1.Clinical observation on pemetrexed disodium in the treatment of advanced pulmonary adenocarcinoma

Objective To evaluate the efficacy and drug-related toxicities of pemetrexed disodium in the treatment of advanced pulmonary adenocarcinoma.Methods A total of 41 patients who had received pemetrexed disodium and cisplatin therapy were retrospectively reviewed.The patients were given respectively pemetrexed disodium (500 mg/m2,1st day) and cisplatin (200 mg/m2,1-4 day) until disease progressed.The clinical outcomes and adverse reactions were observed.Results The CR rate was 7.3 % (3/41),PR rate was 22.0 % (9/41),SD rate was 31.7 % (13/41),PD rate was 39.0 % (16/41),the DRR was 29.3 % (12/41),DCR rate was 61.0 % (25/41),the median PFS was 5 months.The age,sex,smoking history,staging and timing of treatment had not the statistics difference on ORR and DCR.The sex,smoking history,staging and timing of treatment had not the statistical difference on PFS (P > 0.05).The main toxicities were fatigue,nausea,vomiting and myelosuppression.Conclusion The pemetrexed disodium and cisplatin are feasible and welltolerated for advanced pulmonary adenocarcinoma patients.
2.Effects of neural stem cells transplantation and FK506 on pathologic and ultrastructural changes of injured spinal cord

ObjectiveTo investigate the impacts of neural stem cells (NSCs) transplantation on spinal pathology and ultrastructure after spinal cord injury (SCI) in rats and probe into the protective role of tacrolimus (FK506) on neural regeneration.MethodsCompressive SCI at T8 was induced in the adult SD rats,which were randomly assigned to the control group,FK506 group,NSCs group and NSCs + FK506 group.The differences of neural regeneration in each group were compared at days 7,14,28 and 56 after injury by motor evoked potentials ( MEP),HE staining,immunohistochemical staining,ultrastructure observation and image analysis of the myelinated fiber. ResultsThe MEP latency in the NSCs + FK506 group was significantly shorter than that in other groups at day 28 ( P < 0.05 ).HE staining revealed that only local necrosis presented in the NSCs + FK506 group at day 56.More BrdU and NF-200 positive cells were detected with immunohistochemical staining in the other three groups as compared with the control group.Moreover,the positive cells in the NSCs + FK506 group also outnumbered the FK506 group and NSCs group.Electron microscope scan showed edema under the membrane of large myelin sheath in the control group,and classic new myelin sheath and neuraxis in the NSCs + FK506 group at day 56.The regeneration of the nerve fiber in the NSCs + FK506 group was better than that of other three groups (P <0.01 ).ConclusionAfter NSCs transplantation for SCI rats,the early combination use of FK506 can improve the pathology and ultrastructure of the regenerative nerve fiber and is conducive to nerve regeneration.
3.Effect of the levels of serum gonadal hormone and plasma calcitonin gene-related peptide on the hot flushes symptom of perimenopausal women

Objective To study the effect of the levels of serum gonadal hormone and plasma calcitonin gene-related peptide(CGRP) on the hot flushes symptom of perimenopausal women. Methods 65 perimenopausal women (35 with hot flushes and 30 without hot flushes) and 25 healthy fertile women were enrolled. The subjects were analyzed for serum estradiol (E2), follicle-stimulating hormone (FSH) ,luteinizing hormone (LH) and plasma calcitonin gene-related peptide (CGRP). Results. ① There were no significant differences of the E2 levels between the perimenopausal women with and without hot flushes (P>0.05). The levels of FSH and LH were significantly higher in the women with hot flushes than women without hot flushes and fertile women(P<0.05),② The levels of plasma CGRP were significantly higher in the women with hot flushes than women without hot flushes (P<0.05), and significantly lower than fertile women,③The levels of plasma CGRP were significantly higher in severe hot flu-shes group than that in the mild hot flushes group and moderate hot flushes group(P<0.05), the severity of hot flu-shes was positively related to the level of plasma CGRP(rs=0.823, P<0.01), but there was no relationship be-tween serum E2 and the severity of hot flushes (P>0.05). Conclusion The occurrence of perimenopansal hot flu-shes might be closely related to the decline and fluctuation of serum E2,increase of FSH and LH and the concentra-tion variety of plasma CGRP.
4.Correlation factors for hot flashes in climacteric women

