1.Clinical efficacy of somatostatin combined with ulinastatin in the treatment of upper gastrointestinal hemorrhage and its effects on IL-2, IL-6 and TNF-α

Objective To investigate the clinical efficacy of somatostatin combined with ulinastatin in patients with upper gastrointestinal bleeding and the effects of interleukin -2 (IL-2), interleukin -6 (IL-6) and tumor necrosis factor (TNF-). Methods From May 2015 to June 2017, 90 patients with gastrointestinal bleeding were treated in the first people's hospital in Wenling, which were divided into the control group (n=45) and the observation group (n=45) by random number method. The control group was treated with simple somatostatin, and the observation group was treated with ulinastatin on the basis of the control group. Results The curative effect in the observation group was significantly better than that in the control group (P<0.05). Conclusion Somatostatin combined with ulinastatin in the treatment of patients with upper gastrointestinal bleeding is effective and helpful to improve the levels of IL-2, IL-6 and TNF- alpha, so it is worthy of popularization and application.
2.Regulation of telomerase activity and gene therapy for cancer

The hTERT gene promoter is a tumor-specific promoter, regulated by a number of factors.Modulation of the chromatin structure via DNA methylation or histone acetylation plays an important role in regulating the hTERT promoter. Recent study also found that glycogen synthase kinase -3 cytokines, macrophage colony-stimulating factor receptor 1, bone inducing factor -7, transcription factors c-myc inhibitor, transforming growth factor β, cyclopentenone prostaglandin regulate the activity of hTERT through various pathways. Telomelysin, the telomerase-specific replication competent oncolytic adenovirus, has been used in clinical settings.Various gene therapies based on hTERT are current under-way.
3.Roles of regulatory T cells and Th17 cells in pathogenesis of Henoch-Schonlein purpura nephritis in children

Henoch-schonlein purpura nephritis (HSPN)is the most common pediatric secondary renal glomerular disease.It could play an important role in the pathogenesis and progression of HSPN that regulatory T cells decline in the children and that the differentiation of Th17 cells are maybe not able to be effectively inhibited,leading to the insufficient immunosuppressive effect and the Th17 cells overexpression.We could further study the correlation among HPSN and the quantity and functional imbalance of Th17 cells and regulatory T cells to provide more theoretical basis for clinical treatment of HSPN.
4.Trends in cancer mortality in Wuyi County from 2015 to 2019

Objective:
To investigate the trends in cancer mortality among residents in Wuyi County, so as to provide insights into the development of the cancer control strategy.
Methods:
The deaths due to cancer among residents in Wuyi County from 2015 to 2019 were collected from Zhejiang Provincial Information Management System for Surveillance of Chronic Diseases. The crude mortality and standardized mortality of cancer and annual percent change (APC) of were calculated to analyze the trends in cancer mortality and causes of death.
Results:
A total of 3 424 deaths occurred due to cancer in Wuyi County from 2015 to 2019, and the crude and standardized mortality rates of cancer were 199.65/105 and 185.87/105, respectively. The crude mortality of cancer was significantly higher in men than in women (271.53/105 vs. 125.40/105;χ2=459.374, P<0.001). The standardized mortality of cancer showed a tendency towards a reduction among all study subjects, men and residents at ages of 45 to 59 years (APC=-3.87%, -4.44%,-11.46%, all P<0.05), and the crude mortality of cancer appeared a tendency towards a rise with age in all study subjects, men and women (χ2=7 063.842, 2 155.609, 5 492.128, all P<0.001). Lung cancer, liver cancer, gastric cancer, bowel cancer and esophageal cancer were the five leading causes of cancer-related mortality, which accounted for 71.41% of all cancer mortality. The crude mortality of central nervous system cancer (2.07/105) and leukemia (1.66/105) was high among residents at ages of 0 to 14 years, and the crude mortality of liver cancer (3.02/105) and lung cancer (2.01/105) was high among residents at ages of 15 to 44 years, while the crude mortality of lung cancer (51.57/105) and liver cancer (36.22/105)was high among residents at ages of 45 to 59 years, and the crude mortality of lung cancer (305.48/105) and gastric cancer (114.65/105) was high among residents at ages of 60 years and older.
Conclusions
The standardized mortality of cancer appeared a tendency towards a reduction in Wuyi County from 2015 to 2019, and men and the elderly were high-risk populations for cancer. Screening, early diagnosis and treatment of lung cancer, gastrointestinal cancer and cervical cancer should be given a high priority.
5.Distant metastases to maxillary sinus from an unknown lung adenocarcinoma: a cases report.

