact angle implying new-type ADM was hydrophilic substance,(5.48 ± 0.44) Pa of tensile strength implying its moderate level of pulling force,in vitro degradation period reduced to (28.7 ± O.76) h,and >75% relative growth rate (RGR).Cells grew and proliferated on new-type ADM and could be replaced by original tissue after degradation.Conclusions New-type ADM have overcome disadvantages of traditional preparation method in sabotaging dermal matrix structure and incompletely removing cells from matrix,which is qualified with higher level of collagen content and porosity.With improved biological property,greatly reduced inflammation immunoreactions,and accelerated degradation rate,new-type ADM is of higher level of clinical application value.
10.The role of T helper type 17 cells in the pathogenesis of HIV/tuberculosis-coinfected patients

ObjectiveTo evaluate the IL-17 expression in HIV/tuberculosis-coinfected patients and its role in the pathogenesis of this coinfection.MethodsFifty-four HIV infected patients were divided into three groups:simple HIV infected group,HIV with latent tuberculosis infection (HIV+ LTBI) group and HIV coinfected with active tuberculosis (HIV+ ATB) group.The whole blood intracellular cytokine staining was performed and samples were then detected by BD FACSCanto.The expressions of CD4+ IL-17+ T cells and CD4+ IFNγ+ T cells were analyzed using FACSDiva software.Comparison between groups was done by independent sample t test.ResultsThe CD4+ T cell count and viral load among these three groups were comparable.There were no significant difference of the expression of CD4+ IL-17+ T cells between simple HIV infected group and HIV+ LTBI group (1.40 ± 1.01) % vs (1.29±0.86) %,(t=0.336,P>0.05),but both of these two groups were much higher than HIV+ATB group (t=3.680,t=2.516,P<0.05).There were no significant differences of the expression of CD4+ IFNγ+ T cells among these three groups [(32.8±24.0)% vs (40.3±1 21.9) % vs (46.1±31.2)%,(t=-0.939,t=-1.602,t=-0.646,P>0.05)].ConclusionThe Th17 response is down-regulated in HIV/tuberculosis-coinfected patients,which may play an important antitubercular role in the pathogenesis of coinfection.