Objective To investigate the correlation factors for hot flashes in climacteric women so as to improve their quality of life. Methods A questionnaire was distributed among 792 climacteric women. The questionnaire items included general information, characteristics of menstruation, reproduction history, information of hot flashes, lifestyle and sexual life. Weight, height, blood pressure, waist and hip circumferences were measured. Logistic regression analysis was made to find out the correlation factors for hot flashes. Results Univariate logistic regression analysis showed that age, education, income, present characteristics of menstruation, dietary habit, soy products intake, physical exercise, satisfaction of sexual life, body mass index (BMI) and systolic blood pressure were significantly associated with hot flashes. Multivariate logistic regression analysis revealed that irregular menstruation or menopause, frequent exercise and higher systolic blood pressure were risk factors for hot flashes in climacteric women. However, higher education, soy products intake and satisfaction of sexual life were protection factors. Conclusion Guiding climacteric women to take moderate physical exercise, knowing more about climacterium, consuming more soy products and controlling diastolic blood pressure are likely to decrease the incidence of hot flashes.
5.Relationship Between Plasma Level of B-type Natriuretic Peptide and Prognosis in Patients of Sudden Cardiac Arrest With Successful Cardiopulmonary Resuscitation

Objective: To explore the relationship between plasma level of B-type natriuretic peptide (BNP) and short term prognosis in patients of sudden cardiac arrest with successful cardiopulmonary resuscitation (CPR). Methods:A total of 60 relevant patients were divided into 3 groups based on their plasma levels of BNP. Group A, the patients with plasma level of BNP < 200 pg/ml at immediately, 3 hours and 12-24 hours after CPR,n=16. Group B, the patients with consistently increased BNP and at 3 hours, 12-24 hours after CPR and the BNP level > 200 pg/ml , n=22. Group C, the patients with obviously increased BNP at 3 hours after CPR, while at 12-24 hours after CPR, BNP level decreased to lower than 3 hours level,n=22. All patients were followed-up for 6 months to compare the mortality incidence among different groups. Results: There was no patient died in Group A, the mortality incidence in Group B was 11 and in Group C was 3. The 6 months survival rate in Group A was higher than that in Group B (χ2 = 11.337,P=0.001), the survival rates were similar between Group A and Group C (χ2 = 2.330,P=0.127), and the survival rate in Group B was lower than that in Group C (χ2=7.435,P= 0.006). Conclusion: Consistently increased plasma level of BNP may imply heart failure in patients of sudden cardiac arrest with successful CPR, those patients could have poor short term prognosis. It is critical to improve the cardiac function and increase the important organ infusion to make better recent clinical prognosis.
6.Sleep structure and cognitive function in stoke combined with obstructive sleep apnea hypopnea syndrome