A 59-year-old man was admitted to the Department of Ear, Nose and Throat with a complaint of six-month history of left facial numbness and toothache. There was no special previous medical history in addition to smoking. No obvious abnormality in routine electrocardiogram, chest X-ray, abdominal B ultrasound were found. (1) CT scans showed heterogeneous shadows in maxillary sinus with the lesions on the left max- illary bone and evidently destruction of alveolar bone. The histopathological examination revealed bone tissue which was partly covered by an intact adenocarcinoma cell. (2) Immunohistochemical staining foe CK7, CD117, thyroid transcription factor-1, and novel aspartic proteinase A were positive and thus compatible with metastatic lung adenocarcinoma. (3) Chest CT scans showed a 1 cm x 2 cm mass on the superior lobe of the left lung, with destruction of sternum and rib, confirming that the lesions in the paranasal sinuses were lung cancer metastases. Therefore, this patient conclusively diagnosed as lung adenocarcinoma with multiple bone metastases.

6.Investigation and analysis for impact factors of distress in patients with first diag-nosed lung can-cer

Objective:To study the impact factors of psychological distress in patients with first diag-nosed lung cancer.Methods:The cross-sectional study was applied to newly diagnosed lung cancer pa-tients who received treatments in Cancer Center of West China Hospital of Sichuan University in Chengdu from June 201 3 to March 201 5 by distributed questionnaires.The general information of the patients,the hospital anxiety and depression scale (HADS)and distress management screening measure (DMSM) were included in the questionnaire to evaluate the states of distress,pain and the factors related to the distress of the patients.Results:The survey investigated a total of 390 patients with first diagnosed lung cancer,including 291 male patients and 99 female patients.The proportion of the patients with positive anxiety symptom was 26.7% (1 04 /390),with positive depression symptom was 27.7% (1 08 /390), and with positive distress symptom was 30.0% (1 1 7 /390).On the top five problem list of DMSMwere worry,disease treatment,breathing,pain and sleep.The Spearman correlation analysis showed that an-xiety score,depression score,and the pain intensity were positively correlated with the distress.Multiple linear regression analysis showed that gender (β=-0.209,P =0.003),age (β=-0.098,P =0.042),chronic disease (β=0.378,P <0.001 ),and pain score (β=0.1 00,P =0.029)could affect the distress of the patients.Smoking (β=0.1 1 1 ,P =0.041 )could affect the anxiety of newly diagnosed lung cancer patients.Conclusion:The gender,age,chronic disease and pain score of the patients are the independent factors of the psychological distress of first diagnosed lung cancer patients.Smoking is the predictive factors of the patients with anxiety.Controlling the pain intensity and the clinical process of chronic disease of the patients actively,solving the sleep and breathing problem and helping the patients to quit smoking progressively could alleviate the psychological distress of the patients.

8.Analysis of Antipsychotic Drugs Prescriptions in Outpatient and Emergency Department of Our Hospital dur-ing 2013-2014

OBJECTIVE:To provide reference for rational use of drugs in patients with mental disorders. METHODS:In retro-spective investigation,47 386 prescriptions of psychiatric department in our hospital during Sept. 2013 to Aug. 2014 were analyzed statistically. RESULTS:Totally 39 496 cases were enrolled in the 47 386 prescriptions. The median age of patients was 31.42,with male to female ratio of 1:1.07;they were diagnosed as schizophrenia and other mental disorder,accounting for 65.34%,and fol-lowed by mood disorders,accounting for 15.20% . They were mainly given single drug and two-drug use,accounting for 30.80%and 61.68%respectively. Clozapine,quetiapine,risperidone were top 3 prescription drugs in the list of frequency;olanzapine,que-tiapine,risperidone were top 3 drug in the list of consumption sum. Among top 20 psychiatric prescription drugs,there were 7 kinds of drugs for schizophrenia and other psychiatric disorders,3 kinds of drugs for mood disorders. CONCLUSIONS:The use of antipsychotic drugs in our hospital is rational basically.