Objective To explore characteristics of sleep structure and the correlation with cognitive function in cerebral infarction combined with obstructive sleep apnea hypopnea syndrome (CI-OSAHS).Methods The patients with CI-OSAHS and OSAHS in Department of Neurology and Breathing Sleep Monitoring Room of Tianjin Medical University General Hospital from December 2009 till March 2011 were collected All the patients completed polysomography(PSG).Sixty patients were selected and divided into 3 groups based on PSG.These 3 groups were combined group 20 persons (CI-OSAHS),OSAHS group 20persons (OSAHS) and control group 20 persons (without cerebral infarction obstructive sleep apnea hypopnea syndrome).All the patients completed image examinations ( CT and MRI ) evaluation of the cognitive function by Mini-Mental State Examination(MMSE) and Montreal Cognitive Assessment(MoCA).Results Sleep structure:the awake time,non-rapid eye movement sleep (NREM) 1,NREM 2 and NREM periods in combined group and OSAHS group were significantly longer,the NREM3 + 4 and rapid eye movement(REM) periods were shorter than the control group.The NREM and NREM 1 periods in combined group were longer,the NREM 3 +4 and REM periods were shorter than the OSAHS group.The correlation analysis of cognitive function and breathing disorders and low oxygen related index:there was negative correlation between the total scores of cognitive function (MMSE and MoCA)and apnea hyponea index,oxygen desaturation index (ODI) ( MMSE r =-0.450,-0.671,MoCA r =-0.486,- 0.494,all P <0.05) while,was positive correlation between them and noctumal average hypoxemia and minimum hypoxemia ( MMSE r =0.477,0.485,MoCA r =0.507,0.482,all P <0.05) in the OSAHS group.There was negative correlation between ODI,arousal index and the total scores of MoCA in the combined group (MoCA r=-0.463,0.480,both P<0.05),there was correlation between the total scores of MMSE and the other sleep parameters,but,there was no difference in statistics.The correlation analysis of cognitive function and sleep stages:There was positive correlation between the total scores of cognitive function ( MMSE and MoCA) and the NREM 3 + 4 periods ( r =0.521,0.474,both P < 0.05 ) while,there was negative correlation between the total scores of MMSE and the N REM 1 + 2 periods (r =-0.458,P < 0.05 )in the OSAHS group.There was positive correlation between the REM period and the total scores of MoCA (r =0.472,P < 0.05 ).There was correlation between the total scores of MMSE and the sleep structure,but,there was no difference in statistics in combined group.Conclusions Patients with OSAHS have obvious sleep structure disorder.The awake time and light sleep periods are significantly longer than the control group,while,the deep sleep and REM periods are significantly shorter than the control group.The NREM 1 of the patients with CI-OSAHS is longer than the patients with OSAHS.The higher the AHI,the lower the night blood oxygen,the more obvious cognitive dysfunction The longer the awake time,the longer the light sleep,the shorter the deep sleep and REM periods,the more serious cognitive dysfunction.The correlation between the cognitive impairment and low oxygen is more apparent than sleep structure.There is apparent correlation among the total scores of MoCA,the degree of hypoxia and sleep structure in the patients with CI-OSAHS.The total scores of MoCA are more sensitivity than MMSE in mild vascular cognitive impairment.

RNA interference(RNAi)is a process by which introduced small interfering RNA(siRNA)can cause the specific degradation of mRNA with identical sequences. The human herpes simplex virus type 1(HSV-1)RR is composed of two distinct homodimeric subunits encoded by UL39 and UL40, respectively. In this study, we applied siRNAs targeting the UL39 and UL40 genes of HSV-1. We showed that synthetic siRNA silenced effectively and specifically UL39 and UL40 mRNA expression and inhibited HSV-1 replication. Our work offers new possibilities for RNAi as a genetic tool for inhibition of HSV-1 replication.
8.Clinical study on psychosomatic model in the management of 60 cases of the functional symptoms complicated with ulcerative colitis in remission

Objective To investigate the efficacy of combination of reattribution cognitive psychotherapy and low-dose antidepressants in the treatment of ulcerative colitis (UC) complicated with functional symptoms , and to explore the psychosomatic digestive disease pattern that incorporated psychosocial variables into clinical thinking and practice .Methods From March 2016 to April 2017 ,sixty patients with UC who met the admission criteria were randomly divided into control group and observation group with 30 patients in each group .Both groups were treated with oral mesalazine ,and the observation group was further treated with reattribution cognitive psychotherapy and antidepressants . Clinical symptoms ,self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were applied to assess the efficacy at the time of enrollment and four weeks after treatment in both groups .Chi square test or t test was performed for statistical analysis .Results Among the 60 patients ,47 (78 .3% ) met four syndromes of diagnostic criteria for phychosomatic research , and there was no statistically significant difference in the case number of each syndrome between the two groups (P> 0 .05) .After four weeks treatment ,the total effective rates of observation group and control group was 93 .3% (28/30 ) and 53 .3% (16/30) ,respectively ,and the difference was statistically significant (χ2 = 22 .667 ,P< 0 .05) . Before and four weeks after treatment , the SAS scores of observation group were 54 .50 ± 6 .70 and 41 .07 ± 3 .72 ,respectively ,and the difference was statistically significant (t=9 .595 ,P= 0 .005) .The SAS scores of control group were 54 .30 ± 6 .06 and 51 .90 ± 7 .92 ,respectively ,and the difference was not statistically significant (P=0 .195) .Before and four weeks after treatment ,the SDS scores of observation group were 50 .63 ± 6 .29 and 34 .40 ± 4 .58 ,respectively ,and the difference was statistically significant (t=11 .426 ,P=0 .025) .The SDS scores of control group were 50 .03 ± 6 .02 and 43 .47 ± 6 .81 ,respectively , and the difference was not statistically significant ( P= 0 .307 ) . Conclusions Psychosomatic model can significantly improve the functional symptoms complicated with UC .It is meaningful to incorporate psychosocial variables into clinical thinking and practice in psychosomatic digestive disease pattern .
9.IL-17A secreted by Th17 cells recruited by CXCL16 against B-ALL cell apoptosis

Objective To investigate the effect of T helper cell 17(Th17)on the progression of B cell-acute lymphoblastic leukemia(B-ALL)and its possible mechanism.Methods We established the B-ALL mouse model and detected the proportion of Th17 cells in peripheral blood,bone marrow,spleen and lymph nodes by flow cytometry.Further,we used cytokine microarray to analyze the expression of cytokine in peripheral blood of B-ALL mice,and used ELISA to verify the IL-17A expression in peripheral blood of B-ALL mice.Th17 cells were co-cultured with leukemia cells,and the apoptosis ratio of Ki-67 positive cells and B-ALL cells was detected by flow cytometry,the recruitment effect of B-All cells on Th17 cells was analyzed.The expression of apoptosis-related proteins was detected by Western blot.The development of B-ALL by secreting IL-17A from Th17 cells was evaluated in vivo in a mouse model of human acute leukemia.Results Th17 cells were increased in peripheral blood(P<0.0001),bone marrow(P<0.0001),spleen(P<0.0001)and lymph nodes(P<0.0001)of B-ALL mice.The expression of IL-17A was increased in peripheral blood of B-ALL model mice(P<0.0001),and Th17 cells promoted the proliferation of B-All cells and inhibited their apoptosis through IL-17A(P<0.0001).B-All cells recruit Th17 cells through CXCL16(P<0.0001).The animal experiments showed that IL-17A treatment significantly increased the proportion of leukemia cells in peripheral blood,bone marrow and spleen of B-ALL mice,and shortened their survival time.Conclusion The proportion of Th17 cells in B-ALL increases,B-All cells recruit Th17 cells through CXCL16,and Th17 cells secrete IL-17A to promote B-All progression.
10.Value of ultrasound combined with pathological parameters in predicting axillary lymph node metastasis in breast cancer

Objective:To evaluate the value of conventional ultrasound, shear wave elastic parameters and immunohistochemistry in predicting axillary lymph node metastasis of breast cancer.Methods:The ultrasonographic features and pathological results of 172 masses in 152 breast cancer patients who underwent surgery in the First Affiliated Hospital of Shihezi University Medical College from May to October 2020 were analyzed retrospectively. The patients were divided into metastatic group and non-metastatic group according to the status of axillary lymph nodes. The conventional ultrasound characteristics, shear wave velocity (SWV) and immunohistochemical indexes (ER, PR, HER-2, Ki-67) of 2 groups of breast cancer masses were analyzed. Finally, the parameters with statistically significant difference between groups were selected and the Logistic regression model was established.Results:There were significant differences in the aspect ratio, calcification, SWVmean and HER-2 expression between metastatic group and non-metastatic group (all P<0.05). A prediction model was constructed with aspect ratio >1, calcification, high SWVmean and HER-2(+ ). The area under receiver operating characteristic curve (AUC) of the subjects was 0.891, which was larger than the single parameter (all P<0.05), and was in good agreement with pathological results (Kappa=0.731). Conclusions:The joint prediction model can be used to predict the status of lymph nodes, and the axillary lymph node metastasis is more likely to occur in breast cancer with the aspect ratio >1, calcification, high SWVmean and HER-2(+ ).
